Abstract

Diabetes Mellitus is a chronic disease with increasing prevalence worldwide, significantly impacting patients' physical, psychological, social, and spiritual well-being. Conventional medical approaches have limitations in providing a holistic solution, thus highlighting the need for integrative strategies that improve patients' quality of life. This review aims to explore the implementation of the Tri Hita Karana concept, a Balinese philosophy that considers harmony between humans and God (Parhyangan), humans and others (Pawongan), and humans and the environment (Palemahan) in the management of diabetes mellitus. Using a qualitative review method, this study analysed literature and case studies related to the integration of spiritual, social, and environmental dimensions in the care of chronic diseases. Findings indicate that adopting Tri Hita Karana fosters better self-management, enhances treatment adherence, and provides stronger social support networks while promoting psychological resilience and spiritual coping. Furthermore, the environmental aspect encourages healthier lifestyle practices aligned with natural balance. The integration of this philosophy into healthcare practice not only strengthens patient-centred care but also supports culturally sensitive interventions in primary health services. In conclusion, Tri Hita Karana offers a transformative, holistic framework that can significantly improve the quality of life for patients with diabetes mellitus. Future studies are recommended to develop culturally adaptive models and interventional designs to optimise their application in clinical and community health settings.

Abstract

Dewandaru (Eugenia uniflora) has long been used in traditional medicine due to its antimicrobial and antihypertensive properties. Beyond its phytochemical constituents, this plant hosts endophytic microbes, especially fungi from the genera Aspergillus and Pestalotiopsis, which are capable of producing bioactive secondary metabolites.This review explores the diversity of endophytic microbes in Dewandaru and their bioactive compounds to grasp their potential application in biotechnology and medical field. Studies have shown that isolates from Dewandaru stems exhibit antibacterial activity against Staphylococcus aureus and Escherichia coli. The metabolites produced include alkaloids, flavonoids, xanthones, and spiroketals, demonstrating pharmacological effects such as antimicrobial, anticancer, anti-inflammatory, antioxidant, and antidiabetic activities. Moreover, compounds like chloropupukeananin and pestalotheol have shown inhibitory effects on HIV replication. In biotechnology, these endophytes hold promise for sustainable metabolite production through fermentation and biosynthesis. Endophytic microbes from Dewandaru possess great potential as sources of natural therapeutic agents. The biosynthetic diversity of Aspergillus and Pestalotiopsis supports the development of nature-based medicines. However, further research is needed to ensure consistent production, clarify mechanisms of action, and evaluate biosafety for broader medical applications.

Abstract

This narrative literature review aims to analyze and synthesize the latest scientific evidence on the effectiveness and benefits of yoga for managing osteoarthritis in the elderly. The review focuses on examining the impact of yoga on various aspects of physical, mental, and emotional health, specifically targeting pain reduction, improved physical function, and enhanced quality of life. A systematic approach was used to gather studies published in the last five years from major databases, including PubMed, Scopus, and Web of Science. The review includes studies of various designs, such as randomized controlled trials, quasi-experimental studies, and observational studies, that evaluate the effects of yoga interventions on elderly individuals with osteoarthritis. Key findings from the review show that yoga significantly contributes to reducing pain intensity, improving physical function (such as mobility, balance, and muscle strength), and enhancing quality of life. Additionally, yoga interventions were associated with improvements in mental health, including reduced anxiety, depression, and stress. While yoga was found to be a safe and effective non-pharmacological intervention, further research is needed to standardize yoga protocols, including the frequency, duration, and type of yoga, to maximize therapeutic benefits for elderly osteoarthritis patients.

Abstract

Anthropometry is a scientific method for studying physical variations in the human body through the measurement of specific dimensions. One important parameter in anthropometric studies is the nasal index, which is the ratio between the width and the height of the nose, describing differences in nasal morphology between populations. Variations in the nasal index are influenced by geographic and climatic factors, with populations in warmer regions tending to have larger nasal indices (wider noses), while populations in cooler regions tend to have smaller nasal indices (narrower noses). Knowledge of the nasal index has broad implications, ranging from anthropology and forensics for individual identification to clinical practices such as rhinoplasty and the diagnosis of dysmorphic syndromes. Understanding variations in the nasal index is also useful in tracking human migration patterns, environmental adaptation, and their potential association with respiratory disorders. Thus, the study of the nasal index is a crucial aspect of understanding human biological diversity and its applications in the medical and social sciences.

Abstract

Tropical Diabetic Hand Syndrome (TDHS) is a rare but severe acute complication in patients with poorly controlled diabetes mellitus. It characterized by rapidly progressive hand infections leading to cellulitis, ulceration, abscess, osteomyelitis, necrosis, and gangrene. TDHS is often underrecognized and resulting in delayed treatment and high morbidity and mortality. Early trauma, can trigger TDHS in the presence of neuropathy, vascular insufficiency, and immune dysfunction. In this case reported A 55 year old Indonesian woman with history of type 2 diabetes mellitus and chronic complications presented with progressive necrosis of the right third finger following a minor injury. Initial wound management was inadequate, and the lesion worsened progressively. Clinical evaluation revealed localized necrosis with absent digital oxygen saturation and laboratory findings of anemia, impaired renal function, and signs of infection. Radiographic imaging confirmed osteomyelitis of the affected phalanx. She underwent surgical amputation and received intravenous broad spectrum antibiotics along with strict glycemic control. Multidisciplinary management facilitated recovery and prevented systemic complications. This case underscores the diagnostic challenges of TDHS. Conventional clinical assessment may underestimate severity, and early recognition is critical to prevent irreversible damage. The pathogenesis involves a combination of hyperglycemia induced immune dysfunction, peripheral neuropathy, and vascular insufficiency, exacerbated by local environmental factors. Effective management requires timely surgical intervention, antibiotics, and rigorous metabolic control. TDHS should be suspected in diabetic patients presenting with hand infections post minor trauma. Multidisciplinary and prompt treatment including surgical management is essential to improve outcomes and reduce the risk of amputation and mortality.

Abstract

Distal radius fractures are the most common upper extremity fractures, especially in children, young adults, and the elderly, accounting for about 17.5% of all fractures worldwide. They are mainly caused by traffic accidents and low-energy trauma in older adults and are treated either conservatively with casting or operatively. This descriptive cross-sectional study used secondary medical records of distal radius fracture patients treated at Buleleng Regional General Hospital, Bali, from January to December 2024. A total of 85 patients met the inclusion and exclusion criteria, and data were collected from medical records and analyzed using Microsoft Excel and SPSS with descriptive statistics. In this study, most patients were aged 10–19 years (21.2%) and male (51.8%). Closed fractures predominated (75 patients; 88.2%), most of which were treated non-operatively (62 patients; 82.6%), whereas all open fractures (10 patients; 100%) were managed operatively. Additionally, 59 patients (69.4%) had no concurrent diseases or complaints. Distal radius fractures in this study were most common in adolescents, and these patterns align with previous reports linking adolescent and male predominance to high-risk activities and low-energy falls in older adults. However, the retrospective, single-center, cross-sectional design using secondary medical record data and descriptive analysis limits causal inference and generalizability. Overall, distal radius fractures in this study commonly affected male adolescents aged 10–19 years, were predominantly closed injuries, and were mainly managed non-operatively, with most patients having no comorbidities and DRUJ dislocation as the most frequent associated condition.

Abstract

Pharyngitis is an inflammation of the pharynx, one cause of which is the bacterium Streptococcus pyogenes. Although antibiotics are the primary therapy, high rates of resistance necessitate the search for alternative natural-based agents. This study aimed to evaluate the in vitro antibacterial activity of a combination of katuk leaf extract (Sauropus androgynus (L.) Merr.) and lemon juice (Citrus limon L.) against Streptococcus pyogenes. The study was experimental, employing a true experimental design (post-test only with a control group). Antibacterial activity was assessed using the Kirby–Bauer disk diffusion method. The results showed that the combination produced mean inhibition zone diameters of 6.02 mm at 25% concentration, 7.16 mm at 50% concentration, and 7.40 mm at 75% concentration, while the positive control produced a mean inhibition zone of 32 mm. In conclusion, the combination of katuk leaf extract and lemon juice exhibited moderate antibacterial activity against Streptococcus pyogenes and has potential for further development as a natural antibacterial candidate.

Abstract

Rheumatic heart disease (RHD) is a major burden in nations with low or middle incomes. Malnutrition is a typical issue that sets off a vicious cycle in which increasing cardiac dysfunction worsens nutritional status. As a result, patients' clinical conditions rapidly deteriorate. We report the case of a 12-year-old boy, previously diagnosed with RHD but lost to follow-up, who came to the ER with symptoms of acute heart failure in a chronic condition. Clinical examination showed extensive anasarca, a typical heart murmur, and signs of muscle atrophy. Echocardiography confirmed severe mitral regurgitation and moderate tricuspid regurgitation. Severe hypoalbuminemia (2.8 g/dL) also confirmed the patient's poor nutritional status, indicating a poor catabolic state, which contributed to the acute cardiac event. Because of the complexities of handling both of these concurrent medical conditions, the patient was transferred to a tertiary hospital for further treatment after stabilization in the ICU. The increasing complexity of RHD, non-adherence with treatment, and malnutrition will considerably worsen the patient's prognosis. Integrated and multidisciplinary management, emphasizing nutritional support and intense treatment adherence education, is a long-term strategy for breaking the vicious cycle in vulnerable populations.

Abstract

Physical fitness during early childhood supports growth and long-term health. Balinese Traditional Dance, particularly Pendet, may enhance fitness through movements engaging multiple body components. This cross-sectional study involved 24 girls aged 6 years (12 dancers, 12 non-dancers). Physical fitness was assessed using Recovery Heart Rate, the 30-Second Chair Stand Test, and the Sit and Reach Test. Data were analyzed using independent t-test and Mann-Whitney U test. Dancers showed a significantly higher mean Recovery Heart Rate (26.92 ± 11.5 vs. 12.42 ± 3.37 beats/min, p < 0.05), indicating a greater number of heart rate beats returning toward normal within a set recovery period. They also performed better in the Chair Stand Test (21.55 ± 2.58 vs. 13.5 ± 2.5 repetitions, p < 0.05), while flexibility did not differ significantly. Conclusion: Dancers demonstrated better cardiorespiratory fitness and muscle strength than non-dancers.
