Abstract

Background : The use of herbal medicine in Indonesia is getting increase. People try to keep their health or to prevent some diseases using herbal medicines. However, the safety of herbal medicine used in community has not known yet, due to the limited data and believe that herbal medicine is safer than modern medicines. Objective : This study was aimed to understand the safety of herbal medicine used in Yogyakarta. Methods : We used retrospective data of patients who were prescribed herbal medicine by the physicians in two public health centers in Yogyakarta. We searched for the patients identity in the medical records then we did the interview and gave questionnaire to the patients at their homes. The Adverse Drug Reaction was identified using Naranjo algorithm. This study has been approved by ethical committee of Universitas Ahmad Dahlan, Yogyakarta. Results : We identified 47 patients prescribed with herbal medicine as complementary treatment. There are 10 patients (21.3%) experienced ADR. The category of ADR are: probable (7 patients) and possible (3 patients). Tensigard is the most phytopharmaca cause pruritus in this study. Conclusion : Around 20% patients experienced ADR due to the herbal medicine prescription in complementary treatment. Thus, the study of pharmacovigilance is needed to be conducted in bigger sample size and cohort design to understand the safety profile of herbal medicine used.

Abstract

Background : Nowadays, cardiovascular disease is increasing in number each year. It is triggered by high cholesterol diet consumed by urban people. Green algae (Ulva lactuca L.) contains phytomelatonin levels that can ease cholesterol. Objective : To know about the effect of giving green algae ethanolic extract in male strain Wistar mice's cholesterol level. Methods : This research involved 36 male strain Wistar mice divided to six groups. Negative control injected with 2 ml/200 gBW lard, positive control injected with 0,18 mg/200 gBW simvastatin and lard, group III, V, V each given lard and injected with ethanolic extract of green algae at a dose of 50 mg/kgBW, 100 mg/kgBW and 200 mg/kgBW respectively, and normal control given AD 2 dietary treatment. Each group was given the same treatment peroral in 56 days and the blood was taken every 14 days through sinus orbitalis. The blood cholesterol level was analyzed by using enzymatic CHOD-PAP method. Data obtained were statistically examined using Two way ANOVA , Kolmogorov Smirnov, Levene's method and LSD with confidence level at 95%. Results : The blood cholesterol level were not significanly different between the negative and normal control. The result of cholesterol level for 14, 28, and 42 days for all group did not increase compared to negative control, but cholesterol level in group with 200 mg/kgBW dose extract indicated significant difference with negative control for 56 day. Conclusion : Giving ethanolic extract of green algae for 56 days decreased blood cholesterol level in male rats.

Abstract

Background : Combinations of Fluorouracil (FU) and biomodulator Leucovorin (LV) established as a standard regimen for therapy of colorectal cancer with metastases. To give better antitumor activity in colorectal cancer therapy, oxaliplatin is combined with FU/LV and give significant improvement. Fluorouracil can only be given by intravenous administration. This limitation raised effort to find alternative drugs that can be given orally, such as capecitabine. Capecitabine is an oral FU prodrug, with high oral bioavailability, highly accumulated in neoplastic tissue to be converted in FU, and well tolerated. Some clinical studies revealed effectivity of capecitabine plus oxaliplatin (XELOX) compared to FU/LV plus oxaliplatin (FOLFOX). Objective : This article is aimed to compare non inferiority of XELOX to FOLFOX in colorectal cancer with metastases, viewed form primary outcomes and secondary outcomes. Results : XELOX was comparable to FOLFOX with some benefitsover FOLFOX. Conclusion : XELOX could be considered as FOLFOX replacement as a standard therapyfor colorectal cancer with metastases.

Abstract

Nutraceutical is the fusion of nutrition and pharmaceutical. This products, in broad, are food or part of food that provides medical or health benefits including the prevention and or treatment of a disease. Kefir is renowned nutraceutical dairy products produced through fermentation of bacteria and yeasts and naturally present in grains of kefir. The nutritional attributes are due to presence of vital nutrients such as carbohydrates, amino acids, proteins, minerals, phosphorus, vitamin, calcium, and certain biogenic compounds. Intestinal immunity, antimicrobial, anticarcinogenic, hypocholesterolemic effect, antibiabetic, effect on blood pressure level, antioxidant, wound healing, and lactose intoleran can be achieved using this products. The purpose of this review is to gather information about composition, methods of production, and therapeutic properties of kefir products to provide justification for its consumption. This review confirms that kefir can be a new role of nutraceutical products.

Abstract

Disseminated nocardiosis is a rare and complicated opportunistic bacterial infection with propensity of delay in establishing the correct diagnosis and high frequency for misdiagnosis and due to the non-specific clinical presentation and the inherent difficulty in cultivating the organism. The infection is caused by gram-positive aerobic actinomycetes in the genus Nocardia. A definitive diagnosis requires the isolation and identification by Gram stain and modified acid-fast stain from a clinical specimen. Treatment heavily relies on species and susceptibility testing. There is a real paucity of prospective studies in nocardiosis infection, resulted in severely limited understanding of its diagnosis and treatment among the medical community. We report a clinical case of a 42-year old HIV-infected Burmese man with culture proven Nocardia infection.

Abstract

Background : Breastmilk is the ideal food for babies. There are various inhibiting factors for the formation of awareness of parents in providing exclusive breastfeeding for example ignorance from the mother about the importance of breastfeeding, how to breastfeed properly, and aggressive marketing launched by infant formula manufacturers. Objective : TThis study aimed to determine the association between knowledge and techniques of breastfeeding with exclusive breastfeeding in the area of Puskesmas Pengasih II, Kulonprogo. Methods : This research employed cross-sectional design. The population was all mothers with babies six to 12 months with total sample 142 mothers. The data was analyze using Chi-square test with alpha significance=5%. Results : This study found more subjects with a low level of knowledge (57%), the same goes with breastfeeding technique (52,1%). There was statistically significant association between knowledge with exclusive breastfeeding (p=0,006). There was a statistically significant association between breastfeeding technique with exclusive breastfeeding (p=0,002). Conclusion : There were relationships between knowledge (p = 0.006) and feeding techniques (p = 0.002) with exclusive breastfeeding.

Abstract

Background: Cilembu Village - Sumedang, West Java is the producer of sweet potato that tastes sweet. The sweet taste was obtained from potato that has been through the process of incubation for 3 weeks, with a result of the change of starch by amylase enzyme into reducer sugars in the sweet potato Cilembu. Objective: The aim of the study is to verify the levels of reducer sugar of sweet potato Cilembu the analysis of reducer sugar itself is required and the comparison of the methods used. Methods: Among many calorimetry methods that used to analyze reducer sugar, the most widely used is Anthrone. The analysis of total carbohydrates, especially reducer sugar is quite simple and sensitive. but the analysis method for reducer sugar is quite simple SNI 3457.1:2008 using Luff Schoorl method that based on titrimetric, it is consuming time, difficult to do for untrained analyser and the reduction reaction is not stoikiometric. the candidate method that using Anthrone method is recommended in order to replace Luff Schoorl method of reducing sugar in SNI 3457.1:2008. Results: The average levels of reducer sugar in sweet potato Cilembu that used Luff Schoorl as a method was 14.88 ± 0.44 (%) whereas Anthrone method was 21,60 1,31 (%). The average levels of reducer sugars in sweet potato Cilembu from Luff Schoorl method completed the Indonesia Pharmacopoeia conditions, that is has a recovery value a 92,49 % % and but has 3,25% CV value. The average levels of reducer sugar in sweet potato Cilembu with Anthrone method did not complete the Pharmacopoeia Indonesia conditions, which has a recovery value a 67,58 % and has CV 6,56 % value. Conclusion: Anthrone method not recommended for analyze the reducer sugars in sweet potato Cilembu.

Abstract

Background : Diabetes Mellitus (DM) is a disease that can be known by increasing a blood glucose level and caused many kinds of complications if it don't properly treatment, one of those complications is a diabetic ulcer. There are many types of treatments created to overcome the diabetic ulcer, but there are not effective yet. Therefore, ethanolic extract gel of binahong (Anredera cordifolia (Tenore) Steen) leaves is used to make a new innovation of diabetic ulcer treatment Objective : The objective of this research wasto know the concentration of antibacterial and anti-infection activity from ethanolic extract of binahong leaves as wound healing on diabetic ulcer and also to know the changeover of wound diameter. Methods : Binahong leaves were extracted with 96% ethanol by maceration. Then the extract was formulated to be gel product with the concentration of 10% and 30%. The gel product was administrated to diabetic rats which had been made ulcer wound by excision. The result of wound diameter and the percentage of wound healing were analyzed by One Way Anova and then continue analyzed by LSD (Least Significant Different) with significant level of 95%. Results : The result showed that binahong gel with concentration variation of 10% and 30% only affected the organoleptic and doesn't affect the homogeneity, pH, irritation, spreadability and consistency. The result of the effectiveness test of binahong leaves gel is 10% more effective to changeover of wound diameter but there is not significantlydifferent if compared with 30% gel of binahong leaves. Therefore, gel of binahong leavesof 10% is able to provide slightly effective than chloramphenicolâ„¢. Conclusion : The concentration of 10% and 30% of binahong gel were effectively usage for wound healing diabetic ulcer in rats.
