Abstract

Cyclin D1 is a protein that plays a role in the transition from the G1 to S phase of the cell cycle. Cyclin D1 expression has been found to increase in various malignancies and, in many studies, was associated with tumor growth, stage, lymph node involvement, distant metastases, and poor prognosis. Until now, studies on the association of cyclin D1 expression level with chemotherapy response have shown different results. An in-depth understanding of the cell cycle will allow doctors to develop target therapies that work when specific interventions are carried out at certain stages. Some studies reported that cyclin D1 expression was inversely related to chemotherapy response, while others showed opposite results. A significant number of studies have attempted to elucidate this ambiguous effect of cyclin D1. The suggested mechanism involves the difference of cancer cell types, the effect of chromosome instability in a few malignancies, trigger to an excessive DNA repair protein expression stimulus, and the response to DNA damage severity. The ambiguous effect of cyclin D1 towards chemotherapy was thought to arise from the difference in tumor type, chemotherapy agents used, and cell damage severity caused by cytostatics as per different research works. More in-depth research with parallel evaluation of other possible mechanisms such as DNA repair should elucidate the reason behind the inconsistent findings.

Abstract

Background: Thyroid Stimulating Hormone (TSH) and Free Thyroxin (FT4) concentration are currently highlighted in their relation to thyroid carcinoma development in thyroid nodule patients. This study aimed to identify the difference in the pre-operative mean serum concentration of TSH and FT4 in patients with benign thyroid nodules and thyroid carcinoma at Sanglah General Hospital Denpasar.Methods: : This study was a case-control study that involved 110 patients with thyroid nodules who underwent thyroidectomy at Sanglah General Hospital from January until December 2019, whose specimens were examined histopathologically at the Anatomical Pathology Laboratory of Sanglah General Hospital. Cases were patients with thyroid carcinoma. Controls were patients with benign thyroid nodules. The data were collected from medical records. Results: Most subjects in the thyroid carcinoma group were female (72.7%). The thyroid carcinoma group had a higher mean age at the time of thyroidectomy (47.33 ± 13.4) compared to the benign thyroid nodule group (46.07 ± 12.5) (p = 0.61). Significant difference was found regarding the nodule size between the two groups (p < 0.001). There were no significant differences in terms of nodule lateralization (p = 0.56) and the number of nodules (p = 0.58). Papillary thyroid carcinoma was the most common type of thyroid carcinoma (89.1%). A significantly higher pre-operative mean serum TSH concentration was found in cases (1.0 ± 0.23 IU/mL) compared to controls (0.8 ± 0.23 IU/mL) (p < 0.001). The pre-operative mean serum FT4 concentration was significantly lower in cases (1.1 ± 0.25 IU/mL) compared to controls (1.2 ± 0.22 IU/mL) (p = 0.006). The optimal TSH and FT4 cut-off values for thyroid carcinoma were > 1.0 IU/mL (61% sensitivity, 71% specificity) and < 1.1 IU/mL (61% sensitivity, 51% specificity), respectively Conclusions: : Higher pre-operative mean TSH concentration and lower mean FT4 concentration were found in patients with thyroid carcinoma. There were statistically significant differences between patients with thyroid carcinoma and benign thyroid nodules in terms of these two laboratory parameters.

Abstract

Timoma merupakan neoplasma epitelial kelenjar timus yang paling sering ditemukan di mediastinum anterior, pada usia dekade keempat sampai dengan keenam dan memiliki predileksi yang hampir sama antara laki-laki dan perempuan.Lokasi, morfologi dan ada/tidaknya invasi tumor pada pencitraan radiologis dapat membantu mengarahkan diagnosis. Pemeriksaan tomografi komputer toraks dengan kontras (enhanced chest CT scan) berperan besar dalam menilai morfologi dan menentukan stage timoma.Laporan kasus ini menampilkan seorang laki-laki 59 tahun yang datang dengan keluhan utama suara serak, batuk lama dan sesak napas. Pada pemeriksaan foto toraks didapatkan gambaran massa mediastinum, dan pada pemeriksaan CT scan toraks dengan kontras ditemukan massa mediastinum anterior yang telah menginvasi perikardium dan pleura. Hasil temuan sitopatologi TTNA dengan tuntunan CT scan sesuai dengan timoma tipe campuran. Clinical stage menurut Masaoka sesuai dengan timoma stadium IVA. Pada kasus ini surgical stage sesuai dengan clinical stage. Islamun hasil histopatologi pasca bedah sedikit berbeda dengan hasil sitopatologi TTNA, yaitu karsinoma timik.Kata kunci: timoma, foto toraks, CT scan toraks dengan kontras

Abstract

Background: Cervical and breast cancers are the two top leading cases of female cancer in Indonesia. Nowadays, many survivors with various degree of cancer-related fatigue (CRF) were found in the community context. This study aimed to compare and analyse the differences of CRF intensity in cervical cancer survivors (CCS) and breast cancer survivors (BCS) generally, and in each stage of cancer survivorship, specifically.Methods: This cross-sectional study involved 47 CCS and 55 BCS in the district of Rangkah, Gading, and Pacar Keling, Surabaya, Indonesia (n=102). Instrument of Fatigue Symptom Inventory (FSI) was used for data collection. Descriptive statistic, independent sample t-test, and one way ANOVA test were used for data analysis (α<.05).Results: Most respondents were short term survivors (43.14%) with mild CRF (66.67%) in both cases. Higher intensity of CRF was found in CCS compared to BCS. Worst CRF was found more in acute and short term survivorship in BCS compared to more in long term survivorship in CCS. There was no significant difference of CRF found between CCS and BCS (p=.63), and CRF was not significantly different between each stage of cancer survivorship in both cases (p=.883 and p=.117 for CCS and BCS respectively), but CRF intensity in the worst and lightest time possible was significantly different between CCS and BCS (p=.000).Conclusions: There was no significant difference of CRF between cases and stages of survivorship in CCS and BCS, but CRF intensity in the worst and lightest time possible was significantly different between those groups.

Abstract

ABSTRACTThe aim of this study to evaluate clinical and management of penile cancer patients at Sardjito Hospital. Data were collected from medical record of Sardjito Hospital from 2006 - 2013. The clinical factors and therapy were studied from each patient. Correlation between age and stadium of disease were analyzed using Chi-Square Test while the correlation among pathological grade and T, N and M stage were analyzed by using Fisher Test. There are 35 cases of penile cancer with mostly of 40-60 years old man (45.7%) , circumcised (42.9%), squamos cell type (91.4%) and perform partial penectomy (45.7%) followed by chemotherapy (6 patients) and radiotherapy (4patients). There are no correlation between age and stadium of cancer (P>0.05), also among pathological grade with T, N and M Stage (P>0.05). Penile cancers at Sardjito Hospital mostly found on 40-60 years old, and already developed into advanced disease. Thetherapy is consist of partial penectomy followed by chemo- radiotherapy.ABSTRAKPenelitian ini bertujuan untuk mengevaluasi klinis dan manajemen kanker penis di Rumah Sakit Sardjito. Data dikumpulkan dari rekam medis RS Sardjito tahun 2006-2013. Faktor klinis dan terapi dipelajari dari tiap pasien. Hubungan antara usia dan stadium penyakit dianalisis menggunakan tes Chi-Square, sedangkan hubungan antara tingkatan histopatologi dengan stadium T, N, M dianalisis dengan tes Fisher. Ada 35 kasus kanker penis dengandominan pada usia 40-60 tahun (45,7%); telah disunat (42,9%); tipe karsinoma sel skuamosa (91,4%); dan dilakukan penektomi parsial (45,7%) diikuti dengan kemoterapi (6 pasien) serta radioterapi (4 pasien). Tidak ada hubungan antara usia dengan stadium penyakit (>0,05) dan juga antara tingkatan histopatologi dengan stadium T, N, M (p>0,05). Kanker penis di RS Sardjito kebanyakan ditemukan pada usia 40-60 tahun dan telah berkembang menjadi tingkat lanjut. Terapi terdiri dari penektomi parsial diikuti kemo-radioterapi.

Abstract

Introduction: Chronic Myeloid Leukemia is a hematological malignancy driving from myeloproliferative process. It is typified by the presence of the Philadelphia chromosome manifesting in certain distinct complications, including pathological fracture. Pathological fracture is recognized as an extramedullary disease that occurs as a result of transformation of CML into blast crisis phase.Case Presentation: Here, we report a case of pediatric male CML. After being failed with imatinib therapy, he turned to nilotinib and was unable to achieve a major molecular response. He presented with high blast count and pain in the left arm. He was diagnosed with pathological fracture and blast crisis phase CML. Taken the young age and displacement of fracture into consideration, he was conservatively treated by a combination of immobilization and a higher dose of targeted therapy, nilotinib. The 2-month evaluation revealed clinical union and reduction of blast cells.Conclusions: Regarding the minimal displacement and age presentation, pathological fracture in pediatric CML requires non-invasive treatment and optimization of antileukemic therapy.

Abstract

Penelitian ini bertujuan membuktikan pengaruh model interdisiplin pasien kanker serviks dengan gangguan fungsi ginjal terhadap efektivitas dan biaya perawatan. Efektivitas diukur dengan lama masa rawat, status fungsional, dan kualitas hidup penderita. Analisis biaya efektivitas dilakukan untuk menilai apakah biaya yang dibutuhkan pada pendekatan interdisiplin lebih efektif.Penelitian dilakukan menggunakan desain randomized clinical trial terhadap 40 pasien kanker serviks stadium lanjut dengan gangguan fungsi ginjal/KSSLGFG dari Komite Etik Fakultas Kedokteran dan Fakultas Kesehatan Masyarakat Universitas Indonesia, Januari 2007 hingga Januari 2008. Kriteria inklusi adalah semua pasien KSSLGFG menurut FIGO, disertai dengan gangguan fungsi ginjal, yaitu LFG < 60 mL/menit/1,73m2; kreatinin > 2 mg%; dan ureum > 50 mg%. Sampel diambil secara random alokasi dengan sampel random dilakukan pencatatan dan pemeriksaan identitas, tanggal masuk, skor ICF, skor ADL, skor Karnofsky, skor EQ-5D, dan QLQ-C30. Setelah fase akut teratasi dan dapat dinyatakan boleh pulang maka dicatat tanggal kepulangan, biaya perawatan, skor karnofsky, skor ADL, skor ICF, skor EQ-5D, atau QLQ-C30. Penelitian cost effectiveness dilakukan dengan menggunakan perangkat Treeage Pro 2008.Hasilnya, lama rawat kelompok interdisiplin (14,2 [+1,87] hari) lebih singkat jika dibandingkan dengan kelompok konvensional (23,59 [+3,61]hari), p: 0,0280. Kualitas hidup terkait kesehatan pasien KSSLGFG yang diukur dengan instrumen QLQ-C30 menunjukkan bahwa skor pada kelompok interdisiplin lebih baik dari konvensional (8,44 [0,49] vs 6,32[0,33]; p = 0,0013). Sementara itu, untuk penilaian status fungsional dengan Barthel Indeks pada kelompok interdisiplin lebih baik dari konvensional (11,05 [1,16] vs 8,09 [0,68]; p= 0,036). Rerata total biaya selama perawatan pada kelompok konvensional lebih besar jika dibandingkan dengan kelompok interdisiplin (Rp 6.726.100,00 vs Rp 3.632.700,00).Analisis cost effectiveness total biaya menunjukkan pendekatan interdisiplin lebih baik daripada pendekatan konvensional. Pada kelompok interdisiplin, setiap pasien mengeluarkan biaya rata-rata Rp 3.625.378,00 dan mendapatkan 4.23 QALY. Sementara, pada kelompok konvensional setiap pasien harus mengeluarkan biaya rata-rata sebesar Rp 6.703.557,00 dan hanya mendapatkan 3.16 QALY.Kesimpulannya, pengelolaan pasien KSSLGFG dengan model interdisipliner memiliki efektivitas biaya yang lebih baik jika dibandingkan dengan pendekatan konvensional. Pada kelompok interdisiplin, masa rawat lebih singkat, status fungsional lebih baik, dan kualitas hidup lebih baik.Kata kunci: interdisipliner, kanker serviks, status fungsional, kualitas hidup, cost effectiveness

Abstract

Penelitian ini bertujuan melaporkan hasil Retropubik Radikal Prostatektomi (RRP) oleh Tim Urologi FKUI/RSCM, mengevaluasi hasil onkologi pasca-operasi, serta menganalisis hubungan antara data klinis sebelum operasi dan data onkologi setelah operasi denganbiochemical failuresatu tahun pasca-RRP.Materi dan metode: antara Januari 1995 sampai Juli 2009 dikumpulkan 41 pasien pasca-RRP yang telah memenuhi kriteria inklusi. Data pre-operasi meliputi: usia pasien,Prostate Specific Antigen(PSA) pre-operasi, volume prostat,stagingtumor secara klinis,Gleason Score(GS), dan riwayatTrans Urethral Resection of Prostate(TURP) pre-operasi. Data pasca-operasi meliputi: lama pemasangan kateter, lama rawat pasca-operasi, hasil onkologi pasca-operasi (invasi vesikula seminalis, batas sayatan,stagingtumor, keterlibatan kelenjar getah bening pasca-RRP menurut klasifikasi TNM 2002, danGleason Scorepasca-RRP), migrasi staging, migrasiGleason Scoredanbiochemical failuresatu tahun pasca- RRP. Dilakukan analisis hubungan antara data pre-operasi dan data onkologi terhadapbiochemical failuresatu tahun pasca-RRP.Hasil: rerata usia pasien adalah 62,44 tahun; rerata volume prostat 41,16 cc; median PSA pre-operasi 15,9 ng/ml; 61% cT1 dan 39% cT2 atau lebih. Setelah dilakukan RRP, pasien denganstagingklinis cT1 dan cT2 didapatkan 45% memilikistagingpatologis ? pT3.Kesimpulan: dari data onkologi pasca-operasi kita dapat memprediksi kemungkinan akan terjadinyabiochemical failurepada tahun pertama pasca-RRP, namun dari data pre-operasi kita belum bisa memprediksi kemungkinan tersebut.Kata kunci: radikal retropubik prostatektomi,oncologycal result,biochemical failure

Abstract

Introduction: Kimura’s disease (KD) in Indonesia is quite rare. It is a distinct benign reactive process; however, it is usually mimicking malignancy. It usually affects young adults with a peak incidence in three decades, and men are mostly affected compared to women with a 3:1 ratio. KD is a chronic inflammatory disorder characterized by lymphoid hyperplasia, eosinophilia, and associated with soft tissue swelling that usually arises in head and neck regions with regional lymphadenopathy.Case Presentation: Herein, we report a case in a 55-year-old man from East Indonesia who presented a huge lump on his left head and neck regions. The lump started about 8 years ago. Physical examination found a tumor mass of 20 x 10 cm with indistinct border and multiple lymphadenopathies left coli with the biggest size of 1.3 x 0.7 x 0.5 cm. Peripheral blood examination shows marked eosinophilia. Nasopharyngeal CT scan revealed soft tissue masses and multiple lymphadenopathies suspected of malignancy; however, thorax plain radiology did not show any sign of metastatic process. Histopathological examination shows reactive follicular proliferation, extensive eosinophilia, focal area Folliculolysis, and polykaryocytes of the WarthinFinkeldey type.Conclusion: Based on a complete history, laboratory findings, and histopathological examination, this case is concluded as Kimura’s disease. Although clinical findings and radiological examination suspected it as malignancy, those discrepancies were confirmed as a benign lesion by specific histopathological and laboratory findings. Commonly, most patients have a favorable prognosis and good response to therapy.

Abstract

Marilah kita panjatkan puji-syukur ke hadirat Allah SWT atas segala rahmat dan karunia yang telahdilimpahkan kepada kita semua, sehingga kita dapat berkumpul disini untuk menghadiri peringatan Hari UlangTahun ke 16 Rumah sakit Kanker Dharmais dalam keadaan sehat walafiat. Selanjutnya, pada kesempatan yangberbahagia ini perkenankanlah saya mengucapkan selamat ulang tahun ke 16 kepada jajaran direksi, pejabatstruktural dan fungsional, dan segenap karyawan/karyawati Rumah Sakit Kanker Dharmais. Semoga rumah sakitini makin maju dan semakin mendapat tempat di masyarakat Indonesia sebagai rumah sakit khusus kanker yangberkualitas tinggi. Saya mengamati bahwa pada usia 16 tahun ini, yaitu dalam usia yang masih cukup muda,Rumah Sakit Kanker Dharmais telah menunjukkan prestasinya dalam menjalankan peran rumah sakit denganfungsi utamanya melayani para pasien kanker dan melaksanakan peran penelitian serta pendidikan.
