Abstract

Right-sided abdominal pain caused by gallbladder inflammation (cholecystitis) is a common clinical condition that can significantly affect patients’ quality of life. Conventional management generally focuses on pharmacological therapy and surgical intervention, but these approaches may present limitations such as adverse effects and high medical costs. Therefore, complementary therapeutic approaches such as acupuncture have gained increasing attention as potential supportive treatments. This study aimed to describe the implementation and clinical outcomes of acupuncture care in reducing right-sided abdominal pain in a patient with cholecystitis treated at a therapy center in Banjarmasin. The research employed a qualitative case study design involving a 45-year-old female patient diagnosed with cholecystitis. Data were collected using a structured client data sheet based on the Traditional Chinese Medicine (TCM) diagnostic framework, including observation (Wang), listening and smelling (Wen), inquiry (Wen), and palpation (Qie). The participant received six acupuncture therapy sessions over a one-week period. Data analysis was conducted descriptively by comparing clinical findings before and after treatment. The results showed significant improvement in both subjective complaints and objective clinical indicators. The patient initially presented with severe right upper abdominal pain accompanied by nausea, decreased appetite, bitter taste in the mouth, and radiating pain to the scapula. After six therapy sessions, the main complaint of abdominal pain disappeared, accompanying symptoms decreased markedly, and physical examination findings improved. Ultrasonography also indicated the disappearance of previously detected gallstones. These findings suggest that acupuncture therapy may contribute to symptom reduction and functional improvement in patients with cholecystitis.

Abstract

Plantar fasciitis is a common musculoskeletal disorder characterized by degeneration of the plantar fascia due to repetitive mechanical stress that causes microtears, tissue irritation, and persistent heel pain, which can significantly impair daily activities and quality of life. Although conventional treatments are widely used, acupuncture has increasingly been explored as a complementary therapy for pain management. This study aimed to describe the implementation of acupuncture care for a patient with plantar fasciitis at the Bumi Sehat Acupuncture Clinic in Ubud, Gianyar, Bali. A qualitative case study design was applied to document the clinical management process of acupuncture treatment. The study involved a 55-year-old male patient who underwent six acupuncture treatment sessions during April 2025. Data were collected through the traditional four diagnostic methods of Traditional Chinese Medicine, including observation, listening and smelling, inquiry, and palpation, and were recorded using a structured client data sheet. The patient was diagnosed with plantar fasciitis associated with Cold-Bi syndrome. The therapeutic approach included acupuncture point stimulation combined with moxibustion to remove meridian obstruction, warm the channels, and promote the circulation of Qi and blood. Clinical evaluation across treatment sessions demonstrated progressive improvement in symptoms, including a gradual reduction in plantar pain, decreased heel elevation during walking, improved functional mobility, and resolution of calf stiffness and sleep disturbance. By the fifth treatment session, the patient reported complete relief from pain and regained normal walking ability, which was maintained at the sixth session without recurrence. These findings suggest that structured acupuncture care may contribute to pain reduction and functional recovery in patients with plantar fasciitis. The study provides clinical insight into the application of acupuncture as a complementary therapeutic option in the management of plantar fasciitis.

Abstract

Introduction: This study analyzes the disparity between Personal Protective Equipment (PPE) regulations and field practices among Live-Line Maintenance workers (PDKB) at PT PLN (Persero) UP3 Sukoharjo. The research was motivated by a fatal incident in Lubuk Pakam in 2023 and three near-miss incidents at the research location caused by negligence in the use and supervision of PPE. Research Method: This study employed a qualitative approach with a case study design. Data were collected through in-depth interviews with five informants selected purposively (technicians, preparators, team leaders, and OHS officers), field observations, and documentation review. The data were analyzed using the Miles and Huberman model. Data validity was ensured through source and method triangulation. Research Results: The findings indicate a strong organizational commitment through the provision of internationally standardized PPE, certified training, periodic testing every six months, and multi-layered supervision in accordance with Law No. 1/1970, Government Regulation No. 50/2012, and Minister of Manpower Regulation No. 8/2010. The level of worker compliance is relatively high due to awareness of the risks associated with 20 kV electrical voltage and the safety culture of “one opens, three remind,” with PPE internalized as a “second life.” However, two main disparities were identified. First, PPE storage in operational vehicles and basecamps has not yet met the ideal standard of controlled storage facilities; this condition is compensated through adaptive mechanisms such as special storage boxes, air-conditioned rooms, and psychological ownership among workers. Second, regarding PPE materials, safety sleeves that should be made of insulating rubber are still made of plastic due to previous procurement. The multi-layered supervision system, supported by peer monitoring, has proven effective in detecting three near-miss incidents before they escalated into fatal events. Conclusion: This study concludes that the effectiveness of OHS implementation is not solely determined by formal compliance based on regulations (rules-based approach), but also by organizational safety culture (safety culture-based approach) and the synergy of social capital. Recommendation: The study recommends accelerating the transition to PPE that meets current standards, improving storage facilities, and strengthening the quality of double-check procedures.

Abstract

The rapid expansion of social media has significantly influenced adolescents’ mental health, yet most existing studies rely on quantitative approaches that fail to capture adolescents’ subjective experiences. This study aims to explore adolescents’ lived experiences of social media use and its impact on mental health from a psychiatric nursing perspective. A qualitative study with a phenomenological design was conducted involving 10 high school students in West Jakarta, Indonesia, selected through purposive sampling. Data were collected using semi-structured in-depth interviews and analyzed using thematic analysis following Braun and Clarke’s approach. Data credibility was ensured through member checking. The findings reveal that social media use among adolescents has a dualistic impact. Positive effects include enhanced social connectivity, access to information, and opportunities for self-expression. However, negative impacts are more dominant, including anxiety, stress, depressive feelings, social comparison, sleep disturbances, and decreased concentration. These experiences indicate maladaptive coping mechanisms and emotional vulnerability in navigating digital interactions. Additionally, adolescents demonstrated emerging adaptive strategies such as limiting usage, avoiding negative content, and increasing offline engagement. This study highlights that the impact of social media is not solely determined by usage duration but by how adolescents interpret and manage their digital experiences. Therefore, psychiatric nursing interventions should focus on strengthening digital literacy, emotional regulation, and adaptive coping strategies to promote adolescent mental well-being in the digital era.

Abstract

Leprosy (Hansen’s disease) remains a major public health problem in endemic countries, where the decline in new-case detection has been slow, suggesting ongoing transmission, particularly among household members and other close contacts who face a higher risk of disease. This study aimed to evaluate the effectiveness of rifampicin as post-exposure prophylaxis (PEP) for preventing leprosy among contacts of index cases, while also describing variations in prophylactic regimens. The review was conducted in accordance with PRISMA guidelines through literature searches in PubMed and Google Scholar using combinations of terms related to leprosy/Mycobacterium leprae and rifampicin. Study selection involved title/abstract screening followed by full-text assessment based on predefined inclusion and exclusion criteria. Six studies met the eligibility criteria; these were predominantly randomized trials and evaluated diverse interventions, including single-dose rifampicin (SDR), double-dose rifampicin PEP at 20 mg/kg (SDDR-PEP), SDR plus BCG vaccination, and enhanced prophylactic regimens under protocol evaluation, which is rifampicin–bedaquiline and PEP++ (rifampicin–clarithromycin). Follow-up durations were evenly distributed between 24 and 48 months. The synthesized evidence indicates that SDR reduces leprosy incidence primarily within the first two years (≈57%), although the protective effect tends to attenuate during years 2–4. In contrast, SDDR-PEP (20 mg/kg) was associated with a reduction in individual-level risk (IRR 0.55; 95% CI: 0.36–0.83), with stronger protection among household contacts (IRR 0.35; 95% CI: 0.15–0.82). Overall, rifampicin-based PEP appears to reduce leprosy occurrence among close contacts, particularly in the early post-intervention period; however, the magnitude of impact is influenced by transmission dynamics, the breadth of contact targeting, and the quality of screening and program implementation. Evidence for enhanced PEP regimens remains preliminary and requires definitive effectiveness results before broad recommendation.

Abstract

Unilateral facial paralysis, also known as Miantan, refers to paralysis affecting one side of the face. One type of Miantan is Shi Syndrome, which is caused by pathogenic factors such as wind-cold and wind-heat. These factors lead to obstruction of the flow of Qi in the Yang Ming and Shao Yang meridians, resulting in a “wind-phlegm” syndrome. This condition manifests with various associated symptoms, including weakness of the facial muscles, and may lead to the characteristic clinical features of Bell’s palsy. Bell’s palsy is a peripheral facial nerve paralysis characterized by weakness or paralysis on one side of the face due to dysfunction of the seventh cranial nerve. This condition is characterized by inability to close the eyes completely, drooping of the mouth corner, and facial asymmetry. This study aimed to determine the effectiveness of acupuncture care in patients with Bell’s palsy at the Integrative department of St. Carolus Borromeus Hospital Kupang. This study used a qualitative case study design. The participant was a 35-year-old female patient diagnosed with left-sided Bell’s palsy. Data were collected using acupuncture diagnostic methods including observation (Wang), listening and smelling (Wen), questioning (Wen), and palpation (Qie). Acupuncture therapy was administered based on the established diagnosis of disease and syndrome. The results showed facial asymmetry on the left side, inability to raise the forehead, and incomplete eyelid closure. After several acupuncture therapy sessions, improvements were observed in facial muscle function, eyelid closure ability, and facial expressions. This study concludes that acupuncture therapy can improve facial nerve function and accelerate recovery in patients with Bell’s palsy.

Abstract

Chronic obstructive pulmonary disease (COPD) is a chronic respiratory disorder with persistent airflow limitation and prone to acute exacerbations, in which blood eosinophil levels have been proposed as a marker of type 2 inflammation and response to therapy. This study aimed to determine the relationship between clinical-demographic characteristics of patients with acute exacerbations of COPD hospitalized at Waled Regional Hospital and blood eosinophil levels during the period of September 2023–September 2024. This study was a quantitative observational analytical study with a cross-sectional design conducted on 132 inpatients with exacerbations of COPD who met the inclusion criteria based on medical records, including diagnosis, first-time blood eosinophil laboratory results, and the absence of active helminth infections. Data were collected retrospectively from medical records and analyzed using univariate descriptive statistics and Spearman's rank correlation test in SPSS. The results showed that 62.1% of patients were male, 68.9% were aged ≥60 years, 75.0% had normal eosinophils (0–300 cells/μL), and 25.0% had elevated eosinophils (>300 cells/μL). Statistical tests found a significant but weak association between male gender and blood eosinophil levels (p = 0.023, rs = 0.198), while no significant association was found between age or occupational history and eosinophil levels. The results showed that in this regional referral hospital setting, elevated eosinophils were more common in male patients with acute exacerbation of COPD, while age and occupation did not show a significant association. The conclusions of the study underscore the need for larger prospective studies to clarify the role of eosinophils as a marker of severity and response to therapy in low-middle-income areas.
