Abstract

Waiting time in pharmacy services is one of the key indicators of hospital service quality that influences patient satisfaction and perception. Perceived waiting time is not solely determined by the actual duration of service but is also affected by operational and psychological factors experienced by patients during the waiting process. This study aims to analyze the influence of service process efficiency, pharmacists’ communication, waiting room comfort, and drug availability on patients’ perceived waiting time at the Outpatient Pharmacy Installation of RS X, Bekasi City Indonesia. This study employed a quantitative approach with a cross-sectional design. The population consisted of outpatients who redeemed prescriptions at the pharmacy installation, and samples were selected using an accidental sampling technique. Data were collected through a structured questionnaire that had been tested for validity and reliability. Multiple linear regression analysis was used to determine both partial and simultaneous effects among variables. The results indicate that service process efficiency, pharmacists’ communication, waiting room comfort, and drug availability have a significant partial effect on patients’ perceived waiting time (p < 0.05). Simultaneously, all four variables also significantly influence perceived waiting time. Service process efficiency was identified as the most dominant factor affecting patients’ perceptions. These findings suggest that improving pharmacy service quality requires a comprehensive strategy, including workflow optimization, enhancement of pharmacists’ communication skills, improvement of waiting room facilities, and strengthening drug inventory management to create a better patient service experience.

Abstract

Patient waiting time is one of the key indicators in assessing the quality of hospital services, particularly in specialized services that require operative procedures such as urology care. The increasing number of urology patients that is not supported by an integrated surgical scheduling system may lead to service delays, operational inefficiencies, and decreased patient satisfaction. This study aims to evaluate the effectiveness of integrated surgical scheduling implementation in reducing patient waiting time at the Urology Clinic of RSUD dr. Drajat Prawiranegara Serang Indonesia. This study employed a descriptive qualitative design with an evaluative approach. Data were collected through in-depth interviews, direct observations, and document reviews related to the scheduling and delivery of urology services. Informants were selected using purposive sampling and included urologists, nurses, administrative staff, and hospital management personnel directly involved in the surgical scheduling process. Data analysis was conducted thematically through data reduction, data presentation, and conclusion drawing, supported by source and method triangulation to ensure data validity. The findings indicate that the implementation of an integrated surgical scheduling system improved coordination among service units, enhanced the operational flow of surgical services, and contributed to a reduction in patient waiting time at the Urology Clinic. However, the effectiveness of the system has not yet been fully optimized due to limitations in supporting infrastructure, incomplete integration of hospital information systems, and varying levels of human resource readiness in operating digital scheduling systems. In conclusion, integrated surgical scheduling is an effective managerial strategy to improve service efficiency and the quality of urology care. System optimization requires strong managerial commitment, enhancement of health information technology infrastructure, and continuous capacity building of human resources.

Abstract

Chronic kidney disease (CKD) is a global health problem with increasing prevalence, making haemodialysis a critical service in hospitals. Patients' decisions to reuse dialysis services are influenced not only by medical factors, but also by ethical communication between healthcare professionals and patient trust. This study aims to analyse the influence of ethical communication and patient trust on the decision to reuse dialysis services at Dr. Dradjat Prawiranegara Regional General Hospital, Serang, Indonesia. The study used a quantitative approach with a survey of 133 active haemodialysis patients. Instrument validity was tested using Confirmatory Factor Analysis (CFA) with a loading factor >0.5 and Cronbach's Alpha reliability >0.7. Data analysis was performed using Structural Equation Modelling (SEM) using AMOS. The results of the study proved that ethical communication had a positive and significant effect on patient trust (CR = 7.669; p = 0.000), patient trust had a significant effect on the decision to reuse dialysis services (CR = 6.388; p = 0.000), and patient trust was proven to be a mediating variable that strengthened the influence of ethical communication on patient decisions (indirect effect = 0.476). This study emphasized the importance of integrating ethical communication and strengthening patient trust as a managerial strategy to improve service quality and support the sustainability of dialysis services.

Abstract

Growth and development of children aged 2–4 years are critical indicators in determining long-term health outcomes and school readiness. This period is considered the golden age, as rapid brain development occurs, making early detection of growth and developmental disorders essential. However, developmental screening coverage in Indonesia remains suboptimal, and monitoring in early childhood education settings often focuses more on physical growth than developmental aspects. This study aimed to describe the growth and developmental status of children aged 2–4 years using the Maternal and Child Health (MCH) Handbook and the Denver Developmental Screening Test (DDST) at PAUD Vinolia Mojolangu. A descriptive observational design was applied to 23 children. Growth was assessed using height-for-age (HFA), weight-for-age (WFA), weight-for-height (WFH), and body mass index-for-age (BMI-for-age) indicators. Developmental status was evaluated using the DDST, which measures four domains: gross motor, fine motor, language, and personal-social skills. The results showed that most children were classified as having normal growth based on WFH and BMI-for-age indicators. In terms of development, the majority demonstrated age-appropriate outcomes, with one child categorized as “suspect” in the personal-social domain. These findings indicate that integrated monitoring using the MCH Handbook and DDST is effective in identifying children's growth and developmental conditions comprehensively. Strengthening early detection through collaboration among healthcare providers, early childhood educators, and parents is recommended to ensure timely and appropriate interventions.

Abstract

This study aims to analyze the effect of workload, job stress, and work motivation on nurse performance in the surgical inpatient ward of RSPAL dr. Ramelan Surabaya in 2025. Nurses in surgical units face high job demands, physical and psychological pressure, and shift work systems that may affect the quality of nursing services. Excessive workload may increase job stress, while work motivation serves as an important driving factor in maintaining optimal performance. This research employed a quantitative approach with a cross-sectional design. The population consisted of all nurses working in the surgical inpatient ward, with samples selected through random sampling. Data were collected using structured questionnaires measuring workload, job stress, work motivation, and nurse performance. Multiple linear regression analysis was applied to determine both partial and simultaneous effects among variables. The results indicate that workload and job stress significantly influence nurse performance, while work motivation has a positive effect in improving performance. Simultaneously, these three variables contribute to changes in nurse performance. The study concludes that hospital management should optimize workload distribution, reduce job stress, and enhance motivation through organizational support and professional development programs.

Abstract

Pain during arteriovenous fistula (Cimino) puncture in chronic kidney failure patients undergoing regular hemodialysis can cause discomfort, anxiety, depression, and low self-esteem. This study aims to determine the effectiveness of using ice cold pack therapy and slow deep breathing on pain during puncture in patients with arteriovenous fistula (Cimino). The research design used was a quasi-experimental research design with two groups pre-post test conducted at Efarina Etaham Berastagi Hospital. One group was given experimental treatment, namely ice cold pack therapy and slow deep breathing, while the other group as the control group was only given slow deep breathing treatment. Data analysis was carried out to see the relationship between the independent and dependent variables using the independent samples t-test at a significance level of α = 0.05. The independent samples t-test results obtained a p value> 0.05, namely 0.104, meaning that statistically there was no significant difference in the average pain scale between patients given ice cold pack therapy and slow deep breathing with patients only given slow deep breathing. Nurses are advised to continue to carry out slow deep breathing therapy and ice cold packs to overcome pain during puncture in patients with arteriovenous fistula (Cimino).

Abstract

Hospitality in health services encompasses friendliness, effective communication, empathy, and facility comfort. The policy on Hospitality in health services in DKI Jakarta Province is based on the Decree of the Head of the Jakarta Health Office No. 407 of 2021 concerning Health Service Guidelines that prioritize hospitality. The implementation of this policy requires evaluation to determine the achievement of its objectives. This study aimed to analyze the implementation of the Hospitality policy and individual characteristics in relation to patient satisfaction at Matraman Community Health Center, East Jakarta, in 2025. The research method employed was descriptive analytic with a quantitative approach and cross-sectional design. A total sample of 100 respondents was drawn from the population using the Slovin formula, with inclusion criteria being respondents aged ≥16 years, male or female, and patients or individuals utilizing health services at Matraman Community Health Center. Data were collected through a questionnaire that had undergone validity and reliability testing. Data analysis included univariate, bivariate, and multivariate tests. The results showed that patient satisfaction levels were generally high, with the majority reporting “satisfied” and some “very satisfied.” Bivariate analysis indicated significant relationships between friendliness (p = 0.0001; OR = 40.289), communication (p = 0.0001; OR = 19.167), empathy (p = 0.0001; OR = 43.714), and facility comfort (p = 0.0001; OR = 12.500) with patient satisfaction. Individual characteristics showed no significant relationship. Multivariate analysis identified empathy (p = 0.0001; OR = 13.850) as the dominant factor, followed by friendliness (p = 0.002; OR = 11.075). In conclusion, patient satisfaction at Matraman Community Health Center is primarily associated with staff empathy and friendliness.

Abstract

Anxiety is a common psychological condition experienced by patients before undergoing medical procedures, especially surgical procedures, which can affect physiological stability, comfort, and patient readiness to receive the procedure. One non-pharmacological method to reduce anxiety levels is rhythmic breathing distraction, which has the potential to reduce the body's stress hormone levels, such as adrenaline and cortisol, in preoperative patients. This study aimed to analyze the effect of rhythmic breathing distraction on the anxiety levels of preoperative patients under general anesthesia. This study used a pre-experimental method with a one-group pretest-posttest design. The study was conducted at Wijayakusuma Hospital, Purwokerto, from October to November 2025, and included a total of 38 respondents. Data analysis was performed using the Wilcoxon signed-rank test with a significance level of α = 0.05. The results showed a significant decrease in anxiety levels after the rhythmic breathing distraction intervention (p <0.001). The average anxiety score before the intervention was 21.05 and after the intervention was 12.03. It can be concluded that rhythmic breathing distraction significantly influences preoperative anxiety levels in patients undergoing general anesthesia. This non-pharmacological intervention, rhythmic breathing distraction, can be used as an alternative for managing anxiety in preoperative patients.

Abstract

Preeclampsia is one of the main causes of maternal morbidity and mortality in Indonesia which is characterized by increased blood pressure and impaired organ function after 20 weeks of gestation. This study aims to determine the relationship between diabetes mellitus, anemia, and the distance between pregnancies and the incidence of preeclampsia in pregnant women who give birth at Sunan Kudus Hospital in 2024. This study used a quantitative design with a retrospective case-control approach, involving 252 respondents consisting of 63 pregnant women with preeclampsia (cases) and 189 pregnant women without preeclampsia (control). Data were collected secondary through medical records and analyzed using the Chi-Square test with a 95% confidence level and p< significance of 0.05. The results showed that diabetes mellitus was not significantly associated with the incidence of preeclampsia (p = 0.096; OR = 2,737; CI 0.884–8.475), but mothers with DM have a 2.7-fold greater risk of developing preeclampsia. There was a significant association between anemia and the incidence of preeclampsia (p = 0.001; OR = 2,794; CI 1.520–5.136) and between the distance between pregnancy and the incidence of preeclampsia (p = 0.022; OR = 2,053; CI 1,146–3,679). In conclusion, anemia and the distance between pregnancies at risk have a meaningful relationship with the incidence of preeclampsia, whereas diabetes mellitus is not statistically significant but still has the potential to increase risk. Prevention of preeclampsia needs to be focused on monitoring hemoglobin levels, setting ideal pregnancy distances, and managing metabolic diseases during pregnancy through comprehensive antenatal care examinations.

Abstract

Outpatient services are the primary gateway for patients entering healthcare facilities, yet declining visits at regional hospitals in Indonesia indicate challenges in service utilization. This study aims to describe outpatient service utilization in relation to hospital image and human resource availability at Arjawinangun Regional Hospital, Cirebon. A quantitative descriptive study was conducted with 128 outpatients selected through simple random sampling. Data were collected using a structured questionnaire adapted from previous studies, consisting of 12 items for hospital image, 12 items for human resource availability, and 20 items for outpatient service utilization, all measured on a five-point Likert scale. Descriptive analysis was performed using IBM SPSS Statistics to calculate mean scores, which were interpreted using categories of very poor, poor, good, and very good. The results show that hospital image achieved an overall mean of 4.11 (very good), with functional image scoring highest (4.27) and institutional image lowest (3.98). Human resource availability achieved an overall mean of 4.06 (very good), with discipline and work ethic scoring highest (4.11) and waiting time concerns scoring lowest (3.88). Outpatient service utilization achieved an overall mean of 4.05 (very good), with service accessibility scoring highest (4.15) and visit frequency scoring lowest (3.96). The study concludes that while patient perceptions of hospital image and human resources are generally very positive, institutional transparency, waiting time management, and visit frequency require strategic attention. These findings provide empirical evidence for hospital management to strengthen institutional image and optimize staff distribution to enhance consistent outpatient service utilization.
