Abstract

Heart disease is a high-risk cardiovascular disease that occurs due to narrowing or blockage of the coronary arteries by plaque, so that blood flow to the heart is disrupted and can cause chest pain, heart attacks, and sudden death. This study aims to strengthen the role of families in encouraging healthy lifestyle changes as an effort to prevent coronary heart disease in the work area of ​​the Kalumpang Community Health Center. Research Methods: This study used an observational analytical design with a cross-sectional approach to analyze factors related to strengthening the role of families in healthy lifestyle changes as an effort to prevent coronary heart disease in the work area of ​​the Community Health Center. The study population included all heart disease sufferers recorded in the register book at the Kalumpang Community Health Center during 2025. The sampling technique used was total sampling, so that the entire population was used as a research sample. Results: The physical activity variable has an Odds Ratio (OR) value of 12,600, which means that individuals who do not do physical activity have a chance of about 12.6 times greater. Dietary patterns have a significant relationship with the incidence of heart disease, indicated by p value = 0.014 and OR = 4.148, which means that individuals with poor diet have a chance of about 4.1 times greater. Smoking habits have a significant relationship with efforts to prevent heart disease, with p value = 0.018 and OR = 3.838. This means that individuals who have a smoking habit are at risk of about 3.8 times greater. Compliance has a very significant relationship with efforts to prevent heart disease, with p value = 0.000 and OR = 13.964. Family attitudes have a very significant relationship with efforts to prevent heart disease, indicated by p value = 0.000 and OR = 13.444. Conclusion: This study shows that strengthening the role of the family is very important in encouraging healthy lifestyle changes as an effort to prevent heart disease in the working area of ​​the Kalumpang Community Health Center, Ternate City. Compliance, attitude, physical activity, and knowledge are the main factors, while unhealthy behavior and lack of family support increase the risk. Therefore, family-based interventions are needed through education at the Kalumpang Community Health Center, accompanied by counseling, mentoring, policy support, cross-sector collaboration, and increased outreach for heart disease prevention.

Abstract

Indonesia, cases of Dengue Hemorrhagic Fever (DHF) are increasing, with the Bantul Regency having the highest number of cases in Yogyakarta. The method of controlling dengue vectors based on Wolbachia has been proven to be effective in reducing DHF cases. Public knowledge and prevention efforts are very important in reducing the disease. This study aims to compare the level of knowledge and number of dengue fever incidents between areas that have been exposed to the Wolbachia program and those that have not been exposed to it in Bantul Regency, Yogyakarta. The research used an analytical observational approach with a cross-sectional design. The study was conducted in 2023 - 2024. Data DHF incident was from 2018-2023, and knowledge data obtained from questionnaire. Data analysis was carried out using the Mann-Whitney test and the point-biserial correlation test. The knowledge of non-exposed to Wolbachia (83.77) was higher than the exposed to Wolbachia (82.49), but not significant (p=0.149). There was no significant difference in DHF incidence (2018-2023) between two areas (p=0.229). There was a significant correlation between knowledge and DHF incidence in areas exposed to Wolbachia (p=0.015; r = -0.236), and no significant correlation in non-exposed to Wolbachia (p=0.164; r = -0.136). There was no difference in the knowledge and incidence of dengue fever between groups that had not been exposed to Wolbachia and those that had been exposed. Wolbachia release programs together with increased knowledge can sharpen the decline in dengue fever incidence.

Abstract

Introduction: Cystic hygroma is a congenital disorder in the form of a cyst filled with lymphatic fluid arising from abnormalities in the embryonic development of the lymphatic system. Cystic Hygroma typically manifests in the neck area (75-90%) with; 20% in the axillae, 5% in the mediastinum, retroperitoneal region, and can also be in the thoracic wall. Cystic hygromas are often associated with chromosomal aberrations such as Trisomy syndrome; 21, 18, and 13, and Turner syndrome (45, X0). It can also be associated with cardiac anomalies and nonimmune hydrops fetalis. Cystic hygromas can usually be seen on sonographic examinations during the first trimester. Generally, these lesions are associated with a poor prognosis, especially if there is an abnormal karyotype. Reported a case of cystic hygroma colli in a woman who was pregnant at 18 weeks' gestation. In this case, termination of pregnancy was performed at 22 weeks of gestation. Fetuses with cystic hygromas have a high risk of poor outcomes. Therefore, once the diagnosis has been established, it is necessary to consider terminating the pregnancy. This case report demonstrates the importance of routine pregnancy evaluation. Finding the exact cause also needs to be done to avoid recurring events in subsequent pregnancies.

Abstract

Background: According to the International Diabetes Federation (IDF), in 2021, an estimated 537 million people worldwide will suffer from diabetes, and this number is expected to continue to increase (International Diabetes Federation, 2019). The prevalence of diabetes mellitus (DM) in Indonesia increased by 2% in 2018 compared to 2013. The highest prevalence was in DKI Jakarta Province (3.4%) and the lowest was in NTT Province (0.9%) among residents aged ≥15 years. The prevalence of DM in all ages was lower, at 1.5%, than the prevalence of DM in those aged ≥15 years (Kemenkes RI, 2018a, 2018b).Complications in diabetes mellitus patients can have an impact on the quality of life of diabetes mellitus patients. Various physical health problems that can be caused by diabetes mellitus include diabetic neuropathy, diabetic retinopathy, heart disease, and vascular problems. Objective : The aim of the study was to determine the clinical indicators and quality of DM patients participating in the Prolanis program at Kasihan I Community Health Center. Methods : This study was observational with a cross-sectional design on outpatient DM patients at Kasihan I Community Health Center, Bantul, DIY. Sampling of diabetes mellitus patients was carried out using a non-probability sampling technique, namely purposive sampling was carried out by taking samples based on certain criteria. This research was conducted at Kasihan 1 Community Health Center, Kasihan, Bantul, from December 2023 to August 2024. Data collection form containing a questionnaire on the subject's personal biodata (demographics), medical history and treatment, and a questionnaire on lifestyle habits. Patient quality of life was measured using the EQ-5D-5L questionnaire. In-time blood sugar testing kit. Chi-Square analysis was used to determine factors related to the quality of life of prolanis DM patients. Results and Discussion : There were 40 respondents (97.6%) who were female and only 1 respondent (2.4%) who was male. This indicates the increasing involvement of women in Prolanis. There were no significant differences in systolic and diastolic blood pressure and quality of life of DM patients participating in Prolanis at the Kasihan Community Health Center based on gender. Blood sugar levels in male DM patients were higher than in female DM patients (normal, 186.00±63.48). The normality of blood sugar levels of Prolanis participants at Kasihan 1 Bantul Health Center showed that more of them had controlled blood sugar levels. Conclusion: There are differences in the values ​​of clinical picture indicator data and quality of life of patients. Physical exercise interventions for the elderly have a very big influence in changing their quality of life.

Abstract

Degenerative diseases, particularly neurodegenerative disorders, pose significant global health challenges. Traditional medicine systems offer valuable leads for novel therapeutic development. This ethnopharmacological study investigated Keumiki leaf (Glochidion spp), a traditional medicinal plant from Aceh Province, Indonesia, as a therapeutic candidate for degenerative diseases. Comprehensive ethnobotanical surveys revealed high recognition (89.3%) of Keumiki for treating cognitive impairment, inflammatory conditions, and age-related disorders. Phytochemical analysis demonstrated rich composition with total phenolic content of 347.0 ± 4.12 mg GAE/g and total flavonoid content of 163.2 ± 2.78 mg QE/g in ethanol extract. Antioxidant assays revealed potent activity with DPPH IC₅₀ of 23.45 μg/mL, comparable to standard antioxidants. Molecular mechanism studies using Western blot and RT-PCR demonstrated significant activation of the Nrf2/HO-1 antioxidant pathway (2.3-fold and 1.8-fold increases, respectively) and suppression of pro-inflammatory pathways, including downregulation of NF-κB, TNF-α, IL-1β, and IL-6. Strong correlations between phytochemical content and biological activities (r > 0.87) suggest phenolic and flavonoid compounds as primary contributors to therapeutic effects. This integrated ethnopharmacological approach validates traditional knowledge and provides scientific evidence supporting Glochidion spp as a promising therapeutic candidate for degenerative diseases through dual antioxidant and anti-inflammatory mechanisms.

Abstract

The increasing complexity of healthcare systems has intensified challenges in hospital financial management, particularly within Islamic healthcare institutions. Muhammadiyah hospitals are expected to maintain financial efficiency and sustainability while adhering to Islamic ethical values and fiqh principles. Similarly, sharia health insurance requires governance founded on justice, transparency, and mutual assistance (ta'awun). However, existing studies addressing hospital financial governance and sharia health insurance from Muhammadiyah perspectives remain fragmented. This study aimed to synthesize the ethical and fiqh principles underlying hospital financial management and sharia health insurance within Muhammadiyah healthcare through a systematic literature review. A Systematic Literature Review (SLR) was conducted following the PRISMA 2020 guidelines. Literature searches were performed in Scopus, PubMed, Google Scholar, Garuda, and DOAJ databases covering publications from 2015 to 2026. Studies meeting predefined inclusion and exclusion criteria were selected and analyzed using thematic synthesis. A total of 32 studies were included in the final review. Five major themes emerged: ethical principles of financial governance, fiqh-based financing mechanisms, implementation of takaful, Muhammadiyah perspectives on BPJS Health, and the integration of maqashid al-shariah into healthcare financing systems. The findings identified five key foundations of Muhammadiyah hospital financial governance: justice (‘adl), transparency, accountability (amanah), public benefit (maslahah), and avoidance of prohibited financial elements such as riba, gharar, and risywah. Sharia health insurance was consistently viewed as a cooperative mechanism based on tabarru’ and risk-sharing principles. Muhammadiyah also supports participation in Indonesia’s National Health Insurance system (BPJS Kesehatan) provided it upholds justice, transparency, and social welfare. These findings suggest that integrating sharia-based financial ethics into hospital governance may contribute to sustainable, equitable, and socially responsible healthcare systems

Abstract

Breast cancer is the most common malignancy among women worldwide. While most cases present with a palpable mass, cutaneous involvement such as ulceration and serosanguinous discharge is less commonly emphasized and may delay diagnosis, particularly in elderly patients. A 65-year-old female presented with a five-year history of a progressively enlarging right breast lump that had recently ulcerated with serosanguinous and purulent discharge, accompanied by fever, nausea, and anorexia. She had no prior medical evaluation for the mass. Vital signs were stable except for tachycardia (110 bpm). Laboratory findings revealed marked leukocytosis (31.2 × 10³/µL), mild hyponatremia (133 mEq/L), and stage 2 chronic kidney disease (eGFR 60 mL/min/1.73 m²). Core needle biopsy of the breast mass demonstrated invasive carcinoma of no special type (NST), grade III, with no lymphovascular invasion and low tumor infiltrating lymphocytes (<5%). Initial management included intravenous ceftriaxone, analgesics, antiemetics, and wound care. This case highlights the importance of prompt histopathologic diagnosis in ulcerated breast lesions to differentiate malignancy from benign conditions. Delayed presentation remains a challenge in elderly populations. Comprehensive oncologic staging and definitive multimodality therapy are essential for optimal outcomes.
