Abstract

Abstract

Emergency hemodialysis (HD) can happen because of numerous factors such as delayed referral to the nephrologist doctor and inadequate care of patient comorbidities. It was challenging because this procedure had a higher mortality than planned hemodialysis. This study aims to present the characteristics of patients who underwent emergency hemodialysis. An observational description exploration was applied in this retrospective cross-sectional study of all patients receiving emergency hemodialysis at Dr. Soetomo General Academic Hospital in January 2023. The inclusion criteria of having complete evidence data in medical reports indicate the patient’s need for this therapy. We collected data on the patient's age, gender, type of renal failure, HD status, comorbidities, and indications of hemodialysis procedure. A univariate analysis performed for this study. A total of 104 patients undergoing emergency hemodialysis were evaluated in this study. The majority of patients were male (50.96%), in the age of 40-59 range (50%), from Surabaya (51%), and had CKD V (88.46%). The most potential causes of declined kidney function were hypertension (65.38%) and diabetes mellitus (37.5%). This study also examined most indications for emergency hemodialysis: hypervolemia (75.96%) and uremia syndrome (68.26%) with 89.42% patient with multiple indications. The study population was dominated by CKD V patients who had previously undergone regular HD and several patients underwent emergency hemodialysis more than once in January 2023. The majority of patients are in the productive age range and have hypertension and diabetes mellitus. This study also found that hypervolemia and uremic syndrome are the most common indications for emergency hemodialysis.

Abstract

Abstract

Musculoskeletal disorders and trauma-related conditions remain major global health challenges that contribute significantly to disability, chronic pain, reduced mobility, and healthcare burden. In many healthcare systems, general practitioners (GPs) serve as the first point of contact for patients with orthopaedic and traumatology complaints, particularly in rural and resource-limited areas where access to specialists is limited. This study aims to examine the role of general practitioners in orthopaedic and traumatology services, including their responsibilities, barriers to effective practice, and contributions to healthcare integration. This study employed a qualitative narrative literature review approach using scientific articles, policy documents, and institutional reports obtained from PubMed, Google Scholar, and other academic sources published between 1998 and 2025. Data were analyzed using thematic analysis to identify recurring themes related to musculoskeletal care and trauma management. The findings indicate that GPs play essential roles in early assessment, conservative treatment, referral coordination, rehabilitation follow-up, and patient education. However, inadequate musculoskeletal training, limited diagnostic facilities, inconsistent referral systems, workforce shortages, and the persistence of traditional bonesetter practices remain significant barriers. The study concludes that strengthening primary care competence, improving referral pathways, enhancing rehabilitation support, and expanding government involvement are necessary to improve orthopaedic and traumatology services and promote more accessible, efficient, and integrated healthcare systems

Abstract

The rapid advancement of digital technologies and volatile market dynamics have challenged mining companies to optimize supply chain management (SCM) processes. PT Sentosa Laju Sejahtera (SLS) experiences inefficiencies in procurement and inventory, causing decision latency and increased operational costs. This study aims to explore how integrating Machine Learning, specifically the Random Forest algorithm, into the company’s Enterprise Resource Planning (ERP) system can enhance predictive capabilities, reduce costs, and strengthen operational resilience. A qualitative case study approach was employed, combining in-depth interviews with key managerial stakeholders, participatory observations of operational workflows, and analysis of 63,818 historical Purchase Order documents. Triangulation techniques ensured data credibility and reliability. Findings reveal that Random Forest integration transforms decision-making from reactive to data-driven, reducing procurement cycle times from seven days to 2–3 days and lowering Total Cost of Ownership by 15% through fewer emergency orders. Feature Importance metrics enabled management to identify key inefficiency drivers, improving strategic interventions. Organizational readiness and data governance were identified as critical factors for successful implementation. In conclusion, the integration of predictive analytics into ERP systems provides significant operational and strategic benefits, enhancing efficiency, agility, and competitiveness. The study contributes both empirically to the literature on digital business transformation and practically as a model for multi-site mining enterprises seeking data-driven SCM optimization.

Abstract

Abstract

Micro, Small, and Medium Enterprises (MSMEs) play a crucial role in Indonesia’s economy, contributing over 60% to GDP and absorbing nearly 97% of the workforce. Despite this significant contribution, MSMEs face challenges such as low digital literacy, limited access to formal financing, and unequal regional development, which hinder economic convergence and inclusive growth. Addressing these issues is critical in the context of an increasingly digital and interconnected economy. This study aims to design a transformation model for MSMEs as engines of local economic convergence through digitalization, financial inclusion, and sustainable regional development spillovers. Employing a community-based participatory research approach, the study integrates data from Statistics Indonesia, the Ministry of Cooperatives and SMEs, Bank Indonesia, and the Financial Services Authority. A purposive sample of 250 MSMEs was selected to represent varying levels of digital adoption, financial access, and geographic distribution. Data were collected using structured questionnaires, interviews, and secondary sources, with validity and reliability ensured through expert review and Cronbach’s alpha analysis. Data analysis employed descriptive, comparative, and interpretative techniques to evaluate pre- and post-intervention outcomes. The results indicate that MSME digitalization accelerates market, income, and spatial convergence, while financial inclusion supports sustainable growth but remains unevenly distributed. The implementation of the Integrated MSME Convergence Model (IMCM) generates regional spillover effects, including increased demand for local inputs, expanded distribution networks, job creation, and knowledge transfer. These outcomes demonstrate that MSMEs, when supported by integrated policies and digital infrastructure, can function as strategic agents of regional development. In conclusion, the study confirms the potential of digitalized and financially included MSMEs to promote inclusive and sustainable economic growth, highlighting the need for coordinated policy interventions and continuous community-based support.
