Abstract

Purpose: The link from food insecurity to chronic diseases is well-known. However, the contribution of the number of days eaten fruits and vegetables consumption in the association between food insecurity and the incidence of diabetes among Indonesian adults is rather vague. Methods: This study was obtained data from 3955 participants of the Indonesia Family Life Survey (IFLS) in 2007 and 2014. The food insecurity measurement was using a World Food Programme (WFP) concept using food frequency questionnaire on a food consumption score analysis. A generalized estimating equation (GEE) test were used to test the hypotheses model while accounting for the health behaviors and sociodemographic characteristics in this study. Results: The GEE test was confirmed the negative association between food insecurity and diabetes in the adjusted and unadjusted model (p = 0.005 – < 0.001). The number of days eaten vegetables consumption was statistically negative associated with diabetes in both model (p = 0.006 – < 0.001). The number of days eaten fruits consumption was negatively associated with diabetes but not statistically significant in both models. The contribution of number of days eaten fruit and vegetables was related to the ability and/or knowledge of individuals to choose their meals, especially for people with diabetes. Conclusion: Food insecurity is negatively associated with diabetes, with the contribution of less number of days eaten vegetables consumption. Strategies to improve the prevention of diabetes among adults may need an enrichment of fruit and a vegetable tailored-meal on the menu of the restaurant or food court or canteen together with the enrichment of nutrition education, into account.

Abstract

Purpose: Indonesia has grown huge number of internet users thus becomes novel health promotion target in digital era. As Google Trends allows observing public health seeking information rapidly along with our concern towards public cancer interest which is affected by public figure news and cancer campaign, we studied cancer search oscillations in Indonesia in relation to both factors.Methods: Search queries comprised of cancer topics (Kanker; Kanker Darah; Leukemia; Kanker Paru; Kanker Getah Bening; Kanker Nasofaring), public figures news (Ani Yudhoyono; Arifin Ilham; Adira Taista; Anak Denada; Istri Indro; Sutopo; Valentino Nahak), and cancer awareness days (Hari Kanker Sedunia; Hari Kanker Anak; Hari Kanker Payudara) were generated to get their search volumes from 2018 to first trimester of 2019 subsequently grouped them into 3 main topics (cancer, public figure news, and cancer awareness day). Trend-to-trend analysis among main topics is evaluated using Pearson’s correlation, multiple linear regression, and cross-correlation.Result: Strong correlation is evidenced between cancer and public figure search (r = 0,752; p= <0,001) but lower in cancer awareness day (r=0,296; p = 0,017). Multivariate analysis shows significant impact of public figure, secular trend, and cancer awareness day on cancer search volume in Indonesia (R2 = 0,646; p < 0,001).Discussion: This study demonstrates strong influence of public figures on cancer-seeking despite temporary for about 4 weeks with public figure search leads 1 week before evidenced cancer search. Positive yet lower effect of cancer campaign requires some improvement towards this intervention.Conclusion: Compared with cancer campaign, public figure news would ‘blow-up’ most Indonesian internet users towards cancer searching significantly but temporarily, which demands health promoter to maximize this ‘golden period’ to give informative and reliable cancer content in conjunction to fight cancer hoax, or possibly cooperate with cancer survivor public figures for promoting public awareness towards cancer.

Abstract

Background : Indonesia as the highest number of tuberculosis (TB) cases is burdened by both classic unsolved problem and latest issues of TB cases. Innovation using mobile phone application may be potential to integrate several TB control efforts to reach the tuberculosis-free target in year 2030Aim : to review the benefit and applicability of mobile phone application to support the three pillar of end TB strategy, especially in Indonesia.Method : This is a narrative literature review study. Extracting the component and occured problems from each pillar of end TB strategy was firstly conducted. For each findings, the approachment using mHealth were searched in published papers, pilot tested, and GO/NGOs or company website to be reviewed whether it could be performed and applied as one stop integrated mobile application that suitable for Indonesian population.Result : mobile phone and internet penetration in Indonesia is fast increasing, predominantly among lower class society. Separated and various applications found to deal with health services related problems, those could also be implemented to overcome TB based on three pillar strategy in one integrated application. Government played a big role to promote investation, wide use of the application, and creating a connected platform where different type of health startup can enter to support the system. More suggestions are needed on how this integrated application should deal with the ethical clearance to use the big data for medical research, users’ age, rural area implementation, and economic gap.Conclusion : “One stop” concept in one integrated application will enhance the will of wide population to use it and the sustainability of the application. To maximize its benefit to overcome TB in Indonesia, gross participation from individual, community, private sector, until the national government level should be endeavored.

Abstract

Latar belakang: Insidensi kanker serviks di Indonesia menepati urutan ke 2 sejumlah 10,7% dan angka kematian menempati urutan ketiga dengan jumlah 10,3%. Berkaitan dengan hal tersebut, deteksi dini kanker serviks menggunakan metode IVA tes masuk dalam Renstra Kemenkes 2015-2019. Target yang diharapkan pada 2019 mencapai 50%. Kota Jambi mulai melaksanakan program IVA tes pada 2015 di 20 Puskesmas.Tujuan: Menilai capaian pelaksanaan program deteksi dini kanker serviks melalui pemeriksaan IVA tes di Kota Jambi tahun 2015-2018.Metode: Penelitian ini merupakan penelitian observasional dengan rancangan deskriptif. Data diperoleh dari laporan bulanan pelaksanaan IVA tes dari 20 puskesmas.Hasil: Pemeriksaan IVA tes dilakukan melalui beberapa cara, diantaranya melalui kegiatan KB. Tenaga kesehatan menawarkan pemeriksaan IVA test kepada peserta KB. Selain itu pemeriksaan IVA tes juga dilakukan pada acara tertentu seperti pada saat dies natalis Kota Jambi, hari Kanker Sedunia, Dies natalis KORPRI, Hari Kesehatan Nasional, Dies natalis Bhayangkara, Dies natalis KODIM, dan acara PKK. Pemeriksaan IVA tes juga dilakukan oleh bidan puskesmas dengan cara turun langsung ke lapangan. Kegiatan pemeriksaan IVA dilaporkan setiap bulan oleh puskesmas ke Dinas Kesehatan Kota Jambi. Berdasarkan data yang diperoleh dari laporan bulanan  20 puskesmas, capaian pemeriksaan IVA di Kota Jambi tahun 2015-2018 berturut-turut adalah 8.04 %; 10,7%; 7,16% ; 10,6%. Angka ini masih jauh dari target capaian  yang di tetapkan oleh Restra Kemenkes. Kurangnya promosi serta sosialisasi menyebabkan rendahnya pengetahuan masyarakat yang berdampak pada rendahnya partisipasi untuk melakukan IVA tes.Simpulan: Capaian pelaksanaan IVA tes di Kota Jambi belum memenuhi target yang ditetapkan Renstra Kemenkes 2015-2019. Diperlukan adanya penguatan diseminasi informasi mengenai pentingnya pemeriksaan IVA tes melalui berbagai kegiatan kemasyarakatan oleh tenaga dokter dan bidan yang ada di puskesmas.

Abstract

Tujuan: Penelitian ini bertujuan untuk mempelajari proses dan dampak dari program Pos Gizi untuk mengatasi permasalahan gizi kurang. Metode: Penangangan gizi kurang pada 53 balita dari 2 desa dengan menggunakan program Pos Gizi. Program Pos Gizi dilakukan dengan memberikan kapasitas kepada relawan dan kader posyandu untuk mendampingi balita dengan status gizi kurang selama 3 bulan. Pada sesi Pos Gizi selama 10 hari, balita berkumpul untuk makan bersama menu padat gizi dan orang tua balita mendapatkan edukasi mengenai pengasuhan positif pada anak. Menu padat gizi untuk balita merupakan kontribusi dari masing-masing orang tua dan pengasuh balita yang berasal dari bahan pangan lokal yang ada di sekitar rumah. Berat badan balita diukur pada hari pertama, hari kesepuluh, bulan pertama dan bulan ketiga. Hasil: Dari 53 balita yang mengikuti program Pos Gizi, 7 diantaranya dicatat sebagai drop outkarena pindah dari lokasi penelitian, sehingga total peserta yang meneruskan program hingga bulan ketiga sebanyak 46 balita. Sebanyak 43 dari 46 balita (93%) mengalami kenaikan berat badan (mean = 702 gram)  dan 20 balita diantaranya mengalami kenaikan berat badan lebih dari 900 gram. Selama proses implementasi program, relawan mendampingi orang tua dan pengasuh balita untuk mempraktekkan menu padat gizi dan perilaku pengasuhan positif yang dipelajari saat sesi Pos Gizi. Praktik baik yang muncul adalah orang tua dan pengasuh memiliki inisiatif untuk meneruskan kegiatan balita makan bersama dalam kelompok lebih kecil di rumah yang lebih berdekatan. Selain menu padat gizi, praktik baik ini berkontribusi signifikan meningkatkan berat badan balita peserta Pos Gizi. Simpulan: Praktik baik kelompok balita makan bersama meingkatkan dampak dari program Pos Gizi dalam meningkatkan berat badan balita.

Abstract

Barrier of health services access among dwarfishPurpose: Study describes how people with dwarfism seek access and the barriers of accessing healthcare services.Method: Participatory observation method with in-depth interviews among people with dwarfism, family, healthcare officers, midwives and the head village were used in this study. The location was in Sigedang Village, Kejajar District, Wonosobo Regency, Central Java for 2 months between  August-September 2017.Results: The study reveals stigma surrounding dwarfism in Sigedang society that dwarfism was hereditary. According to Sigedang people, abnormality was considered natural. Access to healthcare for people with dwarfism was limited. There were no efforts that has been made by either the village government nor the sub-district government for an increased prevention program for people with dwarfism in Sigedang village. People with dwarfism had limited information about healthcare services available for them. They must travel far to the city or across the regency just to reach the standard health facilities. Therefore, an increase of an integrated health access in Sigedang village is needed, especially for people with dwarfism.In addition, the study shows that health-seeking behavior of the locals was still influenced by the ancestors, where traditional medical practitioners played an important role. The awareness of people suffering from dwarfism and their community towards risk factors should be empowered.Conclusion: The absence of access to health services influenced the poor condition among people with dwarfism. They need empowerment from the local government, public health office and community to support their health service needs.

Abstract

Objektif: Kajian ini bermaksud  memberikan gambaran tentang pelaksanaan  program kesehatan jiwa terutama penanganan Orang Dengan Gangguan Jiwa (ODGJ) pasung di Kabupaten Cilacap Provinsi Jawa Tengah. Metode: Review data sekunder yang berasal dari Data Profil Dinas Kesehatan Kabupaten Cilacap Tahun 2018, jurnal-jurnal terkait serta  wawancara tidak mendalam dengan keluarga dan masyarakat. Hasil: Penduduk Kabupaten Cilacap sebanyak 1.785.971 jiwa, ODGJ mencapai 1.643 penderita, merupakan angka yang banyak tetapi sedikit perhatian karena tidak menyebabkan outbreak, ataupun kematian langsung tapi berdampak ekonomi jangka panjang (kurang produktif). Pemerintah Kabupaten Cilacap melalui Dinas Kesehatan mulai melakukan pendataan untuk menemukan ODGJ pasung, ditemukan 88 penderita dari 2010-2011. Berdasarkan wawancara sederhana dengan keluarga dan masyarakat ada beberapa alasan kenapa mereka melakukan pemasungan : 1) Ketidaktahuan keluarga tentang perawatan ODGJ,  2) Keterbatasan dana,  3) Dorongan mayarakat sekitar, 4)  Keputusasaan keluarga,  5) Ketidaktahuan mencari pertolongan  (Health Literacy). Saat itu rujukan ke RSJ menjadi pilihan tercepat menyelesaikan masalah tetapi ternyata tidak menyelesaikan masalah karena penderita cenderung kambuhan, kurangnya follow up dari puskesmas, ketidakteraturan minum obat, dan belum optimalnya dukungan lingkungan. Simpulan: Penanganan masalah kesehatan jiwa dibutuhkan kerjasama dengan pengambil keputusan, organisasi profesi, institusi pendidikan kesehatan, LSM, tenaga kesehatan, pelayanan kesehatan swasta melalui proses advokasi, pemberdayaan masyarakat melalui kegiatan promotif, preventif, kuratif dan rehabilitatif.

Abstract

Latar belakang: Rekam medis yang baik dan bermutu harus memenuhi indikator-indikator kelengkapan, keakuratan, tepat  waktu  dan memenuhi aspek hukum. Kelengkapan rekam medis juga memperngaruhi kecepatan dalam pengklaiman BPJS. Tujuan: Mengetahui Hubungan  Karakteristik Dokter berdasarkan Jenis Kelamin, Umur dan Masa Kerja dengan Kelengkapan Catatan Laporan. Metode: Penelitian menggunakan rancangan observasional analitik dengan pendekatan cross sectional. Populasi penelitian ini adalah semua berkas laporan operasi tahun 2015. Sampel penelitian ini adalah 166 berkas rekam medis dengan menggunakan metode purpossive sampling. Uji Statistik menggunakan Uji Chi Square (X2). Hasil: Penelitian menunjukan bahwa ada hubungan karakteriktik dokter berdasarkan Jenis Kelamin (nilai p 0,02) umur (nilai p 0,02)  dan masa kerja (nilai p 0,00) dengan kelengkapan laporan operasi. Ada hubungan antara karakteristik dokter dengan kelengkapan laporan operasi dengan nilai p (0,00). Jumlah laporan operasi yang pengisiannya lengkap oleh dokter yang berkarakteristik jenis kelamin perempuan, umur 25-45 dan  memiliki masa kerja > 3 tahun lebih banyak dibanding dokter yang berkarakteristik laki-laki, umur 46-65 tahun dan masa kerja 1-3 tahun. Simpulan: Kedisiplinan dan ketaatan dokter untuk kelengkapan pengisian rekam medis menunjang penyelenggaraan rekam medis yang baik serta tertib administrasi.

Abstract

Android and web-based application for monitoring patients with tuberculosis for puskesmas level programMulti-drug resistant tuberculosis (MDR TB) is a challenge in every country with a high prevalence of TB, including Indonesia. MDR TB itself is influenced by many factors including the lack of discipline in taking medication due to forgetfulness or rejection from the patient. This study aims to design a prototype of the drug-taking companion management information system (PMO) of pulmonary tuberculosis patients. The stages of this design are system requirements analysis, logical design, physical design, and interface. The results of the prototype design will be tested with officers responsible for TB and supervisors taking medication. This android platform-based application helps supervisors to remind patients to take their medicine regularly and the dose. This application continues to be developed by fixing bugs and creating a channel for user complaints

Abstract

Tujuan: Memberikan masukan terhadap pembaharuan system penyusunan Renbut SDMK agar aplikasi renbut SDMK online dapat berbasis transparansi. Metode: Kajian ini menggunakan review study dari berbagai hasil penelitian terkait masalah SDM Kesehatan. Hasil: Sistem perekrutan SDM Kesehatan melalui aplikasi Renbut SDMK dapat diakses secara online, bersifat satu arah dan berjenjang dari instansi terendah sampai ke pusat. Setiap pengelola pada institusi terendah/fasilitas kesehatan hanya bisa menghitung, menyusun, dan mengusulkan renbut SDMK ke tingkat Badan Kepegawaian Daerah (BKD) kabupaten/kota, tanpa bisa memantau apakah usulan tersebut diakomodir oleh BKD kabupaten/kota, provinsi dan pusat. Begitu juga dengan pengelola aplikasi renbut di tingkat BKD kabupaten/kota dan provinsi, mereka tidak bisa memantau usulan renbutnya di tingkat pusat. System aplikasi seperti ini menimbulkan ketidaktransparansi usulan rencana kebutuhan, sehingga terjadi perbedaan usulan pengadaan SDMK di tingkat pusat dengan usulan dari fasilitas kesehatan, yang berdampak pada tidak terpenuhinya kebutuhan SDMK di daerah. Perbedaan usulan SDMK antara tingkat pusat dan fasilitas kesehatan terjadi di seluruh Indonesia karena system aplikasinya berlaku secara nasional. Kondisi ini menunjukkan pentingnya pembaharuan system penyusunan renbut. Kesimpulan: Kebutuhan SDMK di fasilitas pelayanan kesehatan sulit terpenuhi jika aplikasi penyusunan renbut SDMK belum berbasis transparansi. Kementerian kesehatan perlu memperbaharui Permenkes nomor 33 tahun 2015 khususnya pada pasal 1 mengenai pedoman penyusunan kebutuhan SDMK dan lampiran pedoman penyusunan renbut, perlu menambahkan poin penjelasan mengenai hak akses dari setiap pengelola aplikasi renbut pada setiap tingkatan yang ada, dari instansi terendah sampai ke tingkat pusat, untuk dapat memantau, memberikan informasi dan bertanya mengenai usulan renbutnya.
