Abstract

Modalitas konvensional CR ( Computer Tomography) sangat berguna memperoleh diagnosa pada apendix . Appendicogram adalah pemeriksaan appendix berupa foto yang dapat membantu melihat adanya sumbatan atau kotoran pada usus buntu. Tujuan penelitian ini adalah untuk mengetahui penatalaksanaan pemeriksaan Apendikogram dengan apendisitis klinis di Instalasi Aulia Hospital , untuk mengetahui mengapa terdapat perbedaan teknik pemeriksaan apendikogram antara teori dan Aulia Hospital. dan melihat hasil radiografi Appendicogram dengan waktu tunggu enam jam. Penelitian ini bersifat kualitatif dengan desain penelitian deskriptif dan pendekatan studi kasus. Teknik pengumpulan data dengan observasi langsung ,wawancara dengan informan dan dokumentasi yang di lakukan di Aulia Hospital .Objek penelitian adalah 1 orang pasien yang telah menjalani pemeriksaan apendikogram dengan klinis apendisitis di RS Aulia Hospital. Hasil penelitian ini adalah tentang prosedur pemeriksaan appendicogram dengan klinis appendisitis di Instalasi Radiologi Aulia Hospital. Hasil penelitian menunjukkan bahwasanya dalam penatalaksanaan pemeriksaan appendicogram pada setelah minum media kontras pasien tidak diperbolehkan buang air besar hingga pemeriksaan dilakukan. Waktu tunggu yang di gunakan di Instalasi Radiologi Aulia Hospital yaitu minimal 6-7 jam sedangkan 12 jam merupakan waktu maksimal. Dalam waktu tunggu 6-7 jam tersebut kontras sudah mencapai area appendix dan sudah dapat untuk menegakkan diagnosa. Proyeksi yang di gunakan yaitu plan foto, AP Kontras, RPO, LPO, dan jika masih kurang menegakkan diagnosa maka di tambah proyeksi lateral.

Abstract

Background: Esophageal obstruction is a condition that causes dysphagia due to narrowing or blockage of the esophageal lumen, requiring contrast radiographic examination to evaluate the anatomy and function of the upper gastrointestinal tract. Esophagography is a fluoroscopic examination using barium sulfate contrast medium that can demonstrate both morphological abnormalities and esophageal motility disorders. Theoretically, the examination is performed using several standard projections, namely anteroposterior (AP), right anterior oblique (RAO), lateral, and left anterior oblique (LAO). However, in clinical practice at several hospitals, the examination procedure is often modified according to the patient's condition and diagnostic requirements. Therefore, an evaluation of its implementation is necessary. This study aimed to analyze the management of the esophagography examination technique in patients with clinically suspected esophageal obstruction at the Radiology Department of Arifin Achmad Regional General Hospital, Riau Province. Methods: This study employed a descriptive qualitative design with a case study approach. Data were collected through direct observation of the examination procedure, interviews with radiographers, and documentation of radiographic findings conducted from June to July 2024. The data were analyzed descriptively by comparing the actual examination procedure with the established theoretical standards. Results: Esophagography in patients with clinically suspected esophageal obstruction was performed using AP plain, post-contrast AP, and lateral projections with barium sulfate contrast medium under fluoroscopic guidance. The findings showed that the use of the two primary projections (AP and lateral) was sufficient to demonstrate the esophageal anatomy and identify the location of the obstruction, thereby fulfilling the diagnostic requirements. However, the procedure did not fully comply with the theoretical standard, which recommends the additional use of RAO and LAO projections to provide more optimal visualization of the esophagus.

Abstract

Background: Colon in Loop examination is a lower gastrointestinal radiographic procedure using contrast media to evaluate various colonic abnormalities, including colitis. In clinical practice, the examination protocol may be modified according to the patient's condition and technical challenges encountered during the procedure. One rarely reported situation is the replacement of barium sulfate with a water-soluble contrast medium to obtain optimal visualization of the colon. This study aimed to describe the management of the Colon in Loop radiographic examination in a patient with clinically diagnosed colitis and to explain the rationale for using a water-soluble contrast medium as an alternative when barium sulfate failed to provide adequate diagnostic visualization. Methods: This study employed a case study approach conducted at the Radiology Department of Arifin Achmad Regional General Hospital, Pekanbaru. Data were collected through direct observation, documentation, and interviews with radiographers during the examination procedure. Results: The examination began with a plain anteroposterior (AP) abdominal radiograph, followed by the administration of barium sulfate contrast medium through a catheter and radiographic acquisitions in the left lateral, post-contrast AP, and right posterior oblique (RPO) projections. During the post-contrast AP examination, obstruction of barium sulfate flow was observed in the rectosigmoid region, resulting in suboptimal visualization of the colon. Based on the evaluation by the radiologist and radiographers, the contrast medium was replaced with a water-soluble agent diluted with normal saline (NaCl), allowing the contrast to pass through the obstructed segment and providing improved visualization of the colon.

Abstract

BNO-IVP (Blass Nier Overzicht Intravenous Pyelogram) adalah pemeriksaan Traktus Urinaria dengan cara memasukkan kontras media positif kedalam Traktus Urinaria melalui pembuluh darah vena. Media kontras yang digunakan adalah iodium bersifat water soluble (Haliza, 2021). Tujuan dari penelitian ini adalah untuk mengetahui penatalaksanaan pemeriksaan BNO-IVP (Blass Nier Overzicht Intravenous Pyelogram) dengan klinis Colic Abdomen Di Rumah Sakit Islam Ibnu Sina Pekanbaru dengan hanya menggunakan proyeksi AP supine abdomen dengan post kontras 5 menit, post kontras 15 menit, post kontras 45 menit dan post miksi proyeksi AP supine. penatalaksanaan pemeriksaan BNO-IVP (Blass Nier Overzicht Intravenous Pyelogram) dengan klinis Colic Abdomen Di Rumah Sakit Islam Ibnu Sina Pekanbaru dilakukan guna untuk menegakkan curiga adanya masalah pada traktus urinarius selain pada appendix. Jenis penelitian ini adalah kualitatif dengan pendekatan studi kasus Penatalaksanaan Pemeriksaan BNO IVP (Blass Nier Overzicht Intravenous Pyelogram) Proyeksi AP Dengan Klinis Colic Abdomen Di Rumah Sakit Islam Ibnu Sina Pekanbaru Penelitian ini ditujukan dengan cara observasi, dokumentasi, wawancara dengan radiographer. Didapatkan hasil bahwa penatalaksanaan pemeriksaan BNO IVP (Blass Nier Overzicht Intravenous Pyelogram) yang dilakukan pada Nn. M A H di Instalasi Radiologi Rumah Sakit Islam Ibnu Sina Pekanbaru adalah menggunakan Proyeksi AP pada pemeriksaan Abdomen polos, Proyeksi AP pada pemeriksaan 5 menit setelah pemasukkan media kontras, Proyeksi AP pada pemeriksaan 15 menit setelah pemasukkan media kontras, Proyeksi AP pada pemeriksaan 45 menit setelah pemasukkan media kontras, Proyeksi AP pada pemeriksaan post miksi yang terdapat perbedaan.

Abstract

Background: T-tube cholangiography is a radiographic examination of the biliary system using contrast media administered through a T-tube catheter to evaluate the bile ducts, assess the smooth passage of contrast media, and detect obstruction or abnormalities of the biliary system. In clinical practice, examination procedures may differ from theoretical guidelines, particularly in terms of patient preparation and projection selection. Objective: This study aimed to determine the procedure for performing cholangiography examinations in patients with a postoperative T-tube clinical condition at the Radiology Department of Arifin Achmad Regional General Hospital, Riau Province. Methods: This study employed a descriptive qualitative method with a case study approach. Data were collected through direct observation, interviews, documentation, and evaluation of radiographic findings from examinations performed at the Radiology Department of Arifin Achmad Regional General Hospital between June and July 2024. Results: The cholangiography examination procedure in patients with a postoperative T-tube clinical condition did not require special preparation and was performed using preliminary Anteroposterior (AP) and post-contrast AP projections. Contrast media were carefully administered through the T-tube catheter until the duodenum was reached. The radiological report showed contrast filling of the right and left hepatic ducts, common hepatic duct, and common bile duct, with smooth passage into the duodenum. No filling defects or additional shadows were identified. Right Posterior Oblique (RPO) and lateral projections were performed as additional projections when the AP images were insufficient to establish the diagnosis.

Abstract

Background: Transient Tachypnea of the Newborn (TTN) is one of the causes of respiratory distress in neonates and is associated with delayed clearance of fetal lung fluid after birth. Babygram radiography can assist in evaluating both the thoracic and abdominal conditions of neonates. Theoretically, babygram examination may be performed using anteroposterior (AP) and lateral projections; however, in clinical practice, the selection of projections may be adjusted according to the patient's condition and the purpose of the examination. To determine the management of babygram examination in neonates diagnosed with TTN at Pekanbaru Medical Center Hospital.Methods: This study employed a qualitative descriptive method with a case study approach. Data were collected through observation, interviews, and documentation of the babygram examination performed on a patient with a clinical diagnosis of TTN. Data were analyzed interactively and presented narratively.Results: Babygram examination in patients with a clinical diagnosis of TTN does not require special preparation. The patient is positioned supine on the cassette using an anteroposterior (AP) projection, with collimation adjusted to the area of interest and the central ray directed toward the xiphoid process. The exposure factors used were 48 kV and 7 mAs. The examination used only the AP projection because it was considered sufficient to demonstrate the respiratory and digestive systems on a single radiograph while taking into consideration the neonate's respiratory distress.Conclusion: The management of babygram examination in patients with a clinical diagnosis of TTN at Pekanbaru Medical Center Hospital is performed using a single AP projection without an additional lateral projection. The use of a single projection is adjusted to the patient's clinical condition and diagnostic requirements.

Abstract

Background: Sinonasal cancer is a malignancy that originates in the nasal cavity or paranasal sinuses. One treatment modality is external radiotherapy, which uses radiation to control or destroy cancer cells. Radiotherapy implementation requires several stages to ensure targeting accuracy and therapy safety. Objective: To determine the management of radiotherapy in patients with clinical sinonasal cancer in the Radiotherapy Installation of Arifin Achmad Regional Hospital, Riau Province. Methods: This study used a descriptive qualitative method with a case study approach. Data collection was conducted in June 2024 through observation, documentation studies, and interviews with medical physicists and radiotherapists. The study sample was one patient diagnosed with stage T4N0M0 sinonasal cancer. Results: Radiotherapy management included consultation with a radiation oncologist, simulation using a CT simulator, fabrication of fixation devices, determination of reference points, target contouring, dose planning using the Treatment Planning System (TPS), geometry verification, portals, and irradiation using a Linear Accelerator (LINAC). The patient received a total dose of 70 Gy, delivered in three phases: 27 × 2 Gy, 3 × 2 Gy, and 5 × 2 Gy, with two isocenters. Conclusion: Radiotherapy management for sinonasal cancer at Arifin Achmad Regional Hospital, Riau Province, was carried out in stages, starting with consultation, CT simulation, contouring, TPS planning, geometry verification, portal placement, and irradiation using the LINAC. Each stage was carried out to ensure the accuracy of radiation delivery to the target therapy.

Abstract

Background: Chronic rhinosinusitis (CRS) is a chronic inflammation of the nasal cavity and paranasal sinuses lasting more than 12 weeks and can be caused by various factors, one of which is anatomical variations of the paranasal sinuses. Computed Tomography (CT) Scan is an imaging modality that is capable of providing detailed anatomical visualization of the nasal cavity and paranasal sinuses, including the ostiomeatal complex, anatomical abnormalities, and pathological changes within the sinuses. In clinical practice, CT scan examination of the paranasal sinuses must be adjusted to the patient’s condition and the procedures implemented in healthcare facilities. This study aims to determine the CT scan examination technique of the paranasal sinuses in cases of chronic rhinosinusitis at the Radiology Department of Awal Bros A. Yani Hospital. Methods: This study used a descriptive method with a case study approach. Data were collected through observation, documentation, and interviews with radiographers. The study subject was a patient with clinical chronic rhinosinusitis undergoing a CT scan examination of the paranasal sinuses. Results: The examination was performed without special preparation, except for the removal of metallic objects in the examination area. The patient was positioned supine with a head-first position and arms placed alongside the body. The scan range included from the frontal sinus to the maxillary sinus. The examination used parameters of 120 kV and 175 mA with a 19.8 cm FOV. Image reconstruction was performed with a slice thickness of 3 mm in axial, coronal, and sagittal planes. Radiation protection optimization was carried out by adjusting examination parameters according to clinical needs. The CT scan technique of the paranasal sinuses in cases of chronic rhinosinusitis at the Radiology Department of Awal Bros A. Yani Hospital is performed through stages of preparation, positioning, scanning, image reconstruction, and radiation protection optimization according to examination requirements.
