Abstract

Introduction: Both xanthogranulomatous pyelonephritis (XP) and ureteritis cystica (UC) are rare diseases, and no case of coexistence with ureteral urothelial carcinoma has ever been reported. The purpose of this article is to present a case of ureteral urothelial carcinoma complicated by XP and UC and to discuss the CT and MRI findings. Case presentation: A 72-year-old woman with a history of diabetes mellitus, recurrent urinary tract infections, and bladder cancer was referred for evaluation of multiple ureteral and renal masses on the left side. No urinary tract stones were found on CT. The tumor in the lower ureter was hyperintense on diffusion-weighted image (DWI), and the tumor in the middle ureter was hypointense on DWI, showing polycystic wall thickening. DWI of the kidney showed multiple hyperintense lesions within the kidney. Pathological examination revealed tumors in the lower ureter, middle ureter, and left kidney as urothelial carcinoma, UC, and XP, respectively. Conclusion: In a patient with a history of diabetes mellitus and recurrent urinary tract infections, XP and UC should be considered in the differential diagnosis of urothelial carcinoma. Abstract

Blunt abdominal trauma accounts for the most abdominal injuries. A quick yet accurate examination must be carried out immediately to identify and promptly address the cause of the internal bleeding. In this case report, we describe A 17-year-old girl who was taken to the emergency room after she lost consciousness following a traffic accident. The patient presented with unstable hemodynamics indicating signs of hemorrhagic shock. The patient then underwent a CT examination with contrast to determine the diagnosis. The patient was diagnosed with internal bleeding with unstable hemodynamics due to liver injury AAST grade IV and renal trauma AAST grade III. In this case, signs of kidney and liver trauma can be recognized, such as fluid accumulation and the severity of injury to each organ. This case report aims to increase awareness of the signs of intra-abdominal bleeding and its severity so that appropriate treatment can be determined immediately. Abstract

Background: Endometrial Stromal Sarcoma (ESS) is a part of uterine sarcoma that can clinically manifest with abnormal vaginal bleeding, postmenopausal bleeding, uterine enlargement or mass, dysmenorrhea, pelvic pressure, or pain. ESS is divided into four main categories: endometrial stromal nodule, low-grade ESS, high-grade ESS, and undifferentiated uterine sarcoma. Management for ESS includes surgery, chemotherapy, radiotherapy, and hormone therapy. We want to review the role of radiation therapy for endometrial stromal sarcoma, from several pieces of literature. Material and Methods: We are conducting a literature review to describe the role of radiation therapy for ESS. Results: Radiotherapy serves as an adjuvant treatment in ESS, potentially improving prognosis and survival rates. While early-stage ESS may not necessitate radiotherapy, it can be considered for ESS cases with metastasis or when surgery is not an option. Conclusion: According to previous studies, radiotherapy for ESS can improve disease-free survival and increase patient survival. On the other hand, there are studies that still require large-scale prospective studies to validate the effectiveness of radiotherapy for ESS. Abstract

Systemic lupus erythematosus is a complicated autoimmune disease that presents in various organs, one of which is the lungs. We reported a 20-year-old female patient complaining of cough with phlegm, shortness of breath, chest pain radiating to the right shoulder, and a fever two weeks before admission. The patient was examined by thoracic radiography and thoracic CT scan with contrast. They found cavities with air-fluid levels and ground-glass opacity around them, with the results of sputum microbiological examination showing Klebsiella pneumoniae infection. The patient was then given antibiotic therapy. During an evaluation a month after therapy, the air-fluid level reduced, but the consolidation remained. The patient underwent sputum microbiological examination again and presented with MDR Klebsiella pneumoniae, which was then given therapy according to the results of antibiotic sensitivity. At the evaluation, the consolidation reduced, and the patient experienced clinical improvement. Risk factors for lung abscess in SLE patients are immune dysregulation, disease activity, and immunosuppressive therapy given to patients. Both conventional radiographs and CT scans have their place for approaching the diagnosis of lung abscesses in SLE patients. Thus, they should be performed to consider the appropriate management for SLE patients.

Abstract

BACKGROUND AND OBJECTIVE: Ultrasound plays an essential role in the initial assessment of breast cancer. This paper review the breast ultrasound report based on ACR BI-RADS 5th edition 2013, with additional information based on a related updated journal. METHOD: A literature review was followed while conducting this study with the keywords “BI-RADS Update”, “Breast Ultrasound”, and “Update Breast Ultrasound Report”. RESULT: Minimal progress has been made since ACR BI-RADS 5th edition. Meanwhile, the clinical applicability of breast ultrasound reporting could adapt to local policies as needed. CONCLUSION: The fifth BI-RADS  widely awaited version includes updates to the ultrasound. The revisions serve multiple key purposes: aligning terminology across imaging techniques such as mammography, ultrasound, and MRI; anticipating the growing use of supplementary screening methods and specialized technologies like automated breast ultrasound and elastography; and facilitating the transition toward more effective and efficient interpretation and reporting.

Abstract

Hemoglobin is an important protein in erythrocytes that is useful for transporting oxygen throughout the body. Changes in hemoglobin levels are often associated with medical disorders, which are diagnostic indicators for several diseases, including pulmonary tuberculosis. Tuberculosis diagnosis is often carried out through a thorax radiography examination by looking at the extent of the lesion (minimal, moderate advanced, or far advanced). Low hemoglobin levels (anemia) are associated with biomarkers of tuberculosis severity. The objective of this research is to ascertain the correlation between hemoglobin levels and the extent of lesions in thorax radiography examination of patients with pulmonary tuberculosis. In this observational research, secondary data from patient medical records is analyzed. The subjects of this research were pulmonary tuberculosis patients at Dr. Saiful Anwar General Hospital, East Java, Indonesia. Using the Chi-square test, the data from 55 subjects were analyzed, and the findings showed a p-value < 0.001. With a p-value < 0.05, this research shows that hemoglobin levels are significantly correlated with the extent of lesions from thorax radiography examinations of pulmonary tuberculosis patients.

Abstract

Brain cancer is one of the top ten causes of mortality globally and is classified as a malignant condition. An essential aspect of assessing the stage of malignancy when brain metastases occur is radiologic examination. Research on the demographic characteristics and CT-MRI imaging of cancer patients with brain metastases in Malang City is still limited; therefore, this study was conducted at Dr. Saiful Anwar Hospital. This study employed descriptive observational research methods, with the population consisting of patients diagnosed with brain metastatic tumors as recorded in medical records from June 2023 to May 2024. Data collection was based on secondary data obtained from medical records and the Radiology Information System (RIS). The data will be analyzed descriptively and presented in tabulated form using the SPSS program. Research on brain metastases in 92 patients revealed that the majority were aged between 41 and 70 years, had a median age of 51 years, and women were more likely to have it. Brain metastases were most frequently caused by lung cancer, then breast cancer. Metastases were most commonly found in the supratentorial region, particularly in the frontal lobe, with most patients having multiple nodules. Intratumoral hemorrhage occurred in more than half of the patients, especially those with aggressive lung cancer. Additionally, 8.3% of patients had leptomeningeal metastases.

Abstract

Infertility is the fifth highest serious global health issue affecting both males and females. Globally, more than 186 million people suffer from infertility and the number has increased considerably. A diagnosis of infertility is typically made when a couple fails to achieve pregnancy after one year of regular, unprotected intercourse. A 23-year-old nulliparous woman presented with a chief complaint of infertility for the past 2 years. She also complained of abdominal pain for the past 6 months. Ultrasonography (USG) revealed a left ovarian cyst measuring 4.12 cm. Hysterosalpingography (HSG) showed bilateral hydrosalpinx with bilateral patent fallopian tubes. Her husband has not undergone a male infertility evaluation. Problems associated with infertility include abnormal follicle growth, structural malformations, fallopian tube impairment, and uterine defects. The most common imaging test for evaluating tubal patency is HSG, and ultrasound can further assist in visualizing the uterus and surrounding adnexal regions. In this case, USG revealed an ovarian cyst. Cysts affect fertility based on their size, type, and quantity. HSG allows imaging of the uterus and fallopian tubes. Blockage of fallopian tube can inhibit reproduction. The worst manifestation is hydrosalpinx, characterized by fluid filled dilation of the fallopian tube caused by a blockage at distal part, which impairs the fertilization process. Symptoms of hydrosalpinx can be preceded by abdominal pain, similar to this patient’s complaint. Diagnostic imaging plays a vital role in assessing infertility, and selecting the most suitable imaging method is crucial because the results guide the direction of patient treatment.
