Abstract

The interaction between viral and host immune factors plays an important role in natural history of chronic HCV infection .We aimed to determine serum level of MBL and its relation to grades of liver fibrosis in chronic HCV patients.we studied the impact of serum level of MBL on early virological responsein chronic HCV patients.subjects and methods: included 70 HCV patients (40 HCV patients with liver fibrosis grades <F3 (group Ι) &30 HCV patients with liver. fibrosis ≥F3 (groupΙΙ) according to METAVIR score), who fulfilled inclusion & exclusion criteria of the National Egyptian treatment protocol of treatment of CHC, in addition to 16 control subjects. All studied cases were subjected to history taking, thorough clinical examination, routine investigation(fasting blood sugar, serum creatinine, alkaline phosphatase, ALT, AST, total bilirubin, albumin ,complete blood picture ,prothrombin activity, alpha feto-protein, TSH) ,  abdominal ultrasonography and liver biopsy in addition to ELISA for MBL. Results: mean values of MBL was significantly increased among group 2 patients more than that of group 1(P=0.001). After 12wk of combined therapy, it was found that 61 patients (87.14%) were responders to antiviral therapy, while9 patients (12.85%) were non responders.  The mean value of MBL among responders was 64.44± 26.75 while it was 90.41± 82.41 without any statistically significant differences. So it can be concluded that MBL plays an important role in the pathogenesis of liver fibrosis in HCV liver disease. MBL has no impact on early virological response among patients with chronic HCV infection.

Abstract

Background: Gastric bleeding due to stress-related mucosal damage (SRMD) is a condition that is caused due to erosive gastritis that often occurs in critically ill patients in intensive care. One condition that is known as a risk factor for SRMD is traumatic brain injury. Two common agents used as gastric ulcer prophylaxis are proton pump inhibitor (PPI) and H2 antagonist receptor (H2AR). The goal of this study was to compare the effectiveness of PPI and H2AR administration as prophylaxis of SMRD in patients with TBIs who were treated in the ICU. Patients and Methods: This is a single-blind, randomized, controlled trial with pre and post-test measurements. All subjects were measured for baseline gastric pH before being given gastric ulcer prophylaxis. Gastric pH was measured using a pH-meter. The subjects were divided into two groups: omeprazole group (omeprazole 40 mg every 12 hours) and ranitidine group (received ranitidine 50 mg every 12 hours). The pH levels were measured regularly twice daily for five days.  Results: 56 subjects were involved in this study and divided equally into two groups. For each gastric pH measurement, the pH in both groups did not significantly differ. The optimal gastric pH was achieved in 24 hours after the first administration of gastric ulcer prophylaxis.  Conclusions: The administration of ranitidine or omeprazole is equally effective in maintaining the acidity of gastric acid in TBI patients in ICU. There was no significant difference in the incidence of gastric bleeding in ranitidine and omeprazole groups. Keywords: ICU, Gastric pH, PPI, H2-receptor antagonist.

Abstract

Background: Leprosy is a disease caused by M. leprae infection, may affect the peripheral nervous system, skin and other tissues. Until recently, leprosy is still a health problem in Indonesia. Phenolic Glycolipid-1 (PGL-1) is a stable and unique component of M. leprae, and an antibody examination against this antigen may play a role in assisting early detection of subclinical stage leprosy (SSL) and treatment monitoring. Objective: This study aims to define the level of IgM anti-PGL-1 antibody in leprosy patients, household contacts and controls and their serology status  Methods: The examination of IgM anti-PGL-1 antibody was carried out on 52 leprosy patients, 95 people household contacts and 95 healthy controls. The data from subjects were then analyzed and tested using Kruskal-Wallis statistical test and a p-value <0.001 was obtained. Results: It was obtained that the levels of IgM anti-PGL-1 antibody significantly differed between leprosy, household contact and control groups (p<0.05). Most of the leprosy patients had seropositive status, contrary to dominant seronegative that was found in household contacts and control groups. In subgroup analysis, the IgM titer level did not significantly differ between household contacts and control groups (p=0.122). Conclusion: the IgM anti-PGL-1 antibody differed between leprosy and household contacts and control. IgM antibody titer may also depict a person's exposure status to M. leprae bacilli.Keywords: M. leprae, Leprosy, IGM anti-PGL-1 antibody, Household contacts.

Abstract

Tamarind (Tamarindus indica L.) has long been used as a spice, food component and traditional medicine. It has high content of total phenols, proanthocyanidin and tannins Tamarindus indica has a high commercial value affordable functional foods. Howeever it has short shelf-life during post-harvest handling owing to high moisture content. It’s necessary to preserve this valuable fruit for long-term use. Objective of this research focused on the effectiveness of various processing parameters such as pretreatment with KMS solution, sugar concentration, immersion time, and agitation speed during the osmotic dehydration of Tamarindus indica into jam. Our results showed that the optimal dehydration process should be pre-treated with 0.25% KMS solution, sugar 60oBrix, immersion in 75 min, agitation 500 rpm. From this approach, the added value of Tamarindus indica fruits would be enhanced. Osmotic dehydration showed minimum heat degradation to nutritional components owing to low temperature moisture removal. It contributed some advantages such as limiting the thermal damage to the aroma, color, preventing the browning of enzymes and decreasing the energy consumption.Keywords: Tamarindus indica, Osmotic dehydration, Pretreatment, Immersion, Agitation, Sugar.

Abstract

Objectives To determine percentage of case finding of Staphylococcus aureus, methicillin resistant Staphylococcus aureus (MRSA) and panton–valentine leukocidin (PVL)-positive Staphylococcus aureus (S. aureus) isolated from skin and soft tissue infections in the community settings at Primary Healthcare Centres in Sidoarjo region, Indonesia and the relationship of risk factors of Staphylococcal skin and soft tissue infections. Methods S. aureus isolates from clinical cultures of patients in 2 Primary Healthcare Centres were included. Wound cultures were performed to identify the MRSA phenotypically using VITEK® 2 system. The mecA gene and genes encoding PVL were detected using Polymerase Chain Reaction (PCR). Clinical data including gender, age, hospitalization history in primary health care center and hospital, comorbidity (diabetes mellitus) history and antibiotic history, were collected from questionnaires and medical record. Statistical analysis was done by Chi-square-Fisher’s exact test. Results In total, 45 patients involved in this study. The 20 patients consisted Staphylococcal strain, and 25 patients did not. Of the 20 Staphylococcal strains, 3 samples were Staphylococcus coagulase negative, 1 sample was Staphylococcus gallinarum and another sample was Staphylococcus haemolyticus. There were 15 patients (33.3%) were infected by S. aureus. Of these, 13 isolates were MRSA-positive (28,9%) and 1 isolate was PVL-positive (2,2%). Analysis showed no relationship between the risk factors and finding of S. aureus, neither MRSA and PVL in all patients (p > 0.05).Conclusions S. aureus is common in the community. The MRSA is clinical importance in Indonesia, not only in hospitals, but also in the community settings. High case of MRSA was found in this study. However, the case of PVL-positive S. aureus was low. It is very important to promote case finding by detecting MRSA in the community settings. Abstract

Cayratia trifolia is a weak herbaceous climber. Its fruit is fleshy, juicy, dark purple or black. Cayratia trifolia Linn. Is a medicinally important plants and used in the treatment of various diseases. In order to utilize this valuable fruit, objective of this study focused on major technical parameters such as sugar supplementation and yeast ratio in the primary fermentation; different clarifying agents such as gelatin, kaolin, egg white, bentonite  in the secondary fermentation. Results showed that sugar concentration and yeast ratio in the initial must were very important to the ethanol, titratable acidity and aroma in wine. Gelatin was suitable in fining step to control haze turbidity in wine. Abstract

Background: Rabies is still endemic in the working area of the Denpasar Veterinary Center. For this reason, it is still necessary to conduct ongoing rabies surveillance activities aimed at: detecting the presence of rabies virus in dogs at risk of contracting rabies which related to efforts of liberation rabies in Bali Province, detecting the possibility of rabies virus presence in dogs in NTB Province so that this area remains rabies-free, detected rabies virus in dogs in the Flores Island and surrounding areas related to rabies control activities in NTT Province. Method: Rabies surveillance in dogs is mainly carried out by taking samples of dog brains that are at risk of transmitting rabies. The sample examined by the Fluorescent Antibody Test (FAT) method. Results: In 2016, the number of animal brain samples examined by the Denpasar Veterinary Center was 2,066. In Bali Province, the number of animal brain samples examined was 1,480 samples, 206 / 1,480 (13.92%) samples were positive for rabies. Positive cases of rabies came from dogs 205/206 (99.51%) samples and cats 1/206 (0.49%) samples. The average number of rabies positive cases per month is 17 cases. This number decreased sharply compared to 2015 there were 44 cases, per month. The most common rabies cases were found in Buleleng Regency as many as 41 cases, caused by dogs that have not vaccinated. The number of dog brain samples from districts/cities in NTB Province was 417 samples; there was no rabies positive. While animal brain samples from districts/cities on Flores Island and Lembata, NTT Province were examined as many as 169 samples, 45/169 (26.63%) samples were positive for rabies. Positive cases of rabies came from 44/45 dogs (97.78%) samples and 1/45 goats (2.22%) samples. Conclusion: The results of this surveillance show that rabies is still endemic in the Province of Bali and islands around the island of Flores, NTT. Mass vaccination programs, a collaboration between government agencies, communication, information and education about rabies to the community still need to be improved. Until now the NTB Province is still free of rabies. Control of rabies infectious animal traffic to the NTB Province and rabies-free areas in NTT is still needed.Keywords: Brucellosis, BPT, CFT, Bali, NTB. NTT.

Abstract

Echinococcus granulosus is one of the small tap worms that causes Hydatid cyst.  This study was planned for detection of the incidence among cancer patients at Tumor unit in Baghdad medical city. Eleven samples of liver biopsy out of 40 specimens taken from patients with liver cancer in this study. All patients were suffering from Hydatid cyst infection Patients of migratory encampment in and around Baghdad city Kirkuk, Diyala, and Tikrit Hospitals, suffering from hydatidosis. The present study showed that the predominance of hydatidosis was in the patients whose ages ranged between (≤20-60) years. The sensitivity and specificity of ELISA with hydatid cyst IgG were 96%. While Casoin test (immediate hyper sensitivity skin test) was 100%.  The majority incidence was recorded between (50-60) years. The study showed that the predominance of hydatidosis was higher in females 28(70%), than males12 (30%). Analysis of the gene sequence showed that there were genetic mutations occurred in the hepatic cells, and several locations underwent change in genetic sequence including TCC to TCT in exon 147(5), GGT to AGT in exon 152(5), ACT to CCT in exon 160(5), TAT to TGT in exon 203(6),ATG to CTG in exon 235(7).Keywords: TP53 gene, Mutation, Tumor, Hydatid cyst, Gene sequencing.

Abstract

Introduction: Benign Prostatic Hyperplasia (BPH) occurs due to an imbalance between cell proliferation and the apoptotic process. Many factors influence the growth of the prostate. This study aims to determine which factors among TGF-b, TNF-a,, PSA, testosterone, correlate the most the stromal-to-epithelial ratio of the prostate. Methods: The study was conducted in 83 BPH subjects in several hospitals in Denpasar. From all subjects, blood was taken for PSA and testosterone examination. TGF-b and TNF-a were examined from the histopathological examination of prostate tissue specimens. Data were collected, then statistical analysis was done with SPSS 22.0. Results: From all parameters, only PSA was significantly negatively related to stromal-to-epithelial ratio, although the strength was negatively weak (r -0.28). Multivariate analysis showed that the combination of PSA and TGF- levels had an effect of 10.8% on the epithelial-stromal ratio. Conclusion: PSA showed a significant association with the ratio of prostate epithelial stroma and had an effect value of 10.8% when combined with TGF-bKeywords: Prostate enlargement, Epithelial-stromal ratio, TNF-α, TGF-β, PSA, Testosterone.

Abstract

Background: Central venous pressure (CVP) is popular for assessing fluid estimation, especially critically ill patients. Another method in assessing the CVP today is by using ultrasonic cardiac output monitors (USCOM) using transaortic or transpulmonary Doppler’s ultrasound. The objective of this study was to determine the correlation between CVP and IJV collapsibility index to monitor the adequacy of the intravascular volume status in critically ill patients in ICU. Patients and Method: This analytic, cross-sectional study involved 70 patients of 19 to 64 years. The internal jugular vein (IJV) collapsibility index was examined with ultrasound cardiac output monitoring (USCOM) to assess the patient's intravascular volume status, then the data was compared to CVP. The Spearman rank correlation was used to test the hypothesis. Results: The subjects consisted of 38 males and 32 females, with an age range of 19-64 years. The Spearman’s rank correlation test showed a statistically negative correlation between the CVP value and the internal jugular venous pressure index collapsibility value (R=-0.844, p <0.001). Conclusion: There is a strong negative correlation between IJV collapsibility index and CVP in patients treated in the ICU.Keywords: Hemodynamics, Volume status, CVP, USCOM, Collapsibility index.
