Abstract

Hyaline membrane disease (HMD) is commonly found in preterm infants. This disease occurs as the result of surfactant deficiency due to prematurity. Surfactant deficiency results in increased alveolar surface tension, with resistance to inflation and alveolar collapse at the end of expiration. As a result, the alveoli are injured due to shear stresses on the alveolar walls. The injury could be seen in chest x-ray, as the most common radiological modality to help differentiate diagnosis. Plain chest x-ray findings of HMD are low lung volumes, diffuse, bilateral and symmetrical granular opacities, bell-shaped thorax, and air bronchograms. This case study showed chest x-ray finding of preterm newborn that diagnosed with respiratory distress syndrome clinically.

Abstract

Meningitis is defined as inflammation of the meninges, which consists of dura mater, arachnoid mater, and pia mater. Meningitis can be caused by infectious and non-infectious processes such as autoimmune disorders, cancer, drug reactions. Meningitis can have a varied clinical presentation. Symptoms such as fever, neck pain or stiffness, photophobia, headache, dizziness, confusion, delirium, irritability, and nausea or vomiting may present. Signs of increased intracranial pressure such as altered mental status, neurologic deficits, and seizures indicate poor prognosis. Meningitis is diagnosed through cerebrospinal fluid analysis. However, there is controversy that the lumbar puncture could lead to brain herniation in the presence of increased intracranial pressure. Thus, Computed Tomography Scan (CT scan) could be alternative modality. In this case, we present meningitis diagnosed with CT scan of brain.

Abstract

Diabetic foot ulcer is one of the complications of patients who have diabetes mellitus which is not well controlled. It is resulted from poor glycemic control, underlying neuropathy, peripheral vascular disease, and poor foot care. It could lead to osteomyelitis of the foot and amputation of lower extremities. Detailed history taking should be done in order to get a holistic view of disease progression and risk factors. Clinical presentations such as the presence of callus, loss of hair, muscle atrophy, ulcer, paresthesia, hypo- or hyperesthesia, anhydrosis could be present. Ulcers are most common in the weight-bearing areas such as plantar metatarsal head, heel, tips of hammer toes, and other prominent areas. Radiological investigations including plain x-rays should be done in order to look for any underlying osteomyelitis, the presence of air in the subcutaneous tissue, any signs of underlying fractures, and presence of a foreign body. In this case study, we presented X-ray finding of a diabetic foot ulcer case that complicated with fracture and osteomyelitis.

Abstract

Tubo-ovarian abscess (TOA) is one of acute complications of pelvic inflammatory disease that usually occur in woman of childbearing age. Factors associated with TOA are reproductive age, Intra-Uterine Device (IUD) insertion, multiple sexual partners, and a history of a prior episode of PID. The classic presentation of a TOA includes abdominal pain, pelvic mass on examination, fever, and leukocytosis. However, prompt diagnostic evaluation must be undertaken to specify the diagnosis. Tubo-ovarian abscess can be found on imaging with ultrasonography, computed tomography (CT), or magnetic resonance imaging (MRI). Ultrasonography as the cheaper of all could be used as first-line diagnostic modality. This study presents an example of ultrasonography examination of 42-year-old woman presenting with stomach pain and fever. Abdominal ultrasonography was performed and we found abscess formation in adnexa. Emergency surgery was performed and antibiotics were given after surgery. This study showed us how abdominal ultrasound could diagnose tubo-ovarian abscess.

Abstract

Acute appendicitis is one of the most common diagnosis suspected in patients with acute abdominal pain. Classic symptoms such as right iliac fossa pain, anorexia, nausea, constipation, and vomiting only occur in 50% of cases. Thus, other diagnostic modalities are still needed to help doctors conclude the definitive diagnosis that lead to prompt treatment. Ultrasound examination of the abdomen could be good choice since it is relatively faster, cheaper, and quite accurate in diagnosing appendicitis. This study presents an example of ultrasonography examination of patient with abdominal pain suspected of appendicitis. Ultrasound imaging showed echoic inflamed fat with appendix diameter of 13,1 mm indicates acute inflammation of appendicitis. Dilated appendix which is more than 6 mm in size was one of the supportive sign to propose appendicitis. This finding was in line with the patient’s signs and symptoms, thus acute appendicitis was diagnosed. The patient went to surgery and appendectomy was performed. This study showed us how abdominal ultrasound could add diagnostic value to diagnose acute appendicitis.

Abstract

The National Health Insurance Program (Jaminan Kesehatan Nasional - JKN) is a social health insurance managed by the National Health Insurance Agency (Badan Penyelenggara Jaminan Sosial Kesehatan – BPJS Kesehatan), aiming to provide health insurance for all Indonesian people. Community groups that receive subsidies from the government are referred to as Contribution Assistance Recipients (Penerima Bantuan Iuran - PBI) participants. One of the issues regarding the JKN program so far is the inaccuracy of PBI targets. Therefore, a study is needed to evaluate the relationship between JKN PBI ownership with their socioeconomic status and history of receiving social aid.

Abstract

Cellulitis is deep dermal and subcutaneous tissue infections. Cellulitis typically manifests as a warm, erythematous area that is poorly defined, edematous, and tender to palpation. Cellulitis may act as a trigger for the diagnosis of diabetes mellitus, and occult diabetes may be a significant actor in the development of cellulitis.

Abstract

Varicocele is a condition with abnormality in growth and dilatation of veins that drains blood from the testicle, the scrotal venous pampiniform plexus. Abstract

Pediatric inflammatory multisystem syndrome, temporally associated with SARS-CoV-2 (PIMS-TS) or multisystem inflammatory syndrome in children (MIS-C) is a condition identified recently. Children who were previously healthy may experience cardiac, neurological, respiratory, gastrointestinal, and dermatological dysfunction. Majority of patients (73%) require intensive care treatment, and more than half (56%) of patients arrive in shock. Different management are used for MIS-C patients, but they all primarily focus on immunomodulatory drugs such as IVIG and glucocorticoids as initial therapy. Biologic agents like anakinra, tocilizumab, and infliximab should be intensified when the patients are resistant to IVIG and glucocorticoids.
