Abstract

Introduction: Stretch marks or striae distensae are linear atrophic scars that form in areas of skin damage as a result of skin stretching. Mast cell degranulation and activation of macrophages occur as well as changes in the components of the extracellular matrix, including fibronectin. Fibronectin as a dimeric glycoprotein and components contained in the extracellular matrix, functions as a regulator of cellular processes, to maintain tissue and functions in the process of wound healing in tissues. The aim of this study was to determine the relationship between plasma fibronectin levels and stretch marks Methods: This study was an observational study with a cross-sectional design of 40 females with stretch marks and 40 controls. Each patient underwent history, dermatological examination, and blood sampling to assess plasma fibronectin levels by ELISA test. These data were analyzed statistically using the Chi square test. Results: The mean plasma fibronectin level in stretch marks was 259.541±165.937 ng/ml. The highest age with stretch marks was 18–25 years 33 (82.5%) people. Most of them had a family history of stretch marks from their mother 20 (50%) people. The majority of stretch marks are located on gluteus regions by 10 people (25%). The result of this study showed that there was a relationship between plasma fibronectin levels causing a risk of 2.85 times for stretch marks (p = 0.041). Conclusion: There is a relationship between plasma fibronectin levels and stretch marks.

Abstract

Introduction: Coronary heart disease continues to be the leading cause of morbidity and death worldwide. The cardiac dysfunction known as chronic coronary syndrome is caused by atherosclerosis-related obstruction or constriction of coronary arteries. Currently, One interesting non-invasive technique is GLS assessment, which has shown to be useful in predicting coronary lesions in chronic coronary syndrome patients. This study aims to evaluated the correlation between myocardial strain based on 2-D Speckle-Tracking Echocardiography and the severity of coronary lesions in chronic coronary syndrome patients at Haji Adam Malik Hospital Medan Method: This research is an analytical observational study with a cross sectional design on patients who diagnosed with chronic coronary syndrome and treated at H Adam Malik General Hospital Medan which met the inclusion and exclusion criteria. Sampling was carried out starting in January to December 2023. In bivariate analysis, the Fisher Exact test is used if the data is not normally distributed, and the chi square test is used if the data is normally distributed. P values less than 0.05 indicate statistical significance in data analysis using computer statistical methods. Results: Myocardial global longitudinal strain (GLS) based on 2-D Speckle-Tracking Echocardiography was significantly associated with the severity of coronary lesions as measured by syntax score in chronic coronary syndrome patients (p value  0.0001, OR 2.732, 95% CI 1, 81-4,11). As a predictor of coronary lesions severity based on the Syntax score in patients with chronic coronary syndrome, the global longitudinal strain (GLS) cut-off value of -17.37 demonstrates a sensitivity of 92.9% and a specificity of 83.6%. Conclusion: Myocardial global longitudinal strain (GLS) with a cut-off value of -17.37 can be used as a predictor coronary lesions severity in chronic coronary syndrome patients with a sensitivity of 92.9% and a specificity of 83.6%.

Abstract

Introduction: P wave dispersion is a more sensitive cardiovascular marker which obtained by the ECG to elaborate structural and electrophysiological abnormalities in patients. This study aims to elucidate P wave dispersion prevalence in heart failure due to coronary heart disease patients and the correlation with their quality of life. Method: This analytical cross-sectional study is done in RSUP Haji Adam Malik in 133 patients with heart failure from May to June 2023. Patients had undergone clinical evaluation, physical examination, 12-lead electrocardiography (ECG), and then were given the MLFHQ to assess the quality of their lives. Data were analyzed using fischer-exact test, with p value less than 0.05 considered to be significant. Results: From the total of 133 sample of patients, there were 55 (43.2%) patients with positive p wave dispersion, among them 2 had low quality of life, 25 had moderate quality of life, and 28 had good quality of life. With mean of PWD is 27.9±15.1 ms. The group of patient with negative p wave dispersion 78 had moderate and good quality of life. P value is less than 0.01. It’s concluded that there was a positive correlation between p wave dispersion and quality of life of heart failure patients in Haji Adam Malik general hospital Conclusion: There is positive correlation between p wave dispersion and quality of life of heart failure due to coronary heart disease patients in Haji Adam Malik general hospital.

Abstract

Background: Guillain-Barré syndrome (GBS) is an autoimmune disease of the peripheral nervous system, which is clinically characterized by tetraplegia with or without sensory disturbances. Manifestations include weakness, progressive muscle areflexia and can cause weakness in the respiratory muscles. This causes sufferers to need mechanical ventilation assistance. ICU complications such as lung infections and sepsis occur if treatment is prolonged. Plasmaparesis in GBS is a therapy other than IVIG (IntraVenous ImmunoGlobulin) administration. Case report: We compared 2 cases, 42 years old (man A) and 31 years old (man B), with respiratory failure due to GBS were treated with plasmaparesis for 5 cycles. Patient A showed faster clinical improvement results. It took 6 days to get off mechanical ventilation on patient A, while on day 11 patient B still needed mechanical ventilation, with persistent tetraparesis. Conclusion: This case highlights the importance of using TCD and NIRS in the intensive care unit as guiding therapies in maintaining patient blood pressure, administering blood components, and early detection of complications such as cerebral vasospasm.

Abstract

Introduction: Cerebral infarction accompanied by brain edema is a life-threatening condition and requires decompression craniectomy to overcome increased intracranial pressure (ICP). Comprehensive post-operative management using non-invasive transcranial doppler (TCD) monitoring and Near Infrared Spectroscopy (NIRS) is a modality for conducting management in the intensive room (ICU). Case Report: We reported the case of a 56-year-old man diagnosed with cerebral edema due to stroke infarction of the right carotid system accompanied by comorbidities of coronary heart disease with a heart ejection fraction of 31.6% performed craniectomy decompression. The patient was treated for 12 days in intensive care and then transferred to a semi-intensive room Conclusion: This case highlights the importance of using TCD and NIRS in the intensive care unit as guiding therapies in maintaining patient blood pressure, administering blood components, and early detection of complications such as cerebral vasospasm.
