Abstract

Sarkoidosis merupakan kelainan inflamasi multisistem yang ditandai dengan terbentuknya granuloma dan terjadi pada berbagai organ, terutama paru dan kulit. Granuloma berisi sel imun berupa makrofag yang juga berperan dalam metabolisme vitamin D. Makrofag pada granuloma sarkoidosis terbukti mampu memproduksi 1,25-dihidroksi vitamin D (kalsitriol) di luar ginjal yang berasal dari prekusor 25-hidroksi vitamin D (calcifediol). Selain itu, metabolisme 1,25-dihidroksi vitamin D di makrofag tidak memiliki mekanisme umpan balik yang efektif dalam menjaga keseimbangan kadar vitamin D di tubuh. Penurunan ini dapat mengganggu keseimbangan kadar kalsium pada pasien sarkoidosis. Pemberian suplementasi vitamin D dianggap sebagai terapi adjuvan dalam tata laksana sarkoidosis, namun diketahui dapat menyebabkan hiperkalsemia. Perubahan kadar vitamin D pada sarkoidosis tidak selalu menjadi indikasi pemberian suplementasi vitamin D, diperlukan pemeriksaan laboratorium yang tepat sebelum memberikan suplementasi vitamin D guna mengurangi risiko terjadinya hiperkalsemia pada sarkoidosis. Pemberian suplementasi vitamin D dalam dosis rendah diperbolekan bagi pasien sarkoidosis yang tidak disertai kondisi hiperkalsemia, namun perlu dilakukan pemeriksaan kadar vitamin D dan kalsium secara rutin.

Abstract

Introduction: Recessive dystrophic epidermolysis bullosa (RDEB) is a rare bullous disease caused by mutations in the COL7A1 gene encoding anchoring fibril collagen that maintains skin integrity. Case: Male, 67-year-old, presented with blisters and wound on various parts of the body, worsening in the recent year. They had been known to easily appear after trauma since. Vesicles-bullae, erosions-excoriations, milia, and atrophic scars were found on the axillae, groins, and limbs, as well as hyperkeratosis and anonychia of the hands and. Histopathological examination showed subepidermal cleft, milia and fibrosis, without immunoglobulin deposits on immunofluorescence. Discussion: Contrary to classical RDEB, blisters in mitis variant do not always cause impairment. The pronounced symptoms in advanced age might be mistaken for epidermolysis bullosa acquisita. Negative immunoglobulin deposition is the diagnostic key, confirming that blister does not result from an autoimmune process, but rather, from trauma to the abnormal skin, more so by flattening of the rete ridges in the elderly. In the absence of definitive therapy, treatment is symptomatic. Conclusion: The diagnosis RDEB-mitis might be missed in geriatric. Correct diagnosis has implication on management because not only does RDEB-mitis require no immunosuppressant, measures to minimize blister formation is more important to save patient from complication.

Abstract

Pendahuluan: Disfungsi ereksi menjadi perhatian di golongan homoseksual dan sering dikaitkan dengan depresi sebagai salah satu pemicunya. Tujuan: Mengetahui hubungan antara tingkat depresi dengan kejadian disfungsi ereksi di komunitas Lelaki Seks Lelaki (LSL) di Surakarta. Metode: Penelitian analitik observasional dilakukan di Surakarta periode Juni-Juli 2021. Subjek adalah 41 laki-laki berusia 20-50 tahun, orientasi seksual dengan sesama jenis dan menyetujui untuk mengikuti penelitian. Penilaian tingkat depresi menggunakan kuisioner Patient Health Questionnaire-9 (PHQ-9). Penilaian disfungsi ereksi menggunakan kuisioner International Index of Erectile Function (IIEF) dalam domain fungsi ereksi. Analisis data menggunakan uji korelasi spearman rank SPSS versi 21 dengan nilai signifikasi p<0,05. Hasil: Tingkat depresi dan faktor usia tidak memiliki hubungan yang signifikan dengan disfungsi ereksi (r=0,158 dan p=0,323; r=0,020 dan p=0,901). Variabel yang berhubungan signifikan dengan kejadian disfungsi ereksi adalah frekuensi hubungan seksual (r= -0,389 dan p=0,012) dimana semakin jarang subjek penelitian melakukan hubungan seksual maka semakin tinggi kemungkinan terjadinya disfungsi ereksi. Tingkat koefisien korelasi antara frekuensi hubungan seksual terhadap kejadian disfungsi ereksi adalah kategori lemah (r=0,200-0,399). Kesimpulan: Tingkat depresi tidak berkorelasi terhadap kejadian disfungsi ereksi, namun kejadian disfungsi ereksi berkorelasi negatif dengan frekuensi hubungan seksual pada komunitas LSL di Surakarta.

Abstract

Dermatitis atopik (DA) merupakan salah satu dermatosis kronik yang ditandai dengan eksema berulang yang disertai dengan rasa gatal yang tidak tertahankan dan dapat mengakibatkan gangguan kualitas hidup yang bermakna. Studi terdahulu melaporkan faktor genetik, sawar kulit, disregulasi sistem imun, dan faktor eksternal saling memengaruhi dalam menimbulkan penyakit. Meskipun gambaran klinis DA pada tiap pasien relatif sama, studi terkini menyatakan tiap pasien memiliki perbedaan dalam komponen yang dominan berperan dalam patogenesis penyakit. Terapi standar DA saat ini meliputi kortikosteroid, inhibitor kalsineurin, dan pelembap. Pemahaman mengenai patogenesis DA penting untuk mengembangkan modalitas terapi DA yang baru. Uji klinis obat terhadap sitokin (dupilumab, tralokinumab, dan lebrikizumab), molekul kecil (apremilast, crisaborol, tofacitinib, dan baricitinib), reseptor (tapinarof), dan mikrobiom yang terlibat pada DA telah banyak dilakukan dan diharapkan dapat memberi solusi pada tata laksana DA. Tinjauan pustaka ini bertujuan untuk memaparkan studi terkini mengenai patogenesis dan berbagai modalitas terapi yang telah dikembangkan untuk tata laksana DA.

Abstract

Psoriasis is a chronic residive inflammatory skin disease caused by an autoimmune process. The worldwide prevalence of psoriasis ranges from 2-3% with the prevalence in Asia tends to be low, which is less than 0.5%. Psoriasis consists of erythematous scaly plaques and occurs most commonly on the elbows, knees, scalp, and lower back. Because of the reported frequent recurrences, psoriasis requires maintenance therapy in the form of systemic therapy such as retinoids, methotrexate, alefacept, and efalizumab. Coronavirus disease 2019 (COVID-19) is a respiratory infection caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) and is highly contagious. The COVID-19 pandemic has a negative impact on psoriasis management and healthcare delivery. Infection of COVID-19 and its related medications can also affect the clinical severity and exacerbation in patient with psoriasis. Inadequate management of COVID-19 can trigger a cytokine storm and cause acute respiratory distress syndrome (ARDS), sepsis, and other complications. COVID-19 patients who experience a cytokine storm with elevated IL-6 and IL-17 may potentially experience psoriasis exacerbations. Biological therapy for psoriasis during pandemic should be carefully considered according to the individual patient’s condition.

Abstract

Introduction: Hematological-related malignancy-induced eosinophilic dermatosis or He Remained is a rare paraneoplastic cutaneous reaction particularly associated with various hematological malignancies. It has a vast clinical presentation ranging from papules, plaque, to blisters and bullae and resembles other dermatoses. We report a case of He Remained on patient with chronic lymphocytic leukimia on remission. Case: A 49-year-old women who had been diagnosed with chronic lymphocytic leukimia presented with a pruritic rash which had been present for a month. Histopathology examination revealed eosinophilic infiltrate and the diagnosis of He Remained was established. She was treated with systemic steroid and achieve relieve of symptoms. Discussion: The pathogenesis of the dermatosis has not been fully interpreted. The cutaneous symptoms have been found to be unrelated to the hematologic malignancy course. Histopathology studies have an important role as one of the diagnostic criteria. Various treatments have obtained some response in short term; however treatments to control the chronic and relapsing course of the disease has been lacking. Conclusion: He Remained has a negative impact for patients and remains a diagnostic and therapeutic challenge for Dermatovenereologist. This case reports hopes to raise awareness given the rarity of the dermatoses.

Abstract

Background: Leprosy is one of the oldest and most neglected diseases in the world. During the COVID-19 pandemic, leprosy reaction triggered by COVID-19 has not been widely reported. Case: A 38-year-old woman, complained of erythema nodules in the eyelids for 1 week, increasing in size and pain accompanied by fever and nasal congestion. There is no previous history of disease or treatment. Physical examination revealed increased body temperature and anemia with bilateral madarosis. Dermatological status showed multiple erythematous nodules with regional distribution on bilateral superior palpebrae that were palpable with pain. There were multiple lesions of erosion and excoriation in the bilateral plantar pedis region. Sensory function is impaired with stocking hypoesthesia of the digits and lateral plantar pedis bilaterally. The results of the laboratory examination were SARS-CoV-2 PCR positive, ELISA anti PGL-1 IgM 8,159 u/ml, IgG 1,421 u/ml and the Bacterial Index (IB) 3+. Histopathology HE demonstrating lobular panniculitis with vasculitis and FF negative AFB. Discussion: Based on the medical history, physical examination and laboratory, the patient was diagnosed with an Erythema Nodosum Leprosum (ENL) reaction in a new case of Morbus Hansen (MH), suspected to be triggered by a COVID-19 infection. Neutrophil activity and cytokine storms due to COVID-19 are thought to trigger the ENL reaction. Conclusion: In the pandemic era, COVID-19 infection needs to be watched out for as a possible trigger for leprosy reaction.

Abstract

Introduction: Primary cutaneous aggressive epidermotropism CD8+ cytotoxic t-cell lymphoma (PCAE-CD8+ CTCL) is an aggressive variant of cutaneous T-cell lymphoma which has poor prognosis. Because of its rarity, its relatively variable clinical dan histopathological presentation, and no diagnostic criteria for PCAE-CD8+ CTCL, establishing diagnosis can be challenging. Case: A case of PCAE-CD8+ CTCL in a 67-year-old man whose early histopathological features resembling parapsoriasis was reported. Physical examination showed multiple lesions in form of erythematous plaques with ulcers and crusts in almost part of the body. Histopathological features showed epidermotropism and infiltrates of atypical also pleomorphic lymphocytes. Immunohistochemistry result was positive for CD8, CD4, Granzyme, CD3, and Ki-67 supported PCAE-CD8+ CTCL. Discussion: Clinical manifestation of PCAE-CD8+ CTCL varies among patients. Histopathological and immunohistochemistry examination are needed to establish diagnosis. Conclusion: The understanding of clinical manifestation of PCAE-CD8+ CTCL is needed. Because of its poor prognosis and high proliferative activity, early diagnosis may be helpful in improving the likelihood of patient survival.

Abstract

Abstract Introduction: Vitamin D is known to have many functions in the body, one of them is acts as an immunomodulator. This vitamin works through gene transcription of certain cells that express vitamin D receptors (VDR). Objective: To more understand the role of vitamin D in vitiligo based on RVD. Methods: This study using observational analytic study with cross-sectional method. The case group consisted of vitiligo patients whose skin was taken from the lesion area and perilesion through a plong biopsy procedure and stored as stored biologic material, while the control group were burn patients and chronic wounds from Plastic Surgery Outpatient, that underwent a split-thickness skin graft from the healthy skin. RVD expressions were examined by immunohistochemical using monoclonal antibodies and assessed by two reviewers. Results: The results showed that the mean VDR histoscore in the skin lesions of vitiligo (4.96 ± 1,852) was higher than controls (2.47 ± 0.405). The mean skin perilesion of vitiligo (5.07 ± 1,321) was higher than controls (2.47 ± 0.405). The mean histoscore of lesion was higher (4.96 ± 1,852) but was not statistically significant compared to perilesion vitiligo (5.07 ± 1,321). Conclusion: The expressions of RVD in perilesion vitiligo patients are lower than lesion. However, RVD expression of vitiligo patient was not lower than control group. This result shows that RVD plays little role in pathogenesis of vitiligo.

Abstract

Virus merupakan patogen infeksius yang dapat menyebabkan berbagai penyakit life-threatening, dan memiliki kemampuan untuk berevolusi seiring dengan evolusi manusia sebagai pejamu. Virus new-emerging, memiliki patogenitas yang lebih tinggi karena belum terbentuk imunitas alamiah pada pejamu. Selain itu, berbagai virus re-emergingjuga menjadi ancaman tersendiri karena kemampuannya yang tinggi untuk berevolusi, sehingga keberadaan virus emergingini menyebabkan outbreak, epidemi, dan pandemik global. Indonesia dengan letak geografis yang strategis dan habitat fauna yang beragam, berpotensi besar bagi transmisi zoonosis dan tempat berkembang yang ideal bagi vektor pada penyakit virus. Beberapa infeksi virus new emergingcontohnya virus swine fludan SARS-CoV-2, serta virus re-emergingmisalnya virus chikungunya dan measles yang dilaporkan di Indonesia, tidak hanya memberikan gejala klinis yang biasa, namun dapat disertai dengan manifestasi pada kulit. Beberapa manifestasi pada kulit tersebut juga dapat menjadi penanda khas terhadap jenis inveksi virus yang berperan sebagai patogen. Identifikasi terhadap lesi kulit pada penyakit-penyakit akibat infeksi virus penting untuk dilakukan, dan memiliki peran untuk mengarahkan pada kemungkinan diagnosis dan tata laksana yang tepat.
