Abstract

Aphasia is a neurological dysfunction that arises due to disturbances in the brain regions responsible for language processing. These areas are predominantly localized within the frontal cortex and the temporoparietal regions, which regulate both motor and sensory speech functions. Disruptions in these areas may result from ischemic conditions affecting the brain. Isolated cases of aphasia without other accompanying neurological deficits are rarely reported. This article presents a case study of a 66-year-old female patient with a history of chronic hypertension who presented to the emergency unit with a sudden onset of speech difficulties without any other neurological impairments. Initial examination revealed significantly elevated blood pressure at 220/110 mmHg, accompanied by severe headache. A diagnosis of Transient Ischemic Attack (TIA) was supported by clinical findings that resolved within 24 hours and imaging from a non-contrast head CT scan, which showed normal brain parenchyma. The primary focus of initial management was the restoration of cerebral blood flow through the administration of neuroprotective agents and antiplatelet therapy, aiming to optimize cerebral perfusion and achieve clinical recovery of aphasia. This case analysis highlights the importance of prompt and appropriate management in hypertensive emergencies, emphasizing the detection of clinical signs of TIA or complete stroke, even in localized neurological deficits without additional accompanying symptoms.

Abstract

Background: According to the World Health Organization (WHO), chronic energy deficit is associated with 40% of maternal mortality in underdeveloped nations. An assessment of pregnant women's nutritional health may be done through several methods, including the Upper Arm Circumference (UAC) test. If the UAC measurement comes out less than 23.5 cm, it indicates that pregnant women's nutritional status is inadequate. Pregnant women's nutritional health might be positively or negatively impacted by their mothers' knowledge of meal selection. Babies born to mothers who do not get enough nourishment may be born with a host of issues. Objective: to learn how the nutritional status of pregnant women at the Johan Pahlawan Puskesmas is affected by factors including age, education, and knowledge level. Methods: This type of research is an analytic survey with a cross-sectional approach. Data collection techniques are quantitative research. This research was conducted on 48 samples and was carried out from June 26 to July 4, 2024, on pregnant women at the Johan Pahlawan Puskesmas. The measuring instruments used were questionnaires and UAC test. The data analysis method used Spearman's rank correlation coefficient for the statistical test. Research Results: there is a relationship between age, education, and knowledge level to the nutritional status of pregnant women at Puskesmas Johan Pahlawan with a P-value age 0.014 < α ( 0.05), education with P-value 0.047 < α (0,05), knowledge with P-value 0.003 < α ( 0.05), respectively. Conclusion: there is a relationship between knowledge, age, education, and nutritional status in pregnant women in the work area of the Johan Pahlawan Puskesmas, West Aceh. Suggestion: It is expected that health services will provide health promotion activities for pregnant women in the work area of the Puskesmas and need to monitor the nutritional status of pregnant women every month, and it is necessary to identify other variables related to the nutritional status of pregnant women.

Abstract

Commercial noodles are generally made from wheat flour and eggs without adding fish, fruit, and vegetable extracts. Adding fish and red beet extract can increase the nutritional content and antioxidants in the resulting noodle product. Adding Sodium Tripolyphosphate (STPP) can improve the desired noodle texture, namely chewy and elastic. This study aims to identify the effect of the concentration of STPP addition on the physical characteristics and sensory acceptability of Lumi-lumi wet noodles fortified with red beet extract. The study used a single factorial Completely Randomized Design (RAL) with four different STTP concentration treatments (F1: 0%, F2: 0.50%, F3: 0.75%, F4: 1% STPP) with a ratio of wheat flour, Lumi-lumi fish, and red beet extract, namely 80%: 20%: 8%. The observations were physical quality tests, including rehydration, cooking time, cooking loss, and elasticity of noodle. 60 untrained panelists carried out sensory tests. There was a significant effect (P <0.05) of the addition of STPP concentration on the physical quality of noodles. Treatment F1 showed the best rehydration noodle and cooking time, 80.93% and 2.98 minutes, respectively. The best cooking loss and elasticity were obtained in F4, which were 19.93% and 27.08 mm. Sensory results significantly affected color and taste parameters (P<0.05). Aroma and texture had no significant effect (P>0.05). The highest color acceptance was obtained in F3, followed by F2, respectively 3.97 (like) and 3.80 (like). The best taste parameter was received in F2, 3.83 (like). The highest aroma was obtained in F3, followed by F2, respectively, 3.80 (like) and 3.75 (like). The highest texture assessment was in F4, followed by F3 and F2, respectively, namely, 3.55 (slightly chewy), 3.53 (slightly chewy), and 3.40 (slightly chewy). Based on the research results, to produce Lumi-lumi wet noodles with the desired physical and sensory acceptability, an STPP concentration of 0.50% is recommended.

Abstract

A 6-month-old baby requires complementary feeding (MP-ASI) to meet nutritional needs and support growth and development. The mother's knowledge of proper MP-ASI provision influences the baby's nutritional status, including types, composition, and feeding methods. The nutrition received will directly affect the baby's growth. This study aims to analyze the relationship between maternal knowledge of MP-ASI and the nutritional status of babies aged 6-24 months in the working area of Padang Panyang Health Center, Kuala Pesisir District, Nagan Raya Regency. This quantitative study uses an analytical observational method with a cross-sectional design and Purposive Sampling technique. Ninety-three mothers with children aged 6-24 months were selected as samples. Data was collected using a questionnaire to assess maternal knowledge and anthropometric measurements to evaluate nutritional status. The data were analyzed using the Chi-Square test, showing a significant result (p=0.006, p<0.05). The results showed that 55.9% of mothers had sufficient knowledge, 34.4% had poor knowledge, and 9.7% had good knowledge. Additionally, 80.6% of babies had normal nutritional status, while 19.4% had abnormal nutritional status, indicating a strong relationship between maternal knowledge and the nutritional status of babies. Mothers with a better understanding of MP-ASI provision tend to have children with better nutritional status.

Abstract

Introduction: Pulmonary metastases arising from extremity fibrosarcoma are among the most common sites of distant metastasis. The median survival after the occurrence of metastasis is approximately one year. Patients treated with nonoperative therapy have a median survival of 11 months. Case Illustrations: A 70-year-old female, never smoker, admitted to the hospital with a chief complaint of shortness of breath. The patient had a history of fibrosarcoma on the left foot followed by surgery a year ago. Chest X-ray showed central lung mass with massive right pleural effusion. Post-contrast chest imaging revealed a heterogeneous, enhancing, lobulated mass in both lungs. TTNA and core biopsy were performed under USG guidance on the lung mass. The cytologic findings revealed a metastatic neoplasm-suggestive sarcoma. The concomitant core biopsy showed a histologic pattern of spindle cells arranged in short intersecting fascicles suggestive of metastases of fibrosarcoma. The patient showed rapid deterioration resulting in death on the 20th day of hospitalization. Discussion: The incidence of fibrosarcoma in the lower limb accounts for 40% of all cases of soft tissue tumors. Patient with extremity sarcomas are more likely to have distant metastases as their initial site of recurrence. From the histologic pattern, those patients with spindle cell sarcoma were the most likely to have pulmonary metastases. Data on pulmonary metastases arising from extremity fibrosarcoma is poorly available making it difficult to treat metastatic sarcoma. Conclusion: Pulmonary metastases from fibrosarcoma are a challenging issue, with a median survival of less than one year post-metastasis. Pulmonary metastasectomy can extend survival to 33 months, but a multidisciplinary approach, including tailored chemotherapy and radiotherapy, is crucial due to the diverse tumor behaviors. This case highlights the rapid progression and poor prognosis of metastatic fibrosarcoma, underscoring the need for comprehensive treatment strategies.
