Abstract

Introduction: Medical interventions for pulmonary heart resuscitation in patients with DNR raise a conflict situation among nurses, where faced with two equally unsatisfactory decisions that are contrary to ethical principles. The practical approach that can be taken is to build patient and family confidence, define their goals, and apply ethical principles of autonomy: beneficence, nonmaleficence, and justice. This research aims to find out the ethical dilemma of DNR in nursing practice. Methods: A literature review was conducted using three journal databases: ScienceDirect, PubMed, and Google Scholar. Kata kunci dalam tinjauan pustaka ini disesuaikan dengan Medical Subject Heading (MeSH) yang terdiri dari “ethic” AND “principle based” AND “do not resuscitate” AND “nursing ethic”. Several inclusion criteria were set to obtain eligible articles: published between 2014 and 2023, and written in English or Bahasa. Fifteen eligible articles were finally included in the analysis. Results: Four subthemes were found: factors that influence DNR decisions, knowledge and understanding of health professionals related to DNR, ethical dilemmas, and care of DNR patients. Conclusion: The DNR decision requires ethical competence based on DNR information and understanding. The DNR decisions are based on the principles of autonomy, beneficence, nonmaleficence, and justice. The role of a nurse tends to be psychosocial intervention.

Abstract

Introduction: Tornado is the second most common disaster in Lamongan after floods. Elementary school students are one of the groups vulnerable to this kind of disaster. The purpose of the study was to determine the effect of Scrapbook on the knowledge and preparedness of elementary school students in dealing with tornado. Method: This research used quasi-experimental (nonequivalent control group) design with a total sample of 131 students divided into 2 groups, the intervention group (47) and the control group (84), were selected using total sampling. This study was conducted at Muhammadiyah Plus Maduran Elementary School and Bedahan Babat Public Elementary School in March 2024. The intervention group was given education using scrapbook, while the control group was not given any education. The data were collected through knowledge and preparedness questionnaires, then they were analyzed using Wilcoxon test, Paired t-test, and Mann-Whitney (α≤0.05). Result. The results indicated that there was a difference in knowledge and preparedness of students before and after treatment in the intervention group (p=0.001; p=0.001) while there was a difference in knowledge and preparedness of students before and after treatment in the control group (p=0.317; p=0.083). There was a difference in students’ post-test knowledge and preparedness in dealing with tornado disasters between the intervention group and the control group (p=0.001; p=0.001). Conclusion. Scrapbook is proven to significantly increase the knowledge and preparedness of elementary school students in dealing with tornado. The future researchers can develop Scrapbook into digital media so that it is accessible to children through gadgets.

Abstract

Background: Gastritis is one of the health problems in the community with a high prevalence. One of the symptomatic signs of gastritis is epigastric pain, which can interfere with the patient's activities. Pain complaints are still felt in some patients despite pharmacological therapy. Purpose: To determine the effectiveness of lavender aromatherapy to reduce the intensity of epigastric pain in gastritis patients in the emergency room of Klungkung Hospital. Method: This case study investigates two patients with gastric pain who were treated in the emergency room using convenience sampling, a non-probability sampling technique. Findings: Ms. A before the intervention was given a pain scale of 4 out of 1-10, and after the intervention, she was given a pain scale of 2 out of 1-10. Ms. B before the intervention was given a pain scale of 5 out of 1-10, and after the intervention, she was given a pain scale of 3 out of 1-10. Conclusion: Lavender aromatherapy is effective in reducing epigastric pain in gastritis patients. It is hoped that lavender aromatherapy can be applied as one of the non-pharmacological therapies in an effort to reduce the intensity of epigastric pain in gastritis patients.

Background: Hyperthermia or high fever is a condition characterized by an increase in body temperature beyond normal limits. Fever will occur if a person's health condition is disturbed. Body temperature is said to be within normal limits if it is in the range of 36,5-37,50C. Research Objective: To determine the results of giving aloe vera compresses to reduce children's body temperature in hyperthermia with Dengue haemorrhagic fever in the Emergency Room of Klungkung Hospital. Methods: A Case study method using in this research. Two sample are chosen using Purposive sampling technique. Providing nursing interventions in the form of aloe vera compresses to reduce children's body temperature with hyperthermia in dengue haemorrhagic fever. Results: The first patient had a body temperature of 38.9 C before intervention and a body temperature of 37,5oC after intervention. The second patient had a body temperature of 38oC before intervention and a body temperature of 37,9oC after intervention. Conclusions: After being given an intervention in the form of aloe vera compresses proved effective in reducing fever in children.

Purpose: To determine the effectiveness of pericardium 6 acupressure therapy on reducing nausea in gastritis patients at Klungkung Regency Hospital as a prevention of hypovolemic risk. Methods: This final scientific work uses a case study design. The subjects in this final scientific work used two patients in nursing problems with one medical diagnosis of gastritis with the nursing problem of nausea. Results: The results showed that two patients who experienced complaints of nausea after being given acupressure intervention pericardium 6 in gastritis patients said nausea complaints were reduced. Conclusion: From interventions and research related to the effectiveness of giving acupressure therapy pericardium 6, it is proven effective in reducing nausea experienced by patients with gastritis.

Abstract

Introduction: Bengawan Jero is an area encompassing six districts: Turi, Karanggeneng, Kalitengah, Karangbinangun, Glagah, and Deket, which are highly susceptible to flood disasters. Disaster preparedness is crucial for ensuring community safety in facing such events. The aim of this study is to assess the preparedness of the community in the Bengawan Jero area in dealing with flood and tornado disasters. Method: This research employed a quantitative descriptive design and conducted with 333 participants selected using cluster random sampling. This study was conducted in the Bengawan Jero area, specifically in the Turi and Kalitengah districts of Lamongan Regency, in February 2024. A questionnaire on flood and tornado disaster preparedness was utilized to obtained the data, which previously validated with Cronbach’s alpha values of 0.932 and 0.954 respectively, and reliability coefficients of r = 0.444. Data analysis was performed descriptively. Result: The research findings indicate that 84% of the community demonstrated a readiness level categorized as “very prepared” in facing flood disasters, while 61% showed a readiness level categorized as “prepared” in dealing with tornado disasters. Conclusion: The level of community preparedness in facing hydrometeorological disasters is higher in the Kalitengah district compared to the Turi district. It is necessary to identify the factors contributing to the lower level of preparedness among the community in the Turi district.
