Background: Quality is a total picture of the nature of a product or service which is able to give satisfaction. The quality of hospital services can be measured by using the standard operating procedures (SOPs) which are the instructions made together to carry out routine activities based on professional standards. The purpose of this research is to analyze the efforts to improve the quality of medical services in RSIA (Child and Maternity Hospital) ‘Aisyiyah Klaten related to the doctor’s knowledge of the philosophy of making SOPs for Medical Services. Methods: The study design was a case study with a qualitative and quantitative approach. It was conducted by in-depth interviews with directors, the chairman of the medical committee, the secretary of the medical committee and the special staff of directors relating to the medical committee and making of SOPs for Medical Services. The questionnaire is addressed to all the doctors associated with the knowledge, attitudes and behavior in making the SOPs for Medical Services. Analysis of the data is presented descriptively from the perspective of input, process and output. Results and discussion: The results obtained on the input which is the vision to become the recommended hospital with the mission of providing services in accordance with medical and administrative standards of professional and accredited. There are meeting facilities, budget, commitments director on the decree on the medical committee and the accreditation team. From the process side, the meeting attendance of accreditation team doctors is 62.90%, non- accreditation team specialists by 50%, non-accreditation team general practitioners by 18.18%. The making of the meeting minutes reaches 100%. On the SOPs of Medical Services, the value of all the doctor's knowledge is 90.68%, the value of all the doctor's attitude is 84.78% and the value of the behavior of all doctors is 63.37%. From the output side, the number of SOPs of Medical Services which have been made are 31 types (88.24%). The number of SPOs which was passed are 50 titles (43.10%). There are constraints on the time and the ability of doctors. Conclusion: RSIA ‘Aisyiyah Klaten has undertaken efforts to improve medical services quality in making the SOPs for Medical Services through a systems approach well and the doctors already understand the philosophy of making good SOPs for Medical Services.

Hospitals must meet the facilities and infrastructure technical requirements of the hospital that support health care services completely. The evaluation of Hospital buildings standard's fulfillment can be done by Post Occupancy Evaluation. The study used mix method. Population consisted of all internal user in Obstaetric and Gynaecology Unit. There are 23 samples that determined by shovin formula. The result of study showed that location of Obstetrics and Gynecology Unit is not easy to be reached from Emergency Room, Intensive Care Unit, and Operating Room. Location, room availability, width, lighting, humidity, and temperature in Obstetrics and Gynecology Unit are not fulfill the standard yet. Technical and functional aspects are appropriate with Depkes 2007, process aspect is almost appropriate with Depkes 2007.

Indonesian began to operate health insurance program in 2014 with payment scheme by INA-CBG’s (Indonesian Case-Based Group). That makes hospital to have appropriate tariff calculation. Breast fibroadenoma (FAM)must have an appropriate tariff calculation because it will be increasing. One of a calculating method that can be used is Activity Based Costing (ABC) because this method can measure accurately cost out of any activity, improve the precision and accuracy in the details of charging fees. This study was descriptive quantitative. This study is limited to a patient with FAM cases that occurred in 2015 in patients using BPJS class III and without complications. The unit cost excision FAM at RS PKU Muhammadiyah Yogyakarta which is calculated by the method of activity-based costing is Rp 4,704,877. The difference with the hospital unit cost of Rp 218.923 hospitals are higher, while the difference with INA-CBG rates Rp Rp 679.471 smaller. The unit cost of service excision FAM still under INA-CBG’s rates, calculations by the ABC method is higer than the unit cost PKU Muhammadiyah Hospital. The management should be re-evaluated the tarif of this service.

Managers Hospital admit that Strategy planning is important forces management, but this Strategy planning still become problem because some obstacles that faced to manage it well and exact. Basic obstacle relate to human resource obstacle that very determine quality and axact strategy planning.There are some obstacles that make strategy planning doesn’t yet optimal as important forces management that very needed on face change environment that change quickly. Key factor success strategy planning is formation systematic strategy, implementation strategy that effective, and restrain even follow up from Implementation (Brenes, et al.2008). According to view of resource, the resource based view of the firm(RBV), Resource that had Hospital can become source that enable to approximating and implementation that can increase efficiency and effectiveness of Hospital (Barney, 1991). In Rizki Amalia Medika Hospital there are variations in the treatment of patients with dengue haemorrhagic fever, so that it is requires better governance. This study is purposed to develop a clinical pathway of dengue haemorrhagic fever and to formulate clinical governance which are required to develop in Rizki Amalia Medika Hospital. This study is a qualitative research in Rizki Amalia Medika Hospital from September to November 2014. The data were obtained from medical records, standards of medical services and hospital reports, and interviews. The data analysis was performed with qualitative data analysis techniques. This research resulted in clinical pathways dengue fever based on the agreement of medical and paramedical staff in Rizki Amalia Medika Hospital, which is a template containing the information : a) the name of the patient, No. RM, date of birth, and special notes, b) information services every day for 6 days, c) variations in service with a date, reason and signature, d) the date of entry and exit, and e) the main diagnosis and the concomitant, accompanied ICD 10 code. Clinical pathway of dengue fever was used as the basis of examination and medical care of patients. This experiment also produced clinical governance which should be built in Rizki Amalia Medika Hospital among others: clinical audit activity, evidence-based clinical practice, and continuing education for medical staff. Clinical governance is necessary to achieve an appropriate health care standards.

RSKIA PKU Muhammadiyah Kotagede Hospital Type C Special, that in order to maintain the quality and safety of hospital patients, especially the handling of High Alert Medication and prepare for the accreditation of the 2012 version, then RSKIA PKU Muh Kotagede working to improve implementation one of that the Goals Patient Safety with training. Occurrence data and research in pharmaceutical care, as well as general data risk high incidence of errors in the pharmaceutical field especially high alert medications make it a priority to take precedence understanding for officers and implemented. This type of research is descriptive research using a type of mixture (mixed methodology), with a quasi-experimental. The study population were hospital health workers, the sampling technique by purposive sampling. Test analysis using descriptive statistics and qualitatively by comparing with the results of observation, interviews and document research. Test Results Kolmogorov Smirnov normality assumption both knowledge and attitude variables to pre-test and post test both normal distribution. Results Paired Samples Test A (Knowledge) note that t is -6.672 with a significance value of 0.000. Hence the significance of 0.000 <0.05, then Ho is rejected, it means that there is a difference between the average variable knowledge of pre-test and post test. Paired Samples Test / T-test B (attitude), it is known that t is -2.932 with a significance value of 0,010. Hence the significance of 0.010 <0.05, then Ho is rejected, it means that there is a difference between the average variable attitude of pre-test and post test. From interviews with respondents felt more knowledgeable and increase in knowledge after the training given. From the aspect of attitude because respondents feel more knowledgeable after being given training, it leads to a positive attitude in handling the implementation of High Alert Medication. There were changes in knowledge to healthcare workers at RSKIA PKU Muhammadiyah Kotagede after training on High Alert Medication. There is a change in attitude to healthcare workers at RSKIA PKU Muhammadiyah Kotagede after training on High Alert Medication.

The background of the study was the implementation of standardization of health service quality stated in ISO 9001: 2008 in Kalasan Health Center effectively applied on July 9th, 2009. It was hoped that the implementation of standardization of basic health service quality increase the quality of health service and human resources. The data collection was conducted by interview and questionnaire, and the collected data were in the form of primary and secondary data. The number of the research sample was 40 respondents. The data analysis method used was descriptive. The result of the research includes: (1) Based on the result of descriptive analysis in the implementation of ISO 9001: 2008 certification, the implementation of employee duty needs job knowledge and skill so that they can work as their responsibility. (2) The sub-variable of Employee Work Satisfaction was included in employee involvement in all lines so that the goal of the health center was achieved. (3) The sub-variable of employee work environment means having cooperation with other employees in all lines. (4) The sub-variable of Kalasan Health Center was obtained from work practice experience, in all service departments that focus on the increase of the service quality in Kalasan Health Center appropriately. The conclusions of the research result and analysis were (1) all employees increase the service quality for all patients in Kalasan Health Center. The curative aspect was emphasized on outpatients, and the stabilization of emergency and referral patients. (2) Acknowledging qualified work achievements by giving the opportunity to have position promotion as procedure assigned. Work satisfaction was from the existing of formation facility or space condition that gave comfort feeling. (3) Work team and colleague feeling needed to be maintained so that priority gap got decreased. The coordination between the employer and the employee is useful to finish the work. (4) The institution gave guarantee on health service quality which was as the hope of the consumer (patient) and strengthened the competitiveness. Therefore, the implementation of quality management system based on the concept of ISO 9001:2008 as the effort to strengthen the competitiveness was urgent.

as a global problem, patient morbidity and mortality in hospital caused by Healthcare Acquired Infections (HAIs) can be prevented by implementation of Hand Hygiene (HH). This research aims to know the influence of Health Belief Model variables to HH compliance. there was a quantitative study by survey approach and cross-sectional design. The population consisted all of nurses in high risk HAIs ward (ICU and surgical ward, 30 respondents). HCW’s perceptions were measured through self-administered questioner. The compliance of HCWs was measured by observation form of HH. compliance rate of HH based on moment was 46,29 %. Compliance of ICU nurses where higer than surgery ward nurses. Only workplace variable that had significant influence HH compliance (p=0,000). Perception variables can not used as predictor to HH compliance. HH compliance should be increase. Workplace factor had significant influence to HH compliance.

The background of this research is Hidayatullah Islamic Hospital (RSI) Yogyakarta and is done through leadership interferential, employee’s loyalty gives influence toward employee’s performance. (1) The phenomenon factor of harmonious relationship between leader and subordinate. (2) Factor of employee’s performance which has not yet indicating employment conduciveness. (3) Factor of employee’s loyalty toward the lack of leader’s concern on employee’s performance and opportunity to receive compensation. (4) The phenomenon of employee’s performance; the employee’s incompetence in comprehending leader’s instruction and the individual employee’s minimum responsibility which hinder the work performance. Data were compiled through interview and questionnaire, and were classified into primary and secondary data. The sample of the research consisted of 40 respondents. Method used in analyzing the data was linearity test, significance test of multiple linear regression coefficient, t test, F test, R2. The results of the research include: (1) The leader of Hidayatullah Islamic Hospital Yogyakarta doesn’t always impose sanction to its employee but instead puts trust on the head of unit to manage employee, conducts control/supervisory toward subordinate’s performance periodically in every line of work. (2) Employee’s loyalty is correlated with employee’s job and total self-image, as well as the purpose similarity between the individual and the hospital management. (3) The compatibility among willingness, achievement, and result gained is believed to create an individual with high work ethic, professionalism and commitment for its institution and also for its work achievement. The conclusions of the research are; (1) There is a positive and significant influence of leadership style toward the employee’s performance at Hidayatullah islamic hospital Yogyakarta. (2) There is a positive and significant influence of employee’s loyalty toward employee’s performance at Hidayatullah islamic hospital Yogyakarta. (3) The independent variable (leadership style and employee’s loyalty) gives positive and significant influence toward the employee’s performance at Hidayatullah Islamic hospital Yogyakarta.

Infection prevention and control programs areinclude to the important programs because the transmission of infection are particularly vulnerable occur in hospitals, especially from body fluid spills. Related to infectious body fluids, spill kit is used to handle it. The use of spill kits are made by the officer who first discovered the infectious body fluids. The study assessed the effectiveness of training with video media on ability of PKU Muhammadiyah Hospital Unit II officers to use spill kit. Qualitative research with action research that consisted of planning, action, observation, and reflection. The training was conducted by video media. The ability of officials assessed by a checklist based on Standard Operating Procedures (SOP) when pretest and posttest. Focus Group Discussion (FGD) also held after the training. The study was conducted during the months of November to December 2015. Data analysis of training value using paired T-Test as support data. Only 31.6% participants that can use spill kit properly before training and after training 100% participants can use spill kit properly that appropriate to SOP in the hospital. Additionally, many suggestion submitted by officers when FGD associated with the stages of use spill kit at PKU Muhammadiyah Hospital Unit II. There are differences in the ability of PKU Muhammadiyah Hospital officers Unit II in use spill kit before and after training with video media. The SOP that already exist should be revised. There should also be disseminated more adequately on the use of spill kit at PKU Muhammadiyah Hospital Unit II.
