Abstract

Medical record is a description of patient identification, patient examination, diagnosis, treatment, actions and other services, whether carried out by doctors and dentists or other health workers in accordance with their competence. In conducting statistical analysis of health care can be done by using quantitative analysis which explains information in the form of numbers so that the results will be easier to use in decision making. Data collection was carried out at the hospital by recording the data of inpatients who went home, so that data were obtained as many as 554 patients with a total of 179 beds. Data processing is done using a computer with Microsoft Excel application program. Furthermore, the data were analyzed and the results of statistical processing of health care were presented in the form of tables and graphs of Barber Johnson. Most diagnoses were Bronchopneumonia with code J18.0 as many as 24 patients. The main diagnosis grouping based on defecation in ICD-10 the most diagnosis was CHAPTER XI (Disease of Digestive system) of 15.88% as many as 88 patients. The most procedures were Lithotomy Kidney with procedure code 55.01 totaling 14 patients. The main diagnosis according to the most DTD with code DTD 215 (urolithiasis) as many as 30 patients. Based on the RL4a report, the most age group was the age group of 25-44 years with 165 patients, based on female sex as many as 102 patients and male as many as 63 patients, while based on the disposition of life 551 patients, and 3 patients died. After processing the data, BOR was obtained as much as 35.66% or 36%, this BOR value was not efficient. BTR as much as 3.09 times for the efficient number of BTR is per year, then BTR cannot be calculated. IT is 6.44 days or 6 days, the obtained IT value is not efficient and ALOS is 3.57 days or 4 days, the ALOS value obtained is inefficient.

Abstract

Hospital reporting is an organizational tool that aims to be able to produce information quickly, precisely and accurately to hospital management or those who need information. In the process of making the RL 4a report at the Mohammad Natsir Solok Regional Hospital, there were still obstacles. One of the obstacles in processing RL 4a reporting is that inpatient medical record files are not completely filled in and are still being worked on manually. Data collection was carried out at RSUD Mohammad Natsir Solok by recording data on inpatients who returned home from March 1 to March 31, 2018 with a total of 259 patients. The data was processed using a computer using the Microsoft Excel application program. The grouping of reports is based on RL 4a, where after processing the data, it is found that the most age group is the age group 45 to 64 years 54 women and 32 men, followed by the age group 24 to 44 years, where 38 women and 25 men -Male. For the sexes of patients the most were female as many as 158 and male 101 people, then for the disposition 249 were alive and 10 patients died.

Abstract

Medical records are files that contain records and documents including patient identity, examination results, treatment that has been given, as well as actions and other services that have been provided to patients. Data collection was carried out at the Regional General Hospital Dr. Rasidin Padang by recording data on inpatients who were discharged from the period March 01 - March 31, 2018, so that data were obtained as many as 251 patients with 103 beds. Then data processing was carried out using a computer with the Microsoft Excel application program. Furthermore, the data were analyzed and the results of health care statistical processing were presented in the form of tables and Barber Johnson charts. From the grouping of the most common diagnoses based on ICD-10 at the Regional General Hospital Dr. Rasidin Padang for the period March 01 - March 31, 2018, it was found that the most common code was code A91 as many as 21 patients with a diagnosis of Dengue Haemorrhagic Fever (DHF).Grouping the main diagnoses based on bowel movements in ICD-10 resulted in the highest frequency of diagnoses being BAB I (Certain infectious and paracitic diseases) and BAB XV (Pregnancy, chilbirth and pueperium) as many as 12.75% with a total of 32 patients each. The procedure grouping based on ICD-9 CM in RSUD dr. Rasidin Padang for the period March 01 - March 31, 2018 was SC with procedure code 74.99 as many as 14. Based on the DTD code and its definition, it is known that the most common diagnosis is DTD 032.1 (Dengue hemorrhagic fever) as many as 21 patients. In the RL4a Format table, it is known that the most age group as an inpatient at RSUD dr. Rasidin Padang for the period March 01 - March 31, 2018 is the age group 45 - 64 years as many as 70 patients with female gender as many as 37 patients and male as many as 33 patients, while based on the disposition died as many as 2 and live as many as 249. After data processing, it was obtained that the percentage of BOR at RSUD Dr. Rasidin Padang for the period March 01 - March 31, 2018 had not reached an efficient number because the value obtained was 26.65% or 27%, while the BOR efficiency rate was 60-85%. BTR is 2.44 or 2 times, because the calculation of the BTR number is per year, the BTR efficiency cannot be determined. TI is 9.33 or 9 days. This means that the TI value is not yet at an efficient point, because the TI efficiency value is 1 - 3 days.

Abstract

Bed Occupancy Rate (BOR) data can be used to determine the level of service, to know the quality of hospital.The purpose of this paper is to determine the correlation or drill value with number patient visits at Lubuk Basung Hospital for the January-Februay 2017 periode.The data processed is secondary data of inpatients at Lubuk Basung Hospital for the January-February 2017 period, data obtained for 300 patients with a total of 109 beds, then data processing is carried out using the Microsoft Exel application program and utilizing the Pivot Table feature.Statistical processing was analyzed and presented in the form of tables and graphs by Barber Johnson.The results of data processing obtained a BOR of 28.80%, not efficient because the BOR is 60-85% efficient.BTR is 33 times, not efficient because BTR is 40-50 times a year.TI is 8 days, inefficient because it is 1-3 days efficient and ALOS is 4 days, inefficient because it is 6-9 days efficient. Based on the RL4a report,the highest age group was the 45-64 year age group with 78 patients, the highest gender grouping was male 153 patients and female 147 patients.Based on the living disposition of 295 patients, 5 patients died.The distribution of patients by region was mostly in Agam with 259 patients. The highest diagnosis was Pneumonia, Unspecified with a code of J18.9 with 27 diagnoses.The main diagnosis grouping based on defecation in ICD-10, the highest diagnosis was Chapter I, namely Certain Infectious and Parasite Disease with 20.67% totaling 62 diagnoses. There is a positive correlation between the number of patient visits and the number of bed usage.

Abstract

Siti Rahmah Islamic Hospital, Padang is a type A private hospital located in Padang in preparation for implementing electronic medical records and also improving the quality of human resources and health services. This study aims to determine the preparation for implementing electronic medical records at RSI Siti Rahmah Padang. The research used was qualitative using interviews, the method of taking the subject of informants using purposive sampling with several informants 5 people and data collection using analysis techniques by interviewing informants. The results of the research that has been carried out to prepare for the implementation of electronic medical records at RSI Siti Rahmah Padang have been running from the start of the enactment of the PMK Law 24 of 2022 by increasing human resources through training on electronic medical records and providing facilities and infrastructure to support the successful implementation of medical records. electronic medical from the aspect of technology, and information technology (IT). From this research, it can be concluded that preparations for the implementation of electronic medical records have started with human resources increasing insight into medical record officers and medical staff in other healthcare facilities.

Abstract

The government's efforts in achieving the target of exclusive breastfeeding have been stated in Government Regulation no. 33 of 2012. In Padang city, Perwako Regulation Number 7 of 2015 on Provision of Space for Breastfeeding and or Expressing Breastmilk has been issued. This is a form of efforts to accelerate the achievement of the target coverage of exclusive breastfeeding. This study aims to analyze the implementation of the exclusive breastfeeding policy in achieving the target in the city of Padang. In this study, an analytical study will be carried out on the achievement of exclusive breastfeeding coverage and the implementation of PP No. 33/2012 and Perwako in Padang City. Achievements from the exclusive breastfeeding program coverage in the city of Padang, obtained an average of 79.40%. However, this figure cannot be generalized because there is still an equity gap that occurs in several working areas of Puskesmas in Padang City. In the last three years (2017- 2019) the achievements in several health centers have experienced significant fluctuations and decreases, including at Ulak Karang Puskesmas where in 2019 it was 54.5%. The same thing happened at Puskesmas Alai, and Puskesmas Lubuk Buaya experienced a sharp decline from 2017 to 2019 from 77.22% to 52%. Policy implementation has not been carried out properly. So it is necessary to carry out evaluation and follow-up related to the achievements over the last few years (2017-2019), increase cooperation and coordination between the government, health agencies, health workers, and community leaders, and affirmation in determining administrative and written sanctions to support efforts to accelerate the achievement of breastfeeding targets.

Abstract

The use of the ICD-9 CM in coding medical actions will make it easier for us to classify patient action codes and facilitate reporting and facilitate BPJS claims in the JKN era which are useful as supporting administration, management and health research. This research was conducted at the Pariaman Regional General Hospital located Jalan Prof. Yamin SH No. 5, Kp. Baru, Pariaman Tengah, Kota Pariaman, West Sumatra on April 1 to April 14 2019. This study aims to determine the highest medical treatment based on ICD-9 CM at Pariaman Regional Hospital in 2019. This study uses a retrospective descriptive method with a quantitative approach. The research design used in this study was a descriptive cross-sectional study. The population used by all patient medical record files from April 1 to April 14 2019 was 303 medical record files. From the results of this study, it is known that there are 4 highest medical actions, namely SC action of 3 patients with code 74.99, Cataract aspiration action of 3 patients with code 13.41, VE action of 2 patients with code 72.7 and Thyroidectomy action of 2 patients with code 06.39.

Abstract

Abortion is a sensitive issue that involves law, ethics, and human life. This research investigates the legal framework governing abortion, including changes in the 2023 Health Law that updated previous provisions. Abortion is a medical procedure often used to terminate unwanted pregnancies, but it is also considered a hazardous medical practice for pregnant women. The study also reveals that in Indonesia, maternal deaths are primarily caused by hemorrhage, hypertension during pregnancy, and infections in pregnant women, with abortion being one of the major contributors to maternal hemorrhage. Amid controversies surrounding abortion, several illegal clinics have been uncovered, performing thousands of unauthorized abortions in recent years. In terms of law, the research discusses relevant regulations in the Indonesian Penal Code (KUHP) and the Health Law. Legal matters in Indonesia are marked by debates and contradictions in perceptions and understandings of existing laws, leading to underground abortion practices carried out by both formal and informal healthcare providers, whether compliant with medical standards or not. Method: This research employs a systematic review method, utilizing data sources obtained from the internet, including research journals, academic papers, and news articles focused on the ethics and legal aspects of healthcare in the context of abortion. Results: The findings of this research indicate that abortion significantly impacts women's health, with unsafe illegal abortion practices posing life-threatening risks. Additionally, legal interpretations of abortion, particularly in cases involving rape victims, vary. Conclusion: This research underscores the need for clear and harmonious abortion-related policies in Indonesia, as well as a strong understanding of ethics and morals among medical professionals. Consequently, efforts to implement more comprehensive and supportive legislative changes must persist to effectively address the ethical and legal issues surrounding abortion.

Abstract

Diagnosis codes and medical procedures need to be given accurately. An accurate code will have an impact on the accuracy of the code grouping report and is related to financing. This research was conducted at Pariaman Hospital to determine the grouping of diagnosis codes and medical procedures based on ICD-10 and ICD-9-CM. Data taken from Pariaman Hospital for the period 01 April – 30 April 2019, namely the number of discharged patients was 649 patients for a total of 30 days and the number of beds was 163. The research method is direct observation of medical record documents. The results obtained were that the highest diagnosis and code based on ICD-10 was a diagnosis of pneumonia with code J18.9 in 32 patients. The highest distribution of patient diagnoses based on bowel movements was BAB XV (Pregnancy, Childbirth and Puerperium) at 21.27% with 140 patients. The most common procedure performed was SC (Sectio Caesarea) with procedure code 74.99 for 8 patients.

Abstract

According to PMK No. 24 of 2022, a medical record is a document containing data on patient identity, examination, treatment, actions, and other services that have been provided to the patient. Through this policy, health service facilities are required to run an electronic patient medical history recording system. The transition process is carried out until no later than 31 December 2023. This type of research uses quantitative methods. Quantitative research methods are used to determine the picture of hospital readiness. The method used to analyze hospital readiness is DOQ-IT with indicators of organizational culture readiness, governance and leadership readiness, human resource readiness, and infrastructure readiness. Based on the results of the research that has been carried out, information can be obtained that in the aspects of human resources, organizational work culture, leadership governance, and infrastructure as a whole can be seen from the answers of 18 ready respondents (46.06%) and very ready as many as 20 people (49.87%) then with the respondent's answer not ready as many as 1 person (2.22%) the answer is very unprepared as many as 1 person (0.35%) and for the answer hesitant as many as 1 person (1.44%), it is confirmed from the informant that the preparation for the implementation of electronic medical records is ready to be implemented.
