Abstract

Abstract  This study aims to analyze the effect of digital transformation on employee work efficiency, and to explain the mechanisms linking this variable within the hospital’s operational context. The study uses a mixed-method approach with a sequential explanatory design, beginning with a quantitative phase through an employee questionnaire survey, followed by a qualitative phase through in-depth interviews to clarify and enrich the quantitative findings. Conceptually, the model is built on the integration of the Technology Acceptance Model (TAM) and the Resource-Based View (RBV), which position digital transformation as a strategic resource that influences work efficiency at the individual level. Quantitative data were analyzed using multiple linear regression with classical assumption tests, while qualitative data were analyzed thematically. The results show that digital transformation has a positive and significant effect on work efficiency through improved workflows, use of information systems, and reduced time wastage, highlighting the need for continuous investment in the integration of technology-based work processes in hospitals.

Abstract

Pending BPJS Health claims are a recurring problem that affects hospital cash flow, cost efficiency, and service quality. At Rama Hadi Hospital Purwakarta, the average pending claim rate in 2025 reached 12.5% for outpatient services and 20% for inpatient services, with monthly fluctuations that did not indicate sustained improvement. This study aimed to analyze the profile of pending claims, strategies for improving the quality of medical documentation, the implementation of clinical pathways, the involvement of the casemix team, their impact on cost efficiency and service quality, and the formulation of effective management strategies. A descriptive qualitative approach was used, involving seven purposively selected participants consisting of an internal verifier, case manager, claims officer, coder, hospital director, general practitioner, and specialist physician. Data were collected through in-depth interviews, observation, and document review, and were analyzed through data reduction, data presentation, and conclusion drawing. The findings showed that pending claims were mainly caused by incomplete medical summaries, operative reports, and supporting examination results. Clinical pathways contributed to reducing variations in care and the risk of pending claims, although their implementation remained constrained by patient comorbidities and differences in perception among specialist physicians. The casemix team played an important role in ensuring coding accuracy based on ICD-10 and INA-CBG, but continued to face challenges related to documentation compliance and differences in interpretation with BPJS verifiers. High pending claim rates disrupted cash flow and potentially affected the continuity of service quality. This study concluded that reducing pending claims requires an integrated strategy involving stronger EMR-based documentation, improved compliance with clinical pathways, optimization of the casemix team, continuous monitoring, and the use of digital technology.

Abstract

Clinical communication failures and low patient involvement are the primary causes of patient safety incidents in hospitals. The asymmetrical communication in the inpatient wards of RSUD Dr. Moewardi potentially reduces patient understanding, particularly in chronic cases involving high-risk therapies. This study aims to explore the implementation of a structured communication system based on Shared Decision-Making (SDM) and to measure its impact on enhancing patient understanding (health literacy) and strengthening personalized-care-based patient safety. This study employs a Mixed Methods Sequential Exploratory design. The qualitative exploratory phase involves in-depth interviews with 11 informants (healthcare professionals, head nurses, and patients/families) to investigate the implementation of communication and its barriers in the field. These findings serve as the foundation for developing quantitative instruments, which will be distributed to 96 healthcare professionals using a proportionate stratified random sampling technique. The data will be analyzed using inferential statistics to evaluate the effects and mediation among variables. The results are expected to demonstrate that health literacy significantly mediates the impact of structured communication on patient safety. The implications of this study are projected to provide a strategic foundation for hospital management to reconstruct bedside handover and educational guidelines, making them more personal and participatory to prevent Adverse Events (AEs).
