Abstract

Abstract Introduction & Objectives : Thyroid Eye Disease (TED) is an autoimmune inflammatory disease of the orbit that affects orbital fat, extraocular muscles, periocular tissue and lacrimal glands. TED activity can be assessed with a Clinical Activity Score (CAS) to identify active or inactive TED conditions. Dry Eye Diseases (DED) is one of the manifestations in patients with TED with a prevalence that varies from 27%-96%. The Asia Dry Eye Society (ADES) 2017 proposed a new concept for DED diagnosis and therapy, namely Tear Film Oriented Diagnosis (TFOD) and Tear Film Oriented Diagnosis Therapy (TFOT) with Tear Film Breakup Pattern (TBUP) examination. TBUP examination help in determine specific management of DED. The aim of this study is to determine the differences in TBUP in TED patients based on active and inactive. Methods : This is an analytical observational study with a cross-sectional design. The subjects were TED patients who came to the Cicendo Eye Hospital and met the inclusion criteria. The study was conducted at the National Eye Center of Cicendo Eye Hospital in April - June 2022. The data were analyzed with the Chi-Square Test. Results : This study was conducted on 23 patients or 46 eyes. This study found a statistically significant differences in TBUP features in active and inactive TED patients (p=0.0001). The most common features of TBUP was found in active TEDs was dimples (55%). Conclusion : There are differences of TBUP features in active and inactive TED patient.

Abstract

Abstract Introduction & Objectives : Ocimum sanctum extract (OSE) has been shown to have antioxidant, anti-inflammatory, and antibacterial properties which are responsible for wound healing. This study aims to analyze the effect of 5%, 10%, and 15% OSE ointment on changes in collagen density and fibroblast count in Wistar rats with partial-thickness palpebral laceration. Methods : Laboratory experimental study with randomized post-test only control group design. The subjects were 30 rats divided into 5 groups: The placebo; Gentamicin 0.3%; OSE 5%; OSE 10% and OSE 15% group. The intervention was administered 2 times/day for 4 days. On the fifth day, the eyelid tissue was obtained to make histopathological preparations and to observe the collagen density and fibroblast count. Data were analyzed using Mann-Whitney, One-Way ANOVA, Kruskal Wallis, and Tamhane test. A p-value <0.05 was assigned to the statistically significant. Results : The placebo group revealed loose collagen tissue and OSE 15% and gentamicin group revealed dense collagen tissue. The low fibroblast count was in the gentamicin and OSE 15% group and the highest fibroblast count was in the placebo group with a mean of 19.77±3.29; 22,67±0,73 and 60.9±10.66 cells, respectively. There was a significant difference (p<0.001) in the collagen density and the fibroblast count based on the treatment groups. Posthoc tests revealed differences in the collagen density level and the fibroblast count between the placebo and the whole group. Conclusion : There was a significant difference between placebo and all groups of collagen density changes and fibroblast count. There was no difference between OSE 15% and gentamicin both microscopically and statistically.

Abstract

Abstract Introduction & Objectives : Centella asiatica (CA) is considered as major herbal plant that beneficial on wound healing process. Wound healing process will stimulate inflammatory cells to release growth factors and cytokines, e.g., interleukin-6 (IL-6). This aim of this study is to determine the effectiveness of CA on IL-6 levels as a biological marker of postoperative wound healing in the periorbital region. Methods : A double-blind, randomized controlled study on the first day of postoperative periorbital patients was conducted at the Mohammad Hoesin Hospital in Palembang from March to June 2022. There were 30 patients who met inclusion criteria. The effectiveness of CA and placebo were analyzed using Wilcoxon test, while comparisons of the effectiveness were analyzed using Mann-Whitney test (SPSS ver. 22.0). Results : No difference in age, sex, and IL-6 levels before treatment between the CA and placebo groups (p>.05). There was a decrease in IL-6 levels before and after CA administration (p=.005). There was an increase in IL-6 levels before and after giving placebo (p=.031). There was a difference in IL-6 levels between the CA and placebo groups (p=.004) after the intervention. Conclusion : There is a difference in IL-6 levels after administration of systemic centella asiatica on periorbital postoperative wound healing in the first week.

Abstract

Abstract Introduction & Objectives : Eyelid laceration are common in cases of trauma involving the eye. This study will discuss cases of eyelid lacerations, procedures, and the follow-up of patients who underwent surgery at Kariadi General Hospital Semarang. Methods : This study used an observational analytic design and was conducted retrospectively. All patients who underwent eyelid surgery April 2019 to January 2023. Age, gender, laceration location, mechanism of trauma, canalicular involvement, and surgery were recorded, subjective complaints and patient complications after surgery. Results : Of the 76 patients (94 eyelid lacerations) included in the study, men exhibited a greater vulnerability (56,75%) and mostly aged around 16-<50 years (43, 57%). In terms of the laceration location, it occurred in right lower (39,36%), right upper (35,10%), left lower (13,82%), and left upper (11,70%) eyelids. Blunt trauma (83,78%) was the main mechanism, with motor vehicle accidents leading (40,54%). Canalicular involvement was present in 35 cases (47,29%). All patients underwent surgery, with eyelid repair to all of the patient following by recanalization (39,18%). The most common complaint of patients after surgery in the first week of control was pain (47,29%) followed by a feeling of lump (13,51%). A total of 25 patients (33,78%) stated that they had no complaints. Conclusion : Eyelid lacerations mainly involve young adult people, right lower eyelid. Blunt trauma, particularly motor vehicle accidents, is the main cause. Most of the patients had no complaints in the1 week postoperative control, and they increased in subsequent controls.

Abstract

Abstract Introduction & Objectives : Incidence of orbital tumors is relatively low, but the delay in diagnosis, even if benign, can lead to vision loss and deformity. This study aims to review clinical and demographic information of orbital tumors in Sardjito General Hospital, Yogyakarta. Methods : We retrospectively reviewed clinical data of 138 patients who diagnosed with orbital tumors during July 2021 - March 2023. Data extracted included clinical characteristics, operative procedures, and histopathologic characteristics of orbital tumors. Results : There were 138 orbital tumors managed at hospital during a 2-year period. Among 138 patients, 82 patients (59%) were female and 56 patients (40%) were male. Twenty-one patients (15%) were in childhood age and 117 patients (84.8%) were in adulthood age. The common clinical manifestations were proptosis (83.3%) and decreased vision (10.4%). The main procedure used was anterior orbitotomy (54.34%). Eighty-eight patients (63.7%) have primary tumors and 51.4% of the orbital tumors were malignant. The most common orbital tumor in childhood age was a cyst (15%). On the other hand, in adult patients, the most common orbital tumor was meningioma (18%) and Non- Hodgkin Lymphoma (15.7%). There was an overall 8% complication rate after orbitotomy tumor removal, including decrease in vision (3.3%), ptosis (2.6%), and strabismus (1.3%). Conclusion : Orbital tumors are challenging group of tumors with proptosis become the most frequent clinical characteristic that found. The orbital tumors in childhood and adulthood differ significantly in their incidence and tumor type with the percentage of malignant tumors increasing with age.

Abstract

Abstract Introduction & Objectives : Graves ophthalmopathy (GO) is associated with visual impairment and appearance changes, affecting the quality of life. Intraorbital injection of triamcinolone acetonide (TCA) in GO resulted in clinical improvement, but its effect on the quality of life is unknown. The objective of this study was to compare the changes in quality of life after intraorbital injection of TCA between different groups based on changes in disease severity. Methods : Intraorbital injection of TCA was performed on 30 subjects with GO. Disease severity was assessed according to the European Group of Graves Ophthalmopathy (EUGOGO) criteria. Quality of life was measured using the Indonesian version of Graves Ophthalmopathy Quality of Life (GO-QOL) questionnaire. Changes in disease severity and GO-QOL scores were recorded before intraorbital injection of TCA and 1 month after. Subjects were divided into groups based on changes in disease severity: decreased, remained constant, and increased. Changes in quality of life between the groups were compared statistically. Results : Changes in GO-QOL appearance scores in patients with decreased disease severity were significantly higher (p= 0.016) than those in patients whose disease severity remained constant. Changes in GO-QOL visual function scores in patients with decreased disease severity were higher but not statistically significant (p> 0.05) compared to those in patients whose disease severity remained constant. Conclusion : Changes in the appearance aspect of quality of life after intraorbital injection of TCA in patients with decreased disease severity are better than that in patients whose disease severity remained constant, whereas changes in the visual function aspect are not significantly different.

Abstract

Abstract Introduction & Objectives : Chemical ocular injury is an emergency case and commonly occured among children. Alkaline chemical ocular injury is more destructive especially due to saponification. One of the rare chemical ocular injury is alkaline trauma by Potassium Permanganate (KMNO4) Methods : A case of a three-year-old boy patient brought by his mother in emergency department at Cicendo Eye Hospital with severe pain, redness, and swelling of the left eye. Ten hours earlier, the patient was playing and accidentally exposed to KMNO4. Visual acuity of the right eye was 6/6 and the left eye was difficult to examine. Ophthalmological examination revealed superior and inferior palpebra are swelling, KMNO4 adhered tightly on conjunctival tarsal superior, inferior, and conjunctival bulbi inferior until fornix and temporal area. Cornea was hazed and iris shadow test positive so he was diagnosed with Chemical Trauma Ropper Hall grade II OS. Treatment begins with irrigation 2 litres of normal saline until pH 7 along with debridement of KMNO4 from conjunctival tarsal superior inferior and bulbi. The patient underwent installation of transplanted amniotic membrane and debridement under anasthesia Results : Management in the initial phase is irrigation. The definitive therapy requires surgery to speed up the healing process. Complications that often occur are corneal limbal cell deficiency, dry eye, symblepharon formation, secondary glaucoma, and complicated cataracts Conclusion : Alkali chemical injury is hazardous and can lead to severe vision threatening complications because of saponification. Prompt and aggressive management is required in such cases to give them the best chance of a more favourable outcome

Abstract

Abstract Introduction & Objectives : Intravenous corticosteroid has been used in management of Thyroid Eye Diseases (TED). Many previous studies have shown the therapeutic benefits of corticosteroid therapy. Recently, the efficacy of high-dose intravenous corticosteroid pulse therapy has been reported. Recent studies have highlighted the potential role of interleukin-6 (IL-6) in the pathogenesis of TED. However, the correlation between IL-6 levels and the effects of IVMP on rectus muscles in patients with TED remains unclear. This study aims to investigate the correlation between IL-6 levels and the effects of IVMP on rectus muscles in patients with TED Methods : Retrospective analysis were used in the data collection of patients with active Thyroid Eye Disease who undergone high-dose intravenous methylprednisolone pulse therapy (500mg per week for 6 successive weeks) at Sardjito General Hospital between January 2022 and December 2022. Data on the Level of IL-6 were obtained at the start of treatment, and quantitative computed tomography (CT) of the orbit and were performed before and after treatments. Demographics data were presented in tables and graphs. Results : This study included 62 eyes of 31 patients (61,3% female) with mean age of 37.8+13.47 years old. There was significant inverse correlation in levels of IL-6 and ?SR (P=0.037) which implies a reduced effect of IVMP on SR. However, other statistical analyses on different rectus muscle groups bare no statistical significance. Conclusion : The use of IL-6 as a single predictor of responsiveness of the treatment of high dose intravenous methylprednisolone is not endorsed by the result of this study. However, further studies with better established level of IL-6 both pre and post treatment, and more inclusive parameters are needed to better provide better understanding on the role of IL-6.

Abstract

Abstract Introduction & Objectives : Grade II palpebral burns are burns that reach the epidermis and part of the dermis of palpebrae and can have impaired wound healing. TGF β1 has the broadest spectrum of action, affecting all cell types involved in all stages of wound healing. Decreased expression of TGF β1 is associated with impaired wound healing. Aloe Vera has long been known in Indonesia as a healing plant. Aloe Vera extract contains a variety of bioactive compounds that play a role in stimulating wound healing. The administration of Aloe Vera extract on thermal burns aims to reduce inflammation, increase collagen content, and increase the rate of re-epithelization. The administration of Aloe Vera gel extract allows faster healing of thermal burns and rebuilds the vascularity of the wound tissue. Methods : Objective: To determine the expression of Transforming Growth Factor Beta 1 (TGF β1) in a rat model of thermal burn injury grade II of the eyelids treated with topical Aloe Vera gel extract with the concentrations of 20%, 40%, Balanced Salt Sodium (BSS) and a control group that was not given burn intervention. Results : Methodology: an experimental study with the post-test-only approach with a control group design. 24 Rats have induced grade II eyelid thermal burns, divided into four groups: Normal (without treatment), negative control (BSS), given Aloe vera 20%, 40%. Then TGF β1expression was quantified Conclusion : The highest rise in TGF β1expression was found in the group with 40% Aloe vera gel, followed by 20% Aloe vera gel compared to the negative control (BSS Balanced Salt)

Abstract

Abstract Introduction & Objectives : This study aims to compare the effectiveness of Levofloxacin ED 0.5% with Povidone iodine 2.5% in reducing normal bacterial flora of the conjunctiva in preoperative procedures. Methods : Clinical trials will be conducted in the inpatient ward on patients who will undergo intravitreal surgery at Mohammad Hoesin Hospital Palembang from May 2022-July 2022. Subjects were divided into two groups, which is Levofloxacin ED 0.5% and Povidone iodine 2.5% group. Before and after the fornices preparation, the number of bacterial colonies that grow will be counted and evaluated using bivariate analysis. Results : There were 32 samples that fulfilled the inclusion criteria. A significant decrease of conjunctival flora was observed in both groups (p value = 0.000). Comparison of the two groups’ decrease in conjunctival flora was not significant. There was a reduction of up to 100% in 11 samples (68.75%) of both intervention groups. Conclusion : The use of Levofloxacin 0.5% and Povidone iodine 2.5% was equally good in reducing the number of bacterial colonies in the conjunctiva.
