Abstract

Ischemic stroke become the most common cause of mortality and disability for elderly in Indonesia. Territorial ischemic stroke are often associated with the presence of blood clots that clog blood vessels in the brain. There were somehow association in different studies between increased levels of d-dimer as a fibrinolysis factor and the severity of territorial ischemic stroke which evaluated by using National Institutes of Health Stroke Scale (NIHSS) scores. This study aimed to analyze the association of d-dimer profile to the severity of territorial ischemic stroke. A Cross sectional study was conducted by taking secondary data retrospectively through medical records at the Medical Record and Radiolgy Installation at Banten Regional Public Hospital. D-dimer profile was categorized with a cutoff point 1000 ng/mL and NIHSS score was categorized with a cutoff point 10. The statistical analysis test used in this study was Fischer Exact Test. Forty-One of territorial ischemic stroke patients data were included in this study. Territorial ischemic stroke patients were mostly in the age group of 51—60 years with 16 patients (39%), males with 22 patients (53,7%), education in elementary school with 14 patients (34,1%), and territorial ischemic at left brain hemisphere precisely on frontal lobe with 14 patients (34,1%), temporal lobe with 15 patients (36,6%), and parietal lobe with 15 patients (36,6%). There was statistically significant association (p = 0,016) between the d-dimer profile and severity of territorial ischemic stroke (OR 9,6; 95% CI (1,5—60,6)). Early abnormal d-dimer levels on admission cause worsening of the condition in patients with territorial ischemic stroke based on NIHSS scores.

Abstract

Tuberculosis is a disease caused by infection with the bacteria Mycobacterium tuberculosis which can attack the lungs and other organs. Manifestations of TB are characterized by shortness of breath, coughing, increased sputum production so that TB sufferers can experience problems with ineffective breathing patterns due to airway obstruction. This study aims to carry out and analyze nursing care for TB patients with ineffective breathing pattern problems. Case study research method. Data collection techniques use interviews, observation, physical examination and documentation studies. Data collection instruments used observation sheets, interview methods, and nursing care formats. Based on the results of the assessment, complaints of shortness of breath accompanied by coughing were found. The nursing diagnosis determined was ineffective breathing pattern. Nursing intervention with airway management is implemented for 3 x 24 hours. The results of the evaluation on the 3rd day of treatment showed that the breathing pattern had improved, as indicated by no complaints of shortness of breath, no nostril breathing, no use of accessory muscles for breathing, normal breathing pattern with improved respiratory frequency, SpO2 98% and regular breathing rhythm. The conclusion is that the problem of ineffective breathing pattern nursing can be improved with airway management interventions. Airway management interventions can be carried out to overcome problems with ineffective breathing patterns

Abstract

Diabetes Mellitus (DM) is a chronic disease that cannot be cured until now and requires sufferers to undergo long-term treatment. Prolonged physical problems experienced by people with type 2 diabetes mellitus and loss of self-control due to the absence of significant changes to heal make individuals feel helpless. Powerlessness if not overcome will lead to a psychosocial crisis that affects more severe psychiatric problems. This study aims to carry out nursing care for diabetes mellitus patients with helplessness at the South Ponorogo Health Center. This research is descriptive in the form of a case study. Powerlessness can be measured using a powerlessness questionnaire. The subject used was one respondent at the age of 54 years, suffering from type 2 diabetes mellitus since 6 years ago, moderate helplessness score, and cooperative. The results of this study showed that the main intervention was positive affirmation and autogenic relaxation therapy was performed on the first day, a score of 8 (moderate powerlessness) was obtained, while on the fourth day a score (low powerlessness) was obtained. Positive affirmation intervention and autogenic relaxation therapy is a combination that can help patients with diabetes mellitus reduce stress and improve the patient's mental well-being. The synergy of the two implementations that are carried out at each meeting, negative thoughts are changed through positive affirmations and autogenic relaxation therapy is effective in helping patients overcome the anxiety experienced by patients.

Abstract

Metacognitive awareness is a high-level thinking that involves active control over the cognitive processes involved in learning, consisting of two components, namely cognitive knowledge and cognitive regulation. In this study the aim was to assess the relationship between the metacognitive awareness component and student learning outcomes. This study involved 165 medical student respondents at Sultan Ageng Tirtayasa University. The results of the study found that the components of cognitive knowledge, namely declarative (p = 0.410), procedural (p = 0.187), and conditional (p = 0.927) did not significantly influence the learning outcomes of medical students (p > 0.05). In the components of cognition regulation, namely planning (p = 0.221), information management strategies (p = 0.514), understanding monitoring (p= 0.996), and evaluation (p = 0.873) did not significantly influence student learning outcomes. While the correction strategy has a significant relationship with learning outcomes (p = 0.01). In the analysis of the total metacognitive awareness score on learning outcomes, it was found that there was no significant relationship (p = 0.510). This means that there is no significant relationship between metacognitive awareness and learning outcomes of medical students at Sultan Ageng Tirtayasa University, both in the components of cognitive knowledge and cognitive regulation.

Abstract

Lack of learning motivation in discussion group members will cause students to contribute only little and less actively during discussion. Given the importance of group discussion learning method (PBL) in achieving medical learning CPL, the author is interested in conducting research into the relationship between emotional intelligence and the effectiveness of group discussions online in FKIK Untirta medical students. Research design method using cross sectional design, quantitative observational research. The researchers used a stratified random sampling technique on a medical student. The reseach uses two types of questionnaires; the adoption questionnaire of the Emotional Competence Inventory of Ini Kadek’s research, and the Tutorial Group Effectiveness Instrument (TGEI). Based on the results of the study, the majority of emotional intelligence were in the middle category with 50 respondents (53,2%), in the high category 44 respondents (46,8%), and no respondents in the low emotional intelligence category. PBL discussions conducted by medical students of FKIK Untirta have been conducted effectively (98,9%). Bivariate analysis using Fisher Exact Test, showed no relationship (p>0,05) between emotional intelligence and discussion effectiveness. The category of emotional intelligence in students is at a moderate leel, the PBL discussions are already running effectively. There was no relationship between emotional intelligence and effectiveness of PBL discussions.
