Literature Review: The literature on antithrombotic strategies for patients with atrial fibrillation (AF) undergoing percutaneous coronary intervention (PCI) reveals a nuanced landscape characterized by evolving treatment regimens aimed at balancing thromboembolic prevention and bleeding risk. The initial exploration identified significant gaps in the understanding of optimal therapies for AF patients post-PCI, highlighting the need for evidence-based approaches. Subsequent systematic reviews and meta-analyses. Conclusion: In conclusion, the evolving body of literature reflects a significant shift towards optimizing antithrombotic strategies for AF patients undergoing PCI. The consensus points to the need for individualized approaches that consider both thromboembolic and bleeding risks, with a growing preference for dual therapy involving NOACs. Background: The literature surrounding asymptomatic bacteriuria (ASB) has evolved significantly over the past decade, reflecting a complex interplay of epidemiological insights, clinical management, and evolving guidelines. Literature Review: The foundational study by (Olowe et al., 2013) establishes the prevalence of ASB among elderly and middle-aged individuals in rural Nigeria, identifying significant bacteriuria as a pressing concern, particularly in vulnerable populations. This study highlights the need for targeted screening and raises questions about the treatment benefits for these groups. Further exploration by (Prabha, 2016) focuses on antenatal women, emphasizing the increased risk of ASB during pregnancy and the potential complications arising from untreated cases. The identification of urine culture as the gold standard for screening is crucial, reinforcing the need for effective diagnostic strategies in this demographic. Conclusion: In conclusion, the literature collectively underscores the necessity for targeted screening and treatment approaches for ASB, particularly for at-risk populations such as pregnant women and the elderly. The consensus against routine treatment in asymptomatic individuals highlights the importance of evidence-based guidelines in clinical practice. Abstract

Background: The surgical treatment of laryngopharyngeal reflux disease (LPRD) has evolved significantly over the years, reflecting advances in both surgical techniques and our understanding of the disease itself. Literature Review: Laparoscopic fundoplication has emerged as the preferred surgical method due to its favorable outcomes and lower complication rates compared to traditional open surgery (Liakakos et al., 2009). This minimally invasive approach not only effectively controls GERD symptoms but also enhances the quality of life for patients, including those with recalcitrant cases. The ongoing exploration of novel endoscopic techniques, such as endoluminal gastroplication, indicates a commitment to improving surgical options available for reflux management (Liakakos et al., 2009). Conclusion: In conclusion, the surgical treatment of LPRD requires a multifaceted approach that integrates advances in surgical techniques, patient-specific factors, and a robust understanding of the disease's pathophysiology. The interplay between surgical and medical management strategies is essential for optimizing patient outcomes and addressing the complexities associated with LPRD.

Abstract

Background: The literature on topical calcineurin inhibitors (TCIs) for the treatment of vitiligo highlights their promising role as an alternative to traditional therapies. Literature Review: The comparative study by established that the combination of narrowband ultraviolet B (NB-UVB) therapy with 0.1% tacrolimus is both efficacious and safe, especially for localized vitiligo affecting less than 30% of the body surface area. The rapid onset of pigmentation, observed within 4-8 weeks, underscores the immunomodulatory properties of tacrolimus, which enhances melanocyte activity by inhibiting pro-inflammatory cytokines. This study laid the groundwork for further investigations into the mechanisms and effectiveness of TCIs in vitiligo treatment. Conclusion: In conclusion, the body of literature indicates that topical calcineurin inhibitors represent a significant advancement in the management of vitiligo. Their immunomodulatory properties, safety profile, and potential for combination therapies position them as effective treatment options. However, the need for further research to establish robust clinical guidelines and optimize treatment protocols remains evident.

Abstract

Background: Obesity is a multifactorial condition influenced by a combination of genetic, behavioral, socioeconomic, and environmental factors. The first-line approach for managing obesity typically involves dietary interventions. This systematic review aim to evaluate the current evidence on various nutritional management for obesity based on the literatures of the last 10 years. Methods: The study adhered to PRISMA 2020 standards, examining English literature from 2014 to 2024. It excluded editorials, reviews from the same journal, and submissions without a DOI. PubMed, SagePub, SpringerLink, and Google Scholar were utilized as literature sources. Result: Initially retrieving 360 articles from online databases (PubMed, SagePub, SpringerLink and Google Scholar) eight relevant papers were selected after three rounds of screening for full-text analysis. Conclusion: Different nutritional strategies offer distinct benefits for managing obesity, with Mediterranean, anti-inflammatory, and low-carbohydrate diets showing promise. The key lies in promoting adherence and maintaining weight loss through tailored, sustainable interventions.

Abstract

Background: Pyoderma gangrenosum (PG) is a rare, chronic inflammatory skin disease characterized by painful, necrotic ulcers that frequently lead to disfiguring scars and significantly impact patients’ quality of life. This interplay between PG and mental health issues like anxiety and depression underscores the need for holistic approaches to manage both the physical and psychological challenges faced by PG patients. Methods: This systematic review adhered to the PRISMA 2020 guidelines, focusing exclusively on full-text articles published in English between 2015 and 2024. Result: A total of over 600 publications were initially identified through the database search. Following a rigorous three-stage screening process, eight studies were selected for comprehensive analysis. These studies were critically examined to provide an in-depth evaluation of depression and anxiety among adults with pyoderma gangrenosum. Conclusion: Pyoderma gangrenosum, a chronic autoimmune disorder, significantly impacts patients' mental health due to painful ulcers, visible lesions, and social isolation. Untreated depression can exacerbate pain perceptions and reduce treatment satisfaction. Healthcare providers must screen for and address mental health conditions in PG patients, utilizing psychological support and counseling to address both physical and mental health issues.

Abstract

Background: Epistaxis, or nasal haemorrhage, is a common and potentially serious clinical presentation that can range from a minor inconvenience to a life-threatening emergency. This systematic review aims to provide a comprehensive analysis of the diagnosis and management of epistaxis, emphasizing the interplay between epidemiological factors, anatomical considerations, and clinical risk assessment. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2014 to 2024. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: Epistaxis is a multifactorial condition influenced by local and systemic factors, including comorbidities, medication use, and blood markers. Comprehensive treatment, such as effective initial management, bleeding point identification, and control of hypertension and anemia, is essential for improving outcomes. NOACs offer promising alternatives but require careful treatment consideration.

Background: Perioperative hypertension, a common and significant clinical concern, occurs when blood pressure rises above normal levels during the perioperative period, encompassing the preoperative, intraoperative, and postoperative phases. This systematic review aims to analyze the existing literature on the risk factors, management strategies, and clinical outcomes associated with perioperative hypertension. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2014 to 2024. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: Perioperative hypertension management relies on anesthesia induction, monitoring, and antihypertensive medications. Proper control reduces complications like stroke, myocardial infarction, and organ damage, ensuring better outcomes. Consistent blood pressure management throughout the perioperative period is crucial for patient recovery.

Abstract

Introduction : Dry eye disease (DED) is a common, multifactorial condition affecting the tear film and ocular surface, leading to discomfort, visual disturbances, and progressive ocular damage if untreated. DED involves a complex interplay among anatomical structures, inflammatory pathways, and environmental factors. It is classified into aqueous-deficient and evaporative types, often presenting as mixed forms. DED significantly affects quality of life, daily functionality, and psychological well-being. This systematic review synthesizes current evidence on DED pathophysiology, classification, diagnosis, and management while highlighting recent advancements in therapeutic strategies. Methods :This review adhered to PRISMA 2020 guidelines, focusing on original research published since 2018. Inclusion criteria targeted studies on DED’s inflammatory mechanisms, diagnostic tools, and treatment approaches. Databases searched included PubMed, ScienceDirect, Embase, Cochrane Library, and Web of Science. Keywords such as “dry eye disease,” “ocular surface inflammation,” and “meibomian gland dysfunction” were employed. Following a rigorous screening process, eight studies met the inclusion criteria for comprehensive analysis. Results : The analysis reaffirmed the central role of inflammation in DED pathophysiology, driven by cytokines and matrix metalloproteinases. Emerging diagnostics, such as tear osmolarity tests and meibography, enhance disease identification. Treatment advancements emphasize anti-inflammatory therapies, artificial tears, and meibomian gland support. The review highlights the burden of DED on quality of life and identifies gaps in addressing work-related and environmental stressors. Conclusion : DED is a complex disorder requiring a multidisciplinary approach. Future research should prioritize innovative therapies targeting underlying mechanisms, comprehensive patient education, and integration of mental health support. Holistic management can mitigate the burden of DED and improve patient outcomes.

Abstract

Background: Imaging advancements in diagnosing peripheral artery disease (PAD) are crucial for improving patient outcomes. Digital subtraction angiography (DSA) is the gold standard, but non-invasive techniques like computed tomography angiography (CTA) and magnetic resonance angiography (MRA) are preferred. Photon-counting computed tomography (PCCT) offers superior spatial resolution and reduced beam-hardening artifacts, enhancing the accuracy of detecting and quantifying stenotic disease. Methods: This systematic review adhered to the PRISMA 2020 guidelines, focusing exclusively on full-text articles published in English between 2013 and 2024. Editorials and review articles without a DOI were excluded to ensure high-quality sources. A literature search was conducted across reputable databases, including ScienceDirect, PubMed, and SagePub, to identify relevant studies. Result: This review included eight studies encompassing a total of over 1,000 patients. The methodological quality of the studies was moderate, with some risk of misclassification and verification bias. Each study was critically appraised, providing a comprehensive analysis of the diagnostic imaging performance and accuracy in the evaluation of peripheral artery disease. Conclusion: The study highlights the benefits of PCCT in diagnosing PAD, especially in patients with advanced disease, calcifications, or renal impairment. It offers superior spatial resolution, artifact reduction, and lower contrast media requirements, promising improved diagnostic accuracy.
