Abstract

Background: The surgical management of rotator cuff injuries has evolved significantly over the past few decades, with numerous studies exploring various techniques and their efficacy. The early work by Hopstad (Hopstad, 1998) highlighted the importance of postoperative pain relief and the integrity of the rotator cuff in determining functional outcomes.  Literature Review: Early studies, such as those by (Hopstad, 1998) and (J. Kohns, 2000), established that while the size of the tear is important, the integrity of the repair is a more critical determinant of functional success. Kohns further emphasized the importance of early surgical intervention and the role of physical therapists in optimizing outcomes. Recent innovations, such as tissue engineering approaches discussed by (Maffulli et al., 2012), aim to address the limitations of natural healing, suggesting biomechanical strategies to enhance tendon repair. (French, 2018) explored the kinematic assessments of the thoracohumeral joint, revealing that intraoperative factors and postoperative rehabilitation compliance significantly impact recovery. (Emmanuel, 2018) examined surgical options for young, active patients with irreparable rotator cuff tears, advocating for tailored interventions based on demographic characteristics. (Kim et al., 2020) summarized current surgical options for patients with failed rotator cuff repairs, emphasizing the necessity of understanding which patients may benefit from revision surgery. Conclusion: In conclusion, the body of literature collectively underscores the importance of understanding the multifactorial nature of rotator cuff injuries and their surgical management. Factors such as repair integrity, timing of intervention, patient demographics, and rehabilitation strategies are critical in determining outcomes. Continued research and innovation are essential to refine surgical techniques and improve recovery for patients suffering from rotator cuff injuries.

Abstract

Background: Osteoporosis remains a significant public health concern, particularly among the aging population, where the prevalence of the condition and associated fractures continues to rise. The literature surrounding osteoporosis treatment and fracture outcomes has evolved considerably over the past decade, reflecting advancements in pharmacotherapy and the importance of patient-centered care. Literature Review: The foundational article by (Body et al., 2012) highlights the expanding pharmacological options available for osteoporosis management, stressing the complexities involved in treatment selection due to the limited comparative evidence. This article sets the stage for understanding the broader landscape of osteoporosis therapies, where both skeletal and extraskeletal effects of treatments must be considered for informed clinical decision-making. In examining the treatment landscape, (Fontalis et al., 2019) provides insights into the mechanisms of action of various agents, such as bisphosphonates and denosumab, and discusses emerging therapies that may enhance treatment efficacy. This article underscores the importance of understanding the long-term safety profiles of these medications, as adverse effects can significantly influence treatment outcomes. Moreover, (Lin et al., 2021) offers a comprehensive meta-analysis comparing the efficacy and safety of various osteoporosis treatments for postmenopausal women. This work is pivotal in guiding clinicians toward selecting appropriate interventions based on the relative effectiveness of available therapies, particularly in the context of fracture prevention. Conclusion: In conclusion, the literature collectively emphasizes a comprehensive approach to osteoporosis management that incorporates advancements in pharmacotherapy, addresses adherence challenges, and prioritizes patient-centered care. This multifaceted strategy is critical for optimizing fracture prevention and improving the overall quality of life for individuals with osteoporosis.

Abstract

Background: Throughout the COVID-19 pandemic, pregnant women have been classified as a vulnerable population. However, the epidemiological evidence for infection risk during pregnancy on maternal and neonatal outcomes, such as preterm birth, hypertensive disorders in pregnancy, and postpartum hemorrhage of mothers and prematurity, respiratory and neurodevelopmental disorders of neonates, is still undetermined. The aim: The aim of this study to show about the impact of COVID-19 on neonatal outcomes. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. This search approach, publications that came out between 2014 and 2024 were taken into account. Several different online reference sources, like Pubmed, SagePub, and Sciencedirect were used to do this. It was decided not to take into account review pieces, works that had already been published, or works that were only half done. Result: Five publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: The intrauterine transmission of SARS-CoV-2 was possible, although rare, with early postnatal transmission through direct contact with infected persons occurring more frequently.

Abstract

Background: Hepatocellular carcinoma (HCC) is the most prevalent form of liver cancer, and its incidence and mortality have been steadily increasing in recent decades. Numerous studies have shown that individuals with type 2 diabetes are more likely to develop HCC. This systematic review aims to evaluate the association regarding the role of diabetes in the development and progression of HCC based on literatures of the last decade. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2014 to 2024. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: T2DM is a significant risk factor for HCC, especially with metabolic comorbidities. GLP-1RAs show promise in reducing this risk. Public health interventions are needed to improve diabetes management, and further research is essential to validate and explore these findings.

Abstract

Latar belakang : Kehadiran rokok elektrik menyebabkan sebagian pengguna rokok konvensional yang ingin berhenti merokok beralih menggunakan rokok elektrik yang menurut masyarakat lebih aman. Tujuan: Untuk mengetahui perbandingan tekanan darah perokok elektrik dan perokok konvensional pada mahasiswa kedokteran umum Universitas Malahayati. Metode : Penelitian ini adalah analitik komparatif dengan desain cross-sectional. Sampel merupakan mahasiswa kedokteran yang merokok usia 19-22 tahun. Nilai tekanan darah diperoleh dari pengukuran langsung. Uji statistik menggunakan uji perbandingan t-tes independen dengan nilai kemaknaan p<0,05 Hasil : Distribusi frekuensi usia terbanyak pada usia 20 tahun yaitu sebanyak 23 sampel (34,8%). Lama penggunaan terbanyak dengan lama penggunaan 12 bulan sebanyak 28 sampel (42,4%). Tekanan darah sistolik terbanyak pada rentang 120-139 mmHg dengan 55 (83,3%) sampel dan tekanan darah diastolik terbanyak pada rentang 80-89 mmHg dengan 42 (43,75) sampel. Uji bivariat didapatkan nilai p-value 0,007 untuk sistolik dan 0,013 untuk diastolik. Kesimpulan : Terdapat perbedaan yang bermakna antara tekanan darah perokok elektrik dengan tekanan darah perokok konvensional.

Abstract

Background: The review of the literature on the diagnosis and management of multidrug-resistant tuberculosis (MDR-TB) reveals a pressing global health challenge characterized by complex factors influencing its prevalence and treatment. (Chhabra et al., 2012) lay the groundwork by identifying systemic issues such as inadequate healthcare resources and poor case detection as significant contributors to the MDR-TB epidemic.  Literature Review: (J. Sloan & M. Lewis, 2016) further contribute to the discourse by exploring novel treatments and their applicability in low-resource environments. They emphasize the importance of improved diagnostics and the integration of new therapeutic strategies, while also addressing the financial barriers associated with MDR-TB treatment. (Kolloli & Subbian, 2017) shift the focus to host-directed therapies, arguing for innovative management strategies to address the complexities introduced by co-infections and chronic illnesses. They call for a reevaluation of traditional treatment regimens, particularly for patients with MDR- and extensively drug-resistant tuberculosis (XDR-TB), highlighting the need for approaches that enhance effectiveness. (Yadav, 2023) reviews recent advancements in MDR-TB research, particularly in diagnostic technologies and treatment methodologies. The article reinforces the urgency of addressing the global challenge posed by MDR-TB through innovative strategies that facilitate faster and more accurate diagnosis, ultimately aiming for improved patient outcomes. Conclusion: In conclusion, the literature collectively illustrates the dynamic and intricate landscape of MDR-TB management. The articles underscore the critical need for continued research, enhanced diagnostics, and innovative treatment strategies to effectively combat this growing public health crisis. A multifaceted approach that includes international collaboration, financial investment, and the integration of new technologies is essential for addressing the challenges posed by MDR-TB and improving patient outcomes.

Abstract

Background: The literature surrounding the effects of mineralocorticoid receptor blockers on chronic kidney disease (CKD) has evolved significantly over recent years, reflecting a growing understanding of their potential benefits and risks.  Literature Review: (Yanai et al., 2021) reinforced the cardiovascular protective effects of MRAs through their systematic review and meta-analysis, indicating that MRAs could significantly reduce cardiovascular events in CKD patients. In a similar vein, (B. Wish & Pergola, 2022) discussed the advantages of combining MRAs with ACE inhibitors or ARBs, while cautioning against the elevated risk of hyperkalemia. (Hu et al., 2023) provided a meta-analysis focusing on eplerenone, a selective MRA, suggesting its role as an adjunct therapy in CKD management, although they noted the need for further exploration of its safety profile. (Hedlund Møller et al., 2024) examined the paradoxical nature of MRA treatment, where increased hyperkalemia incidence was associated with decreased mortality risk, underscoring the need for a balanced approach in clinical decision-making. This duality illustrates the necessity of vigilant monitoring and patient management strategies to optimize the therapeutic benefits of MRAs while minimizing risks. Conclusion: In conclusion, the collective findings from these studies indicate that MRAs hold significant promise in the management of CKD, particularly in reducing proteinuria and offering cardiovascular protection. However, the associated risk of hyperkalemia poses a critical barrier to their widespread use. Future research is essential to further elucidate the long-term outcomes of MRA therapy, explore novel agents that separate organ protection from adverse effects, and establish guidelines that can enhance patient safety while maximizing therapeutic benefits.

Abstract

Background: The effectiveness of vitamin D supplementation in enhancing physical performance and reducing falls among older adults has been a topic of considerable research and debate over the past several decades.   Literature Review: Subsequent studies, such as the work by (M. Daly, 2010), further elucidated the importance of vitamin D in conjunction with exercise, highlighting that low serum 25(OH)D levels correlate with diminished muscle strength and increased fall risk. This relationship was echoed in research by (López-Torres Hidalgo, 2011), which demonstrated that combined calcium and vitamin D supplementation significantly improved musculoskeletal function and reduced falls among older adults. This points to the potential synergistic effects of these nutrients in promoting physical health. Meta-analyses, such as those conducted by (Bruyère et al., 2014) and (Rosendahl‐Riise et al., 2017), reinforced the notion that vitamin D supplementation can reduce falls and non-vertebral fractures, particularly at dosages of 700–1000 IU per day. These findings suggest a clear dose-response relationship, emphasizing the critical role of adequate vitamin D levels in maintaining muscle strength and balance, which are essential for fall prevention.  However, the literature also highlights significant variability in outcomes, as noted in the systematic review by (Prokopidis et al., 2022). While some studies reported positive effects of vitamin D on muscle function, others yielded inconclusive results, pointing to the need for further research to clarify the role of vitamin D in combatting sarcopenia and its associated risks. Conclusion: In conclusion, the literature presents compelling evidence supporting the effectiveness of vitamin D supplementation in enhancing physical performance and reducing falls among older adults, particularly at higher dosages. Nonetheless, the inconsistencies in findings across various studies underscore the complexity of vitamin D's role in muscle health and fall prevention. Continued research is essential to refine supplementation strategies, address individual variability in vitamin D metabolism, and establish clear guidelines for effective interventions in older populations.

Abstract

Background: The management of hepatocellular carcinoma (HCC) is a multifaceted challenge that significantly impacts patients' quality of life (QoL). The literature reveals a growing body of research dedicated to understanding the intricate relationship between HCC management and health-related quality of life (HRQoL), as evidenced by a series of systematic reviews and studies spanning over a decade. Literature Review: The systematic review conducted by (Fan, 2011) underscores that patients with HCC experience a notably poorer HRQoL compared to those with chronic liver disease and the general population. Further, the global survey by (Gill et al., 2018) highlights significant gaps in patient information at diagnosis, with a majority feeling inadequately informed. The exploration of advanced HCC treatment options by (Li et al., 2019) reveals that while curative therapies exist for early-stage HCC, patients often present with advanced disease, where treatment options are limited. The study by (Zhou & Fountzilas, 2019) further examines the impact of systemic therapies on QoL, revealing that side effects from anticancer agents can lead to treatment interruptions, thereby diminishing the overall efficacy of the treatment. This finding highlights the importance of ongoing research to manage drug toxicity and improve the QoL of patients undergoing systemic therapy. The systematic review by (Wohlleber et al., 2021) advocates for standardized HRQoL measures, emphasizing their importance in informing treatment strategies and enhancing patient care. The review also discusses various HRQoL instruments, underscoring the need for their integration into clinical practice to better capture the patient experience. n examining prognostic factors, (Minami et al., 2022) emphasizes the role of early detection and effective management of HCV in improving both survival rates and QoL Conclusion: In conclusion, the literature collectively emphasizes the intricate interplay between HCC management and quality of life. There is a clear need for continued research and enhanced clinical practices that prioritize patient-centered care, effective communication, and the integration of psychosocial support to improve the overall QoL for individuals living with HCC.

Abstract

Background. Thoracic empyema, a serious complication of pneumonia, often begins with conservative treatments like thoracentesis and antibiotics in its early stages. However, the management of advanced empyema, particularly in stage II and III, remains contentious. Traditionally, open thoracotomy and decortication have been viewed as requiring lengthy recovery and extended hospital stays, leading some to favor thoracoscopic approaches for quicker outcomes.  Objective. This study reports on the management of thoracotomy in patients with thoracic empyema. Case Report. A 64-year-old woman had been experiencing a cough with blood-streaked sputum for the past 2 months, which did not improve with medication. Further examination indicated thoracic empyema. An open thoracotomy was performed to manage the thoracic empyema. Conclusion. Open thoracotomy proves to be a beneficial approach for managing thoracic empyema. The procedure facilitates effective drainage and resolution of the infection, leading to improved recovery outcomes.
