Abstract

Clavicopectoral plane block (CPB) is a relatively new regional anesthesia technique for clavicle fracture surgery. Methods: The study includes three cases of patients undergoing clavicle fracture surgery who received CPB in remote area. Results: The results show that CPB effectively prevents pain and provides high patient satisfaction. There were no complications during surgery, and pain scores remained low in the post-anesthesia care unit. Conclusions: This serial case reports suggest that CPB can be a safe and effective alternative to regional anesthesia for clavicle surgery, particularly in patients with midshaft clavicle fractures. However, further research with larger sample sizes is needed to confirm the effectiveness of CPB in clavicle surgery. Overall, CPB shows potential as a valuable technique in clavicle fracture surgery.

Abstract

This paper presents a case report on the management of post-dural puncture headache (PDPH) after failed subarachnoid block anesthesia through trans-nasal sphenopalatine ganglion block (SPGB). PDPH, a distressing complication following procedures like lumbar punctures, can persist despite conservative management and usually happen in multiple or single attempts at successful subarachnoid injections. For a long period, the treatment for severe PDPH was an epidural blood patch. SPGB, targeting the sphenopalatine ganglion (SPG) involved in cranial pain transmission, has emerged as a promising intervention. The case involves a patient experiencing severe PDPH after failed subarachnoid block anesthesia. Although SPGB initially brought relief, a recurrence of headaches followed, highlighting the need for further optimization. The case shows that SPGB could be used as an extra treatment for PDPH that doesn't seem to be working, but more research is needed to find out how well it works in the long term and what the best technique is. The abstract emphasizes the importance of ongoing monitoring and intervention in managing PDPH, offering insights into the challenges and prospects of SPGB in clinical practice.

Abstract

Epilepsy is a significant health problem in society. Epilepsy prominently affects a person's quality of life, including social and psychological aspects, with the community’s negative attitude and stigmatization also contributing to the decline of quality of life. Psychiatric comorbidities such as anxiety and mood disorders are found to be the most common comorbidities in epilepsy. We reported a 31-year-old male with anxiety, palpitations, headache, and nausea. These complaints were often felt by the patient in a crowded place and his wife did not accompany him. These complaints have been felt by the patient since his father died in 2014. He also had a history of seizures after an accident in 2008, although the seizures had been decreased to 3 to 4 times a month. The patient always worried that the seizure would recur. Examinations showed severe anxiety and moderate depression. EEG showed functional disturbances with epileptogenic potential. CT scan showed gliosis and temporal bone defect. The patient was diagnosed with post-trauma epilepsy and agoraphobia. The patient was treated with antiseizure, sleeping pills, antidepressants, cognitive behavioral therapy, psychotherapy, relaxation therapy, and family psychoeducation. 1 week after treatment, the headache was significantly reduced, and the patient reported sleeping better. This report showed the importance of assessing and treating psychiatric comorbidities in patients with epilepsy to improve their quality of life.

Abstract

Cryptococcal Meningitis (CM) has caused high morbidity and mortality rate among people living with HIV infection (PLWH). This case report showed a 32 years old female with HIV infection who came with chief complaint headache since 1 month before hospital admission (SMRS). Her headache had become worse in the past 2 days SMRS. Had been taking antiretroviral (ARV) therapy for 2 weeks and anti-toxoplasmic therapy for 3 weeks SMRS could not alleviate her complaints. Although the cerebrospinal fluid (LCS) examination has shown high probable Tuberculosis and Cryptococcal meningitis, specific examination for these infections only showed a positive result for CM. Few days after hospital admission, the patient experienced generalized seizures, monoplegia and sixth nerve palsy. Two months after high dose fluconazole therapy, the specific examination for Cryptococcal infection in CSF showed a negative result. This case report will show the difficulties in diagnosing and managing CNS opportunistic infections. Diagnosing CNS opportunistic infections, such as CM, has been a great challenge because of the low sensitivity and positive prediction value of the tests available, especially among PLWH. Managing these infections also poses some limitations, especially for the availability of their medications. Time to start ARVs has also been a big issue since ARVs were being given before 1definite diagnosis was established. This case study also showed that fluconazole alone can be used as an alternative therapy to amphotericin B and flucytosine for C.M. Therefore, this case report has shown the difficulty in diagnosing and managing CM in Indonesia.

Abstract

The increasing emergence of zoonotic diseases originating from vectors, rodents, mammals, and others has increased the potential for outbreaks of pandemics in the Indonesian territory. Although control and prevention have been implemented, these efforts have not yet revealed a bright spot; therefore, elaboration with advanced CRISPR-Cas9 technology is a way to accelerate efforts to control zoonotic diseases in Indonesia. However, there is limited literature on this topic. This review aims to comprehensively describe, identify, and summarize the application of CRISPR-Cas9 genome-editing technology in zoonotic disease control in Indonesia. Our findings show that CRISPR-Cas9 genome editing technology offers an innovative approach for zoonotic disease control by targeting disease vectors, modifying animal reservoirs, improving disease surveillance, enhancing vaccine development, and exploring traditional medicine candidates and immunotherapy. The high level of precision, efficiency, and versatility in targeting genomes capable of disrupting, damaging, and disrupting the disease transmission cycle in pathogens makes CRISPR-Cas9 highly effective. However, challenges such as off-target impacts, regulatory complexity, and ethical considerations must be overcome with inter- and multidisciplinary collaboration to promote transparency, equity, and public engagement throughout the process of implementing this technology in the field, especially in Indonesia.

Abstract

Prosthetic limbs are crucial as assistive devices for individuals with lower limb amputations. This study focuses on designing and analysing prosthetic feet, specifically the telescopic tube component, to achieve optimal material strength and aesthetic appeal. Numerical simulations using ANSYS Workbench 19 are conducted to assess the total deformation and von Mises stress. Design iterations adjust the pylon tube thickness and material selection. Prototypes are developed using aluminium 6061 tube and rod materials for further testing. Various tests, including scanning electron microscopy, metallography, axial tensile testing, and tangential tensile testing (according to ASTM B 557M), are performed to evaluate the material properties. Results indicated that the axial and yield strengths of the aluminium 6061 rod are superior to the tube type. Conversely, the aluminium 6061 tube exhibits higher ultimate and yield stability in tangential testing. Ultimately, the study concludes that the aluminium 6061 tube material is more robust and suitable for prosthetic feet.
