Abstract

Schizophrenia is a chronic, severe, and debilitate mental disorder that causing disability. Factors that causing disability is cognitive impairment. Cognitive impairment caused by disturbance in neurodevelopmental process of the brain that related to Dopaminergic, GABA-ergic, and Acethylcholinergic pathway. Cognitive Enhancement Therapy is additional therapy to recovery the cognitive function. CET facilitated repairmenin socio and non-socio cognitive function, encourage patient behaviour related social condition, develop patient understanding of schizophrenia and the impact on cognitive impair- ment, and rehabilitation program that characterised of an experiential and exercise to repair the non-social cognitive function (attention, memmory, and problem solving). CET shows improvement in early or late onset of schizophrenia. Pa- tients who got CET can back to work. Now, CET is used for early intervention in cognitive impairment in Schizophrenia. CET was also developed in patients with Schizoaffective, Schizophrenia comorbid with drug abuse, and patientswith au- tism. Hopefully, the improvement onquality of life patient with schizophrenia can achieve with combination pharmacother- apy and Cognitive Enhancement Therapy.

Abstract

Introduction – Urban living, chronic smoking is a risk factor in schizophrenia. Pornographic addiction is also a potential risk factor of schizophrenia considering its neurobiological pathways. Schizophrenia is one of the most dangerous mental illness contributing to 2,8% years lived with disability. Considering many people with urban living, chronic smoking, and pornographic consumption, relation of these risk factors to schizophrenia is demonstrated in this case report. Methods – Presented the case of the patient, 39 years old with psychotic symptoms was found to be born and lived most of his life in urban area, smoking since junior high school, and often consumed pornographic contents since senior high school. Data were taken from psychiatric interviews, general, and mental status examinations in psychiatric clinics, then a literature review was carried out. Results – People who born, and live through their live longer in a larger city, have more risk of Schizophrenia. Patient was known to be born in one of the biggest city in Indonesia, before moved out to another urban area in different countries in last two years. Crave with chronic smoking and pornographic addiction become one of contraproductive conduct source which arise difficulty in patient’s side to take an health life style with the risk of poor prognosis and mental health deterioration. Discuss – People who born and live longer in urban areas such as cities are in increased risk of schizophrenia due to social status awareness and fast paced life induce brain stress. Chronic smoking is a risk factor of schizophrenia due to the effect of nicotine on dopamine levels. Pornographic addiction is a potential risk factor due to addiction pathway effect on dopamine threshold. Conclusion – Urban living, chronic smoking, and pornographic addiction are risk factors associated with schizophrenia found in this case report. Epidemiologic and pathophysiological mechanisms behind these risk factors are an interesting view for physician dealing with schizophrenia patients. Awareness of social status and fast paced life in urban lifestyle, pathways of nicotine in dopamine dysregulation, and dopamine-related impairment in addiction are the neurobiological mechanisms behind these risk factors which a physician needs to be aware of.     Keywords: schizophrenia, urban living, chronic smoking, pornographic addiction

Abstract

The COVID-19 pandemic has changed many things in the medical education system. This change requires adaptation and modification from education providers, teachers, and students. A virtual learning system has been implemented instead of a face-to-face learning system. However, this makes several challenges and issues that must be considered. Virtual learning is considered less attractive and interactive and not effective in teaching clinical skills. For student clerks, the reduced time to undergo education in the hospital leads to fewer case exposures. Another problem is the facilities and infrastructure. Several ways to solve this problem have been implemented while minimizing risk and taking strict precautions. Some institutions modify and innovate to maximize the effectiveness of learning activities. In addition to technical and effectiveness issues, changes in the education system also have an impact on the mental health of medical students, therefore it is important for institutions to be sensitive to changes and risks to students' mental health during the pandemic.

Abstract

Kejadian Henti Jantung Mendadak (HJM) semakin sering terjadi seiring dengan peningkatan prevalensi Penyakit Jantung Koroner (PJK) di Indonesia. Tersedianya penolong (bystander) dapat menaikkan angka keselamatan korban terutama pada kasus yang terjadi di luar rumah sakit. Terbatasnya sumber daya dalam memberikan pelatihan mengakibatkan masih sedikitnya jumlah penolong terlatih di masyarakat. Strategi yang dapat dilakukan yaitu dengan melibatkan siswa sekolah sehingga diharapkan mereka dapat melatih orang tua atau saudara di rumah serta teman sebaya sehingga di waktu yang bersamaan semakin banyak orang teredukasi. Tim Teknologi Kedokteran berkolaborasi dengan Tim Kedokteran ITS menyelenggarakan kegiatan PkM berupa Pelatihan Basic Life Support (BLS) yang diikuti oleh 30 siswa dan guru SMA Negeri 1 Probolinggo pada 6 Oktober 2023. Kegiatan diawali dengan pemberian materi lalu dilanjutkan dengan praktik BLS pada manekin. Peningkatan pengetahuan dan keterampilan secara signifikan didapatkan setelah pelatihan BLS. Semakin banyaknya jumlah penolong terlatih diharapkan dapat meningkatkan keluaran HJM.

Abstract

Despite low nicotine levels, cigarettes and vape liquids can cause addiction, which makes quitting smoking difficult owing to withdrawal syndrome. Hence, we created a nicotine-free liquid vape using essential oils (EOs) like peppermint (Mentha piperita), lavender (Lavandula latifolia), and tangerine (Citrus reticulata) to substitute the nicotine in inducing relaxation without addiction. Thus, this study aimed to examine (1) the safety profile of this nicotine-free liquid vape by observing chemical characteristics using gas chromatography-mass spectrometry (GC-MS) and (2) its efficacy in reducing short-term anxiety in healthy volunteers who had previously used nicotine vape. The most complicated vape liquid formula (F4), containing propylene glycol, vegetable glycerin, with those three EOs, was injected into GC-MS, and the mass spectra were compared to the Wiley mass spectral library to identify the compound name. For the efficacy test, subjects who met inclusion criteria and fulfilled informed consent were randomly assigned to five groups (four groups received test liquids formula (F1-F4) while one group received placebo). Before and after using the vape liquid, subjects were asked to complete the State-Trait Anxiety Inventory (STAI) questionnaire to assess its anxiety-reduction effects. The result showed that nicotine-free vape liquids containing three EOs did not show any foreign substances that had never been encountered before, which means our product is safer than nicotine-containing vape liquids. This safety was also supported by all study subjects who did not report any adverse effects. The efficacy test showed that subjects who used vape liquids with essential oils (F1-F4) experienced a significant decrease in heart rate and systolic blood pressure. At the same time, 50% had a lower post-test STAI score, indicating a decrease in anxiety, although it was not statistically significant.

Abstract

Mahasiswa kedokteran tahun pertama menghadapi banyak tantangan dalam menye-suaikan diri dengan lingkungan akademik baru, yang dapat memengaruhi kesehatan mental mereka. Tujuan dari penelitian ini adalah untuk meningkatkan kemampuan siswa dalam mengelola stres dan memberikan dukungan psikologis melalui pelatihan manajemen stres dan dukungan psikologis pertama / Psychological First Aid (PFA). 30 mahasiswa kedokteran semester 1 FKK ITS mengikuti pelatihan, yang berlangsung selama dua hari dan total 16 jam. Pelatihan juga dievaluasi melalui pretest dan posttest menggunakan kuesioner. Melalui simulasi kasus, peserta menunjukkan kemampuan yang baik dalam menggunakan strategi PFA dan manajemen stres. Hasil analisis menunjukkan peningkatan signifikan dalam pemahaman dan keterampilan peserta setelah mengikuti pelatihan (p=0.000). Pelatihan mencakup pemaparan teori dan praktik keterampilan PFA, seperti keterampilan mendengarkan yang aktif, keterampilan komunikasi, keterampilan empati, dan keterampilan bertahan hidup. Dua modul program ber-ISBN dan publikasi di media massa. Kesimpulan dari pengabdian masyarakat adalah bahwa terjadi peningkatan pemahaman dan kemampuan mahasiswa kedokteran tahun pertama terhadap Psychological First Aid dan Stress Management yang diharapkan lebih siap menghadapi tantangan akademik.

Abstract

Keberadaan media sosial di kalangan remaja telah menjadi gaya hidup. Media sosial memberikan dampak positif dan negatif bagi penggunanya, Salah satu dampak negatif yaitu munculnya fenomena cyberbullying. Cyberbullying mengacu pada perilaku perundungan, seperti penghinaan, pelecehan, pencemaran nama baik, dan tindakan serupa yang dilakukan secara daring dan dapat berlangsung terus-menerus. Kurangnya pengawasan terhadap penggunaan media sosial oleh remaja berkontribusi pada keberlanjutan tindakan ini, sehingga menimbulkan dampak psikologis yang signifikan pada korban. Program pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan remaja mengenai cyberbullying guna memberdayakan peran mereka dalam kegiatan pencegahan baik di lingkungan sekolah maupun masyarakat. Metode yang digunakan adalah edukasi kesehatan mental dan penugasan karya video edukasi. Kegiatan edukasi dilaksanakan di MAN Kota Mojokerto secara luring dengan sasaran 100 siswa – siswi. Sebelum dan sesudah edukasi, peserta diminta mengisi kuesioner pre-test dan post-test. Kemudian dilanjutkan dengan lomba video edukasi satu bulan setelah edukasi dengan tema “stop cyberbullying”. Hasil kegiatan didapatkan bahwa pemberian edukasi kesehatan mental berpengaruh pada peningkatan pengetahuan peserta mengenai cyberbullying secara signifikan (p<0,001). Setelah mendapatkan edukasi, siswa berperan membuat media promosi kesehatan yang berupa video edukasi untuk disebarkan media sosial guna mencegah kejadian cyberbullying untuk mencapai kesehatan mental remaja yang optimal

Abstract

Depressive disorder is a disorder which frequently found in adolescence. Although some interventions had already known, many adolescence had difficulty to access care when needed, and adherence to therapy. We aimed to introduce using technology-based interventions for depression in adolescence. We searched PubMed, Springer, Sage, and Science Direct for articles in English using the search terms “adolescent depression” and “depression”, combined with additional search terms, “technology-based” “online” “computer-based” “computerized” “prevention” “treatment” “CBT” “videoconference”. We focused on papers from past 5 years, but also included papers published in the past 10 years (if sparse). We found 25 articles were suitable for this topic. This review resulted some novel development intervention such as Self-Monitoring and mood assessment, Computerized Cognitive Behaviour Therapy, and videoconference therapy that had benefit could increase the coverage of the patient, adhered more to the therapy, more convenient to access from wherever they live, and cost the least for the patient than conventional therapy that showed the same effectiveness. Technological development in medical care, specifically in mental health, was a possible solution to increase the coverage of intervention for depression in adolescence. Further research is needed on the effectiveness of this technology-based intervention in larger population.

Abstract

Monitoring vital physiological parameters such as breathing rate (BR) is crucial for assessing patient health. However, current contact-based measurement methods often cause discomfort, particularly in infants or burn patients. This study aims to develop a non-contact system for monitoring BR using infrared thermography (IRT). This approach permits to detects and tracks the nose from thermal video, extracts temperature variations into a breathing signal, and processes this signal to estimate BR. The estimated BR is then classified into three health categories (bradypnea/normal/tachypnea) using k-nearest neighbors (k-NN). To evaluate system accuracy and robustness, experiments were conducted under three conditions: (i) stationary breathing, (ii) breathing with head movements, and (iii) specific breathing patterns. Results demonstrated high consistency with contact-based photoplethysmography (PPG) measurements, achieving complement of the absolute normalized difference (CAND) index values of 94.57%, 93.71%, and 96.06% across the three conditions and mean absolute BR errors of 1.045 bpm, 1.259 bpm, and 0.607 bpm. The k-NN classifier demonstrated high performance with training, validation, and testing accuracies of 100%, 100%, and 99.2%, respectively. Sensitivity, specificity, precision, and F-measure results confirm system reliability for non-contact BR monitoring in clinical and practical settings.

Abstract

Obstructive Sleep Apnea (OSA) is a potentially life-threatening sleep disorder that often remains undiagnosed due to the complexity of conventional diagnostic methods such as polysomnography (PSG). Currently, there is a lack of accessible, non-invasive diagnostic solutions suitable for home use. This study proposes a novel approach to automate OSA detection using single-lead electrocardiogram (ECG) signals acquired through non-contact conductive fabric electrodes embedded in a mattress, enabling unobtrusive monitoring during sleep. The main contributions of the proposed study are a mattress-embedded contactless ECG monitoring system eliminating the discomfort of traditional electrodes, and an advanced signal processing framework integrating wavelet decomposition with machine learning for precise OSA identification. ECG signals from 35 subjects (30 male, 5 females, aged 27-63 years) diagnosed with OSA were obtained from the PhysioNet Apnea-ECG database, originally sampled at 100 Hz and up-sampled to 250 Hz for consistency with experimental recordings from healthy volunteers tested in various sleep positions. Signals were recorded non-invasively during sleep in various body positions and processed using the Discrete Wavelet Transform (DWT) up to the third level of decomposition. The processing of ECG signals involved Heart Rate Variability (HRV) analysis, which was applied to extract information in the time domain, frequency domain, and non-linear properties. By analyzing HRV on the respiratory sinus arrhythmia spectrum, the respiration signal was obtained from ECG-derived respiration (EDR). Feature selection was performed using ANOVA, resulting in a set of key features including respiratory rate, SD2, SDNN, LF/HF ratio, and pNN50. These features were classified using the XGBoost algorithm to determine the presence of OSA. The proposed system achieved a detection accuracy of 96.7%, demonstrating its potential for reliable home-based OSA diagnosis. This method improves comfort through non-contact sensing and supports early intervention by delivering timely alerts for high-risk patients
