Abstract

Enteral formula helps meet the nutritional needs of patients who cannot eat normally, maintains bowel function, and prevents malnutrition. Formula substitution can provide greater flexibility and nutritional options to meet patient needs.This study aims to analyze the management of standardized nutritional care in patients with pericardial effusion, pleural effusion, lipid storage disorder accompanied by hypertension and anemia using enteral formula substitution. This descriptive study was conducted at X Hospital, on CVCU inpatients. Nutritional assessment was conducted by collecting anthropometric, biochemical, physical, dietary history, and personal history data. The patient was 64 years old with normal nutritional status based on estimated BMI. The patient was given liquid food with commercial formula and hospital formula via nasogastric tube (NGT) as a bolus. The results showed that the use of enteral formula substitution can meet the patient's energy, protein, fat, and carbohydrate needs during hospitalization. There was an increase in intake within 3 days of intervention, reaching >80% of the needs. The patient's body weight and nutritional status did not decrease or change. This study shows that the use of enteral formula substitution can be an effective option in standardized nutritional care in patients with complex medical conditions. Thus, enteral formula can help meet the nutritional needs of patients who cannot eat normally and prevent malnutrition.

Abstract

Background: The oral cavity harbors a highly diverse microbial community, within which nitrate-reducing bacteria (NRB) constitute a functional core group contributing to microbial balance and systemic health through nitric oxide (NO) production. Objectives: The present study aimed to detect and characterize NRB from the oral cavities of healthy individuals using a selective molecular approach. Methods: A total of ten oral samples were collected and cultured on Brain Heart Infusion (BHI) agar prior to DNA extraction. A species-specific primer targeting Rothia mucilaginosa, a well-documented oral NRB, was designed and applied in PCR assays. Results: The analysis revealed the presence of Rothia mucilaginosa in one positive sample (10%). Although the prevalence was limited compared to previous studies reporting broader NRB diversity, the detection of Rothia mucilaginosa underscores its ecological relevance in nitrate reduction within the oral environment. Conclusion: Collectively, these findings demonstrated the feasibility of selective molecular detection, highlight Rothia mucilaginosa as a potential biomarker for oral and systemic health, and emphasize the need to expand primer panels in future research to capture the full diversity of oral nitrate-reducing bacteria.

Abstract

Objectives: determine the types and percentage of Astigmatism in children in Karbala city in Iraq. Patients and methods: This study was cross-sectional comparative study performed in Imam Hussein ophthalmology center in Karbala governorate in Iraq from January 2024 to June 2025. The study included 1834 eyes with equal sex and mean age of 8.95±2.37 years. Results: Regarding the Astigmatism, the higher proportion was for “-2 to 0” Myopic Astigmatism (48.7%), followed by “0 to 2” hypermetropic Astigmatism (35.3%). Most of the children had against the role astigmatism. Conclusion: Myopic Astigmatism was more common in males, while Hypermetropic Astigmatism was more common in females. Most of the children had against the rule astigmatism

Abstract

Background: Infection prevention occupies a central role in dental care because of the frequent exposure to saliva, blood, and other potentially infectious materials during treatment. Objectives: The present study seeks to determine how well dental students understand established infection control procedures and to investigate the extent to which they report following these practices during their preclinical preparation and subsequent clinical training. Methods: We conducted a descriptive cross-sectional study with 51 To capture a broad range of information. The data was analyzed and interpreted using SPSS. Results: Most students appeared to understand the main infection control practices. About 88% reported washing their hands correctly, and roughly 80% used personal protective equipment appropriately. Fewer students were familiar with protocols for sharps injuries and the specific guidelines of their institution, with around six in ten demonstrating awareness. Conclusion: Control practices. However, gaps persist, especially when it comes to following institution-specific policies and reporting injuries. To overcome these gaps, it may be necessary to strengthen the infection control component of the curriculum and incorporate additional practical training opportunities in clinical settings to reinforce safe practices..

Abstract

Elevated perioperative serum lactate is a common finding during craniotomy, but its clinical significance and origin—whether reflecting systemic hypo perfusion or localized cerebral metabolic shifts—remain unclear. This study aimed to prospectively evaluate the association between intraoperative hyperlactatemia and postoperative outcomes in elective craniotomy patients. In this prospective study, 50 adult patients undergoing elective craniotomy for brain tumors or lesions were enrolled. Arterial blood gas analyses, including serum lactate measurements, were performed preoperatively, immediately postoperatively, and at 24 hours. Patients were stratified into High-Lactate (HL, ≥2 mmol/L) and Normal-Lactate (NL, <2 mmol/L) groups based on immediate postoperative levels. Primary outcomes included new postoperative neurological deficits and hospital length of stay (LOS). Secondary outcomes included systemic complications (renal failure, myocardial infarction, 30-day mortality). Data were analyzed using comparative statistics and logistic regression. Elevated immediate postoperative lactate (≥2 mmol/L) was observed in 30% (15/50) of patients. There was no statistically significant difference in the incidence of new neurological deficits between the HL (38.5%) and NL (27.3%) groups (p=0.31). Similarly, no cases of mortality, renal failure, or myocardial infarction occurred within 30 days. However, median hospital LOS was significantly longer in the HL group (6.5 days) compared to the NL group (3 days) (p=0.003). Regression analysis confirmed elevated intraoperative lactate as an independent predictor of prolonged hospitalization (p=0.036), but not of new neurological deficits. Infratentorial lesion location was associated with a higher odds of developing a new neurological deficit (OR=5.5, p=0.027). In patients undergoing elective craniotomy, isolated postoperative hyperlactatemia is not an independent predictor of new neurological deficits or major systemic complications. Its primary clinical correlate is a significantly extended hospital stay, suggesting an impact on resource utilization and recovery pace. These findings challenge the routine use of serum lactate as a specific alarm biomarker for neurologic injury in this context and highlight the need for investigation into more direct cerebral metabolic markers..

Abstract

The importance of handwashing with soap (CTPS) behavior during the COVID-19 pandemic plays a significant role in preventing the transmission of infectious diseases. However, document reviews and interviews at Garuda Health Center in Pekanbaru City indicate that the achievement of CTPS is still low, at only 29%. This low achievement is caused by a lack of cross-program coordination between environmental health program holders and health promotion, as well as the non-implementation of Community-Based Total Sanitation (STBM) triggering activities in 2020 due to the COVID-19 pandemic. This study aims to evaluate the implementation of the Clean and Healthy Life Behavior (PHBS) program through CTPS activities in efforts to prevent COVID-19 at Garuda Health Center in Pekanbaru City, and to formulate alternative problem-solving strategies through the development of an intervention plan (Plan of Action). This study uses a qualitative method with data collection techniques in the form of in-depth interviews, field observations, and document reviews regarding the implementation of the STBM program. The research informants consist of the Head of the Community Health Center (Puskesmas), the Head of Administration, the holder of the STBM program, and related staff. The results of the study show that the achievement of CTPS is still low due to weak cross-program coordination and suboptimal community empowerment activities. Improvement efforts are carried out by preparing a work plan (RUK) and a Plan of Action in the form of strengthening cross-program coordination, training environmental health workers, providing health promotion media, and improving CTPS facilities in public places. It is concluded that the evaluation of the PHBS program through CTPS needs to be focused on strengthening cross-sector collaboration to improve clean and healthy living behaviors of the community in the working area of Garuda Community Health Center, Pekanbaru City.

Abstract

The Hospital Facility Maintenance Installation (IPSRS) at Petala Bumi Regional General Hospital, Pekanbaru, functions under the direct supervision of the Hospital Director and is responsible for maintaining hospital facilities, infrastructure, and medical equipment to ensure safety and service quality. This descriptive-analytic qualitative study (2023) aimed to analyze the medical equipment maintenance system at IPSRS, involving four informants from hospital management and technical staff. Data were obtained through interviews, observations, and document reviews, analyzed using content analysis with triangulation techniques. The findings show that IPSRS faces human resource shortages, with limited staff qualifications and multitasking duties affecting performance. The maintenance budget is available but requires better allocation. Materials and spare parts are mostly sufficient but partly depend on third-party suppliers. SOPs exist but are not fully implemented due to HR constraints and lack of evaluation. The organizational structure functions adequately but needs better HR placement. It is recommended that hospital management add skilled technical staff, update SOPs, and enhance staff competence through training and certification. Strengthening HR, budget management, and maintenance procedures will improve IPSRS efficiency and support high-quality, sustainable hospital services.

Abstract

To enhance the quality of services within the Medical Record Installation, it is necessary to implement a patient registration system that is simple, efficient, and easily accessible. One of the essential components of this system is the patient medical card, which is issued by the medical record unit to facilitate the identification and management of patient data. The main objective of this program is to optimize the hospital’s information management system, particularly within the Medical Record Installation of RSUD Teluk Kuantan, to support accurate, timely, and integrated health data management. The activity was carried out through three main stages: Preparation Phase – including the introduction and orientation at RSUD Teluk Kuantan. Implementation Phase – involving interviews, direct observations, and document reviews within selected medical record units. Reporting Phase – consisting of the preparation of the final report (Chapters I–VIII), followed by consultations with the field supervisor and academic advisor, and concluded with a residency result seminar. The priority problem was identified using the USG (Urgency, Seriousness, and Growth) method, which determined that the main issue was the Patient Medical Card Management System at RSUD Teluk Kuantan. As part of the Action Plan, the proposed solution involved establishing a Memorandum of Understanding (MoU) between the Head of the Medical Record Installation and the Hospital Director, concerning the development and implementation of an improved patient medical card system at RSUD Teluk Kuantan. Additionally, another strategic solution was to recruit new human resources who meet the professional qualifications required for the medical record department. This step aims to strengthen the unit’s operational capacity and ensure that medical record services are conducted effectively and in accordance with hospital standards.

Abstract

The effectiveness of health administration at the Posyandu level is very important in supporting accurate health data recording, monitoring, and service delivery. This study aims to analyze the factors that influence the effectiveness of health administration by Posyandu cadres in Air Terbit Village, Tapung District, Kampar Regency. This study uses a qualitative descriptive design through in-depth interviews, observations, and document review with informants consisting of the Head of the Community Health Center, village officials, and active Posyandu cadres. Data analysis was conducted thematically using the 5M approach (Man, Method, Machine, Material, Money) as well as environmental factors. The results of the study indicate limitations in human resources, lack of administrative training for cadres, conventional implementation methods, and weak cross-program coordination. Facilities, promotional media, and administrative funding are still limited, and public awareness of the importance of health reporting is also low. It is concluded that improving the effectiveness of health administration needs to be carried out through cadre training, digitization of record-keeping, routine supervision, and funding support from the community health centers and village government.

Abstract

General Background: Avian pathogenic Escherichia coli (APEC) remains a major challenge in poultry production, necessitating reliable vaccination strategies. Specific Background: Liposomal and crude antigen vaccines represent distinct formulations with different capacities to stimulate humoral and cellular immune responses. Knowledge Gap: Comparative evidence on their immunogenic profiles in broiler chickens, particularly across key cytokine markers and IgY levels, remains limited. Aims: This study evaluated and compared the immunogenicity of liposome-based (LIPO) and crude antigen (CRU) vaccines against APEC in broiler chickens. Results: Both vaccines significantly increased IL-4, IL-10, IgY, IL-12, IL-1β, and IFN-γ compared to the control (p < 0.0001). LIPO vaccination induced higher IL-4, IgY, IL-12, IL-1β, and IFN-γ levels, indicating strong activation of both Th2-mediated humoral and Th1-mediated cellular responses. CRU vaccination showed increased IL-10 and moderate cytokine responses, suggesting partial immune activation. Novelty: This study provides a direct immunological comparison of liposomal and crude antigen formulations using multiple cytokine indicators in a controlled broiler model. Implications: The findings support liposomal vaccines as a promising strategy for inducing balanced immune responses against APEC, contributing to improved poultry health management and reduced reliance on antimicrobial interventions.
