Abstract

Intensive Care Unit (ICU) services often cause anxiety among families due to patients’ critical conditions and limited access to information. The orientation phase of therapeutic communication plays an essential role in enhancing clarity, safety, and trust. This study aimed to analyze the effect of implementing therapeutic communication in the orientation phase on family satisfaction of patients treated in the ICU of Ibnu Sina Gresik Hospital. This study used a pre-experimental one-group pretest–posttest design without a control group. The population consisted of all family members waiting in the ICU, and 30 respondents were selected through purposive sampling based on inclusion criteria. The independent variable was the implementation of the orientation phase of therapeutic communication, while the dependent variable was family satisfaction. Data were obtained using a validated and reliable family satisfaction questionnaire. The intervention consisted of a structured orientation phase conducted by nurses, including self-introduction, role explanation, time contracting, initial information provision, orientation to ICU procedures and environment, and opportunities for clarification. Data collection was carried out in two stages: pretest before the intervention and posttest after the intervention. Data analysis included univariate distribution and Wilcoxon Signed Rank Test to measure changes in satisfaction. Results indicated a significant improvement in family satisfaction after the intervention (p=0.000; p<0.05). Most respondents reported moderate satisfaction before the intervention and shifted to satisfied to very satisfied afterward. It is concluded that systematic implementation of the orientation phase of therapeutic communication effectively increases family satisfaction in the ICU.

Abstract

Diabetic foot ulcers (DFUs) are a common complication of diabetes mellitus (DM) and cause significant morbidity and mortality. Diabetic feet exhibit various structural and functional changes compared to non-diabetic feet. The purpose of this study was to identify foot complaints commonly experienced by patients with DM. This study employed a descriptive design with a cross-sectional approach. The sample size was 200 adult patients with DM selected using a purposive sampling technique. Patients with DM were asked to provide written consent before completing the questionnaire. This study used descriptive analysis, using a frequency distribution table. Data on foot complaints of patients with DM showed that numbness or thickening of the feet was present in 28.5%, tingling in 32.5%, and numbness in the feet was less common, with only 7.0% reporting consistent experience. Meanwhile, 13.0% experienced itchy feet, and only 3.0% reported persistent reddish feet. Blackish feet, a more serious condition, were recorded with 12.5% reporting persistent blackness. Swelling in the feet was reported by 6.0% and pain in the feet was reported by 15.0%. All patients with diabetes in the study experienced complaints in their feet, some even having more than one foot complaint. Among these foot complaints, numbness/thickness, tingling, and pain are common among patients with diabetes. Patients with DM should receive accurate information to identify foot complaints of developing Diabetic foot ulcer and to prevent them by performing proper and regular foot care

Abstract

Families of ICU patients typically go through anxiety and unpredictability due to the patient’s critical condition, which generates basic needs such as information, support, proximity, comfort, and assurance. When these needs are not optimally met, family satisfaction with hospital services may decrease. Structured information delivery is expected to help families better understand the situation and increase their satisfaction. This study aimed to analyze differences in service satisfaction scores before and after the provision of information related to fulfilling the basic needs of patients’ families. The study utilized a pre-experimental pretest-posttest design with a single group, including 30 family members of ICU patients selected through purposive sampling. The independent variable involved application of information on fulfilling families’ basic needs, and service satisfaction as the dependent variable. Data were obtained through an adapted service satisfaction questionnaire based on the five SERVQUAL dimensions, with analysis performed using the Wilcoxon Signed Rank Test at a significance level of α = 0.05. Result showed higher satisfaction scores after the intervention, with a mean improvement of 2.17 points and a 13-point increase in the minimum score. The Wilcoxon test yielded p = 0.001, showing a significant variation in satisfaction scores between the pre- and post-information phases. In conclusion, structured information delivery is an effective strategy to enhance family satisfaction during the ICU waiting period, as clearer information helps families feel calmer, understand the patient’s condition, and develop a more positive perception of hospital services. These findings support the integration of structured information provision within family-centered nursing care, and hospitals are encouraged to implement routine information delivery, develop educational media, and strengthen communication to maintain family satisfaction.

Abstract

AbstractIntroduction: Vaginal discharge is a problem of female reproductive organs that often occurs in adolescents in Indonesia. This study aims to determine the relationship between knowledge and attitudes of adolescent girls about vulva hygiene and the incidence of vaginal discharge at SMA Negeri 13 Surabaya. Methods: This study used a type of correlational analytical research with a cross-sectional approach. A sample of 65 young women was taken using the proportionete Stratified Random Sampling technique. The independent variable is the knowledge and attitude of adolescent girls, the dependent variable is the incidence of vaginal discharge. The data were analyzed with univariate, bivariate using Chi Square with a value of α = 0.05. Results and Data Analysis: The results of this study showed that out of 65 adolescent girls, most of the adolescent girls (52.3%) had good knowledge, most of the adolescent girls (70.8%) had a positive attitude, most of the adolescent girls (66.2%) experienced physiological vaginal discharge. Most adolescent girls (69.8%) are well-informed to experience physiological vaginal discharge. The results of the Chi Square test were obtained that there was a relationship between adolescent girls' knowledge of vulva hygiene and the incidence of vaginal discharge (p-value = 0.001 < α = 0.05). Almost all adolescent girls (88.4%) have a positive attitude of experiencing physiological vaginal discharge. The results of the Chi Square test obtained that there was a relationship between adolescent girls' attitudes about vulvar hygiene and the incidence of vaginal discharge (p-value = 0.001 < α = 0.05). Conclusion: Adolescent girls' knowledge of vulvar hygiene is mostly in the good category. Adolescent girls' attitudes about vulvar hygiene are mostly positive. There is a relationship between knowledge and the occurrence of vaginal discharge. There is a relationship between attitude and the incidence of vaginal discharge. Advice for young women to do vulva hygiene properly and correctly to prevent vaginal discharge. For schools, it is hoped that they can add books in the library related to the hygiene of the reproductive organs of adolescent girls as educational materials and can carry out counseling on the hygiene of female reproductive organs.Keywords: Knowledge, Attitude, Vulva Hygiene, Vaginal Discharge
