Abstract

Dyslipidemia is a significant global public health concern, particularly in Indonesia. Current dyslipidemia guidelines stated the management should be done with a combination of lifestyle and pharmacological interventions. Treatment with lipid lowering agents should be initiated early for better prognostic benefit. ACS Europath IV project emphasizes the importance of early intensification of lipid-lowering therapy to reduce the risk of recurrent cardiovascular events and improve patient outcomes. This literature review the current state of dyslipidemia management in Indonesia and discussing the potential role of ACS Europath strategies in improving patient outcomes.

Abstract

Introduction: Maternal mental health is an important yet often overlooked issue, both globally and nationally. About 10% of pregnant women experience mental health problems during pregnancy, with prevalence rates tending to be higher in developing countries. Methods: This descriptive study involved 100 pregnant women who attended routine antenatal care at UPTD Puskesmas Manggar between September and October 2023. The study aimed todetermine the prevalence and characteristics of maternal mental health status. Data were collected through interviews and the Self Report Questionnaire-20 (SRQ-20). Results: A total of 24% pregnant women were found to have common mental disorders, with fatigue being the most frequently reported symptom. The majority of those affected was multiparous (58.3%), in the third trimester of pregnancy (58.3%) and had no history of abortion (91.7%). Conclusion: The relatively high prevalence of common mental disorders during pregnancy underscores the importance of implementing routine mental health screening as an integral part of antenatal care at the primary healthcare level.

Abstract

Introduction: Giant cell tumor of bone (GCTB) is a rare, benign but locally aggressive tumor that uncommonly afects distal ulna. Case: This case is a 30-year-old female with worsening pain, swelling, and limited wrist motion. Imaging studies revealed cortical destruction and soft tissue infltration characteristic of an aggressive GCTB. Campanacci type 3 GCTB of the distal ulna was diagnosed and managed through limb salvage surgery using a combination of Darrach procedure and single bone technique.This combined approach aims to achieve tumor clearance while preserving limb function. Post-operative management included soft tissue closure using an abdominal fap. Conclusion: The case highlights the challenges in managing aggressive GCTBs of the distal ulna and demonstrates a viable limb salvage strategy. Long-term follow-up is recommended to evaluate functional outcomes and to monitor for recurrence, because tumor’s aggressive nature.

Abstract

Hand, foot, and mouth disease (HFMD) is a common viral illness primarily affecting children. HFMD is caused by various types of coxsackievirus and enterovirus; the main causes are coxsackievirus A16 and enterovirus A71. This article discusses HFMD surveillance and its spread in several countries, including China, Singapore, and Indonesia, aiming to understand the surveillance processes and HFMD distribution to formulate effective prevention and management strategies.

Abstract

Hepatitis B vaccine has been highly effective in preventing HBV infection. However, a subset of individuals fails to develop adequate protective antibodies after complete vaccination series (anti-HBs ≥ 10 mIU/mL), termed as non-responders. Non-responder phenomenon is influenced by a complex interplay of genetic, immunological, and environmental factors. This narrative review aims to delve into the immunological aspects of the non-responder phenomenon, exploring the factors influencing vaccine response, the immunological mechanisms involved, and potential approaches to managing and overcoming non-responders.

Abstract

Latar belakang: Kejadian gangguan kognitif meningkat seiring dengan pertambahan usia ataupun karena gangguan organik otak. Citicoline diduga dapat memperbaiki fungsi kognitif. Tulisan ini meninjau efektivitas citicoline dalam meningkatkan fungsi kognitif. Metode: Pencarian sistematis pada 3 database, yaitu PubMed, Scopus, dan EBSCO, sesuai kriteria inklusi dan eksklusi. Artikel yang terpilih ditelaah kritis dan dianalisis lebih lanjut. Hasil: Dalam analisis terakhir, didapatkan 2 artikel untuk telaah kritis. Satu studi populasi sehat menunjukkan perbaikan fungsi kognitif terutama pada domain memori setelah pemberian citicoline 500 mg/hari selama 3 bulan, 1 studi lain pada populasi pasca-stroke iskemik menunjukkan perbaikan fungsi kognitif terutama pada domain atensi, eksekutif. dan orientasi temporal setelah pemberian citicoline 1.000 mg/hari selama 12 bulan. Simpulan: Pemberian citicoline dapat memperbaiki fungsi kognitif, baik pada populasi sehat maupun populasi pasca-stroke iskemik.

Abstract

Urinary tract infection is a common disease with high recurrence. UTI is usually treated with antibiotics, however high bacteria resistance towards first-line antibiotic treatment is becoming a health issue. Cranberries have been shown to prevent the adhesion of bacteria to host cell, hence preventing UTI. This systematic review aimed to determine the effectivity and safety of cranberry for prevention of urinary tract infection. Systematic Review was done with search engine from Google Scholar, PubMed, and ScienceDirect, starting with forming clinical question using PICO framework. Studies included was selected based on inclusion and exclusion criteria. The outcomes extracted from each study are the effectiveness and safety of cranberries in UTI prevention. Eight studies were reviewed, seven studies concluded that cranberries can significantlyprevent UTI, while 1 study found that the result was not significant. Three studies reported minimal adverse effect in a small percentage of patients. Cranberries can reduce the incidence of recurrent UTI with minimal adverse effect. PACs (proanthocyanidins) in cranberries can prevent bacteria adhesion, therefore preventing colonization of the bacteria in the urinary tract. Further research on dose and duration are required.

Abstract

Indonesian Ministry of Health detected an increasing trend in suspected hand, foot, and mouth disease (HFMD) in all provinces in Indonesia in 2024. Enterovirus 71 (EV71) is the main cause of severe HFMD and death. There has been no specific therapy for EV71. EV71 vaccine has been developed and approved by regulatory agencies in various countries (China, Thailand, and Indonesia) for the prevention of HFMD due to EV71. Multi-center phase IV study showed a good safety profile with an AEFI of 1,079%. Most AEFIs are grades 1 and 2.

Abstract

Hyponatremia is an electrolyte imbalance disorder commonly found in clinical practice. Hyponatremia is associated with significant mortality and morbidity. Tolvaptan is an orally administered V2 receptor antagonist drug capable of aquaresis without excreting electrolytes, thus increasing plasma sodium concentration. Results from clinical trials showed tolvaptan to be a relatively safe and effective treatment in euvolemia and hypervolemia hyponatremia.

Abstract

Graves’ disease is the primary cause (60%-80%) of hyperthyroidism. The risk factors include genetic, environmental, and immunological aspects, with a higher prevalence in women. The diagnosis is based on medical history and physical examination, supported by Wayne index, as well as appropriate diagnostic tests including thyroid function tests, antibody testing, thyroid scan, or color-doppler ultrasonography. Treatment options include anti-thyroid medication, radioactive iodine treatment (RAI), and surgery. Complications include Graves’ ophthalmopathy, thyroid dermopathy, cardiovascular effects, and thyroid crisis. The prognosis depends on the likelihood of remission and relapse, with remission being more common in adults than in children.
