Abstract

Glucocorticoid receptors (GR) have an important role in the resistance of steroid therapy . Previous studies showed  significant  difference  activation  and  expression  of  GR  between  Steroid  Resistant  Nephrotic Syndrome (SRNS) and Steroid Sensitive Nephrotic Syndrome (SSNS), however the other studies showed contrastive result. Previous studies also revealed Hsp-90 had an important  role in  GR activation. The aim of this study was to determine the differences expression and activation of GR and also Hsp-90 expression in lymphocytes  cytoplasm  and  nucleus  between  SRNS  and  SSNS  patients.  A  Cross-sectional  study  was conducted  to  40  children  (20  SRNS  and  20  SSNS  patients).  Double  stainning  immunocytochemistry  of lymphocytes  was  performed.  The  GR  expression  and  activation  were  determined  by  counting  amount  of lymphocytes  that  express GR and Hsp-90 among 200 under light  microscope with 1000x magnification. The differences  of  activation  and  activation  of  GR  and  Hsp-90  in  cytoplasm  and  nucleus  were  analyzed  with independent t-test and the correlation was analyzed with Pearson correlation.

Abstract

Pulmonary T uberculosis remains  as a major health problem in  South East Asia as well as Indonesia.  The failure  of  tuberculosis  treatment  is  mostly  due  to  drop-out  and  multiple  drugs  resistant.  Previous  study  showed that  Vitamin  D  increase  the  intracellular  cathelicidin  which  has  potency  to  kill  the  Mycobacterium tuberculosis. This research was aimed to identify the effect of Vitamin D as supportive treatment  on the improvement of sputum  conversion,  radiographic  finding  and  TNF-α  serum  concentration.  A  Double-blind randomized  control trial  was  used  in  two  random  allocated  patient  group  with  standard  tuberculosis  treatment.   The  first  group  was treated  with Vitamin  D (800  iu/day)  while the  other was not. Subjects were evaluated using three  parameters i.e.  sputum  conversion,  radiographic  finding  and  TNF-α  serum  concentration.  Compared to  the  group  without vitamin  D  supplementation,  group  with  vitamin  D  has  higher  proportion  of  sputum  conversion  in  the  first  month (67%    vs. 34%, p=0,04) and second month (95%  vs. 77%, p=0,18).    Improvement  in  radiographic  findings  in the  group with vitamin D is  also higher in  the  first  month (67%  vs. 18%, p=0,02) and second month (76%    vs. 45%,  p=0,06).    There is  a  significant  difference  in  the  level  of  25-dihidroxyvitamin  D between  two  groups (p=0,00). However,  there  is  no significant difference in  the  level  of TNF-α between two  groups (p=0,76) after two  months.

Abstract

Low  power  of  Far-Infrared  Rays  (FIR)  emitted  by  artificial  radiators  have  been  used  for  many  diseases associated with blood vessels disorder.  FIR also used to  regulate  temperature  in  incubator  for  infant.  This study aims at exploring correlation pattern between FIR and temperature,  producing a biological simulator model from  a  non-biological  material for  further  research  can  be  initiated.  Differentiation  factors  between biological  and  non-biological  material  that  exposed  to  FIR  also  could  be  determined.  A  radiant  far-infrared  type T -FSR  was placed 20 cm  above the  objects (umbilicus  and gel). Then both surface  and inside  temperature  of the  objects  measured  continuously  using  Thermistor  series  D-S06  and  series  D-RB2.  Room  humidity maintained at constant levels  (60%).  This study found  a significant correlation between FIR and temperature increasing  both on the  umbilicus and on the  gel. Influence  of FIR intensity  on temperature  can be formulated into  the  formula:  T_umb_U1  =  27,6  +  0,00554  FIR  and  T_umb_U2  =  27,8  +  0,00560  FIR,  while  on  the  gel  were T_gel_U1 = 24,9 + 0,00430 FIR and T_gel_U2 = 25,0 + 0,00428 FIR.

Abstract

Resitance  to  steroid  therapy  is  a  poor  prognosis  sign  for  nephrotic  syndrome,  but  this  mechanism is  still unclear. NF-κB has been reported to affect the glucocorticoid at the T cell through the transrepression in  the glucocorticoid  receptor .  Until  know,  this  study  was  still  limited.  The  aim  of  this  study  was  to  show  the  difference expression  and  activation  of  NF-κB  p65  subunit  between  childhood  SRNS  and  SSNS  patient.  Cross  sectional study  was  conducted  during  January  2009  to  December  2009  in  the  Saiful Anwar  General  Hospital  and Biomedic  Laboratory  Brawijaya  University.  Expression  and  activation  of  NF-κB  p65  subunit  were  determine  by immunochemistry  staining  by  counting  the  amount  of  lymphocyte  that  express  and  activate  NF-κB  p65  subunit among  200  lymphocytes  under  light  microscope  with  1000  magnification.  Results  was  analyzed  by independent  t  test  with  95%  CI  (p<0,05)  and  Pearson  correlation.  The  result  of  this  study  revealed  no significant  difference  of  expression  of  NF-κB p65  subunit  between  SRNS and  SSNS  (p=0,153),  but  there  was an increase activation of NF-κB p65 subunit in the SRNS patients (p=0,000).

Abstract

Decrease of lymphocyte  apoptosis is  one factor  that  leads  to  chronic airway inflammation  in  allergic asthma. Previous  studies  revealed  the  role  of  interleukin  (IL)-4  in  preventing  lymphocyte  apoptosis;  however  there  are few  studies  about  the  role  of  lymphocyte-Bax  and  its  relationship  with  IL-4  in  asthma.  The  aim  of  the  study  is  to demonstrate IL-4 role in preventing lymphocyte apoptosis via lymphocyte-Bax inactivation in bronchioli and lungs  of  asthmatic  mice.  It  is  a  randomized  control  group  experimental  study .  The  subject  was  Balb/c  mice  that categorized  into  2  groups:  asthma  and  non-asthma.  Asthma  group  was  sensitized  by  ovalbumin intraperitoneally  in  day  0  and  14,  followed  by  inhalation  every  2-3  days  for  6  weeks.  In  week  8,  all  of  mice were terminated.  The  IL-4  and  lymphocyte-Bax  expression  was  examined  through  immunohistochemistry  method, whereas  lymphocyte  apoptosis  by  TUNEL  method.  Independent  sample  t-test,  Mann  Whitney  U  test, regression  analysis,  and  path  analysis  was  used  in  statistical  analysis  with  confident  interval  95%.  The bronchioli and lungs specimens were obtained from 18 mice (9 in each group). Lymphocyte apoptosis was similar between 2 groups (p=0.1 16), additionally lymphocyte-Bax  decreased in  asthmatic group (p=0.003). This  indicated  low  activity  of  lymphocyte  apoptosis  in  asthmatic  group.

Abstract

Mastocyt  degranulated  can  cause  vasodilatation  of  blood  vessels,  but  it  is  unclear  whether  there  is  a relationship  between the  number of mastocyt with a diameter of blood vessels. This study aims to  determine the  relationship  between the  number of mastocyt pherivascular and diameter of blood vessels.  This research is  in  vivo  exploratory  research  in    Wistar  rat  mesentery which was sensitated  with ovalbumin.  The  diameter  of blood  vessels  at  mesentery  preparate  were  measured  microscopically  using  a  micrometer  scale  standards.  At large  diameter  of  blood  vessels   (2.860  ±  0.348),  the  number  of  mastocyt was 12.3  ±  2.3.  At  medium diameter of blood vessel (1.276 ± 0.198), the  number of mastocyt was 4.4 ± 0.5. Blood vessels with small diameter (0.961 ± 0.043), the  number of mastocyt was 3.5 ± 1.3. Calculations area on the  microscope visual field  was divided into  four  areas (counting area), there  were zone 1, zone 2, zone 3 and zone 4. Statistical analysis showed  that  the  correlation  between  the  number  of  mastocyt  at   zone  1  and  the  diameter  of  blood  vessels  was 0,904 at 1% significance level.  Similar to  that  at zone 2 (r=0,892),  zone 3 and zone 4 (r=0,940).  We conclude there  was  a  significant  relationship  between  the  number  of  mastocyt  and  the  diameter  of  blood  vessels. Keywords:  mastocyt  pherivascular,  diameter  of  blood  vessels

Abstract

Nutritions  plays  an  important  role  in  the  incidence  of  atopic  both  in  children  and  adults.  Several  recent guidelines  recommend  a  delay  of  solid  foods  to  prevent  atopic  disease.  However,  until  now,  there  is  still  much controversy  about  the  influence  a  delay  of  solid  foods  in  children  to  develop  atopic   disease.  This  study  aimed to  determine  the  impact  of  the  start  time  of  solid  foods  to  atopic  children.  A  case-control  study  was conducted in  outpatient  institutions  Pediatrics RSSA  Malang from  May 2008  until  May 2009.  Respondents were divided into two groups: atopic (cases) and non atopic (control) based on history of atopic disease. The age of first given  solid  food  and  familial  atopic  were retrieved  based  on  history .  Statistical analysis  using  Chi-square test at 95%    level  of confidence,  was conducted  to  determine the  impact  of the  start  time  of solid  food  to  the incidence  of  atopic.  There were 236  patients  divided  equally  into  atopic  and  control  group.  Giving solid  foods at 4-6 months of age will increase  the  risk  of atopic compared  with age> 6 months (OR  2.8 (1.29  to  6.07), p = 0.007).  The  results  of  this  study  support  the  recommendations  given  by  WHO, that  the  postponement  of  solid food  until  age  6  months  would  reduce  the  incidence  of  atopic  risk  in  children.Keywords  :  postponement  ,  solid  food,  atopic  diseases,  sensitization

Abstract

We report  one female  newborn baby suffered from  moderate asphyxia, meconium aspiration syndrome, and Hirschsprung  disease.  The  patient  admitted  in  Neonatal  Intensive  Care  Unit  (NICU)  and  ventilated  with Pressure Controlled Mandatory Ventilation (PCMV)  mode ventilator.  Patient suffered from  pneumodiastinum and therefore  mediastinotomy was performed. Sepsis and Necrotizing Enterocolitis (NEC)  made the  case more  complicated.  On  further  examination,  abdominal  was  distended  and  perforated  and  therefore  we decided  to  perform  exploration  laparatomy  and  sigmoidoctomy .  Anaesthesia  management  with  general anaesthesia  had  been  performed.  Premedication  was  not  given.  We  used  oxygen  and  Sevofluran  for induction;  Oxygen, Sevofluran, Atracurium, Fentanyl,  and  Morphin for  maintenance; and  Morphin for  post operative analgesic. On follow up examination, we found wound disruption and therefore relaparotomy was performed.  T ypes  and  doses  of  anesthesia  drugs  were  chosen  according  to  newborn  immature  organ  noticely . Providing adequate oxygenation, preventing hypotermi, and balancing electrolytes and acid base states still be the  most considerations in  management of this  patient.  Anesthetic management and intensive  care of this patient  gained  a  satisfying  outcome.  The  prognosis  was  good. Keywords:  Hirschsprung  disease,  meconium  aspiration  syndrome,  pneumomediastinum,  sepsis,  NEC.

Abstract

Airway  remodeling  is  an  important  thing  in  asthma.The  increase  of  airway  smooth  muscle  mass  is  a prominent  feature  of  asthmatic  airway  remodeling.  The importance  role  of  airway  smooth  muscle cell  (ASM) + apoptosis in airway remodeling is unknown.T cell also had important role in asthma, whereas CD4 T cell +role  in  airway  remodeling  has  already  known,  but  how  CD8   T cell  role  in  airway  remodeling  is  still  unclear.  A randomized  control  group experimental study  used Balb/c mice, that  categorized into  2 groups: asthma and control.  Asthma group was sensitized by ovalbumin intraperitoneally  in  day 0 and 14, followed  by inhalation +every  2-3  days  for  6  weeks.  In  week  8,  all  of  mice  were  terminated.   The  expression  of  CD8   T  cell  lymphocyte was  examined  through  immunohistochemistry  method,  whereas  ASM  apoptosis  by  TUNEL  method. Independent sample t-test and spearman test was used in statistical analysis with confident interval 95%. The  bronchioli  and  lungs  specimens  were  obtained  from  18  mice  (9  in  each  group).  Case  goup  had  significant +increase  in  the  amount of ASM apoptosis and expression of CD8   T cell lymphocyte  (p=0.000;  p=0.001). +There was also positif correlation between  ASM apoptosis and expression of CD8   T cell (r=0.37,  p=0.065). +We  conclude  that  increasing  ASM  apoptosis  has  a  relationship  with  increasing  CD8   T  cell+Keywords:  asthma,  mice,  ovalbumin,  ASM  apoptosis  and  CD8   T  cell
