Abstract

Type 2 diabetes mellitus (T2DM) is a chronic metabolic disorder characterized by hyperglycemia caused by insulin resistance and impaired insulin secretion. This study aimed to analyze the effectiveness of combination therapy with metformin and Sodium-Glucose Cotransporter-2 (SGLT2) inhibitors on glycemic control compared with metformin monotherapy. An analytical observational cross-sectional design with a retrospective approach was applied to 120 T2DM patients treated at the Endocrinology Clinic of Mataram General Hospital from January to December 2024. Medical record data included HbA1c, fasting plasma glucose, body mass index (BMI), and lipid profile. Data were analyzed using an independent t-test and multivariate linear regression. The results showed a mean HbA1c reduction of 1.42 ± 0.38% in the combination group, greater than the 0.87 ± 0.31% reduction in the monotherapy group (p < 0.05). The combination group also showed reductions in fasting plasma glucose and body weight without an increased risk of hypoglycemia. Multivariate analysis identified combination therapy as an independent predictor of improved glycemic control (β = 0.62; p = 0.001). Thus, metformin and SGLT2 inhibitor combination therapy was more effective than metformin monotherapy in improving glycemic control in patients with T2DM.

Abstract

Low fruit and vegetable consumption among adolescents remains a major public health concern worldwide, including in Indonesia, and contributes to an increased risk of non-communicable diseases later in life. The rapid advancement of digital technology offers opportunities to develop nutrition education strategies that are more engaging, interactive, and compatible with adolescents' learning preferences. This study aimed to examine the effectiveness of digital media-based nutrition education in improving fruit and vegetable consumption behavior among senior high school students. A quasi-experimental study using a one-group pretest-posttest design was conducted involving 100 students selected through purposive sampling. The intervention consisted of digital nutrition education delivered through educational videos, interactive infographics, and daily reminder messages over a four-week period. Data were collected using a validated and reliable questionnaire assessing fruit and vegetable consumption behavior and were analyzed using a paired t-test with a significance level of 0.05. The findings demonstrated a statistically significant improvement in students' fruit and vegetable consumption behavior following the intervention (p<0.001). Digital nutrition education effectively enhanced adolescents' knowledge, attitudes, and healthy eating behaviors. These findings provide empirical evidence that integrating digital media into nutrition promotion programs represents an innovative and effective strategy for improving healthy dietary behaviors among adolescents in school settings.

Abstract

Hypertension remains a major public health problem because it requires long-term therapy, risk-factor control, and sustained medication adherence, while patients in urban primary care frequently experience difficulties in understanding health information. This study aimed to examine the relationship between health literacy and medication adherence among patients with hypertension in urban community health centers. An analytic observational study with a cross-sectional design was conducted among 214 hypertensive patients receiving routine care at three urban primary health centers. Data were collected using a respondent characteristics form, a validated and reliable health literacy instrument, and an antihypertensive medication adherence scale. Data were analyzed using chi-square tests and multivariable logistic regression. The findings showed that 57.9% of respondents had limited health literacy and 46.3% had low medication adherence. Patients with adequate health literacy were more likely to adhere to antihypertensive medication than those with limited health literacy (AOR = 2.41; 95% CI: 1.36–4.27; p = 0.002) after controlling for age, education, duration of hypertension, and comorbidity. This article argues that health literacy is a modifiable determinant of medication adherence in hypertension management at the primary care level. The study contributes empirical evidence for strengthening health communication interventions, simplifying medication information, and designing literacy-sensitive patient education strategies in urban primary care settings.

Abstract

Type 2 diabetes mellitus is a chronic disease with a steadily increasing prevalence that requires optimal medication adherence to prevent long-term complications. The rapid expansion of digital health technologies has improved public access to health information, yet differences in digital health literacy may influence patients' ability to obtain, evaluate, and apply health information to support medication adherence. This study aimed to examine the relationship between digital health literacy and medication adherence among patients with type 2 diabetes mellitus attending primary healthcare centers. A quantitative analytical study with a cross-sectional design was conducted involving 180 patients selected through consecutive sampling. Data were collected using the Digital Health Literacy Instrument (DHLI) and the Morisky Medication Adherence Scale (MMAS-8). Statistical analyses included the Chi-square test and multivariable logistic regression with a significance level of 0.05. The findings indicated that patients with higher digital health literacy were significantly more likely to demonstrate good medication adherence than those with lower literacy levels (p<0.001). Furthermore, digital health literacy remained an independent predictor of medication adherence after adjustment for age, educational level, and duration of diabetes. These findings highlight the importance of strengthening digital health literacy in primary healthcare settings to improve diabetes self-management and support the development of evidence-based digital health education programs for patients with chronic diseases.

Abstract

Fruit and vegetable consumption among Indonesian adolescents remains below the recommended dietary guidelines, increasing the risk of various health problems later in life. The rapid development of digital technology has created opportunities to utilize interactive and accessible educational media to improve adolescents' nutritional knowledge and healthy eating behaviors. This study aimed to examine the effectiveness of digital media-based nutrition education in improving fruit and vegetable consumption among senior high school students. A quantitative approach with a quasi-experimental one-group pretest-posttest design was employed. The study was conducted at a public senior high school in Yogyakarta City, Indonesia, involving 100 students selected through purposive sampling. The intervention consisted of digital nutrition education delivered through educational videos, interactive infographics, and daily digital reminders over a four-week period. Data were collected using a validated and reliable questionnaire assessing fruit and vegetable consumption behavior. Data were analyzed using a paired-samples t-test with a significance level of 5%. The findings demonstrated a statistically significant improvement in fruit and vegetable consumption behavior following the intervention (p < 0.001). The mean behavioral score increased from 61.84 ± 8.72 before the intervention to 76.91 ± 7.95 after the intervention. These results indicate that digital media-based nutrition education is effective in promoting healthier fruit and vegetable consumption among adolescents. This study provides empirical evidence supporting the integration of digital educational media into school-based health promotion programs as an adaptive, efficient, and sustainable strategy for improving adolescent nutritional behavior.
