Abstract

In the case of advanced stage gynecological disorders, most patients have a disturbance in the upper urinary tract. Most patients with uterine cervical carcinoma are already in stage 3B and require urine diversion. The purpose of this study is to find and determine the profile of gynecological disorders that require urine diversion, the type of urine diversion, and urine diverted output in patients with cervical carcinoma. This descriptive analytical retrospective study was conducted at Dr. Soetomo Hospital, Surabaya, Indonesia, between 2012 and 2015. The samples of this study were all patients with gynecological disorders who underwent retrograde DJ stent procedures, underwent ureterocutaneostomy and nephrostomy at the Integrated Central Surgery Building, Dr. Soetomo Hospital, Surabaya, for three years between 2012 and 2015. Data was collected based on the medical record number of patients with gynecological disorders who underwent urine diversion procedures and were recorded in the book on the operation schedule. We find that the most common cause of gynecological abnormalities in kidney obstruction is cervical carcinoma. There are three types of urine diversions used, namely DJ stent, percutaneous nefropyelostomy, and ureterocutaneostomy, with DJ as the most commonly performed. Urine diversion can improve creatinine without being influenced by the type of diversity.

Abstract

Human papillomavirus (HPV) infection is one of the most common sexually transmitted infections. HPV high risk (HPVHR) were HPV16,18 related with invasive penile carcinomas, and HPV low risk (HPVLR) were HPV6,11 related to anogenital warts. Male infection is usually asymptomatic that it would be explain increasing the incidence of HPV associated cancers. Identification HPV genotype is very important for predicting the development of the diseases, to be benign or malignant cancer. The objective of this study was to identify the genotype of HPV that infect men with anogential warts. This research used 12 biopsy specimens from men patient with anogenital warts at Outpatient clinic of Department Dermatology and Venereology, Dr. Soetomo General Hospital period 2016-2017. The specimens were diagnozed by pathologist and HPV gentoyping was done to detect 40 HPV genotype including HPVHR and HPVLR. The result showed that 58% (7/12) were positive for HPVLR and 42% (5/12) were positive for HPV LR/HR. The genotype HPV that infected men patient with anogenital warts is HPVLR (HPV6,11) and HPVHR (HPV18,51,52,82) with single infection of HPVLR or mutiple infection HPVLR/LR or HPVLR/HR. The infection of HPVHR would be develops to be malignant transformation. It suggested that HPV genotype needs to be checked the for the anogenital warts cases for predicting the development of the diseases.

Abstract

Head and neck reconstruction serves two purposes, namely to restore function and aesthetics. The priority goals for lip reconstruction should be determined before surgery. In cases of large lip defects, maintaining the goal makes lip reconstruction difficult. This is a case report of a 66 year old man who came to ORL-HNS OPD, Dr. Soetomo Hospital, Surabaya with the main complaint of ulcers on the upper lip since one year ago accompanied by pain. A mass biopsy result on the right vestibule in Malang on October 11, 2016 showed adenosquamous cell carcinoma. The patient planned to have a wide excision and reconstruction of the lip defect. Wide excision and lip reconstruction was performed on 15 September 2016. Total resection of the upper lip was performed up to the vestibule of the nose. The results showed that squamous cell carcinoma was poorly differentiated. The evaluation after myocutaneous flap surgery was well combined, there were no signs of inflammation or infection. Patients can open their mouths, eat, drink and speak without complaint. The patient then received regular chemoradiotherapy two weeks after surgery.

Abstract

The phenomena of increasing total IgE level in Soil Transmitted Helminthiasis and its protective role in resistance of infection remains unclear. The purpose of this study was to determine the correlation between total IgE and the intensity of Soil transmitted helminthiasis among farmers in these area. Total 162 farmer, age ranged 25-80 years participated in this cross sectional analytic study. Kato-Katz method was used to diagnose Soil Transmitted Helminthiasis and to determined infection intensity based on egg per gram (EPG). Total IgE levels were measured by enzyme linked flourescent assay (ELFA). The results of this study showed that 13,56% farmer positive for Soil Transmitted Helminthiasis, with single infection of Ascaris lumbricoides were 1,85%, 9,26% for Trichuris trichiura, 0,61% for Hookworm and mixed infection that were 1,23% for A.lumbricoides+T.trichiura 0,61% for A.lumbricoides+Hookworm. The intensity of infection were in mild category. Total IgE level in Soil transmitted Helminthiasis group was significantly different compared with noninfected group (p<0.05, t-test). There was significant correlation between EPG and total IgE level (r=0.667, p=0.001, Spearman test). As a conclusion total IgE level was significantly correlated with EPG, this fact can be used to develop management of Soil Transmitted Helminthiasis theraphy.

Abstract

Anemia in patient with chronic kidney disease could cause a lot of complication. The first line therapy of this condition is by treating with erythropoiesis-stimulating agents (ESA) or called erythropoietin. The erythropoietin alpha and beta were two types of the human recombinant erythropoietin that are usually used in Indonesia. The aim of this study was to determine the effectivity of erythropoietin alpha compared to erythropoietin beta especially in haemoglobin and haematocrit level. This prospective observational study was conducted in March – September 2016. The inclusion criteria were CKD stage 5 patients with a minimum of 3 months of regular hemodialysis, Hb <10 g/dL with enough iron status ST > 20% and FS > 200ng/mL. The methology of this study had been approved by the Health Research Ethics Committee of the Bhayangkara H.S. Samsoeri Mertojoso Hospital, Surabaya. Patients received 2000 IU subcutaneous erythropoietin twice a week on both groups. Blood sample was withdrawn in pre-treatment and after 4 weeks of post erythropoietin therapy treatment for measurement of haemoglobin and haematocrit. Target for this erythropoietin therapy are increase of Hb 0.5 – 1.5 g/dL (not to exceed 12 g/dL) and increase of Hct level 2 – 4 % in 4 weeks. Based on the inclusion criteria, there were 20 patients in this study (10 patient each of both erythropoietin alpha either beta group) that consist of 7 women and 13 men. After the treatment, the mean of increased haemoglobin level for erythropoietin alpha group was 1.28 ± 0.80 g/dL (p=0.001) and erythropoietin beta was 0.37 ± 0.95 g/dL (p=0.254). The mean of increased haematocrit level for erytropoietin alpha group was 3.56 ± 3.46 % (p=0.010) and erythropoietin beta was 1.34 ± 2.71 % (p=0.152). In comparison of haemoglobin and haematocrit achievement in both groups showed that erythropoietin alpha gave better achievement in haemoglobin parameter (p=0.033), but there were no differences in both groups on haematocrit parameters (p=0.127).

Abstract

Diabetes mellitus (DM) is one of disease that its incidence increases every year worldwide. The condition of DM can cause various complications caused by oxidative stress. Stichopus hermanii (SH) or golden sea cucumber extract contains antioxidant compounds that have been proven to reduce oxidative stress conditions. The purpose of this study was to investigate the effect of Stichopus hermanii extract on condition of diabetes mellitus by looking at changes in fasting blood glucose, plasma insulin, and malondialdehyde levels in animal models of Wistar rats. This study was a laboratory experimental study using Randomized Control Trial Design with Post-test only control group design. Thirty-five male Wistar rats divided into five groups, i.e normal control group, positive control, negative control and two treatment groups with SH extract dose 8.5 and 17 mg / kgBW for 14 days once daily after induction of Streptozotocin at the Biochemistry Laboratory of the Faculty of Medicine, Airlangga University. Fasting blood glucose level was measured by a glucometer, plasma insulin measured by ELISA and MDA level was measured by a spectrophotometer. Data were analyzed statistically by using One Way ANOVA test and Kruskal Wallis. There were significant results of SH extract can reduce fasting blood glucose (Kruskal Wallis, p=0.030) and MDA (Kruskal Wallis, p=0.042) but not in plasma insulin (ANOVA, p=0.130). The lowest MDA level occurs in the K4 group that given SH extract dose 17 mg/kg BW than another experimental group. As the conclusion, this study showed SH extract can decrease fasting blood glucose and oxidative stress in diabetic-induced rats.

Abstract

Foreign objects may be present in the bladder due to deliberate insertion or displacement of the surrounding organs. There have been multiple cases of sharp objects such as nails, hair pins, and screws, as well as large objects such as AAA batteries, toothbrushes, and ballpoint pens found in the urethra and the bladder, which must have been aimed to induce sexual satisfaction associated with mental and psychic disorders. The diagnosis was established by means of radiological examinations, ie thoracic x-ray, KUB, and urethrography. This article reported a 32-year-old man with headset inserted to penile until bladder and could not be pulled out again. The patient complained pain and bleeding out of the urinary tract. We performed vesicotomy and pulling the out headset from bladder. The operation was succesful. In conclusion, the case of a headset in the bladder has been treated successfully with vesicotomy.
