Abstract

Abstract

Maternal mortality in Indonesia remains high, with rural and remote regions disproportionately affected due to systemic barriers in emergency care. Objectives: This study explores the experiences and challenges of midwives providing emergency maternal care in East Kalimantan, focusing on systemic barriers, clinical autonomy, and professional dedication. Methods: A qualitative exploratory design was employed, involving semi‑structured one‑on‑one interviews conducted online via Zoom with eight midwives working in geographically isolated areas. Data were analyzed using Braun and Clarke’s thematic analysis, supported by NVivo coding. Results: Three overarching themes emerged: (1) systemic barriers and geographical isolation that delay referrals and emergency responses, (2) autonomous clinical decision‑making under resource constraints, and (3) the ethos of dedication and expanded professional roles beyond standard midwifery practice. Midwives reported reliance on poor transport routes, minimal equipment, and lack of supervision, often making rapid clinical decisions without institutional support. Despite these constraints, they demonstrated resilience and commitment, frequently assuming roles beyond maternal care, including general health services and community leadership. Conclusion: Midwives in rural East Kalimantan play a critical role in bridging systemic gaps in emergency maternal care. Their experiences highlight the urgent need for policy reforms to strengthen referral systems, improve emergency transport, expand continuing professional development, and ensure equitable distribution of doctors and specialists. Addressing these challenges is essential to reduce maternal mortality and achieve health equity in Indonesia’s rural communities.

Abstract

Introduction: Infant massage provides various benefits for infants, including supporting growth, improving sleep quality, and strengthening mother–infant bonding. Mothers’ knowledge may influence how regularly infant massage is practiced. Novelty: This study examines the relationship between mothers’ knowledge of infant massage benefits and the frequency of practice in Medan Sunggal. Its novelty lies in emphasizing the consistency of infant massage practice within a community setting, an aspect rarely addressed in studies. Methods: This study used a correlational design with a cross-sectional approach. The population consisted of all mothers with infants in Medan Sunggal, totaling 94 people. The sampling technique used was purposive sampling, with 48 respondents who met the inclusion and exclusion criteria. Data were collected using questionnaires and analyzed using the chi-square test. Results: The results showed that mothers with good knowledge who regularly performed infant massage were 4.2%, while those who did not perform it regularly were 4.2%. Mothers with sufficient knowledge who regularly performed infant massage were 22.9%, while those who did not perform it regularly were 27.1%. Meanwhile, mothers with poor knowledge who regularly performed infant massage were 4.2%, while those who did not perform it regularly were 37.5%. The statistical test showed an Asymp. Sig. (2-sided) value of 0.027 < 0.05, indicating a significant relationship between mothers’ knowledge of the benefits of infant massage and the frequency of infant massage practice. Conclusion and Suggestions: Mothers should improve their knowledge of infant massage, while health workers should provide regular education to support correct and consistent practice.

Abstract

Background: Emesis gravidarum commonly affects women during the first trimester and may impair nutritional status and quality of life. Ginger and lemon are widely used as complementary therapies, although comparative evidence remains limited. Objective: To compare the effectiveness of standardized ginger decoction and fresh lemon juice in reducing emesis gravidarum. Methods: A quasi-experimental pretest-posttest two-group study was conducted among 30 first-trimester pregnant women at an independent midwifery practice in Medan, Indonesia. Participants received either ginger decoction (n = 15) or fresh lemon juice (n = 15) once daily for four consecutive days. Emesis severity was assessed using the Pregnancy-Unique Quantification of Emesis and Nausea (PUQE-24) questionnaire. Results: Both interventions significantly reduced PUQE scores (p < 0.001). The reduction was greater in the ginger group (5.60 points) than in the lemon group (3.87 points) (p < 0.001). Conclusion: Both therapies effectively reduced emesis gravidarum, with ginger decoction demonstrating superior clinical effectiveness and greater potential as a complementary therapy during early pregnancy.
