Abstract

Exposure to all pesticide classes can have a disruptive effect on metabolism and energy storage. The bioindicator that can be used to identify farmers exposed to pesticides is acetylcholinesterase (AChE). AChE can be a good indicator for changes in body composition due to its associations with various metabolic and physiological parameters. Abnormal AChE levels stimulate nicotinic and muscarinic receptors in various organs, causing a wide range of metabolic disorders, including changes in the body composition. This study aimed to examine the correlation between long-term pesticide exposure and potential alterations in body composition. This research was conducted in Sagarahiang village, one of the highest vegetable producers in Kuningan, West Java onJuly-August 2024. And it was a cross-sectional study of 46 farmers who sprayed pesticides in vegetable areas and was determined by purposive sampling. Data collected through interview using questionnaires and some tools, like indicators of pesticide exposure were measured by cholinesterase serum and history of exposure, whereas parameters of body composition were measured by bioelectrical impedance analysis (BIA) using a body fat monitor with eight electrodes. Data analysis using the Pearson correlation test. The percentage of subjects with abnormal AChE activity was four persons (8,9%). AChE levels had no correlation with all components of the exposure history, but had a significant correlation with body composition parameters (p<0,05), body weight (p=0,027), Body Mass Index (BMI) (p=0,020), total fat (p=0,038), visceral fat (p=0,030), and resting metabolism (p=0,037) with a weak correlation (R 0,31-0,34). Based on this study, AChE is not strong enough to assess changes in body composition clinically.

Abstract

Latar Belakang : Pruritus vulvae merupakan keluhan rasa gatal pada area genital perempuan yang sering terjadi pada remaja putri, terutama saat menstruasi akibat kurang optimalnya personal hygiene dan rendahnya tingkat pengetahuan reproduksi. Berdasarkan studi pendahuluan di SMA Negeri 1 Darma terhadap 10 siswi, sebanyak 9 orang mengalami keluhan pruritus vulvae. Penelitian ini bertujuan untuk mengetahui hubungan antara personal hygiene, pengetahuan kesehatan reproduksi, dan keluhan pruritus vulvae pada remaha putri. Metode : Penelitian ini merupakan penelitian kuantitatif dengan desain cross sectional yang dilaksanakan pada Kamis, 30 April 2026 di SMA Negeri 1 Darma. Populasi penelitian berjumlah 361 siswi dengan sampel sebanyak 186 responden yang dipilih menggunakan teknik purposive sampling. Instrumen penelitian menggunakan kuesioner tertutup yang diadaptasi dari Sulaikha (2018). Data primer diperoleh melalui pengisian kuesioner secara mandiri oleh responden, sedangkan data sekunder diperoleh dari data administrasi SMA Negeri 1 Darma. Analisis data dilakukan secara univariat dan bivariat menggunakan uji korelasi Spearman Rank. Hasil : Sebagian besar responden memiliki personal hygiene kategori cukup (81,7%), hampir seluruh responden memiliki tingkat pengetahuan reproduksi kategori baik (95,2%), dan sebagian besar mengalami keluhan pruritus vulvae kategori berat (68,3%). Terdapat hubungan yang signifikan antara personal hygiene dengan keluhan pruritus vulvae (p=0,017), sedangkan tidak terdapat hubungan yang signifikan antara tingkat pengetahuan reproduksi dengan keluhan pruritus vulvae (p=0,474). Simpulan : Terdapat hubungan yang signifikan antara personal hygiene dengan keluhan pruritus vulvae, namun tidak terdapat hubungan yang signifikan antara tingkat pengetahuan reproduksi dengan keluhan pruritus vulvae pada remaja putri di SMA Negeri 1 Darma 2026.

Abstract

Introduction: Long-term pesticide exposure can harm farmers. Metabolic syndrome (MetS) occurs due to metabolic disorders associated with the use of pesticides which act as diabetogenic and obesogenic agents. High pesticide use and diverse agricultural activities contribute to the increased incidence of MetS. This study aims to examine the associations between history of direct exposure (HDE) and history of indirect exposure (HIE) to pesticide and MetS, as well as to identify factors associated with MetS among farmers. Methods: This case-control study included 44 cases and 90 controls from 307 male vegetable farmers in Kuningan, Indonesia. Laboratory tests were used for the diagnosis of MetS and pesticide exposure history, which was measured using a questionnaire (coefficients = 0.30 to 0.60; p < 0.05; Cronbach's alpha = 0.718). Data analysis employed binary logistic regression, controlling for age and family history of metabolic disease by examining the Adjusted Odds Ratio (AOR) with a 95% confidence interval (CI). Results: HIE, defined as eating or drinking after spraying, was associated with the occurrence of MetS (p = 0.017; AOR = 3.677; CI: 1.262–10.710). HDE, characterized by watering activities, also showed a significant association (p = 0.010; AOR = 3.012; CI: 1.301–6.974). In total exposure analysis, both factors remained significant (respectively, p = 0.043; AOR = 3.282; CI: 1.041–10.349, and p = 0.043; AOR = 2.430; CI: 1.027–5.747). Conclusion: Frequent watering and the habitual consumption of food and/or beverages after spraying increase the likelihood of developing MetS. Both HIE and HDE significantly contribute to the onset of MetS.
