Abstract

Background : Infeksi pada diabetic foot ulcer menjadi salah satu faktor penghambat penyembuhan luka, penggunaan silver dalam sediaan krim dapat menjadi pilihan dressing primer. Objective : Dalam case study ini penulis melaporkan perkembangan luka infeksi pada diabetic foot ulcer dengan krim koloidal silver sebagai dressing primer. Case: Pria usia 60 tahun datang ke wocare 17 maret 2025. keluhan utama luka dikedua kaki muncul tanggal 11 februari 2025, awalnya lepuhan setelah berjemur didepan rumah jam 13.00 WIB 10 februari 2025. Kemudian berobat ke rumah sakit luka setiap hari dibersihkan namun tidak membaik. Luka tampak multiple pada kedua kaki. Kategori luka unstageable karena dasar luka tertutup slough. warna dasar luka kuning 40%, merah 60%. Eksudat banyak, jenis cairan serosa. Ditemukan eritema, edema dan area sekitar luka teraba hangat. manajemen luka menggunakan pendekatan TIME, yang dilakukan dengan 3M. Dressing primer menggunakan krim kolloidal silver. Balutan diganti setiap 3 hari. Conclusion : Silver dalam sediaan krim efektif untuk mengatasi infeksi pada diabetic foot ulcer. Tanda-tanda klinis infeksi pada luka seperti eritema, udem, dan hangat disekitar luka sudah tidak ditemukan saat evaluasi dilakukan.

Abstract

Breast engorgement is a common problem among postpartum mothers that often causes pain, disrupts comfort, and interferes with the success of exclusive breastfeeding. Non-pharmacological management using aloe vera gel compresses is believed to help reduce pain due to its active compounds with analgesic and anti-inflammatory properties. This study aimed to determine the effect of aloe vera gel compresses on breast engorgement pain in postpartum mothers at Sumber Waras Hospital in 2025. The study employed a pre-experimental one-group pretest–posttest design. The sample consisted of 10 postpartum mothers selected through purposive sampling. Pain intensity was measured using the Numeric Rating Scale (NRS), and data were analyzed using a dependent t-test. The results showed a decrease in the mean pain score after the application of aloe vera gel compresses: from 4 to 3 on the first day, from 3 to 2.10 on the second day, and from 2.20 to 1 on the third day. Statistical analysis revealed a p-value < 0.05 on all treatment days, indicating a significant effect of aloe vera gel compresses in reducing breast engorgement pain. In conclusion, aloe vera gel compresses are effective in reducing breast engorgement pain in postpartum mothers. This intervention can be applied as a safe, inexpensive, and practical non-pharmacological therapy to support the success of exclusive breastfeeding

Abstract

ABSTRACT Background: Post-cataract surgery infection is a complication that can hinder the healing process and reduce the quality of patient's vision. Discharge planning using audiovisual media is considered effective because it is able to present information in an interesting, clear, and easy to understand manner. Objective: to analyze the application of discharge planning using audiovisual media in improving patient knowledge about preventing post-cataract surgery infection. Method: This study used an Evidence-Based Nursing (EBN) design with a case study approach. The research subjects were 10 respondents who were post-cataract surgery patients at Hospital X. Data collection was carried out by measuring the level of knowledge of respondents before and after being given discharge planning using audiovisual media. Results: The paired t-test showed a significant difference between the level of knowledge before and after the intervention with a p value = 0.001 (p < 0.05). The average knowledge score increased from 25.60 ± 8.83 to 46.10 ± 3.81 with a mean difference of 20.50. Conclusion: Discharge planning using audiovisual media is effective in increasing patient knowledge regarding post-cataract surgery infection prevention and can be used as an innovative nursing education method.

Abstract

Background: Stroke is one of the main causes of disability which is often accompanied by limited mobility, thus increasing the risk of decubitus. Decubitus can prolong the treatment period if not prevented optimally. One of the nonpharmacological nursing interventions that can be used to prevent decubitus is Effleurage massage with olive oil. Objective: This study aims to determine the effect of Effleurage massage with olive oil on the risk of decubitus in stroke patients at the Bina Daksa Budi Bhakti 2 Social Home, West Jakarta Method: This study uses a quantitative method with a pre-experimental design through a one group pretest–posttest design. The population in this study is stroke patients at the Bina Daksa Budi Bhakti 2 Social Home, West Jakarta, with a sample of 5 respondents selected using purposive sampling techniques. Data collection was carried out by assessing the risk of decubitus using the Braden Scale before and after the administration of Effleurage massage with olive oil. Results: The results showed that before the intervention, Braden's average score was 12.00, with most respondents in the moderate to severe risk category. After being given Effleurage massage with olive oil, the average value of Braden's score increased to 17.40, and all respondents moved to the mild risk category. Result Paired Samples T-Test p-value 0,000 (p<0,05). Conclusion: Effleurage massage with olive oil has a positive effect on reducing the risk of decubitus in stroke patients.

Abstract

Background: Phlebitis is a common Healthcare-Associated Infection (HAI) related to the use of Peripheral Intravenous Catheters (PIVCs), particularly in Intensive Care Unit (ICU) patients. Critically ill patients often require prolonged intravenous therapy, increasing the risk of intravenous complications. The implementation of Intravenous (IV) Bundle Care is an evidence-based strategy to reduce phlebitis through adherence to standard procedures during insertion and maintenance. Objective: This study aimed to identify nurses’ compliance with IV Bundle Care during insertion and maintenance and to describe the incidence of phlebitis among ICU patients at Sumber Waras Hospital, Jakarta. Methods: This study used an Evidence-Based Nursing (EBN) case study design. The sample consisted of five ICU patients receiving intravenous therapy via PIVC. Data were collected over three days through direct observation using an IV Bundle Care checklist and the Visual Infusion Phlebitis (VIP) Score. Data were analyzed descriptively. Results: Nurses’ compliance with IV Bundle Care during insertion reached 100% across all components. During maintenance, 100% compliance was observed in hand hygiene, insertion site care with transparent dressings, assessment of PIVC necessity, and patient or family education. Infusion set replacement was performed based on clinical indications in 60% of patients. VIP Score assessment showed that 80% of patients had no phlebitis, while 20% experienced early-stage phlebitis. Conclusion: High compliance with IV Bundle Care was accompanied by a low incidence of phlebitis among ICU patients. Consistent implementation supported by documentation and monitoring is necessary to maintain patient safety.

Abstract

ABSTRACT Background: Electronic Medical Records (EMR) are a key component of digital transformation in healthcare services aimed at improving the quality and completeness of nursing care documentation. Complete nursing documentation supports clinical communication, quality assurance, and legal accountability in nursing practice. However, paper-based documentation systems frequently show incomplete records, particularly in the evaluation component of nursing care. Therefore, EMR implementation is expected to support more systematic and consistent documentation. Objective: This study aimed to identify the completeness of nursing care documentation before and after the implementation of Electronic Medical Records (EMR) and to analyze the impact of EMR on documentation completeness at Anggrek Mas Hospital. Methods: This study used an Evidence-Based Nursing (EBN) design with a case study approach. The sample consisted of five patient medical record files that met the inclusion criteria. Data were collected through document review using a nursing documentation audit checklist based on hospital standards. Descriptive analysis was conducted by comparing documentation completeness before and after EMR implementation. Results: Documentation completeness before EMR implementation ranged from 80–90%. After EMR implementation, completeness increased to 90–100%, particularly in nursing assessment, diagnosis, planning, and implementation. However, documentation of nursing care evaluation was not consistently recorded across all medical records. Conclusion: EMR implementation improves the completeness of nursing care documentation. However, strengthening evaluation documentation remains necessary to ensure optimal recording of all nursing process stages. Abstract

Background: Structured follow-up during the third trimester is believed to influence maternal choice of delivery facility, but empirical evidence remains limited. Objective: To analyze the relationship between third-trimester ANC follow-up (≥36 weeks gestation) and institutional delivery at Hospital X, West Java. Methods: A pre-experimental study with a one-group posttest-only design was conducted on 79 pregnant women with ≥36 weeks of gestation who attended outpatient ANC between April and June 2025. Data were analyzed using Chi-Square and Fisher’s Exact Test. Results: A total of 81% of participants delivered at Hospital X. Statistical analysis showed no significant association between ANC visit frequency (p = 0.365), nurse follow-up frequency (p = 0.365), or ANC follow-up intensity (p = 0.394) and place of delivery. However, there was a proportional tendency for women with higher visit and follow-up frequencies to deliver at the same facility where they received ANC. Conclusion: Although statistically insignificant, routine ANC follow-up during late pregnancy may strengthen institutional attachment and continuity of maternal care. This strategy should be considered in ANC program development to support institutional delivery utilization.

Abstract

Patient satisfaction is one of the key indicators for assessing the quality of hospital services and influences its competitiveness. This study aims to analyze the gap between patient expectations and reality regarding nursing services at the Eka Hospital Cibubur Polyclinic. This study is quantitative and uses the SERVQUAL (service quality) method to measure patient satisfaction based on five dimensions: Tangible, Responsiveness, Reliability, Assurance, and Empathy. The study sample used an accidental sampling technique obtained by 89 respondents participating by completing an online questionnaire. The results showed that the average patient expectation value was 4.52, while the average reality value was 4.39, resulting in a SERVQUAL gap of (-0.13). Based on the results, improvements are still needed related to services to increase patient satisfaction. However, the level of conformity between expectations and reality reached 97%, which indicates that overall, the service is in accordance with patient expectations.
