Abstract

Dyslipidemia or hyperlipoproteinemia is a quantitative change in the concentration of total cholesterol, its fraction, or just triglycerides in plasma. In addition, there is ample evidence to support that pediatric dyslipidemia is associated with atherosclerosis in adulthood. To reduce cardiovascular risk in adulthood, early identification and treatment is needed to reduce morbidity and mortality. Dyslipidemia itself can occur due to changes in primary lipoprotein metabolism due to genetic (primary) causes or other pathologies such as the result of accumulation of exogenous (secondary) factors. Therefore, mixed dyslipidemia results from genetic and environmental associations with risk factors for cardiovascular disease. However, lipid metabolism screening in pediatrics is not carried out in a targeted manner, causing problems where the outcome is not generally accepted. In addition, no one knows the effect of long-term pharmacological therapy. Therefore, this study aimed to review the various available literature resources on pediatric dyslipidemia to present data on a structured approach to pediatric dyslipidemia that focuses on screening, atherosclerotic risk stratification, and therapeutic approaches.

Abstract

Uterine fibroids are benign gynecologic tumors that develop from the myometrium and become entangled in the majority of women. The available pharmacological treatments are useful for reducing the size of the fibroids and relieving the symptoms of uterine fibroids. However, these pharmacological agents are said to be unaffordable for most people and are also associated with considerable side effects. Therefore, botanical medicines stole the attention in the last decade because of their therapeutic effectiveness accompanied by affordable costs and comparable and stronger therapeutic efficacy, with fewer side effects. The aim of this review is to provide information regarding the mechanisms by which various botanical drugs possess uterine anti-fibroid activity. The source search method is carried out through several trusted websites such as Pubmed, Science Direct, and Google Scholar. Based on the literature that has been identified, five botanical drugs were found which will be discussed in this review. These botanical drugs will be explained their mechanism of action in carrying out uterine anti-fibroid activity which has the potential as an alternative treatment for uterine fibroids.

Abstract

The Occupational Health and Safety sector is currently in training methods to minimize the risk of injury and increase safety at work. Various work sectors have implemented a more active method of delivering training materials, namely through the application of digital games. Digital games are considered successful in some sectors of work, but are limited in agriculture. This is because in the agricultural sector this tends to occur with the same movement and flow. Therefore, this literature aims to understand how to deal with various occupational risks as long as game-based safety in the field can be applied in the agricultural sector. Literature search through Pubmed, Scopus, PsycINFO, and Web of Science with a focus on “serious games” and “gamification” as safety training methods. As a result, digital games that can be applied are through computer technology, with games in the form of increasing levels, points, and challenges being the most popular attraction. During the game, the positive feedback generated and after the game is proven to improve the skills and safety of workers. Therefore, digital games can be developed as an effective, satisfying, and interesting training method for workers in the agricultural sector.

Abstract

COVID-19 pandemic has gone through for more than two years, and it has taking millions of lives and has given a catastrophic damage globally in almost every aspect of life. The disease that is caused by SARS-CoV-2 virus is a novel disease that attack respiratory system in a swift manner. Even though respiratory system is the main concern of the disease, in the nature of its own disease, COVID is a systemic inflammatory disease and easily could make a different type of the symptoms in any other system of the body, not limited to respiratory system. It also gives a cutaneous manifestation with a lot of different lesion, but not handled properly and being overlooked. Until now, there is no concrete report of manifestation of cutaneous in COVID-19, this is one reason why clinician kept getting overlooked and do not giving patient a thorough and comprehensive therapy.  The importance of report in cutaneous manifestation in COVID-19 patient will make clinician easier to notice the manifestation and could help them to make a proper comprehensive therapy for patient. Giovani et al has proposed a group of cutaneous manifestation of COVID-19, characterized by its own lesion, the location, pathophysiology and incidence. This grouping of manifestation hopefully can help clinician to identify cutaneous manifestation in COVID-19.

Abstract

yspepsia is one of the health problems that are often encountered in daily practice. Dyspepsia was defined as having one or more symptoms of epigastric pain, burning, fullness after eating, or early satiety. Bloating and nausea are common with dyspepsia but are not specific. Potential lifestyle factors associated with dyspepsia include smoking, alcohol, and consumption of analgesics. Furthermore, dietary habits that include consumption of fast food, salty foods, coffee/tea, and spicy foods are associated with aggravating dyspepsia symptoms and lack of physical activity may contribute to the onset of dyspepsia. In addition, the stress factor is also something that must be overcome because it is one of the trigger factors and also aggravates the condition of dyspepsia patients. This study is a case report. Primary data were obtained through history taking (autoanamnesis and alloanamnesis from family and patients), physical examination, supporting examinations and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Application of family doctor services based on evidence based medicine to patients by identifying risk factors, clinical problems, and patient management based on a patient problem solving framework with a patient centered approach and a family approach. A 56-year-old female patient came to the Kemiling Health Center with complaints of abdominal discomfort such as pain and a lump in the gut, accompanied by nausea and headache that had worsened since 2 days ago. Complaints are felt to come and go but get worse if the patient is late to eat. Similar complaints have been felt to have disappeared since 2017. Management of functional dyspepsia patients in a holistic and comprehensive manner, patient centers, family approved with functional dyspepsia treatment regularly according to EBM is needed in order to eliminate symptoms, trigger factors and change patient behavior so as to prevent complications and recurrence of the disease.

Abstract

Ovarian cystadenoma is a benign neoplasm originating from ovarian epithelium, including serous cystadenoma, mucinous cystadenoma, endometrioid, transitional clear cells (Brenner), and stromal epithelial cells. These epithelial tumors account for 60-80% of all ovarian neoplasms. Symptoms experienced by patients with ovarian cystadenoma are quite varied, depending on the type of cyst itself. The management of ovarian cystadenoma is surgical cystectomy with thorough exploration of the intrapelvic and abdominal organs. Further management is usually carried out with histopathological examination to determine the presence or absence of malignancy. Mrs. M, a 57-year-old woman was referred to Abdul Moeloek Hospital with a suspected diagnosis of ovarian neoplasm. The patient was referred for further examination and treatment. Symptoms include a lump in the stomach accompained by lower left abdominal pain since 2 months ago, often feeling nauseous since 5-6 years ago, weight loss is denied. Physical examination revealed a mass in the left lower quadrant. The results of ultrasound examination revealed a cystic mass in the left ovary. The patient was treated with pharmacotherapy and operative left salpingo oophorectomy. Postoperative diagnosis was left ovarian cystadenoma.

Abstract

The carbonated soft drinks consumption in this world have been increasing rapidly. Several of epidemiology studies have linked chronic consumption a carbonated soft drink with obesity, fatty liver disease, cardiovascular disease, metabolic syndrome, dental caries and osteoporosis. This research aims to determine the effect of carbonated soft drinks consumption to increase blood glucose levels and islets of Langerhans diameter changes rats in male Sprague dawley white rats (Rattus norvegicus). This research is a post test only control group design experimental study using 28 male white rats Sprague dawley which divided into 4 groups. K group only given an aquadest ad libitum 6 ml/200 gr/day, P1 group is given carbonated soft drinks 3 ml/200 gr/day, P2 group is given carbonated soft drinks 6 ml/200 gr/day and P3 group is given carbonated soft drinks 12 ml/200 gr/day for 30 days. The data of blood glucose levels was analysed by using One Way ANOVA test with result in p value=0.00 (p<0.05) then followed by Post Hoc LSD test with result in p value<0.05 for all of groups. The data of diameter Langerhans islets was analysed by using One Way ANOVA test with result in p value=0.394 (p>0.05). There is an effect of carbonated soft drinks consumption to increase blood glucose levels and there is no effect of carbonated soft drinks consumption to Langerhans islets diameter changes in Male Sprague dawley white rats (Rattus norvegicus) all 30 days long.

Abstract

Obstructive sleep apnea (OSA) is characterized by episodes of respiratory arrest or shallow breathing during sleep. These episodes are caused by complete or partial collapse of the upper airway. Most of the time, respiratory events are associated with snoring, oxygen desaturation and decreased sleep duration. Obesity is an important risk factor for the development of OSA and is unique among the major preventable and modifiable risk factors. Many studies have evaluated the effect of weight loss, achieved by behavioral, pharmacological, and surgical approaches, in the management of OSA in obese patients. Initial management includes weight loss, which must be maintained in the long term to prevent relapse, and a 'weight loss program' is mandatory for patients. The success of such programs is usually influenced by the motivation of individuals and their support groups. The increasing prevalence of obesity has led to an increased incidence of respiratory distress during sleep. Obesity is associated with anatomical changes that predispose to upper airway obstruction during sleep. There are several mechanisms by which obesity can cause OSA, and the mechanisms are synergistic. Research shows that increased peripharyngeal fat deposition produces a mechanical load that offsets the maintenance of airway patency by dilator muscles and that this increase in collapsibility is particularly pronounced during sleep when there is decreased neuromuscular activity. In addition, there is some evidence to suggest that central obesity in particular may have a detrimental effect on neuromuscular activity in the upper airway. This literature review discusses the possible mechanisms of obstructive sleep apnea syndrome and the implications of the management that can be recommended to patients.

Abstract

Osteoarthritis (OA) is a degenerative disease of the joints caused by several factors there are age, gender, race, genetics, trauma, occupation (activity), and obesity. Based on data from the WHO 2019, the prevalence of joint disease is reported to be 40% of the world's elderly population will suffer from OA, which 80% have limited joint motion. The causes of pain that occur are multifactorial. Obesity is the strongest modifiable risk factor in OA patients. During activity, half of the body weight rests on the joints. An increase in body weight will double the load on the joints when walking, especially the knee joints. Patient Mrs. N, 69 years old, came to the Panjang health center on November 11, 2021 with complaints of pain and stiffness in the joints in the right and left knees since 1 week ago. The complaints were felt continuously, especially when the patient often walked. Pain is felt in the right and left knees. The duration of complaints of pain appears less than 30 minutes.The pain is felt like a stabbing and comes and goes, especially in the morning after waking up. On physical examination, the body weight was 87 kg, height 168 cm, BMI 33.98. The diagnosis in this patient was Osteoarthritis (OA) and 2nd Grade Obesity.Management is carried out holistically for patients and families through flipchart intervention media in the form of information about OA and obesity and also suggests recommended activities for patients. Holistic management based on Evidence Based Medicine through patient-centered and family approaches can improve knowledge, attitudes and behavior on the patient

Abstract

Typhoid fever is a global infectious disease, especially in developing countries. Typhoid fever in Indonesia is endemic and is a public health problem. Failure of therapy by using antibiotics can bind closely to the patient, and germs. To implement comprehensive and holistic family doctor services by identifying risk factors, clinical problems, and patient management based on evidence based medicine with strategies that are patient centered, family oriented and community oriented. The analysis of this study is a case report. Primary data were obtained through history taking (autoanamnesis), physical examination, supporting examination and home visits. Assessment is carried out based on a holistic diagnosis of the initial, process, and final study quantitatively and qualitatively. Patient Mrs. H, 40 years old, came to the clinic with complaints of fever accompanied by headache, abdominal pain, nausea, vomiting, and a dirty tongue. Patients diagnosed with typhoid fever are in accordance with evidence based medicine. After the intervention, there was a decrease in clinical symptoms, an increase in knowledge and changes in the behavior of patients and their families. The diagnosis and management of typhoid fever in this patient was in accordance with national guidelines.
