Abstract

Objective: This study aims to analyze the effects of hyperglycemia status on the function of testicular spermatology, especially CAT-1. Material & Methods: This study was an experimental pre-clinical study. Twenty-seven rats were divided into 3 groups: normal, 2 weeks, and 4 weeks hyperglycemia. The hyperglycemic state in the Wistar rats was induced by Streptozotocin (STZ). All data were collected and analyzed with SPSS 20.0. At two and four weeks, testicular tissue was extracted and it will be processed using the Total RNA Mini Kit FavorPrepTM then quantitative PCR was performed using the SYBR® Fast qPCR Kit. Results: CAT-1 gene expression in the hyperglycemia induction group increased when induced for 2 weeks and again after 4 weeks compared to controls (18.88 ± 4.7 and 21.45 ± 5.52 vs 10.83 ± 3.4). However only induction after 2 weeks was statistically significant (p= 0.021). Conclusion: CAT-1 (Catalase) gene expression has increased in testicular tissue under conditions of hyperglycemia.

Abstract

Objective: The purpose of this study is to compare which analgesic is better between ketorolac and diclofenac. Material & Methods: A systematic search was conducted in Pubmed, Science Direct, Cochrane, EBSCO, Proquest database, and PICO analysis was used in determining the study question. The included article in the analysis was randomized controlled trials. Results: A total of 6 articles were included in the analysis, with no significant differences between ketorolac and diclofenac both in a direct comparison and indirect comparison (OR 2.74 [95%CI 0.72 – 10.43]; p= 0.14 and OR 0.51 [95%CI 0.29 – 0.91]; p= 0.49). Conclusion: Both ketorolac and diclofenac have the same efficacy in treating renal colic.

Abstract

Objective: In present days, endourology is the hallmark of urology, including in developing countries. This study reviews the surgical experiences of a urology practitioner in the first year of his practice in the underdeveloped province in Indonesia. Material & Methods: This research is a descriptive study that retrieval patients data from the medical record during June 2018 - May 2019. Results: Total of 401 operations were obtained. There were 328 male (81.8%) and 73 female (18.2%) with the average of patients age were 52+17.77 years old. The most common diagnoses were BPH 113 cases (28.2%). bladder stone 83 cases (20.7%), and ureteral stone 52 cases (13%). The most common surgical procedures performed were TURP 120 operations (29.9%), Ureterorenoscopy (URS) 64 operations (16%), and Vesicolithotripsy 58 operations (14.5%). The upper tract procedures were 84 operations (20.9%), mainly URS 64 operations (16%). Whereas the lower procedures were 317 operations (79.1%) mainly TURP 120 operations (29.9%). The complications were bleeding post-TURP 5 cases which require blood clot evacuation (1.2%), bleeding post open nephrolithotomy 1 case which requires blood transfusion (0.2%), and TURP syndrome 1 case (0.2%) with mortality 2 patient (0.4%). All kidney stones were handled by open surgery due to lack of PCNL and ESWL set devices. Conclusion: First year urologist who are interested in serving in such areas may benefit from prioritizing the availability of upper and lower set to performed basic standard care for surgical urology without leaving the hallmark of urology.

Abstract

Objective: To determine the difference in pain intensity between the administration of a combination of tramadol and intraurethral lidocaine gel with only lidocaine gel during the removal of DJ stents. Material & Methods: This study was a prospective randomized clinical trial with 22 patients that were divided into 2 groups. Group 1 was given a combination of tramadol and lidocaine gel urethral injection 3 to 5 minutes before DJ stent removal, whereas group 2 was given lidocaine gel urethral injection 3-5 minutes before the removal. The inclusion criteria in this study was all patients with a DJ stent placement with 1 to 3 months prior to release who were willing to participate. The Wong-Baker scale was used directly to determine the scale of pain during the procedure. Mann Whitney analysis was used to analyze the findings. Results: Most patients in group 1 are shown to have a pain rating of 2. There are 2 patients with a high pain rating scale of 6 and 7. In group 2, most patients claimed that the procedure was painless. The average Wong-Baker pain scale in group 1 (2.9) compared to group 2 (0.36) was significantly different. Conclusion: The combination of intraurethral tramadol and 2% lidocaine gel can significantly decrease pain during DJ stent removal more effectively than sole intraurethral 2% lidocaine gel based on the Wong-Baker pain scale.

Abstract

Objective: To analyze the effect of coconut water administration on the occurrence of urolithiasis in calcium oxalate-induced Wistar rats. Material & Methods: Laboratory experimental study with post-test only control group design, using experimental model of male Rattus norwegicus-Wistar strains. The treatment given is ad libitum administration coconut water to observe its effect on renal histopathological changes, microscopic analysis, and renal function markers. Urolithiasis induction uses exposure of 0.75% ethylene glycol (EG), and 2% Ammonium chloride (AC) in drinking water. Results: Descriptive analysis showed the tendency of decreasing in mean of renal function markers, and histopathological nephron damage in the treatment group versus control group: BUN (11.22 versus 14.29); Creatinine Serum (0.5308 versus 0.5714); degree of nephron damage (5.22 versus 10.36). In microscopic analysis, there were calcium deposits in the kidney tubules in the control group, which were not found in the treatment group. Based on the ANOVA test, obtained significant difference in BUN levels with p value=0.023 (p<0.05), and the degree of nephron damage between study groups with p value= 0.000 (p<0.05). There was no significant difference in serum creatinine with p value= 0.23 (p>0.05). Conclusion: Coconut water is potential modality in inhibiting the deposition of calcium oxalate crystals, and protecting from impaired kidney function against urolithiasis in animal-model of calcium oxalate-induced Wistar rats.

Abstract

Objective: To determine the effectiveness of Modified Guy's Stone Score (GSS) and S.T.O.N.E score (SS) as predictors of stone free rates in patients undergoing PCNL. Material & Methods: The design of this study was a prospective observational analytic. Samples were patients with kidney stones who came to the Urology Polyclinic of Soetomo0 General Hospital Surabaya, which was planned to undergo PCNL surgery, patients who met the inclusion criteria will have a CT stonographic examination then counted for the S.T.O.N.E score and Modified Guy's Stone Score before the PCNL procedure. Postoperative stone size evaluation used KUB X-ray (BOF) to assess Stone Free Rate (SFR). Results: In the ETA statistical test there was a strong relationship between GSS and SFR with a relationship strength value of 0.609, the direction of the relationship between these two variables was positive. This means that the greater the GSS, the less likely the SFR could be achieved. The relationship between these two variables was significant with p= 0.05. While between SS and SFR with a relationship strength value of 0.55, the strength of the relationship in these two variables was positive which indicated the higher the SS, the less likely the occurrence of SFR. But both of them were not statistically significant with p= 0.228. Conclusion: Guy Stone Score (GSS) gives a better predictive value than the STONE score (SS) on the stone free rates in patients undergoing PCNL procedures.

Abstract

Objective: To analyze the effect of curcumin and vitamin E on kidney function and inflammatory response of Wistar strain rats that received cisplatin. Material & Methods: An experimental laboratory study with a post-test only control design, using male Wistar strain rats (Rattus norwegicus). Rats were randomized using the simple randomized sampling method. Samples were treated with cisplatin 5 mg/kg (positive control group), vitamin E 100 mg/kg, curcumin 100 mg/kg body, and a combination of both (treatment group), to evaluate its effect on and kidney function and inflammatory response as measured by tumor necrosis factor-α (TNF-α), blood urea nitrogen (BUN) and serum creatinine. Results: There were differences in TNF-α levels in the positive control group (cisplatin 5 mg/kg) against each treatment group (p<0.05). Further analysis showed that there was a significant difference between the treatment group that received vitamin E and curcumin from the treatment group that received a combination of both (P<0.05). In addition, there were differences in BUN and serum creatinine levels in the positive control group (cisplatin 5 mg/kg) against each treatment group (p<0.05). However, there was no significant difference in BUN levels in the treatment group that received vitamin E with the treatment group that received curcumin or a combination of both (p>0.05). No differences were found in serum creatinine levels between treatment groups receiving vitamin E, curcumin, or a combination of both. Conclusion: Vitamin E 100 mg/kg, curcumin 100 mg/kg, and the combination of both have a nephroprotector feature in Wistar rats exposed to cisplatin.

Abstract

Objective: To investigate whether ADSCs are capable of nerve repair in stress urinary incontinence. Material & Methods: Thirty rats were allocated to five groups for receiving a periurethral injection with Phosphate Buffer Saline (PBS) (B1,B2) and Adipose-derived Stem Cells (C1, C2). Functional assessments with Voided Stain on Paper (VSOP) Method were reassessed at 2 and 4 weeks after injection. The rat SUI model was generated by Vaginal Distention (VD). The regeneration of peripheral nerve was evaluated by PGP 9.5 immunohistochemical staining. Results: Results revealed significant peripheral nerve differences between ADSC group and other groups in 28 days using PGP 9.5 staining. Our functional assessment, Voided Stain on Paper (VSOP), showed that ADSC group had significant improvement in 14 days; unfortunately, our 28 days improvement was not substantial. Conclusion: Periurethral injection of ADSCs improves urine pattern in SUI-induced rats and increase the amount of peripheral nerve. This method may be a potential strategy for SUI treatment.
