Abstract

Bakcground: Tuberculosis (TB) affected adults and children. The incidence of TB in children was increasing in Jember, East Java. A study from RS Paru Jember found that 17 (11%) students were infected by TB. This figure exceeded that of national level. This study aimed to examine the association between lack of house light, overcrowding, humidity, and the risk of TB infection among elementary school children in Jember.Methods: This was a case control study conducted in Jember, from January to June 2008. A sample consisting of 17 children with TB infection, and 51 children without TB infection was selected for this study. The instrument used in this study included questionnaire, lux meter (for light), and hygrometer (for humidity). Logistic regression and ORs were used to estimate the risk of TB infection.Results: Results showed an association between lack of house light (OR= 16.9; 95%CI= 4.1 to 69.8), over crowding (OR= 4.6; 95%CI= 1.4 to 14.8), and humidity (OR= 1.3; 95%CI= 1.1to 1.5), and the risk of TB infection.Conclusion: Lack of house light, overcrowding, and humidity were associated with increased risk of TB infection. Abstract

Background: To investigate the barriers for women in North Sulawesi to accessing a skilled attendant at birth.Methods: A secondary data analysis of the 2002/2003 Indonesia Demographic and Health Survey which involved the most recent live birth in the past five years of 428 ever-married women aged 15-49 in North Sulawesi was performed. Independent variables were captured by the Andersen behavioral model. Multivariate analysis was conducted with logistic regressions.Results: By adjusting other factors, mother’s educational attainment was the only significant determinant for the use of a skilled attendant. The adjusted OR of the change in birth attendant in women with secondary or higher level of education was 2.58 (95%CI 1.35-4.93).Conclusion: Low educational attainment was a significant barrier for women in North Sulawesi to accessing professional delivery care. To improve access, health care authorities and providers would need to educate women about their reproductive health and familiarizing them with maternal health services. Abstract

Background: One of the disorders found in autistic children is immune system aberration that affects gluten and casein metabolism, food intolerance, and vulnerability to infection e. Improved dietary pattern may help cure autism and reduce brain and digestion dysfunction. This study aimed to describe knowledge, attitude, and practice in dietary pattern among mothers with autistic children at Tali Kasih Foundation.Methods: This was a descriptive study. Population in this research is amount to 32 mothers then become total sampling. Data collected by using interview technique then analyzed descriptively in frequency distribution table.Results: The results showed that 31.3% of mothers with autistic children had good knowledge and 68.8% had fair knowledge. All of mothers had good attitude. 62.5% of mothers had good practice, and 37.5% had fair practice.Conclusion: Most of mothers of autistic children at Tali Kasih Foundation have either good or fair knowledge, attitude, and practice in feeding pattern. Abstract

Jurnal Kedokteran Indonesia (Indonesian Journal of Medicine) menerima dan menerbitkan naskah tulisan ilmiah di bidang biomedis, kedokteran klinis, kedokteran komunitas, dan kesehatan masyarakat. Naskah ilmiah meliputi artikel hasil penelitian, editorial, surat kepada Redaksi, dan resensi buku. Naskah bisa ditulis dalam bahasa Indonesia ataupun Inggris. Abstract

World Health Organization dalam Laporan Tahunan Kesehatan Dunia 2008 “Primary Health Care – Now More Than Ever”, mengemukakan lima masalah serius pelayanan kesehatan di dunia: (1) inverse care; (2) impoverishing care; (3) fragmented care; (4) unsafe care; (5) misdirected care (WHO, 2008). Sistem pelayanan kesehatan yang ‘fragmented’ membawa akibat yang tidak diinginkan: inefisiensi, ketidakefektifan, ketidakadilan, komoditisasi, komersialisasi, deprofesionalisasi, depersonalisasi, ketidakpuasan terhadap pelayanan kesehatan (Stange, 2009).

Abstract

Background: Hand dermatogliphic is a pattern of skin ridges in hand radius and palm. Skizophrenia is apsychosis with disturbance in personality, distorted thought, and perception disturbance. However,cognitive and intellectual capacity are usually not disturbed. This study aimed to describe handdermatogliphic profile of skizophrenic patients and compare it with non-skizofrenia persons.Methods: An analytic study was conducted at Sambang Lihum Mental Hospital Martapura, SouthKalimantan. 32 skizophrenic patients were taken as study subjects, and 32 persons without skizophreniaas control. The data was analyed using chi square test for finger ridge pattern and independent t testfor total finger ridge count and fluctuating asymmetry.Results: There was statistically significant difference in finger ridge pattern (p<0.05) . There was nostatistically significant difference in total fingger ridge count (p>0.05) and in fluctuating asymmetry(p>0.05).Conclusion: There is statistically significant difference in finger ridge pattern between skizophrenicpatients and non-skizophrenic persons.Keywords: skizofrenia, finger ridge pattern, total finger ridge count, fluctuating asymmetry

Abstract

Background: Gods leaf plant (Gynura pseudochina LDC) has active substances including saponin, flavonoid,and etheric oil. These substances have insecticide effect against larvae. Therefore Gods leaf plant canprobably used to control Aedes aegypti mosquito population in its larvae form. This study aimed todetermine the efficacy of Gods leaf extract in killing Aedes aegypti instar IV mosquito larvae.Methods: This was an experimental study. The Gods leaf was macerated to obtain the extract form. Inthis study the concentration levels of Gods leaf were varied including 550 ppm, 1100 ppm, 2200 ppm,4400 ppm, 8800 ppm, respectively. The positive control was temefos 1%, and the negative control wasCMC-Na 1%, each with four replication. The Gods leaf extract was then administered to 25 l Aedesaegypti larvae in each concentration group. This sample was observed for 24 hours. The data on thenumber of killed larvae was analyzed using Kruskal-Wallis dan Mann-Whitney tests.Results: There was no statistical difference in the mean number of killed larvae among differentconcentration (p=0.40). All concentrations could kill the larvae. Mann-Whitney test showed statisticallysignificant difference in the mean number of killed larvae between negative control and eachconcentration group. There was no statistical difference in the number of killed larvae betweenpositive control and the God leaf groups.Conclusion: Gods leaf extract (Gynura pseudochina (LDC) has statistically significant effect on thedeath of A. aegypti instar IV mosquito larvae, therefore can be used as a natural larvacide.Keywords: Gods leaf extract, larvacide, Aedes aegypti instar IV larvae

Abstract

Background: Children with history of low birth weight have the risk of atherosclerosis in their future.Vascular cell adhesion molecule-1 (VCAM-1) is a group of immunoglobulin adhesion molecules, a receptorprotein to marginalization process and mononuclear leukocytes movement along the endotholium.VCAM-1 is an early marker of atherosclerosis which can be detected at an early age. This study aimedto analyze the relationship between levels of VCAM-1 and history of low birth weight among childrenaged 15-18 years old.Methods: A historical cohort study was conducted to senior high school students in Surakarta in July2010. A Sample of students aged 15-18 years were selected by fixed exposure sampling. The data wereanalyzed by Mann-Whitney test and multiple linear regression analysis.Results: Of 700 students, 9 students had a history of low birth weight. With the 1:3 ratio obtained, thetotal study subjects were 28. Children with history of low birth weight had VCAM-1 mean level of 224.6ng / ml higher than those with normal birth weight (b = 224.6, p <0.001), after controlling for obesity.Conclusion: VCAM-I is associated with history of low birth weight. Abstract

Background: The incidence of postterm pregnancy accounts 10% of all pregnancies, and causes perinatalmortality 3 to 5 time as many as full-term pregnancy, depending on placenta function. Labor inductioncan lessen perinatal mortality if it is carried out before the emergence of insufficency signs. Oxytocinhas often been used for labor induction, but recently misoprostol has been increasingly used. Thisstudy aimed to compare the effect of misoprostol induction and oxytocin induction on labor time inpostterm pregnancy.Methods: This study was a double-blinded randomized controlled trial (RCT). The target populationwas mothers with postterm pregnancy who undertook induction. The source population was motherswith postterm pregnancy who undertook induction and gave birth at RSU PKU MuhammadiyahDelanggu, Klaten, Central Java. Eighteen postterm mothers undertook oxytocin induction and 18postterm mothers undertook misoprostol induction. Confounding factors that were controlled forincluded maternal age, parity, gestation age, birth weight, Apgar score, and Bishop score. The datawas analyzed by independent t test, by use of SPSS version 16.Results: This study showed that misoprostol induction shortened on average 3.5 hours less thanoxytocin, and this difference was statistically significant (p<0.001). This result was consistent withanother related finding that showed higher Bishop score in misoprostol induction than oxytocininduction.Conclusion: It is concluded that labor time induced by misoprostol is shorter than oxytocin in posttermpregnancy. Misoprostol can be used to replace oxytocin as a faster inductor for postterm pregnancy.Keywords : postterm pregnancy, oxytocin induction, misoprostol induction, labor time

Abstract

Background: A study on the effectiveness of amitriptyline and cognitive behavior therapy (CBT) toshorten healing time in leprosy reaction patients has been conducted at leprosy clinics RSK. SITANALA– Tangerang, on the leprosy reaction patient in periods 1 September 2009 until 31 October 2009. Thisstudy aimed to determine the effectiveness of amitriptyline and cognitive behavior therapy (CBT) onshortening healing time in leprosy reaction patients.Methods: This studi was a randomized clinical trial. A sample of patients diagnosed as contracingtleprosy reaction was divided into three groups. One group received leprosy reaction standard therapyand amitriptyline 25 mg 3 times daily. Another group received leprosy reaction standard therapy,amitriptyline 25 mg 3 times daily, and cognitive behavior therapy. The third group received leprosyreaction standard therapy. Mean differences of the three groups were compared with ANOVAResults: The group with either standard therapy and amitriptyline or amitriptyline and CBT showedfaster rccovery period, as well as reduction in pain, sleep disorder, fever, rash, and depression. Provisionof CBT alone did not show significant improvement in these clinical presentation.Conclusion: Provision of CBT in conjuction with amitriptyline and standard therapi can shorten recoveryperiod, as well as reduce pain, sleep disorder, fever, rash, and depression, more than standard therapyalone. Sole provision of CBT without amitriptyline in conjunction to standard therapy does not showsignificant improvement in these clinical presentation.Keywords : amitriptyline, leprosy reaction, standard therapy, cognitive behavior therapy
