Abstract

Introduction: Coronary artery bypass grafting (CABG) remains a cornerstone intervention in the management of progressive coronary artery disease; however, insufficient post-discharge self-care may lead to complications, delayed healing, and hospital readmissions. Telenursing has emerged as a promising strategy for supporting patients during early recovery. This study aimed to evaluate the effect of a structured telenursing educational intervention on patients’ knowledge and self-care management during early follow-up after CABG. Methods: A quasi-experimental study was conducted on 114 patients undergoing first-time elective CABG at Assiut University Heart Hospital. Participants were divided into intervention (n = 57) and control (n = 57) groups. Data were collected using a structured knowledge interview questionnaire and validated self-care management scale. Statistical analyses included paired t-tests, independent t-tests, repeated measures ANOVA, chi-square test, and Pearson correlation analysis. Results: The intervention group demonstrated significant improvement in knowledge and self-care management scores one month after discharge compared to baseline and the control group (p < 0.001). A significant positive correlation was observed between patients’ knowledge and self-care management (r = 0.804, p < 0.001). Conclusions: Telenursing intervention was associated with enhanced patient knowledge and self-care management during the early follow-up after CABG. The integration of structured telenursing follow-up into routine nursing care is recommended to support safe recovery and promote patient independence.

Abstract

Introduction: Central venous catheters (CVCs) are essential in the management of critically ill patients. However, their use may increase the risk of complications, particularly central line-associated bloodstream infections (CLABSIs). This systematic review aimed to evaluate evidence-based CVC care interventions for reducing the risk of CLABSIs in intensive care units. Methods: This review was conducted in accordance with the PRISMA 2020 guidelines. Specific keywords related to CVC care and CLABSI prevention were used to identify articles published between 2020 and 2024 in PubMed, ScienceDirect, and Google Scholar. A total of 48,541 records were identified and screened according to predefined criteria, including original research articles written in English and available in full text. Review articles, editorials, commentaries, abstract-only articles, and articles with restricted access were excluded. Nine articles met the eligibility criteria and were critically appraised using the Joanna Briggs Institute tool to assess the methodological quality of the included studies. Results: Daily bathing with 2% chlorhexidine, antisepsis of the insertion site using 1-2% chlorhexidine, timely dressing changes, polyhexamethylene biguanide (PHMB) dressings, and consistent adherence to central line care bundles were associated with reduced CLABSI incidence. Conclusions: These interventions emphasize the role of nurses in maintaining CVC care and preventing healthcare-associated infections. The implementation of evidence-based CVC care practices is therefore recommended to reduce the incidence of CLABSIs in the ICU.

Abstract

Introduction: This study examined the association between parental mental health and the development of children’s social skills. Methods: A cross-sectional study was conducted in an urban area of Indonesia using purposive sampling method. The participants were parents of kindergarten children, and all kindergartens in the study area were invited to participate. Parental mental health was self-assessed using the Depression Anxiety Stress Scale-21 (DASS-21), while children’s social skills were evaluated by 15 trained teachers using the Social Skills Scale-24 (SSS-24), which covers assertion, self-control, and cooperation. Regression analysis and multivariate analysis of variance (MANOVA) were performed on the data from 343 parent–child dyads. Results: Higher parental stress was negatively associated with children’s assertion (β = –0.119, p = 0.042) and self-control (β = –0.139, p = 0.040). Lower parental anxiety was associated with higher levels of children’s self-control (β = –0.144, p = 0.032) and cooperation (β = –0.175, p = 0.010). MANOVA demonstrated that the combined effects of parental depression, anxiety, and stress were significantly associated with children’s assertion (F = 3.72, p = 0.025) and their overall social skills (F = 3.12, p = 0.045). Conclusions: Greater parental psychological distress is associated with poorer social development in early childhood. Promoting parental mental health may support optimal social skills development among young children.

Abstract

Introduction: Unhealthy lifestyle behaviours in adolescence are major risk factors for non-communicable diseases. Digital interventions may provide age-appropriate support for preventive health efforts in this population. This review aimed to evaluate the effectiveness of digital interventions in improving adolescent lifestyle behaviours related to non-communicable disease prevention, particularly physical activity, dietary behaviour, sedentary behaviour, and weight-related lifestyle outcomes. Methods: A systematic search was conducted in PubMed, Scopus, Web of Science, and ScienceDirect to identify randomised controlled trials evaluating digital health interventions in adolescents. Two reviewers independently performed study selection and data extraction in accordance with PRISMA guidelines. Data on intervention characteristics, technological platforms, behaviour change strategies, and reported behavioural outcomes were extracted. Methodological quality was assessed using the Joanna Briggs Institute critical appraisal checklist. Results: Eleven randomised controlled trials were included. Interventions were delivered through mobile health applications, wearable monitoring devices, chatbot-based coaching, virtual reality, and web-based programmes. The most consistent findings were observed for physical activity and fitness-related outcomes, although effects were not uniform across studies. Evidence for dietary behaviour was generally favourable but varied, whereas evidence for sedentary behaviour and weight-related outcomes remained limited or mixed. Interventions incorporating self-monitoring, goal setting, gamification, and personalised feedback appeared more likely to support behavioural engagement. Conclusions: Digital interventions may support improvements in adolescents' health behaviours, particularly physical activity. Future studies should prioritise longer follow-up periods, stronger engagement strategies, and the integration of digital tools within broader health promotion environments.

Abstract

Introduction: Caring is fundamental to nursing and ensures an optimal patient experience. However, the complex nature of caring phenomena requires deeper exploration. Therefore, this study aimed to explore nurses' caring behaviors from the perspective of nurses as care providers and patients as care recipients. Methods: A hermeneutic phenomenological qualitative study was conducted in two major public hospitals in Malaysia, involving nurses (n = 16) and patients (n = 14). Participants were recruited through purposive sampling. Qualitative data were collected through focus group discussions and semi-structured interviews, aligned with institutional restrictions, and continued until data saturation was reached. To maintain data accuracy, this study used Lincoln and Guba’s framework of trustworthiness throughout the data collection and analysis. All data were audio-recorded, transcribed, and analyzed using thematic analysis. Results: Both groups identified caring as the essence of nursing. Nurses perceive caring as a professional obligation to ensure patient safety and acknowledge the importance of empathy. Meanwhile, patients define nurses' care as being authentic, with emotional presence and meaningful communication. Systemic and organizational constraints, such as staff shortages and administrative burdens, impose an emotional toll, leading to conflict between nurses' professional obligations and the clinical realities that sustain caring behavior. Conclusions: Continuous education and structured training may benefit nurses by enhancing and sustaining caring behavior and harmonizing professional responsibilities with psychological needs. Future interventional studies to measure their effectiveness are a way forward in this context.

Abstract

Introduction: This integrative review synthesizes the existing literature on transformational leadership in nursing practice and its aggregated influence on patient outcomes, staff performance, and healthcare quality indicators. Methods: Following Whittemore and Knafl's five-stage framework and PRISMA guidelines, a systematic search of PubMed/MEDLINE, Scopus, and Web of Science was conducted for peer-reviewed empirical articles published in English between 2010 and 2025. Eligible studies explicitly addressed transformational leadership and reported empirical results for at least one target domain. Data were extracted using a standardized template, and the methodological quality was appraised using the Mixed Methods Appraisal Tool (MMAT). As all the included studies employed quantitative designs, a structured narrative synthesis was applied, tabulating the findings by outcome domain and identifying within-domain patterns. Results: Eight studies were included, encompassing approximately 1,944 nursing participants (registered nurses, nurse managers, and charge nurses) and 1,209 patients across hospital settings in seven countries; the methodological quality was moderate to high. Transformational leadership is consistently associated with positive patient outcomes, including reduced adverse events, improved patient satisfaction, and higher-quality care. Staff-level benefits include reduced burnout, greater job satisfaction, and lower turnover intention. Healthcare quality indicators, particularly team climate and organizational commitment, improved substantially. Empowerment and social identity emerged as preliminary mediating mechanisms, warranting confirmatory research. Conclusions: Healthcare organizations should invest cautiously in leadership development, and longitudinal and interventional studies are needed to confirm the causal pathways and identify contextual moderators.

Abstract

Introduction: The growing demand for homecare nursing in Indonesia offers important career opportunities; however, how clinical competencies are translated into entrepreneurial choices remains unclear. This study examined the relationship between patient-centered communication (PCC) and social entrepreneurship (SE) motivation with nursing students' entrepreneurial intentions (EI) to establish homecare practices. Methods: A cross-sectional survey was conducted with 208 nursing students in West Sumatra, Indonesia. Data collected with validated instruments for EI (α = 0.90), SE drive (α = 0.94), and PCC (α = 0.92) were analyzed using partial least squares structural equation Modelling (PLS-SEM). Results: PLS-SEM showed that PCC did not directly influence EI (β = -0.050, p = 0.669) but significantly predicted SE motivation (β = 0.604, p < 0.001). SE motivation significantly predicted EI (β = 0.627, p < 0.001) and fully mediated the PCC–EI relationship (β indirect = 0.379, p < 0.001). The model explained 35.8% of EI variance. Conclusions: Clinical communication competencies alone do not predict entrepreneurial intentions; rather, they appear to require activation through motivation for social entrepreneurship. Therefore, nursing education should adopt integrated pedagogical frameworks that connect clinical empathy with social-business models to foster homecare as a viable career pathway.
