Abstract

Health problems with cardiovascular system disorder are still ranked high globally. One way to detect abnormalities in the cardiovascular system especially in the heart is through the electrocardiogram (ECG) reading. However, reading ECG recording needs experience and expertise, software-based neural networks has designed to help identify any abnormalities of the heart through electrocardiogram digital image. This image is processed using image processing methods to obtain ordinate chart which representing the heart’s electrical potential. Feature extraction using Fourier transforms which are divided into several numbers of coefficients. As the software input, Fourier transforms coefficient have been normalized. Output of this software is divided into three classes, namely heart with atrial fibrillation, coronary heart disease and normal. Maximum accuracy rate of this software is 95.45%, with the distribution of the Fourier transform coefficients 1/8 and number of nodes 5, while minimum accuracy rate of this software at least 68.18% by distribution of the Fourier transform coefficients 1/32 and the number of nodes 32. Overall result accuracy rate of this software has an average of 86.05% and standard deviation of 7.82.

Abstract

Indonesia is one of the countries with the high endemic of Dengue viral infection followed by Thailand, Myanmar, India and Srilanka. For more 10-15 years, Dengue Viral Infection/DHF has become a cause of patient who should be hospitalized and was the first cause of death children in south easthern Asia.1,2 Batavia was the first city of Indonesia found Dengue Viral infection which had been written in journal by David Bylon in the 1779. Encephalopathy of dengue (ED) is one unusually complication of dengue viral infection which had been characterized by aberration the arrangement of nerves central (CNS). This paper want to describe of a young teenage with suffer from DHF and seizure. Beside it, pleural effusion and cerebral edema had been found. Seizure most likely due to dengue encephalopathy associated with cerebral edema and was supported by positive IgG and IgM anti dengue. Corticosteroid was given toimprove cerebral edema. By good management as long as admission, she was discharged from hospital with a good condition.

Abstract

Tuberculosis (TBC) is an dangerous disease and many people has been infected. One of many important steps to control TBC effectively and efficiently is by increasing case finding using right method and accurate diagnostic. One of them is to detect Mycobacterium Tuberculosis inside sputum. Conventional detection of Mycobacterium Tuberculosis inside sputum can need a lot of time, so digitallydetection method of Mycobacterium Tuberculosis was designed as an effort to get better result of detection. This method was designed by using combination between digital image processing method and Neural Network method. From testing report that was done, Mycobacterium can be detected with successful value reach 77.5% and training error less than 5%.

Abstract

Drug resistance in leprosy is important for Leprosy Control Program, since the WHO-Multidrug regiment (MDT) has been used for global treatment of leprosy for more than two decades already. A Dapsone resistance case in a Multibacillary (MB) leprosy case is reported. The patient was diagnosed and treated in Tajuddin Chalid Hospital Makassar, South Sulawesi. Previously he was treated in a health center at South Sulawesi and was given a treatment for one year, before referred to the hospital. The leprosy skin lesions are still active with erythematous skin lesions and thickened ear lobe. Bacteriological examination was positive for Acid Fast Bacilli, the Bacterial Index was 3+ and the Morphological Index was 1%. The specimens of M.leprae isolation was sent to the Institute of Tropical Disease Surabaya for drug resistance study. Using the Lp1-2 and Lp3-4 nested primers, PCR test was positive for M.leprae.Sequencing result for folP gene showed a double mutation at codon 53 (ACC / Threonin ) which become (AGG / Arginine). Simultaneous mutation at two nucleotides at one codon has never been reported in Indonesia before and this phenomenon is important for leprosy control policy.

Abstract

Background: Typhus is a disease caused by Salmonella typhi, Salmonella paratyphi A Salmonella parathypi B, dan Salmonella paratyphi C bacteria that attacks digestive tract and caused infection in small intestine. The common test that performed in the laboratory is widal test. The result reading of the widal test still processed manually with looking the turbidity caused by the agglutination. Aim: The research was made to decrease human error by creating a program based on artificial neural network (ANN) with learning vector quantization (LVQ) method. Method: Input of this program is image of blood serum that has reacted with widal reagen. Image procesing start with grayscaling, filtering, and thresholding. Result: Output of this program is divided into two classes, normal and typhus detected. Conclusion: From this experiment result that using 24 testing data, gives the accuracy of this program 95.833% with 1 error result from 24 testing data.

Abstract

Influenza is an acute respiratory diseases caused by various influenza virus which infect the upper and lower respiratory tract and often accompanied by systemic symptoms such as fever, headache and muscle pain. Influenza spreads through the air. Swine influenza comes from swine and can cause an outbreaks in pig flocks. Even this is a kind of a rare case but the swine influenza could betransmitted to human by direct contact with infected swine or through environment that already being contaminated by swine influenza virus. There are 3 types of swine influenza virus namely H1N1, H3N2 and H1N2. Type H1N1 swine-virus had been known since 1918. Avian influenza virus infection is transmitted from one person to another through secret containing virus. Virus is binded into the mucous cells of respiratory tract before it is finally infecting the cells itself. Management patients with H1N1 influenza is based on the complications and the risk. Besides, it is also need to consider the clinical criteria of the patient. Therapy medicamentosa is applied to the patients by giving an antiviral, antibiotics and symptomatic therapy. Prevention can be done by avoid contact with infected animal or environment, having antiviral prophylaxis and vaccination.

Abstract

Objective: Acid fast bacilli (AFB) for sputum smear microscopy is the affordable method used for prompt diagnosis of tuberculosis in Nigeria despite its lack of specificity and limited sensitivity. The study aims to identify Mycobacterium tuberculosis and other acid fast organisms isolated from sputum of of HIV positive adult patients with pulmonary disease in Jos, Nigeria. Methods: Acid fast organisms isolated from 80 AFB positive sputa of HIV positive adult patients suspected for tuberculosis in Jos, Nigeria were identified for members of M. tuberculosis Complex (M tuberculosis, M bovis, M africanum, M canetti M. microti and M. caprae) by use of spoligootyping, Multiplex Gen Probe, Hain genotype assay and gene sequencing for spoligotype negative isolates. Results: Seven different spoligotypes of M. tuberculosis complex were identified from 70/80 (87.5%) total number of isolates. M. kansasii (1), M. dulvalii (1) Nocardia species (1) and Tsukamurella species (2) were detected from 5/10 spoligotype negative isolates. Conclusion and Recommendation: Although M. tuberculosis is the dominant AFB associated with chronic pulmonary disease in Jos, Nigeria, other clinically relevant mycobacteria were observed in the study. This suggests that other AFB positive microorganisms associated withtuberculosis -like symptoms could be misdiagnosed and incorrectly treated as M. tuberculosis. It is therefore necessary for laboratories in TB high burden countries to step up diagnostic procedures beyond routine smear microscopy.

Abstract

HIV/AIDS is a global problem involving industrialized and developing country including Indonesia. Malaria has killed millions of human beings almost 3 million people each year, whereas since 1999, nearly 36 million people in the world infected with HIV and 3 million more have died (Kakilaya, 2006). HIV infection increases the risk and aggravate malaria. In Africa in the area of malaria transmission intensities high and low, HIV aggravate malaria and improve case fatality at any age (Eline 2006). HIV is an RNA viruses whose hallmark is the reverse transcriptation of its genomic. Malaria is a protozoan disease transmitted by the bite of infected anopheles mosquito. Infection malaria can stimulate HIV replication and may cause faster progression of HIV disease.

Abstract

Background: Dengue Virus Infektion is major public health problem in Indonesia. Aedesaegypti is widespread in both urban and rural areas, where multiple virus Serotype are circulating. On 2013 outbreak of dengue virus infection occur in East Java. Therefore study seroepidemiology in Bangkalan and Lombok had been done. Aim: to find a mutated strain of Dengue Virus in 4 cities of Indonesia. Method: On 2011 and 2012 seroepidemiology study had been done in Dr. Soetomo Surabaya and Soerya Sidoarjo Hospital; and on  2013 study had been done in Surabaya, Bangkalan and Lombok Hospital . Diagnosis of Dengue Virus Infection was based on Criteri WHO - 2009. Virus isolation in Surabaya, Sidoarjo, Bangkalan and Lombok had been done. Result: a total of 349 isolate were obtained from dengue patients sera collected in Surabaya and Sidoarjo, 2011–2012 showed that Den V1 (182), Den V2 (20) Den V4 (1) were found in Surabaya on 2011 and Den V 1 (79) , Den V 2 (7) were found in Surabaya on 2012; Den V1 (40), Den V 2 (3) were found in Sidoarjo on 2011 and Den V 1 (17) were found in Sidoarjo on 2012; Virus isolation in Surabaya on 2013, January: 237 serum sample were collected, found Den V 1 (8), Den V 3 (2) and Den V 4 (5). And PCR stereotyping of isolated viruses in Madura found Den V 1 (1) and Den V 4 (23). In Lombok found Den V 4 (4).It is possible to shift predominant strain in Surabaya , Genotype or Serotype shift might increase the number of dengue patients. Conclusion: there were shift predominant strain in Surabaya especially Den V 1. Therefore to continuous surveillance of circulating viruses is required to predict the risk of DHF and DF.

Abstract

Patients of thalassemia who are conventionally treated by a regular transfusion regimen, are at a risk of developing transfusion transmitted infections, including hepatitis. The present study was conducted to evaluate the prevalence of hepatitis virus infection in repeated transfused thalassemia patients. A total of 83 patients of thalassemia who had received at least 10 transfusions were tested for HBs Ag, anti HBs and anti-HCV using ELISA. Amongst these patients, HBs Ag, anti HBs and anti HBC were detected in 1.2%, 26.5% and 12% patients respectively. the prevalence of HBV and HCV infection were in agreement with the findings in other study.
