Abstract

Background: Cataract is the leading cause of blindness worldwide, with surgery as a definitive therapy. Incisions may damage the underlying tissue, including loss of corneal sensibility. The purpose of this study was to determine differences in corneal sensibility decreased in patients after phacoemulsification and SLIMCE. Method: This study was a randomized clinical trial assessing changes of corneal sensibility in immature senile cataract patients after phacoemulsification and SLIMCE at Sanglah and Indera Hospital Denpasar, Bali-Indonesia from March to August 2013.  Surgeons and examiners are experienced ophthalmologists. Examiner did not know the initial data subjects. Preoperative assessment of corneal sensibility on the first, eighth, and fifteenth days were assessed postoperative using Cochet-Bonnet aesthesiometer. Statistical analysis was performed by applying mean difference of two independent groups test with repeated measures of multiple comparisons (ANOVA). Samples from each group were 17 eyes. Results: There were significant differences in corneal sensibility on the first day postoperative (33.2 mm), day 8 (-21.5 mm), day 15 (-11.8 mm) in both groups. Decrease in corneal sensibility in both groups was significant (p = 0.001). There was decreasing in corneal sensibility with SLIMCE technique in the first (21.8 mm) and eighth day (45.9 mm). Conclusion: In phacoemulsification technique, corneal sensibility was also decreased in the first (31.8mm) and eighth day (50.6 mm) but returned to preoperative values on day 15 in both groups. Decrease in corneal sensibility during follow-up period is influenced by cataract surgery technique (p = 0.017) and was higher in SLIMCE technique compared with phacoemulsification.

Abstract

Background: Preoperative fine needle aspiration biopsy/FNAB examination, imprint cytology and frozen section intraoperative has big implications for diagnosis and surgical strategy of thyroid nodules with follicular neoplasm cytology. FNAB and frozen section has its limitations, it is difficult to detect the presence of capsular and/or vascular invasion of thyroid follicular carcinoma. Whereas imprint cytology can preserve cellular overview (especially the cell nucleus), including the capsular and/or vascular invasion. In addition, imprint cytology is faster than frozen section. Frozen section examination could not indicate the presence of capsular and/or vascular invasion in most cases so that imprint cytology is used to replace frozen section as an alternative. Method: This research is a diagnostic test study using a descriptive design. This is a prospective study to assess the sensitivity, specificity, NPV, and PPV of imprint cytology in patients with thyroid follicular neoplasm cytology. Results: In our study; sensitivity, specificity, PPV, NPV, and accuracy of imprint cytology for follicular neoplasm was found as 84.21%, 95.45%, 94.12%, 87.50% and 90.24% respectively. The outcome was based on likelihood ratio value of 18.21 and the ROC curve, area under the curve obtained at 0.879 and Kappa value of 0.802. Conclusion: Imprint cytology has a value of a good diagnostic validity in the diagnosis of follicular neoplasm of thyroid nodules with sensitivity and specifity values of 84.21% and 95.45%. Imprint cytology is a technique that is simple, inexpensive, and has good reliability so that it can be used instead of frozen section.

Abstract

In managing chemistry, manufacturing and controls documents; the larger process is involved for electronic regulatory submissions to health authorities. This is a case of an entire process standard providing significant gains in efficiency. A solution is needed for this area of the biopharmaceutical industry. Within a Regulatory affiars, there are Information Management needs, particularly related to the electronic Common Technical Document (eCTD) submission process. There are a number of methods for adding efficiencies to the submission process, particularly through automation and standards. Consider the overall process of a sponsor producing an eCTD submission and within it the specific process of transferring Chemistry, Manufacturing and Controls (CMC) documents from a manufacturer to the sponsor. Keeping in mind the overall eCTD process, consider the specific CMC process to illustrate where and how inefficiencies can be changed.

Abstract

Osteoporosis in children is rare and usually secondary to an underlying disease process whose diagnosis may be difficult to detect. It can be a manifestation of acute lymphoblastic leukemia (ALL). About a-quarters of children with ALL will have signs and symptoms of osteoporosis. We report the case of a seventh-year-old boy with back pain. His antero-posterior pelvic radiograph showed the osteoporotic bone. The bone age study revealed six-year-old bone. Review of peripheral blood smear showed normochromic anemia with thrombocytopenia. Immunophenotyping of peripheral blood revealed no dominant marker was seen, but the bone marrow aspiration confirms the diagnosis of ALL (L2).

Abstract

Background: The present study was undertaken with an objective to observe effectiveness of oral administration of cinnamon extract to hyperglycemic induced rats using alloxan. Male and female Wistar Rats were injected with alloxan to induce hyperglycemia. Methods: This is an experimental study to determine whether cinnamon has an ability to combat hyperglycemic rats. Blood glucose was estimated by GOD-PAP method using diagnostic kit supplied by Agappe diagnostics, Maharashtra. Results: The present experimental study provides further evidence that oral administration of Cinnamon extract for 21 days produced a significant decrease in the blood glucose level in the model of alloxan induced diabetes rats. Conclusion: From this study, we can conclude that the oral administration of Cinnamon extracts have beneficial effect on blood glucose levels. However further pharmacological and biochemical investigations will clearly elucidate the mechanism of action and helpful in projecting these plant extracts as a therapeutic target in diabetes research.

Abstract

Background: Osteoporosis is a silent metabolic disease characterized by diminished bone mass and change in bone microstructure which cause increment of fracture risk. Until now, osteoporosis still becomes one of major health problems around the world. In Indonesia, the incidence of osteoporosis is 25%. Previous study have shown the relation between osteoporosis and IL-6 gene polymorphism at -572G>C and -174 G>C. There are some controversies about the correlation between these polymorphism and osteoporosis because of different result between each study. Genotype G polymorphism at -572 G>C of IL-6 gene has been correlated with lower Bone mineral density (BMD) and Genotype G polymorphism at -174G>C of IL-6 gene has been correlated with higher BMD value. In Indonesia, there are still no study about the association between IL-6 gene polymorphism and osteoporosis. In the future this IL-6 gene polymorphism could be used as a genetic marker for osteoporosis in postmenopausal woman. The objective of this study is to determine the difference of genotype of -572G>C and -174G>C polymorphism of IL-6 gene and osteoporosis in Balinese postmenopausal women. Method: This research design is a case control study. Sample was obtained at orthopedic outpatient clinic of Sanglah General Hospital, Bali-Indonesia from June 2012 until November 2012. The diagnosis of osteoporosis is described as BMD value with T score ? -2.5 SD using DEXA. All sample’s peripheral blood are taken to be isolated for DNA and analyzed for IL-6 gene polymorphism at -572G>C and -174G>C using Real Time PCR. Data obtained was analyzed with chi square test using SPSS. Results: This research found 11 osteoporosis sample from total 52 with no difference sample characteristic between case and control (p > 0.05). Using Chi square test, There was a significant differences between genotype -572 G>C; IL-6 gene polymorphism in Balinese postmenopausal woman with osteoporosis and in Balinese postmenopausal woman without osteoporosis (c2 = 41.85 and p = 0.001) and no significant differences between genotype -174 G>C;IL-6 polymorphism in Balinese postmenopausal woman with osteoporosis and in Balinese postmenopausal woman without osteoporosis(c2 = 0.283 and p = 0.518). This study discovered significant differences between genotype -572 G>C; IL-6 gene polymorphism in Balinese postmenopausal woman with osteoporosis and in Balinese postmenopausal woman without osteoporosis. Conclusion: We concluded that ethnic genetic profile variations affect the role of IL-6 gene polymorphism. In the future, this IL-6 gene polymorphism could be used as risk factor for osteoporosis in Balinese premenopausal woman

Abstract

Background: Bilateral paramedian thalamic and midbrain infarct is rare and present with varying symptoms. The occlusion of a rare arterial variant called the artery of Percheron results in bilateral thalamic infarcts, with or without midbrain involvement. To this date, the diagnosis of ischemic infarction due to occlusion of the artery of Percheronhas was uncommon. Case: We reports a 50 year-old man with acute ischemic infarcts of both paramedian thalami and midbrain, presenting with complete ophthalmoplegia, arreactive mydriasis pupils, and altered mental status (somnolent and fluctuating hypersomnia).Conclusions: Based on clinical and neuroimaging findings, we concluded this case as acute ischemic infarctions due to occlusion of the artery of Percheron.
