Abstract

Tujuan penelitian adalah melakukan analisis faktor-faktor yang mempengaruhi kepatuhan masyarakat mengikuti program pemberian obat masal pencegahan (pomp) filariasis di Wilayah Puskesmas Maleber Kabupaten Kuningan. Desain penelitian adalah kuantitatif dengan pendekatan cross sectional study. Populasi sebanyak 42.167 orang dengan ukuran sampel sebanyak 240 orang. Teknik pengambilan sampel stratified random sampling. Teknik pengumpulan data melalui wawancara. Analisa data Analisa bivariate menggunakan chi square dan analisa multivariate dengan regresi logistik. Variable motivasi (p 0.006), derajat perubahan gaya hidup (p 0.049), nilai/keyakinan mengurangi ancaman sakit (0.000), pemahaman terhadap tingkah laku specific yang harus dilakukan (0.000), derajat kesukaran menerima dan melaksanakan instruksi (0.000), keyakinan terhadap terapi / instruksi (0.000), kekhawatiran terhadap efek samping obat (0.020), dan derajat kepuasan terhadap pelayanan kesehatan (0.022), memiliki nilai p < dari 0.05, sehingga dapat disimpulkan, bahwa variabel tersebut memiliki pengaruh terhadap kepatuhan masyarakat mengikuti program POMP filariasis. Factor dominan yang mempengaruhi kepatuhan masyarakat mengikuti program POMP filariasis adalah nilai/keyakinan mengurangi ancaman sakit dengan nilai Exp (B) paling tinggi, yaitu 4.251. Perencanaan dan alokasi anggaran, dan pelaksanaan kegiatan promosi / pendidikan kesehatan khususnya program POMP filariasis perlu dtingkatkan. Perlu dilakukan penelitian yang bertujuan untuk mencari metoda/pendekatan maupun media yang lebih efektif dalam pelaksanaan promosi dan pendidikan kesehatan kepada masyarakat

Abstract

The largest population in Indonesia suffering from hypertension in 2023 is the elderly. Ergonomic and isometric handgrip exercises are recommended for managing hypertension. The aim of this study was to investigate the differences in systolic and diastolic blood pressure among hypertensive elderly individuals. The research design was a pre- and post-intervention experimental approach with two treatment groups. The study population consisted of hypertensive elderly individuals in Cirebon, Indonesia. The samples size was calculated using formula (t-1) (r-1) > 15 for each group, resulting in a total of 32 hypertensive elderly, selected through simple random sampling. Data analysis was conducted using a parametric t-test. The results indicated a significant difference in systolic (p=0.024 < 0.05) and diastolic (p=0.045 < 0.05) blood pressure between the isometric handgrip exercise and ergonomic exercise groups following the intervention. Ergonomic exercise yielded a more significant results, with a reduction of 15.3 mmHg in systolic pressure and 10.0 mmHg in diastolic pressure, compared to the isometric handgrip exercise, which resulted in reductions of 5.75 mmHg in systolic pressure and 4.0 mmHg in diastolic pressure. Ergonomic exercise can be recommended as a safe and accessible form of exercise for hypertensive elderly individuals.

Abstract

Background: In autistic children, temper tantrums can occur in association with other challenging behaviors like sleep problems, selective eating, anxiety, irritability, self-harm, and aggression. Families who have adult autistic children experience burdens in their lives. Poor communication and interaction with the child at home provoke an emotional tantrum. This study aims to explore family experiences in reducing children's autistic tantrums through family support. Methods: This phenomenological study involved seven parents of adolescents with autism. The study was carried out over a period of six months, from September 2022 to March 2023, covering the stages of preparation, participant recruitment, in-depth semi-structured interviews, and verbatim transcription. Data were analyzed using Colaizzi’s phenomenological method, which comprised verbatim transcription, identification of significant statements, coding, clustering of themes, and the development of an essential description of the parents’ lived experiences. Results: Six themes were constructed from the study: (1) The process of accepting their children with special needs; (2) Parents' expectations of their children; (3) Internal and external support in reducing autism tantrums; (4) Children join with activities; (5) Enforcement of rules in the family; (6) Family isolation. Conclusion: This study highlights the central role of families in caring for children with autism spectrum disorder (ASD), particularly in managing tantrums. Families apply spiritual values, collaborate among members, and use practical strategies such as providing a safe space, allowing calming time, and redirecting children to meaningful activities. The findings emphasize that family-centered approaches can enhance children’s adaptive abilities and quality of life while reducing caregiver burden.

Abstract

Ergonomic exercise has been recommended for older adults with hypertension. However, initially, it was carried out without music. Kacirebonan gamelan music is a traditional music that has characteristics of music therapy. The research objective is to determine the effects of ergonomic exercises accompanied by kacirebonan gamelan music and those not accompanied by kacirebonan gamelan music on systolic and diastolic blood pressure in hypertensive elderly in Cirebon. The research design is a quasi-experimental pretest-posttest intervention with two treatment groups: the first received ergonomic exercise accompanied by kacirebonan gamelan music, and the second received ergonomic exercise only. The intervention was carried out daily for 12 days, with two sessions per day, each lasting about 30 minutes, every 9 hours in the morning. The sample size was formulated by (t-1) (r-1) > 15, so the sample for each group consisted of 16 hypertensive elderly and 32 people for the whole sample, selected by simple random sampling and meeting the inclusion and exclusion criteria. Data analysis was performed using a t-test based on the Shapiro-Wilk normality test, with a p-value> 0.05. The result: there was no significant difference between before and after the intervention in systolic pressure (p=0.787>0,05) and diastolic pressure (p=0.229>0,05) in hypertensive elderly in the treatment group. There was no significant difference between the treatment and control groups in systolic pressure (p = 0.468 > 0.05) or diastolic pressure (p = 0.761 > 0.05) after treatment in hypertensive elderly. Ergonomic exercise without the accompaniment of kacirebonan gamelan music produces a greater effect on systolic (0.036 < 0.05) and diastolic (0.045 < 0.05) blood pressure in hypertensive elderly.  The research conclusion is that the accompaniment of kacirebonan gamelan music during ergonomic exercises did not have a significant effect on systolic or diastolic blood pressure in elderly hypertensives. Ergonomic exercise without the accompaniment of kacirebonan gamelan music had a greater impact on reducing systolic and diastolic pressure in hypertensive elderly in Cirebon. Future research should formally operationalise and measure participants' cultural perceptions and expectations of an intervention before implementation.
