Abstract

Artritis gout akut merupakan kondisi inflamasi persendian yang disebabkan oleh kelainan metabolisme purin, di mana terjadi pengendapan kristal monosodium urat (MSU) di sendi, ditandai dengan rasa nyeri parah yang datang secara mendadak, disertai bengkak, kemerahan, serta keterbatasan gerak sendi. Selain pengobatan farmakologi seperti obat antiinflamasi nonsteroid (NSAID), pendekatan nonfarmakologi berupa kompres bubuk rimpang jahe merah (Zingiber officinale var. rubrum) menunjukkan efek yang mampu mengurangi hiperalgesia pada penderita inflamasi sendi, termasuk kasus artritis gout. Penelitian ini bertujuan menguji efektivitas gabungan kompres bubuk jahe merah dengan NSAID dibandingkan penggunaan NSAID tunggal terhadap tingkat intensitas nyeri pada pasien artritis gout. Metode penelitian menerapkan desain True Experimental Design pendekatan Randomized Controlled Trial (RCT), dilaksanakan di Rumah Sakit Royal Prima Medan pada periode Oktober hingga November 2025. Populasi penelitian meliputi pasien nyeri akut artritis gout sebanyak 74 responden, dipilih melalui random sampling, kemudian dibagi secara acak menjadi kelompok intervensi (37 responden) dan kelompok kontrol (37 responden). Analisis data dilakukan dengan uji paired t-test dan independent sample t-test. Hasil penelitian mengindikasikan adanya perbedaan signifikan dalam penurunan intensitas nyeri antara kelompok intervensi dan kontrol dengan nilai signifikansi (p) = 0,001 (p < 0,05), di mana kelompok intervensi menunjukkan pengurangan nyeri lebih tinggi daripada kelompok kontrol. Kombinasi kompres bubuk jahe merah dan NSAID terbukti efektif dalam menekan skala nyeri akut pada pasien artritis gout.

Abstract

Abstract Acute postoperative pain following long bone fractures is a common clinical problem that may hinder the healing process and delay early patient mobilization. This study aimed to analyze the effectiveness of Progressive Muscle Relaxation (PMR) techniques in reducing the intensity of acute postoperative pain in patients with long bone fractures. A randomized controlled trial with a pretest–posttest control group design was employed. The sample consisted of 72 respondents, who were divided into an intervention group (NSAIDs + PMR) and a control group (NSAIDs only). Univariate analysis showed that the mean pain intensity in the intervention group decreased from 68.2 ± 8.5 mm before the intervention to 32.4 ± 7.6 mm after the intervention, while in the control group, mean pain intensity decreased from 67.9 ± 8.1 mm to 48.6 ± 8.9 mm. Bivariate analysis using the paired t-test demonstrated a statistically significant reduction in pain intensity before and after treatment in both the intervention group (p = 0.000) and the control group (p = 0.001). Furthermore, the independent t-test revealed a significant difference in post-intervention pain intensity between the intervention and control groups (p = 0.000). These findings indicate that Progressive Muscle Relaxation is effective as a complementary therapy in reducing acute postoperative pain following long bone fractures and is recommended for integration into postoperative nursing care. Abstract

Background: Skeletal muscle hypertrophy is an adaptive process influenced by molecular signals and metabolic conditions. Sirtuin 1 (SIRT1), a NAD-dependent deacetylase, plays a crucial role in regulating skeletal muscle growth and regeneration through its interaction with key pathways such as mTOR, PGC-1?, FOXO, and myogenic factors. Aim: This literature review summarizes the current scientific evidence on the role of SIRT1 in muscle hypertrophy, including its effects on mitochondrial efficiency, oxidative stress reduction, and suppression of catabolic genes such as atrogin-1 and MuRF1. Method: A literature search was conducted using databases such as PubMed, ScienceDirect, and Google Scholar, focusing on articles from the last 10 years. Result: These findings indicate that SIRT1’s effects are highly context-dependent, varying with cellular energy status and external stimuli such as exercise or caloric restriction. Rather than directly inducing hypertrophy, SIRT1 facilitates a favorable cellular environment that enables sustained muscle growth. Conclusion: These insights offer promising potential for the development of molecular-based therapies to preserve or enhance muscle mass, particularly in aging populations or pathological conditions.

Abstract

Dysmenorrhea is a burden on women's health in the world due to its high prevalence. Dysmenorrhea is often associated with several risk factors, one of which is eating habits. The consumption of fast food, which has become a habit of people in the era of globalization, is reported to affect the incidence and intensity of dysmenorrhea. This Community Service aims to socialize the composition and nutritional value of healthy food, the composition and nutritional value of fast food, dysmenorrhea and how it affects daily life as well as the effect of excessive consumption of fast food on dysmenorrhea in the community which was carried out at Prof. Dr. Chairuddin P. Lubis Hospital, Medan, North Sumatra. This community service activity consists of two stages, namely providing education in the form of posters containing material and questions and answers or discussions on related health topics, then handing over the material to the community to be able to practice the knowledge gained so that people's knowledge increases and can be applied to daily life, with the hope that in the future dysmenorrhea disorders caused by excessive fast food intake which causes limitations In carrying out daily activities can be avoided and no longer become one of the largest contributors to health problems in the world and no longer a burden to women's health in the world.

Abstract

Rumah sakit merupakan fasilitas pelayanan kesehatan yang berpotensi menimbulkan risiko kesehatan lingkungan apabila pengelolaannya tidak dipantau secara terintegrasi. Di RSI Malahayati Medan, monitoring risiko kesehatan lingkungan masih dilakukan secara manual, data kualitas lingkungan belum terintegrasi, dan pengambilan keputusan belum berbasis sistem digital sehingga program Green Hospital belum berjalan optimal. Kegiatan pengabdian kepada masyarakat ini bertujuan mengembangkan dashboard digital berbasis web untuk monitoring risiko kesehatan lingkungan sekaligus mendukung pengambilan keputusan berbasis data. Kegiatan menggunakan pendekatan Community-Based Research dan Participatory Action Research melalui enam tahapan: identifikasi kebutuhan, analisis masalah, perancangan dashboard, pelatihan dan implementasi, monitoring dan evaluasi, serta penyusunan rekomendasi. Sasaran kegiatan adalah petugas sanitasi, tim K3RS, komite mutu, dan manajemen rumah sakit. Hasil menunjukkan peningkatan rata-rata pengetahuan peserta dari 58,4 menjadi 84,7 dan keterampilan dari 51,2 menjadi 86,9, percepatan waktu pelaporan data lingkungan, serta tingkat kepuasan pengguna yang tinggi. Dashboard berhasil mengintegrasikan enam indikator kesehatan lingkungan dan indikator Green Hospital. Disimpulkan bahwa implementasi dashboard digital efektif meningkatkan kapasitas mitra dan kualitas pengambilan keputusan kesehatan lingkungan.

Abstract

Background: Knee osteoarthritis (OA) is the most common degenerative joint disease with a high global prevalence. It causes pain, joint stiffness, muscle weakness, and reduced functional activity, significantly affecting patients' quality of life. Inflammatory factors, particularly interleukin-6 (IL-6), play a role in the pathogenesis of knee OA, contributing to inflammation and pain sensitization. Non-pharmacological management of knee OA includes physiotherapy manual traction therapy, aimed at reducing pain and improving joint function. However, the relationship between manual traction and the reduction of IL-6 levels as well as improvements in the Knee Injury and Osteoarthritis Outcome Score (KOOS) has not been widely studied. Objective: To analyze the effect of physiotherapy manual traction on KOOS values and serum interleukin-6 (IL-6) levels in patients with knee osteoarthritis. Methods: This experimental study used a pre-post test design involving 36 patients with knee OA who received manual traction interventions twice a week for four weeks. KOOS values and IL-6 levels were measured before and after the intervention. Data were analyzed using the Wilcoxon test and Spearman correlation. Results: A significant increase in KOOS scores was observed after the intervention (p = 0.001), but no significant reduction in IL-6 levels was found (p = 0.491). The correlation between changes in IL-6 levels and KOOS scores was very weak and not statistically significant (r = 0.122; p = 0.760). Conclusion: Manual traction physiotherapy significantly improves knee joint function and quality of life in patients with knee OA, but does not significantly reduce serum IL-6 levels over a short intervention period.
