Abstract

Hallucinations are a type of mental disorder where a person cannot distinguish between real life and fake life. Based on data collection obtained from Prof. Hb Saanin Mental Hospital, Padang, the most common nursing diagnosis in 2026 was Hallucinations with a total of 6,062 patients. Researchers are interested in taking because is one of the patients who shows the most signs and symptoms of hallucinations. This researcher aims to apply Mental Nursing Care to with hallucinations at Wisma Flamboyan, Prof. Hb Saanin Mental Hospital, Padang. The implementation of Nursing Care to was carried out on February 5-11, 2026 with a visit of five days of meetings. The method of this Scientific Paper is in the form of a Case Study. The results of providing Mental Nursing Care were obtained by was able to control hallucinations independently by using implementation strategies (SP) and group activity therapy (TAK), namely SP 1 rebuke practice, SP 2 taking medication regularly, SP 3 having conversations, SP 4 carrying out scheduled daily activities. In this study, it is expected that understands the provision of Mental Health Nursing Care, as well as researchers, Prof. Hb Saanin Padang Mental Hospital and institutions are expected to be able to increase insight as a reference in providing optimal Mental Health Nursing Care.

Abstract

Hallucinations are a perceptual disorder in which the client perceives something that is not actually happening, resulting in a loss of self-control, where the client experiences panic and behavior is controlled by hallucinations. The purpose of this study is to explain the application of psychiatric nursing care to patients experiencing auditory and visual hallucinations at Wisma Flamboyan. The method used is a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The results of the assessment indicate that the client experienced signs and symptoms of hallucinations, namely hearing whispers, anxiety, and talking to himself. The main nursing diagnosis is hallucinations with an additional diagnosis of self-care deficit. After receiving nursing care, the client showed progress in overcoming his nursing problems and was able to control hallucinations independently by applying implementation strategies (SP 1-4). The interventions implemented included reprimanding exercises, regular medication adherence, conversations, and carrying out scheduled daily activities. These results indicate that the application of SP is effective in helping clients manage hallucinations and increasing independence in self-care. This study is expected to increase public understanding and awareness of the importance of maintaining mental health in order to achieve maximum well-being.

Abstract

Abstrak Risiko perilaku kekerasan adalah jenis gangguan jiwa dimana seseorang dapat membahayakan diri sendiri, orang lain secara fisik ataupun psikologis, dan merusak lingkungan. Berdasarkan data dari rekam medis di RSJ. Prof. Dr. HB. Saanin Padang, masalah keperawatan terbanyak kedua dalam 3 tahun terakhir adalah Risiko perilaku kekerasan yang berjumlah 523 pasien. Tujuan studi kasus ini dilakukan untuk memberikan dan melaksanakan asuhan keperawatan pada pasien dengan risiko perilaku kekerasan di wisma flamboyan RSJ. Prof. Dr. HB. Saanin Padang. Metode yang digunakan dalam penulisan karya tulis ilmiah ini adalah studi kasus melalui pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, intervensi, implementasi, evaluasi dan dokumentasi. Pada hasil pengkajian pasien menunjukkan tanda gejala risiko perilaku kekerasan berupa emosi labil. Diagnosa utama yaitu risiko perilaku kekerasan dan diagnosis pendukung adalah deficit perawatan diri. Setelah dilakukan pemberian asuhan keperawatan pasien mampu mengontrol emosi dan perilaku kekerasannya secara mandiri dengan menerapkan latihan terjadwal strategi pelaksanaan pasien risiko perilaku kekerasan dengan melakukan SP 1 latihan tarik napas dalam dan pukul bantal, SP 2 patuh minum obat secara teratur, SP 3 secara verbal, dan SP 4 melakukan kegiatan spritual dan SP 1-4 Defisit Perawatan Diri. Kesimpulan pasien dapat mengontrol resiko perilaku kekerasan dengan terapi yang di ajarkan oleh peneliti secara mandiri dan terjadwal. Pasien juga dapat melakukan secara mandiri latihan untuk menjaga perawatan diri. Saran ketika dalam memberikan asuhan keperawatan jiwa pada pasien dengan risiko perilaku kekerasan dapat dilakukan dengan meningkatkan komunikasi terapeutik dan pemberian pujian pada pasien pada pasien setiap selesai melakukan latihan.   Kata Kunci: Perilaku Kekerasan, Asuhan Keperawatan Jiwa, Strategi Pelaksanaan

Abstract

Hypertension is one of the non-communicable diseases that commonly occurs in Indonesia and contributes to increased morbidity and mortality rates. Hypertension is often referred to as a silent killer because many sufferers are unaware of their condition until complications such as stroke, heart disease, and kidney failure occur. Based on data from the Naras Community Health Center, hypertension ranked second among the ten most common diseases, with 1,363 cases reported in 2025. This condition indicates that hypertension remains a health problem requiring special attention. This study aimed to implement family nursing care for families with hypertension in the working area of the Naras Community Health Center in 2026. The research method used was a case study with a family nursing process approach, including assessment, nursing diagnosis, intervention, implementation, and evaluation. Based on the results of the study conducted by the researcher from February 28 to March 6, 2026, in the working area of the Naras Community Health Center on Mrs. S’s family, it was found that the family lacked understanding regarding hypertension, its causes, signs and symptoms, home care, and the importance of routine blood pressure monitoring. The nursing problems identified were risk-prone behavior, noncompliance, and pain. The interventions provided included health education, recommendations for a low-salt diet, regular physical activity, medication adherence, and utilization of health services. After the nursing interventions were implemented, the family showed increased knowledge, was able to care for family members with hypertension, became more compliant with health check-ups, and experienced reduced pain complaints. It can be concluded that family nursing care is effective in helping control hypertension and prevent complications.
