Abstract

Dental caries among school-aged children remains a significant health problem in Indonesia, with a prevalence of 82.8% (SKI 2023) and 93% of children aged 5–12 years having experienced caries, far higher than the global prevalence of 60–90%. This condition is exacerbated by poor toothbrushing behavior (only 6.2% of Indonesians brush correctly) and limited access to dental health services in rural areas such as Gurah Village, Gurah District, Kediri Regency. This community service activity aims to improve knowledge, toothbrushing skills, and provide additional protection against caries risk through the FANTASI program (Fluoride for Healthy, Safe, and Beautiful Smiles). The method employed a collaborative participatory approach based on a modified Community-Based Participatory Research (CBPR), comprising three intervention components: Dental Health Education (DHE), toothbrushing practice with medical personnel assistance, and Topical Application of Fluoride (TAF) by professional health workers. The target participants were 5th and 6th-grade students of SDN Gurah 1, conducted on August 5, 2026. The results showed an increase in participants' average knowledge score from 82.40 in the pre-test to 93.07 in the post-test, with the proportion of participants in the good knowledge category increasing from 76% to 90.7%. Toothbrushing practice with medical personnel assistance effectively trained students' psychomotor skills, while TAF application provided additional protection against caries risk. The integration of these three intervention components offers a comprehensive school-based caries prevention model relevant for rural areas with limited access to dental health services, while also supporting the achievement of SDGs goal 3 (good health and well-being) and goal 4 (quality education).
