Abstract

Politik kebijakan anggaran kesehatan adalah elemen penting dalam mendukung upaya percepatan pembangunan sektor kesehatan. Namun, dalam prosesnya, kebijakan soal anggaran justru mengabaikan sektor pembiayaan kesehatan dan lebih difokuskan pada sektor militer dan perang. Situasi ini menjadi semakin rumit di era ketidakpastian yang ditandai dengan peningkatan jumlah konflik, perluasan daerah konflik, dan penggunaan teknologi tempur berbiaya mahal. Persoalan “pengalihan paksa” anggaran kesehatan demi tujuan perang oleh sebuah kebijakan politik harus dianggap sebagai langkah keliru yang bisa membahayakan peradaban manusia, mengingat dalam beberapa dekade terakhir ini telah terjadi peningkatan tajam prevalensi berbagai jenis penyakit yang membutuhkan sokongan dana yang sangat besar. Abstract

Background: Reporting patient safety incidents is important to improve patient safety and quality of care. Barriers to report patient safety incidents in nursing may occur due to lack of knowledge and unscheduled as well as low reporting rates. Unfortunately, nurses’ experiences in reporting patient safety incidents have not been comprehensively reviewed.Purpose: The purpose of this study was to explore nurses’ experiences of reporting patient safety incidents in East Nusa Tenggara, Indonesia.Methods: A descriptive phenomenological approach was used to identify, analyze and describe the experiences of 15 nurses in reporting patient safety incidents.  Nurses having worked for more than two years, not on leave, not being infected with COVID-19 in the last 14 days, and not having a structural position were purposively recruited. Data were collected using in-depth interviews and voice recordings. The collected data were then transcribed verbatim, and thematic analysis was applied for data analysis.Results: Four main themes were found in this study, which included: (1) Priority and responsibility for services, (2) Barriers to incident reporting, (3) Learning for nurses, and (4) Support for nurses.Conclusion: The nurses experience of in reporting safety incidents is still constrained by several obstacles. It is hoped that health care organizations need to provide appropriate strategies to enhance the safety incident reporting efforts made by nurses. Based on the research findings, it is recommended that health service organizations disseminate the use of safety incident reporting forms and assist nursing managers to their subordinates by conducting supervision and motivation related to incident reporting on a scheduled and continuous basis.

Abstract

ABSTRAK Implementasi mengenai discharge planning merupakan kebijakan yang utuh, terintegrasi, dan berkelanjutan sejak pasien masuk rumah sakit, dirawat, serta sampai pasien berada dirumah dan benar-benar tidak membutuhkan perawatan. Tujuan dari penulisan adalah untuk melakukan analisis implementasi discharge planning di Rumah Sakit Umum X Kota Depok. Metodologi yang digunakan adalah pendekatan pilot study yang dilaksanakan dari analisis situasi, pembuatan plan of action, implementasi, evaluasi dan analisis gap dengan menggunakan literature review. Analisis masalah dilakukan melalui diagram fish bone dan analisis prioritas masalah dilakukan dengan menggunakan analisis CARL. Masalah utama adalah belum optimalnya pelaksanaan discharge planning yang terstruktur dan terintegrasi. Implementasi dilaksanakan dengan pendekatan pilot study. Implementasi yang dilakukan adalah pembuatan panduan dan format discharge planning, sosiliasi, uji coba panduan dan format discharge planning, serta evaluasi. Hasil uji coba didapatkan bahwa perawat telah melakukan discharge planning terstruktur dan terintegrasi dengan capaian sebesar 92% berdasarkan pengisian format yang ada. Diperlukan rencana tindak lanjut dari pihak manajemen Rumah Sakit Umum X di Kota Depok untuk menerapkan pelaksanaan discharge planning terstruktur dan terintegrasi. Kata kunci: discharge planning, keperawatan ABSTRACT Implementation of discharge planning is a complete, integrated, and sustainable policy since the patient is admitted to hospital, treated, and until the patient is at home and really does not need treatment. The purpose of writing is to conduct analysis of discharge planning implementation at the Regional General Hospital X in Depok City. The methodology used is pilot study approach which is carried out from situation analysis, plan of action plan, implementation, evaluation and gap analysis by using literature review. Problem analysis was done through fish bone diagram and problem priority analysis was done by using CARL analysis. The main problem is the not yet optimal implementation of structured and integrated discharge planning. Implementation is done by pilot study approach. Implementation is the making of guidance and format of discharge planning, sociation, pilot study and format of discharge planning, and evaluation. The result of the experiment shows that the nurse has done structural and integrated discharge planning with achievement 92% based on the filling of the existing format. A follow-up plan from the management of X General Hospital in Depok City is required to implement a structured and integrated discharge planning. Abstract

Background: Communication is an important part of nursing care. However, Indonesian nurses still feel communication problems due to cultural and linguistic differences, especially in peripheral and border areas. Purpose: This study explores the communication experience of nurses when providing services to patients in three peripheries and two border areas of Indonesia. Methods: A phenomenological approach is used in this study. The purposive sampling technique was used to recruit 22 nurses in Indonesia’s periphery and border areas to be involved in the study. The data collection process was carried out from August to October 2021. Data were collected through interviews using a semistructured questionnaire, and verbatim transcripts of audio-recorded interviews were analyzed using thematic analysis. Results: This research resulted in four important themes, specifically: (1) difficulty in communicating, (2) emotional distress, (3) conflict and understanding, (4) lack of support for competency development. Conclusion: The findings of this study increase the scientific understanding of the communication barriers of nurses in the culturally diverse periphery and border areas. The barriers encountered can be used by nursing managers to construct ineffective nurse communication problem-solving interventions. The main emphasis on solving communication problems should include continuing education and training support so that nurses’ communication competence can be improved and applied in the service area based on the cultural and linguistic conditions of the patient.

Abstract

Penelitian ini bertujuan untuk mengetahui sejauh mana peningkatan kompetensi guru IPA kelas VIII dalam menyusun silabus dengan memasukan nilai karakter melalui bimtek di Sekolah Mengenah Pertama Negeri 4 wolowaru. Pendidikan karakter dalam dunia pendidikan dijadikan sebagai wadah atau proses untuk membentuk pribadi peserta didik  agar menjadi pribadi yang baik. Kompetensi guru IPA terus ditingkatkan melalui sebuah pelatihan atau bimbingan teknis sehingga benar-benar menghasilkan guru yang berkualitas secara administrasi dan juga secara tindakan nyata dalam proses pembelajaran di kelas. Semakin banyak bimbingan yang diberikan kepada para guru maka semakin besar pula perubahan yang dialaminya sehingga inilah yang dinamakan guru berkompetensi atau profesional. Silabus mata pelajaran sebagai sebuah kelengkapan yang tidak terpisah dari guru untuk dijadikan sebagai acuan dalam melaksanakan kegiatan belajar mengajar di kelas. Silabus mata pelajaran yang harus dimiliki oleh guru adalah silabus mata pelajaran yang telah memenuhi sistematika yang benar sesuai dengan standar isi BSNP. Jenis penelitian ini adalah penelitian tindakan sekolah. Subjek penelitian adalah 2 orang guru IPA kelas VIII SMP negeri 4 wolowaru tahun pelajaran  2020/2021. Teknik pengumpulan data adalah observasi dan dokumentasi. Hasil penelitian menunjukan adanya peningkatan kompetensi guru IPA kelas VIII dalam menyusun silabus melalui bimtek di SMP negeri 4 wolowaru sebesar 37,50%, terlihat dari kondisi awal siklus I sebesar 62,50%, dan pada siklus II meningkat menjadi 100%. Hal ini menunjukann bahwa silabus tersebut telah memenuhi kriteria sesuai sistematika yang benar dan juga telah memasukan nilai-nilai karakter kedalam silabus mata pelajaran IPA.

Abstract

Background: Immunisation barriers can increase morbidity and mortality associated with vaccine-preventable diseases. Understanding barriers to childhood vaccination is critical to informing effective interventions to maximise coverage. Purpose: This study aims to explore the experiences of nurses involved in immunisation related to the obstacles they experience in immunisation activities. Methods: This research is qualitative research with a phenomenological approach. Nurse participants who manage immunisation activities at the public health centre were recruited using a purposive sampling technique. In-depth interviews were conducted with 15 nurses who work in immunisation activities. Inclusion criteria are nurses who directly provide immunisation services and have managed the immunisation program for more than one year. Exclusion criteria were nurses who were on leave. The study was conducted from August to September 2022, and face-to-face interviews were conducted. Interview transcripts were analysed using the thematic analysis method. Results: Three major themes were generated in this study, namely: 1) barriers to immunisation governance systems, 2) barriers to vaccine recipients, and 3) policy barriers. Conclusions: Interventions to overcome barriers to immunisation need to be carried out comprehensively at various levels, such as promoting the benefits of vaccines for the community, strengthening the capacity of officers' competence, reducing political bias, and funding supporting facilities for immunisation programs.

Abstract

Background: Parents of children with Congenital Rubella Syndrome (CRS) in rural areas face limited access to medical care and specialised services, financial difficulties, social isolation, and stigma. Research on their experiences is limited.Purpose: This study explores parents’ experiences of caring for their children with CRS in rural areas of Indonesia.Methods: The research used a descriptive phenomenological approach, purposive sampling to collect parents of children with CRS from the shelter, and snowball sampling for other participants. Inclusion criteria included parents who cared for CRS children over than two years old, could express their experiences, and spoke Indonesian. Twenty-five parents participated in an in-depth semi-structured interview. The data were analysed using the Colaizzi method.Results: This research found that parents caring for sick children in rural areas experienced life-altering situations that affected their mental health. In addition, caring for sick children also caused emotional disturbances and tension in family life. Other challenges included getting proper treatment for their child, especially in remote rural areas, and difficulty to reach health services. Therefore, proper mental health treatment and support were essential for parents who cared for sick children in rural areas.Conclusion: This study describes complexities of parents’ experiences of caring for children with CRS, such as difficulty to accept the child's condition and feeling burdened by their child’s case. Further studies are needed to understand cultural influences and expand the study area.

Abstract

Masalah gizi kurang, terutama pada anak-anak di wilayah pedesaan, masih menjadi isu kesehatan global yang signifikan. Di Indonesia, angka prevalensi gizi kurang tetap tinggi, dengan Kabu-paten Timor Tengah Utara (TTU) mencatatkan angka stunting yang mencemaskan. Masalah ini dipicu oleh berbagai faktor, termasuk pola pemberian ASI dan MPASI yang tidak optimal, kebudayaan lokal, serta kondisi ekonomi yang terbatas. Pengabdian masyarakat ini bertujuan untuk meningkatkan penge-tahuan dan keterampilan orang tua, khususnya ibu rumah tangga, dalam memberikan ASI eksklusif, MPASI berbahan lokal, serta memperbaiki ekonomi keluarga melalui produksi manisan asam sebagai produk berbasis pangan lokal. Metode yang digunakan adalah Mission Based Learning, yang meng-gabungkan teori dan praktik, serta pelatihan teknik rolling massage untuk meningkatkan produksi ASI. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan peserta tentang ASI dan MPASI, serta ket-erampilan dalam mengolah bahan pangan lokal. Selain itu, pelatihan pembuatan manisan asam mem-berikan dampak positif terhadap ekonomi keluarga. Keberlanjutan program ini dapat mengurangi prev-alensi gizi kurang dan meningkatkan taraf hidup masyarakat TTU secara berkelanjutan. Pelatihan dan pendampingan berkelanjutan diharapkan dapat menjadi solusi efektif dalam mengatasi masalah gizi kurang di wilayah tersebut.
