Abstract

Introduction: Irrational antibiotic use remains a global public health problem that contributes to the increasing incidence of Antimicrobial Resistance. Low public literacy regarding rational antibiotic use can increase the risk of antibiotic resistance, particularly among housewives who play an important role in family health decision-making. Therefore, effective educational media are needed to improve public understanding regarding proper antibiotic use. Objective: The purpose of this community service was to improve antibiotic literacy among housewives in Karangsari Village through infographic leaflet-based health education and to evaluate the effect of the educational intervention on antibiotic literacy improvement. Method: This public service was conducted using a pre-experimental design with a one group pre-test and post-test approach in April 2026 in Karangsari Village, Purwokerto. Participants consisted of 42 housewives selected using purposive sampling. Educational intervention was carried out through counseling sessions and interactive discussions using infographic leaflets containing information regarding rational antibiotic use, proper antibiotic consumption, and the risks of antibiotic resistance. Antibiotic literacy was measured using pre-test and post-test questionnaires. Data were analyzed using IBM SPSS Statistics version 25 and evaluated using the Paired Sample t-test. Result: The average antibiotic literacy score increased from 58.21 ± 10.34 during the pre-test to 84.12 ± 8.27 during the post-test, with a mean difference of 25.91. Statistical analysis showed a significant difference between pre-test and post-test scores (p-value <0.001). Conclusion: Infographic leaflet-based education effectively improved antibiotic literacy among housewives in Karangsari Village and may serve as an effective community-based strategy to promote rational antibiotic use and prevent antimicrobial resistance.

Abstract

Cardiovascular diseases (CVDs), including hypertension and heart failure, remain major contributors to global morbidity and mortality and frequently require long-term polypharmacy. The concurrent use of multiple medications increases the risk of drug–drug interactions (DDIs), which may reduce therapeutic effectiveness, increase toxicity, and contribute to adverse drug reactions. This study aimed to develop and validate a locally deployed large language model (LLM)-based chatbot for detecting drug–drug interactions involving captopril in both monotherapy and combination therapy settings. A development and validation study was conducted using DDI data obtained from DrugBank and Drugs.com. The chatbot was developed using the LLaMA-3 model integrated with LangChain, Ollama, and FastAPI and was evaluated through iterative testing and 5-fold cross-validation. System performance was assessed using accuracy, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV), while usability was evaluated using the System Usability Scale (SUS) and Single Ease Question (SEQ) questionnaires completed by pharmacists. The chatbot demonstrated progressive performance improvement throughout development and achieved excellent performance during final validation involving 300 drug pairs, with an accuracy of 99.0%, sensitivity of 100.0%, specificity of 98.0%, PPV of 100.0%, and NPV of 92.0%, exceeding all predefined acceptance thresholds. Usability testing indicated only fair to moderate usability, with a mean SUS score of 65.0 and a mean SEQ score of 5.0, suggesting that further refinement of the user interface and workflow may be required. The locally deployed LLM-based chatbot demonstrated satisfactory diagnostic performance and preliminary feasibility for captopril-related DDI screening. Although the system showed promising classification performance, additional usability optimization and evaluation in real-world clinical workflows are needed before broader implementation can be considered as a pharmacist-supportive screening system.

Abstract

Introduction: Irrational use of antibiotics remains a major global and national public health challenge, contributing to the accelerating problem of antimicrobial resistance. Low antibiotic literacy is a key determinant of inappropriate antibiotic use, and misconceptions often develop from an early age due to limited health education. Therefore, promotive and preventive educational strategies targeting children are essential to foster appropriate antibiotic use and long-term behavioral change. Objective: The purpose of this community service activity was to promote and improve antibiotic literacy among elementary school students through the use of infographic and comic-based educational media. Method: This community service activity was conducted using a qualitative descriptive approach with a participatory educational design based on Participatory Action Research. The intervention employed visual educational media in the form of infographics and an educational comic titled “Una and the Mission to Finish Antibiotics.” Data were collected through participatory observation, interactive discussions, and simple written interviews, and analyzed using thematic analysis. Result: The activity revealed three main response patterns among students: positive responses characterized by a clear understanding of antibiotic use and the importance of completing therapy, partial responses indicating basic understanding but conceptual confusion, and limited responses reflecting minimal comprehension. Most students demonstrated high engagement and increased awareness of appropriate antibiotic use. Conclusion: Infographic and comic-based education is an effective and child-appropriate strategy for promoting early antibiotic literacy. This approach supports preventive efforts in pharmaceutical and public health education and has strong potential to encourage rational antibiotic use when integrated into school-based learning.
