Abstract

Background: In Indonesia, low vision poses a significant public health issue, especially to children, which could lead to challenges in performing daily activities and diminished quality of life. This study aimed to describe the clinical characteristics of pediatric patients with low vision diagnosis in Cicendo Eye Hospital Bandung. Methods: All medical records of low vision patients aged 0-17 at Cicendo Eye Hospital Bandung during 2021 were reviewed. Data collected were patient’s age, gender, area of origin, main diagnosis of low vision cause, visual acuity, and low vision aids and/or rehabilitation given. Result: From 172 medical records reviewed, majority of patients were male (54.07%). The age group of 10-17 years had the largest proportion (38.95%). Majority of patients lived in Bandung and its surroundings (51.16%). A total of 52.33% patient experienced moderate visual impairment and 34.3% experienced severe visual impairment. Most common anatomical cause of low vision was posterior segment abnormality, with Retinopathy of Prematurity as the main cause (13.37%), followed by retinal dystrophy (12.79%). Low vision aid that was most commonly prescribed were glasses, followed by telescope. Conclusion: Pediatric low vision patients at Cicendo Eye Hospital were mostly male, in the age group of 10-17 years, and lived in Bandung and its surrounding. The most common classification of low vision cause was posterior segment abnormality, with Retinopathy of Prematurity as the main diagnosis. The most widely used low vision aids were glasses. Abstract

Pendahuluan: Kelainan refraksi yang tidak terkoreksi merupakan penyebab terbanyak gangguan penglihatan yang dapat dihindari. Miopia merupakan kelainan refraksi yang paling sering dan prevalensinya terus meningkat sehingga menyebabkan gangguan penglihatan. Genetik dan gaya hidup akan mempengaruhi perkembangan dan hasil akhir miopia. Metode: Penelitian ini merupakan penelitian deskriptif retrospektif dari pasien yang terdiagnosis miopia pada periode 1 Januari hingga 31 Desember 2020. Sebanyak 301 pasien dipilih menggunakan consecutive random sampling. Usia, jenis kelamin, tempat tinggal, kelainan refraksi, ketajaman penglihatan, derajat miopia, koreksi penglihatan terbaik, lateralisasi mata, dan penatalaksanaan ditinjau secara retrospektif. Hasil: Penelitian ini menunjukkan paling banyak terjadi gangguan refraksi bilateral (92,4%), miopia ringan (57,1%), dan diberi kacamata (99,3%). Pasien sebagian besar tidak mengalami gangguan penglihatan (70,4%), namun masih terdapat kebutaan (3,7%). Koreksi terbaik menunjukkan peningkatan pada kelompok tanpa gangguan penglihatan (92,7%), namun terdapat kebutaan yang tidak dapat dikoreksi (0,7%) yang berhubungan dengan ablasi retina, katarak, dan ambliopia. Kesimpulan: Pada penelitian ini terdapat peningkatan jumlah pasien tanpa gangguan penglihatan sebesar 22% setelah diberikan alat bantu kacamata. Selain itu terdapat 36,9% pasien yang belum pernah memakai kacamata sebelumnya dan 42,5% pasien berasal dari luar kota bandung. Hal ini menunjukkan bahwa masih kurangnya akses dan pelatihan untuk kesehatan mata terutama pada daerah terpencil sehingga banyak pasien dengan kelainan refraksi yang tidak terkoreksi harus menempuh jarak yang cukup jauh untuk mendapatkan alat bantu.

Abstract

Background: Asthenopia, characterized by symptoms following near visual activity, often affects individuals with accommodative insufficiency and prolonged screen time. This study aims to investigate the correlation between demographic, refractive status, accommodative insufficiency, screen time, and asthenopia symptoms among patients at the Indonesia National Eye Centre, Cicendo Eye Hospital Bandung, Indonesia. Method: In this cross-sectional study, consecutive sampling was conducted to obtain subjects. Ocular examinations were performed to determine refractive status, amplitude of accommodation, accommodative insufficiency, symptoms of asthenopia, and screen time. Result: A total of 105 subjects were included in the study, with a median age of 25 years (IQR: 3). The median amplitude of accommodation was 9.09 D (IQR: 0.7), which was found to be lower than normative data. Asthenopia was observed in 47 subjects (44.7%), and accommodative insufficiency was present in 37 subjects (35.2%). Male gender and screen time > 8 hours per day were significantly correlated with asthenopia (p=0.048 and p<0.001, respectively), while refractive status and accommodative insufficiency showed no significant correlation. Conclusion: this study demonstrates a relatively similar prevalence of asthenopia. Additionally, male gender and excessive screen time were identified as risk factors for asthenopia. Abstract

Lensa kontak dan kacamata adalah perangkat optik untuk mengoreksi kelainan refraksi. Lensa kontak lebih sesuai untuk mengoreksi kelainan refraksi tertentu seperti anisometropia tinggi dibandingkan kacamata. Keuntungan menggunakan lensa kontak adalah gambaran retina yang sama meskipun kekuatan lensa tidak sama, berkurangnya efek prisma, dan hasil lapang pandang yang lebih luas. Tujuan dari laporan kasus ini adalah untuk melaporkan kasus penggunaan lensa kontak lunak dan kacamata secara bersamaan sebagai terapi optik dari anisometropia. Seorang pasien wanita berusia 35 tahun mengeluhkan penglihatan buram saat melihat jauh, terutama pada mata kanan. Mata kanan memiliki tajam penglihatan terbaik 0.63 dengan koreksi S -16.00 C -1.00 x 10, sedangkan mata kiri 1.0 dengan koreksi S -8.50 C -1.50 x 180. Pasien didiagnosis menderita miopia tinggi pada kedua mata, astigmatis miopia kompositus pada kedua mata dan ambliopia anisometropia pada mata kanan. Pasien kemudian diberikan terapi dengan kombinasi lensa kontak dan kacamata. Kelainan refraksi yang tinggi dan koreksi anisometropia dapat menjadi tantangan tersendiri. Lensa kontak dapat menjadi alternatif tatalaksana pada kasus kelainan refraksi tinggi yang disertai anisometropia.

Abstract

Introduction: Impaired blood flow is an early sign of retinal dysfunction in diabetes. Impaired blood flow of the radial peripapillary capillaries can affect the Retinal Nerve Fiber Layer (RNFL) or ganglion cell function. Methods: This was an analytic cross-sectional study in February-April 2019 at Bandung. The subjects of this study were 41 people (79 eyes) divided into two groups, group A (healthy subjects) 19 people (37 eyes) and group B (DM type 2 without diabetic retinopathy) 22 people (42 eyes). Peripapillary retinal perfusion was assesed using OCT Angiography and RNFL thickness was assesed using OCT. Pearson or Spearman correlation statistics test was used to analyze the result. P value ≤0.05 was consiedered significant. Results: There was a decrease in peripapillary retinal perfusion density in the inferior quadrant (P = 0.003) and flux index throughout the peripapillary retinal quadrant (P = 0.0001) but an increase in RNFL thickness in the peripapillary temporal quadrant (P = 0.012) compared to control grup. Positive correlation was found between peripapillary retinal perfusion density and RNFL thickness globally (r = 0.480, P = 0.001), superior quadrant (r = 0.436, P = 0.004), and inferior quadrant (r = 0.608, P = 0.000). A positive correlation was also found between the peripapillary flux and RNFL thickness globally (r = 0.517, P = 0.000), superior quadrant (r = 0.630, P = 0.000), and inferior quadrant (r = 0.519, P = 0.000). Conclusions: There was a relationship between peripapillary retinal perfusion and RNFL thickness in DM patients without diabetic retinopathy.
