Abstract

According to data from the World Health Organization (WHO), the prevalence of Chronic Energy Deficiency (CED) among pregnant women is estimated to reach 40% in 2023. The Indonesian Ministry of Health reported that in 2020, 451,350 pregnant women across 34 provinces had a Mid-Upper Arm Circumference (MUAC) of less than 23.5 cm, indicating a risk of CED. This study aims to identify the risk factors associated with CED among pregnant women in the working area of Limboto Barat Public Health Center. This study used a case-control design by comparing case and control groups. The study was conducted in the working area of the Limboto Barat Public Health Center during February–March 2025. The study population consisted of all pregnant women recorded in Limboto Barat Health Center Register Book, which consist of 982 individuals. The sample included pregnant women with and without CED, selected using purposive sampling. Data were collected through structured questionnaires and analyzed using the chi-square test. Significant associated risk factors with CED among pregnant women included maternal age (OR = 3.833, 95% CI: 2.164–6.791), ANC visits (OR = 2.346, 95% CI: 1.348–4.083), and parity (OR = 2.883, 95% CI: 1.623–5.119). Although iron tablet consumption (OR = 1.410, 95% CI: 0.819–2.428) was identified as a risk factor, it was not statistically meaningful. Economic status was also found to be a meaningful factor (OR = 2.400, 95% CI: 1.202–4.792). There is a need to enhance education and regular monitoring of the nutritional status of pregnant women, particularly among those in high-risk age groups, those with low ANC visit frequency, and those with high parity. Health centers should take a more active role in providing nutritional counseling, improving compliance with iron tablet consumption, and expanding comprehensive maternal health services to reduce the incidence of CED among pregnant women

Abstract

ABSTRACT Family Planning (FP) is a key preventive health service for women. Enhancing and expanding family planning services is a strategy to reduce high rates of pregnancy-related maternal morbidity and mortality, as well as to lower birth rates. This study aimed to assess the implementation of the Family Planning program at the Limboto, Biluhu, and Pulubala Community Health Centers (Puskesmas) in Gorontalo Regency. A qualitative descriptive case study design was employed. The three primary informants were the heads of the respective Community Health Centers. Six additional informants—comprising coordinating midwives and village midwives from the three centers—also participated. The collected data were processed using triangulation and subsequently organized into a research matrix. Human resources, equipment, and supplies were found to be adequate. However, a significant obstacle was the lack of specific funding allocations to support the FP program from either the regional budget (APBD) or the National Health Insurance (JKN) scheme. Effective communication, cross-sector collaboration, and quality public education are key to the program's successful implementation and active community participation. The implementation process involves various stakeholders and complementary stages. A bureaucratic structural approach—characterized by inter-agency cooperation and coordination—has yielded significant positive results. To date, the implementation of the Family Planning program in Gorontalo Regency has been successful. The leadership provided by the heads of the Community Health Centers, supported by coordinating and village midwives as frontline service providers, forms a solid foundation for the program's execution.
