Abstract

Background: Rapid changes in air pressure can create a pressure imbalance between the middle ear cavity and the external environment, often resulting in ear pain, a sensation of fullness, ringing in the ears (tinnitus), temporary hearing impairment, and other forms of discomfort. This condition, known as ear barotrauma, occurs due to environmental pressure changes such as during aircraft takeoff or landing, diving activities, or while inside pressurized environments like hyperbaric chambers. The Valsalva maneuver is a method used to alleviate this issue; it involves closing the nose and mouth and attempting to exhale, thereby increasing air pressure within the nasal cavity and Eustachian tubes. Purpose: To optimize the Valsalva maneuver technique as a non-invasive intervention for reducing ear pain caused by air pressure changes in patients undergoing hyperbaric chamber therapy. Method: This activity consisted of interactive education and practical training conducted at the Hyperbaric Unit of Dr. Mintohardjo Naval Hospital (RSAL). The target audience comprised patients at high risk of developing ear pain (ear barotrauma) due to air pressure fluctuations during hyperbaric therapy. Twenty-eight patients participated in the activity. Respondents' knowledge and understanding were assessed using a "one-group pretest-posttest" design, utilizing questionnaires administered before (pretest) and after (posttest) the educational session. Practical proficiency was evaluated through observation of the respondents performing the Valsalva maneuver during a simulation. Evaluation involved comparing pretest and posttest results to measure knowledge gains and observing the practical skills demonstrated during the maneuver. The data were analyzed descriptively to assess the success of the education and training regarding the Valsalva maneuver in patients prone to ear pain. Results: The study showed that the mean age of participants was 52.9 years, with the majority (11 participants or 39.3%) falling within the 45–59 age range. Most participants were male (17 participants or 60.7%). Pre-test and post-test results indicated an improvement in category distribution, rising from 8 individuals (28.6%) to 23 individuals (82.1%) an increase of 53.5%. Conclusion: Community service activities comprising education via standing banners, demonstrations, and hands-on practice were highly effective in enhancing the knowledge and understanding of patients undergoing hyperbaric therapy regarding the application of the Valsalva maneuver. Improved proficiency in performing the maneuver also reduced anxiety associated with confined spaces (claustrophobia), prevented panic caused by ear pain or ringing resulting from pressure changes, and fostered the confidence needed for the procedure to proceed smoothly and without injury. Suggestion: It is recommended that RSAL dr. Mintohardjo specifically the hyperbaric unit provide standing banners and offer ongoing education to support the dissemination of health information regarding the Valsalva maneuver to the wider community. Keywords: Air pressure changes; Ear barotrauma; Hyperbaric therapy; Valsalva maneuver Pendahuluan: Perubahan tekanan udara yang terjadi secara cepat dapat menimbulkan tekanan tidak seimbang antara rongga telinga tengah dan lingkungan luar, sehingga sering menyebabkan nyeri telinga, rasa penuh, berdengung, gangguan pendengaran sementara dan ketidaknyamanan lainnya. Kondisi ini dikenal sebagai barotrauma telinga yang terjadi karena ada perubahan lingkungan seperti saat pesawat lepas landas atau mendarat, aktivitas menyelam dan juga saat berada di dalam ruang bertekanan seperti hyperbaric chamber. Manuver valsalva merupakan salah satu metode untuk mengurangi masalah tersebut dengan menutup hidung dan mulut lalu mencoba menghembuskan napas sehingga tekanan udara di rongga hidung dan tuba eustachius meningkat. Tujuan: Mengoptimalkan teknik manuver valsalva sebagai intervensi non-invasif untuk mengurangi nyeri telinga akibat perubahan tekanan udara pada pasien yang menjalani terapi di ruang hyperbaric chamber. Metode: Kegiatan ini merupakan edukasi interaktif dengan intervensi pelatihan yang dilaksanakan di Ruang Hyperbarik RSAL dr. Mintohardjo. Sasaran kegiatan ini adalah pasien yang memiliki risiko tinggi terjadinya nyeri telinga (ear barotrauma) akibat perubahan tekanan udara selama proses terapi hiperbarik. Kegiatan ini melibatkan 28 pasien untuk menjadi partisipan. Pengukuran tingkat pengetahuan dan pemahaman responden dilakukan dengan pendekatan one group pretest and posttest design menggunakan kuesioner yang diberikan sebelum (pretest) dan sesudah kegiatan edukasi (posttest). Untuk tingkat kemampuan responden di evaluasi berdasarkan observasi dari simulasi ketika melakukan praktik manuver valsalva. Evaluasi dilakukan dengan membandingkan hasil pretest dan posttest untuk melihat peningkatan pengetahuan dan pengamatan dari keterampilan praktik menerapkan manuver valsalva. Hasil evaluasi ini di analisa secara deskriptif untuk menilai keberhasilan kegiatan edukasi dan pelatihan manuver valsalva pada pasien dengan terjadinya nyeri telinga. Hasil:  Menunjukkan bahwa usia rata-rata partisipan adalah 52.9 tahun dan mayoritas berada direntang usia 45-59 tahun yaitu sebanyak 11 partisipan (39.3%). Sebagian besar partisipan berjenis kelamin laki-laki yaitu sebanyak 17 partisipan (60.7%). Dengan hasil pretest dan posttest menunjukkan adanya perubahan distribusi kategori menjadi lebih baik dari 8 orang (28.6%) menjadi 23 orang (82.1%), atau mengalami peningkatan sebesar 53.5%. Simpulan: Kegiatan pengabdian kepada masyarakat berupa edukasi menggunakan media standing banner, demonstrasi, dan praktik langsung sangat efektif meningkatkan pengetahuan serta pemahaman pasien yang sedang menjalani terapi hiperbarik dalam menerapkan manuver valsalva. Peningkatan kemampuan pasien dalam menerapkan manuver valsalva juga mengurangi rasa cemas pasien di ruang sempit (klaustrofobia), mencegah kepanikan saat telinga terasa sakit atau berdengung akibat perubahan tekanan, serta membangun rasa percaya diri agar prosedur berjalan lancar tanpa cedera.  Saran: Diharapkan kepada RSAL dr. Mintohardjo khususnya di ruang hiperbarik untuk menyediakan standing banner dan memberikan edukasi keberlanjutan sebagai upaya mendukung penyebaran informasi kesehatan tentang manuver valsalva agar dapat diterima kepada masyarakat yang lebih luas.

Abstract

Dysmenorrhea is one of the most common complaints experienced by adolescent girls during menstruation and may interfere with their daily activities. Various non-pharmacological approaches have been used to help reduce menstrual pain, including pineapple juice and green coconut water. This study aimed to describe changes in menstrual pain intensity following the administration of pineapple juice and green coconut water among adolescent girls with dysmenorrhea at the Faculty of Health Sciences, Dirgantara Marsekal Suryadarma University, in 2026. This study employed a descriptive case study design involving four participants with dysmenorrhea. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. Two participants received pineapple juice, while the other two received 250 mL of green coconut water twice daily for three consecutive days during menstruation.  The observational findings showed that all participants experienced a reduction in pain intensity during the three-day observation period. Among participants who received pineapple juice, pain scores changed from 6 to 2 and from 5 to 1. Among participants who received green coconut water, pain scores changed from 6 to 3 and from 5 to 1. These findings describe the changes in pain intensity observed in each participant during the observation period. Because this study used a case series design with a limited number of participants, the findings should not be interpreted as evidence of comparative effectiveness or causality.

Abstract

Hypertension in pregnancy is a major maternal health issue risking complications for both mother and fetus. Warm water foot soak therapy can be applied as a complementary non-pharmacological intervention in maternal nursing care. Objective: To describe blood pressure changes in third-trimester pregnant women with hypertension through the implementation of warm water foot soak therapy. This descriptive case study involved two third-trimester pregnant women with hypertension in South Jakarta. Data collection was conducted over 7 days (March–April 2026) via interviews, observation, and blood pressure monitoring. The self-administered intervention used warm water (37–40°C) for 15–20 minutes, twice daily (morning and evening). Both subjects remained compliant with prescribed medical antihypertensive treatments during the study. After 7 days, a decreasing in blood pressure and subjective symptoms was observed. Subject 1's initial blood pressure (150/95 mmHg) changed to 125/80 mmHg (morning) and 120/80 mmHg (evening) on the final day. Subject 2's initial blood pressure (160/100 mmHg) changed to 130/80 mmHg (morning) and 125/80 mmHg (evening). The warm water foot soak therapy indicates a gradual in lowering blood pressure for both subjects. This complementary therapy can be considered a safe, supportive intervention in maternity nursing care.

Abstract

Primary dysmenorrhea is menstrual pain occurring without underlying gynecological abnormalities and may interfere with students’ activities and comfort. This case study aimed to evaluate the implementation of carrot juice and honey as non-pharmacological interventions for reducing primary dysmenorrhea pain among students at Universitas Dirgantara Marsekal Suryadarma. A descriptive case study design was employed using the nursing care process, including assessment, nursing diagnosis, intervention, implementation, and evaluation. The subjects were four students with primary dysmenorrhea. Two subjects received carrot juice twice daily for two days during menstruation, while the other two received honey for five days, comprising three days before and two days during menstruation. Pain intensity was assessed before and after the interventions using the Numeric Rating Scale. Following carrot juice administration, pain scores decreased from 6 to 3 and 2. Following honey administration, pain scores decreased from 4 to 1 and from 5 to 2. Pain reduction was also accompanied by decreased weakness and facial grimacing. The findings indicate that carrot juice and honey may have potential as complementary interventions in nursing care for primary dysmenorrhea. However, controlled studies with larger samples are required to confirm their effectiveness.
