Abstract

Blood glucose testing is one of the most important laboratory examinations for the diagnosis, monitoring, and evaluation of diabetes mellitus management. The accuracy of test results is influenced by various factors, particularly those occurring during the pre-analytical phase, including the type of blood collection tube used and the delay between sample collection and analysis. Delayed examination may reduce blood glucose concentration due to ongoing glycolysis within blood cells, thereby affecting the validity of laboratory results. This study aimed to analyze the effect of delayed examination time on blood glucose levels using plain tubes and Serum Separator Tubes (SST). An experimental study with a Two-Way Repeated Measures design was conducted. Serum samples obtained from seven respondents were collected into plain tubes and SST tubes and analyzed at 0, 5, and 10 hours using a Biobase Automatic Chemistry Analyzer. Data were analyzed using the Shapiro–Wilk normality test and Two-Way Repeated Measures ANOVA. The results showed that the mean blood glucose level in plain tubes decreased from 75.57 mg/dL at 0 hours to 64.29 mg/dL at 5 hours and 65.43 mg/dL at 10 hours. In contrast, blood glucose levels in SST tubes remained relatively stable at 75.71 mg/dL, 76.86 mg/dL, and 78.14 mg/dL at 0, 5, and 10 hours, respectively. The data were normally distributed (p > 0.05). Two-Way Repeated Measures ANOVA demonstrated that examination time, tube type, and their interaction had a statistically significant effect on blood glucose levels (p < 0.05). These findings indicate that delayed examination affects blood glucose levels in plain tubes, whereas SST tubes are able to maintain blood glucose stability for up to 10 hours after sample collection. ABSTRAK Pemeriksaan kadar glukosa darah merupakan salah satu pemeriksaan laboratorium yang berperan penting dalam menegakkan diagnosis, memantau perkembangan penyakit, serta mengevaluasi keberhasilan terapi pada pasien diabetes mellitus. Keakuratan hasil pemeriksaan dipengaruhi oleh berbagai faktor, khususnya pada tahap pra-analitik, seperti jenis tabung yang digunakan dan lamanya waktu antara pengambilan sampel dengan proses pemeriksaan. Penundaan pemeriksaan dapat menyebabkan penurunan kadar glukosa akibat proses glikolisis yang masih berlangsung di dalam sel darah sehingga memengaruhi validitas hasil pemeriksaan. Penelitian ini bertujuan untuk menganalisis pengaruh penundaan waktu pemeriksaan terhadap kadar glukosa darah menggunakan tabung plain dan Serum Separator Tube (SST). Penelitian menggunakan desain eksperimen dengan pendekatan Two Way Repeated Measure. Sampel berupa serum dari tujuh responden yang dimasukkan ke dalam tabung plain dan SST, kemudian dianalisis pada waktu 0 jam, 5 jam, dan 10 jam menggunakan Biobase Automatic Chemistry Analyzer. Analisis data dilakukan menggunakan uji normalitas Shapiro-Wilk dan uji Two Way Repeated Measure ANOVA. Hasil penelitian menunjukkan bahwa rerata kadar glukosa pada tabung plain mengalami penurunan dari 75,57 mg/dL pada 0 jam menjadi 64,29 mg/dL pada 5 jam dan 65,43 mg/dL pada 10 jam. Sebaliknya, kadar glukosa pada tabung SST tetap relatif stabil, yaitu 75,71 mg/dL, 76,86 mg/dL, dan 78,14 mg/dL pada masing-masing waktu pengamatan. Data penelitian berdistribusi normal (p>0,05). Hasil analisis menunjukkan bahwa waktu pemeriksaan, jenis tabung, serta interaksi keduanya memberikan pengaruh yang bermakna terhadap kadar glukosa darah (p<0,05). Dengan demikian, penundaan pemeriksaan menyebabkan perubahan kadar glukosa pada tabung plain, sedangkan penggunaan tabung SST mampu mempertahankan stabilitas kadar glukosa darah hingga 10 jam setelah pengambilan sampel.

Abstract

The accuracy of blood glucose results is influenced by various factors, one of the most important of which is the pre-analytical stage. Blood glucose levels can decrease if plasma separation is delayed, because blood cells continue to carry out the glycolysis process. The method used in this study was an experiment with a posttest-only control group design, employing purposive sampling that involved 5 respondents and 4 treatment conditions for each type of tube based on Federer’s formula. The sample consisted of sixth-semester students in the D3 medical laboratory technology programme who met the inclusion and exclusion criteria. Blood glucose levels were measured using the GOD-PAP method with a BIOBASE chemistry analyser. The results of the analysis showed that blood glucose levels in both types of tubes decreased as the plasma separation delay time increased. For NaF plasma, the One-Way ANOVA test yielded a p-value > 0.05, indicating that variations in the plasma separation delay time had no significant effect. In contrast, the Kruskal-Wallis test for EDTA plasma yielded a p-value < 0.05, which indicated a significant difference in each delay time variation. Based on these results, NaF plasma was better able to maintain stable blood glucose levels during the plasma separation delay compared to EDTA plasma. This is caused by the sodium fluoride content in the NaF tube, which acts as an antiglycolytic agent, so it is able to inhibit the glycolysis process and maintain stable glucose levels in the blood. ABSTRAK Keakuratan hasil glukosa darah dipengaruhi oleh berbagai faktor, salah satunya yang paling penting pada tahap pra-analitik. Kadar glukosa darah dapat mengalami penurunan apabila pemisahan plasma ditunda, karena sel-sel darah tetap melakukan proses glikolisis. Metode yang digunakan dalam penelitian ini adalah eksperimen dengan desain Posttest Only Control Group Design menggunakan teknik purposive sampling yang melibatkan 5 responden dan 4 perlakuan setiap jenis tabung berdasarkan rumus Federer. Pemilihan sampel terhadap mahasiswa Program Studi D3 Teknologi Laboratorium Medis semester 6 sesuai kriteria inklusi dan eksklusi. Pemeriksaan kadar glukosa darah dilakukan menggunakan metode GOD-PAP dengan BIOBASE Chemistry Analyzer. Hasil analisis menunjukkan bahwa kadar glukosa darah pada kedua jenis tabung mengalami penurunan seiring dengan bertambahnya waktu penundaan pemisahan plasma. Pada plasma NaF hasil uji One Way ANOVA diperoleh nilai p>0.05, sehingga variasi waktu penundaan pemisahan plasma tidak memberikan pengaruh yang signifikan. Sebaliknya, plasma EDTA uji statistik Kruskal Wallis menunjukkan nilai p<0.05, yang mengindikasikan adanya perbedaan yang signifikan pada setiap variasi waktu penundaan. Berdasarkan hasil tersebut, plasma NaF lebih mampu mempertahankan stabilitas kadar glukosa darah selama penundaan pemisahan plasma dibandingkan plasma EDTA. Hal ini disebabkan oleh kandungan natrium fluorida pada tabung NaF yang berperan sebagai agen antiglikolitik, sehingga mampu menghambat proses glikolisis dan menjaga kestabilan kadar glukosa dalam darah.

Abstract

Lipid profile examination is used to help assess the risk of cardiovascular disease. LDL level examination can be performed using the indirect method (Friedewald Formula) and the direct method (homogeneous test). The indirect method is more economical and is still frequently used in laboratories, but its accuracy is starting to be questioned at high triglyceride levels. This study aims to determine the accuracy of LDL levels using the indirect method (Friedewald Formula) compared to the direct method (homogeneous test) at triglyceride levels of 200-300 mg/dL. This study used a comparative analytical quantitative design with a paired cross-sectional approach. The study sample used primary data, namely the serum of patients with triglyceride levels of 200-300 mg/dL examined using the indirect and direct methods on the same sample. Data analysis was performed using the Wilcoxon test, mean %Bias, Bland-Altman, and linear regression. The results showed that there was no significant difference between the indirect and direct methods with a significance value (p > 0.05). The average %Bias value of -0.3962% still meets the acceptance limits of the National Cholesterol Education Program (NCEP). Bland-Altman analysis showed that most of the data were within the limits of agreement. Based on the results of the study, the indirect method (Friedewald Formula) has good accuracy and relatively comparable results with the direct method (homogeneous testing) at triglyceride levels of 200-300 mg/dL. ABSTRAK Pemeriksaan profil lipid digunakan untuk membantu menilai risiko penyakit kardiovaskular. Pemeriksaan kadar LDL dapat dilakukan menggunakan metode indirect (Friedewald Formula) dan metode direct (homogeneous assay). Metode indirect lebih ekonomis dan masih sering digunakan di laboratorium, tetapi akurasinya mulai diragukan pada kadar trigliserida tinggi. Penelitian ini bertujuan untuk mengetahui akurasi kadar LDL metode indirect (Friedewald Formula) terhadap metode direct (homogeneous assay) pada kadar trigliserida 200–300 mg/dL. Penelitian ini menggunakan desain kuantitatif analitik komparatif dengan pendekatan cross-sectional berpasangan. Sampel penelitian menggunakan data primer, yaitu serum pasien dengan kadar trigliserida 200–300 mg/dL yang diperiksa menggunakan metode indirect dan metode direct pada sampel yang sama. Analisis data dilakukan menggunakan uji Wilcoxon, mean %Bias, dan Bland-Altman. Hasil penelitian menunjukkan tidak terdapat perbedaan bermakna antara metode indirect dan metode direct dengan nilai signifikansi (p > 0,05). Nilai mean %Bias sebesar -0,3962% masih memenuhi batas keberterimaan National Cholesterol Education Program (NCEP). Analisis Bland-Altman menunjukkan sebagian besar data berada dalam batas kesepakatan. Berdasarkan hasil penelitian, metode indirect (Friedewald Formula) memiliki akurasi yang baik dan hasil yang relatif sebanding dengan metode direct (homogeneous assay) pada kadar trigliserida 200-300 mg/dL.

Abstract

Electrolyte examination is performed to assess the levels of the major electrolytes in the blood. This examination is used to help establish the diagnosis of fluid balance disorders and acid-base imbalances in the body. The test materials commonly used for electrolyte examination are serum and heparinized plasma. Differences in the type of test material may affect the results of electrolyte examinations. This study aimed to determine the differences in electrolyte levels, including sodium, potassium, chloride, and ionized calcium, between serum and heparinized plasma test materials. This study employed a quantitative approach with an analytical observational design and a cross-sectional design. A total of 15 samples were obtained from patients undergoing electrolyte examinations at Muhammadiyah Babat General Hospital in February 2026. Electrolyte examinations were performed using the Erba Electrolyte Analyzer with the direct Ion-Selective Electrode (ISE) method. The results showed that the mean sodium levels were 139.29 mmol/L in serum and 136.49 mmol/L in heparinized plasma, the mean potassium levels were 3.79 mmol/L in serum and 3.58 mmol/L in heparinized plasma, the mean chloride levels were 106.77 mmol/L in serum and 105.63 mmol/L in heparinized plasma, and the mean ionized calcium levels were 1.00 mmol/L in serum and 0.89 mmol/L in heparinized plasma. The examination results were analyzed using the paired t-test and the Wilcoxon signed-rank test according to the data distribution. The results showed significant differences in sodium, potassium, and ionized calcium levels between serum and heparinized plasma test materials, with significance values of p < 0.05. Meanwhile, chloride levels showed no significant difference between serum and heparinized plasma test materials, with a significance value of p > 0.05. Based on the data analysis that had been conducted, significant differences were found in sodium, potassium, and ionized calcium levels between serum and heparinized plasma test materials, whereas chloride levels showed no significant difference. ABSTRAK Pemeriksaan elektrolit merupakan pemeriksaan untuk menilai kadar elektrolit utama di dalam darah. Pemeriksaan ini digunakan untuk membantu menegakkan diagnosis adanya gangguan keseimbangan cairan maupun ketidakseimbangan asam-basa dalam tubuh. Bahan uji yang umum digunakan pada pemeriksaan elektrolit adalah serum dan plasma heparin. Perbedaan jenis bahan uji dapat memengaruhi hasil pemeriksaan elektrolit. Penelitian ini dilakukan untuk mengetahui adanya perbedaan kadar elektrrolit, meliputi kadar natrium, kalium, klorida, dan kalsium terionisasi antara bahan uji serum dengan bahan uji plasma heparin. Penelitian ini menggunakan pendekatan kuantitatif observasional analitik dengan desain cross-sectional. Jumlah sampel sebanyak 15 sampel dari pasien yang menjalani pemeriksaan elektrolit di RSU Muhammadiyah Babat pada bulan Februari 2026. Pemeriksaan elektrolit dilakukan menggunakan alat Erba Electrolyte Analyzer dengan metode Ion Selective Electrode (ISE) secara langsung. Hasil pemeriksaan menunjukkan bahwa rerata kadar natrium pada serum sebesar 139,29 mmol/L dan plasma heparin sebesar 136,49 mmol/L, kadar kalium pada serum sebesar 3,79 mmol/L dan plasma heparin sebesar 3,58 mmol/L, kadar klorida pada serum sebesar 106,77 mmol/L dan plasma heparin sebesar 105,63 mmol/L, serta kadar kalsium terionisasi pada serum sebesar 1,00 mmol/L dan plasma heparin sebesar 0,89 mmol/L. Data hasil dari pemeriksaan elektrolit akan dianalisa menggunakan uji T-dependen dan uji Wilcoxon sesuai dengan distribusi data. Hasil uji pada kadar natrium, kalium, dan kalsium terionisasi antara bahan uji serum dengan bahan uji plasma heparin menunjukkan adanya perbedaan bermakna, dengan nilai signifikansi (p < 0,05). Sedangkan, pada kadar klorida tidak terdapat perbedaan bermakna antara bahan uji serum dan bahan uji plasma heparin dengan nilai signifikansi (p > 0,05). Berdasarkan analisis data yang telah dilakukan, ditemukan perbedaan yang bermakna pada kadar natrium, kalium, dan kalsium terionisasi antara bahan uji serum dengan plasma heparin, sedangkan kadar klorida tidak menunjukkan perbedaan bermakna.
